On today's episode, I'm so excited to be joined by Amy Pershing to talk about Binge Eating Disorder (BED).  Amy is essentially THE expert on treating BED with IFS, and I'm so thankful that she's sharing her wisdom with us!  We've been trying for months to record an episode, and we finally did it! Just a little about her, Amy Pershing, LMSW, ACSW, CCTP-II, is the Founding Director of Bodywise, the first BED-specific treatment program in the United States, and President of the Board of the Center for Eating Disorders in Ann Arbor, Michigan.  She is the founder of Pershing Consulting, which offers training to clinicians treating BED and trauma worldwide. Amy is also the co-founder of "Attune", an online coaching program for attuned eating and recovery support. Amy is an internationally known leader in the development of treatment paradigms for BED, and one of the first clinicians to specialize in BED treatment. Based on 35 years of clinical experience, Amy has pioneered an approach to BED recovery that is strengths-based and trauma informed, incorporating Internal Family Systems (IFS) and body-based techniques to heal the deeper issues that drive binge behaviors. Her approach integrates a non-diet body autonomy philosophy, helping clients create lasting change with food and body image. Amy is also the author of the book Binge Eating Disorder: The Journey to Recovery and Beyond (Taylor and Francis, 2018) and Emotional Eating, Chronic Dieting, Bingeing And Body Image: A Trauma-Informed Workbook, with co-authors Judith Matz and Christy Harrison (PESI Publishing, 2024). She also offers a variety of trainings on BED treatment through PESI. Amy maintains her clinical practice in Ann Arbor, Michigan. Amy begins the podcast episode by discussing her own history with binge eating disorder and how, thanks to the relentless messages from diet culture, she believed that her body "needed to be fixed because it was fat."   Throughout the episode, we touch upon: How IFS views Binge Eating Disorder The role of Binge Eating parts Shame as a coping mechanism How to build trust between Self and parts What Self-led eating looks like Developing trust in your body Amy also emphasizes the fact that Self cannot be damaged by trauma, and Self does not accept body shame narratives.  She states, “There's never a time that you're in-Self and accepting a body shame narrative. It just doesn't happen. Self knows that body shame doesn't exist.” We ended the episode speaking directly to therapists, both those who work with food and body issues and those who don't.  Amy noted that all therapists are working with clients who live in a body and we're therefore all working with body stigma to some degree.  She further notes that all clinicians really need to think about how their parts feel about their own body, food, weight stigma, and health.   And finally, she states, "If you treat BED, you're treating trauma.  We have to understand the binge eating in the context of their history." Amy gives us so many pearls of wisdom in this episode that you'll want to listen to it more than once (and take notes).  Even if you aren't someone who binges, how she talks about body shame is applicable to everyone in a body.  Take a listen! Turner is currently the co-founder and co-CEO of the Body Freedom Project which was formed to realize a world free from body discrimination. Turner is also the founder of the Binge Eating Disorder Association (BEDA) which she successfully merged with the National Eating Disorders Association (NEDA) in 2018 after 10 years of running BEDA's day-to-day operations and programming. A highlight of Turner's work with BEDA was the inclusion of Binge Eating Disorder (BED) in the Diagnostic and Statistical Manual's 5th Edition (DSM-5), published in 2013. Prior to this diagnostic addition, those struggling with the most common and underdiagnosed eating disorder had little hope of being recognized much less accessing treatment and insurance coverage. One of the most forward-thinking and pivotal programs created by Turner and the board of directors of BEDA was Weight Stigma Awareness Week which continues under The Body Freedom Project. This powerful event held every September is critical to the awareness and education necessary to eliminate the weight stigma and discrimination that impedes access to quality healthcare, employment, housing and the basic dignity of not being bullied, harassed or stigmatized based on body shape and size. Turner carries these messages to audiences around the US and abroad in schools, healthcare settings, trade organizations, associations, professional conferences, corporations, and governmental agencies. Turner is also the founder of the Binge Eating Disorder Association (BEDA) which she successfully merged with the National Eating Disorders Association (NEDA) in 2018 after 10 years of running BEDA's day-to-day operations and programming. A highlight of Turner's work with BEDA was the inclusion of Binge Eating Disorder (BED) in the Diagnostic and Statistical Manual's 5th Edition (DSM-5), published in 2013. Prior to this diagnostic addition, those struggling with the most common and underdiagnosed eating disorder had little hope of being recognized much less accessing treatment and insurance coverage. One of the most forward-thinking and pivotal programs created by Turner and the board of directors of BEDA was Weight Stigma Awareness Week which continues under The Body Freedom Project. This powerful event held every September is critical to the awareness and education necessary to eliminate the weight stigma and discrimination that impedes access to quality healthcare, employment, housing and the basic dignity of not being bullied, harassed or stigmatized based on body shape and size. Turner carries these messages to audiences around the US and abroad in schools, healthcare settings, trade organizations, associations, professional conferences, corporations, and governmental agencies. In this episode of Causes or Cures, Dr. Eeks chats with Dr. Trine Tetlie Eik-Nes about her new intervention for obesity with binge eating disorder (BED) that was recently tested in a feasibility study and published here. Obesity is an epidemic affecting many developed nations, and rates only increased during the pandemic, for both adults and kids. In the podcast she will discuss in detail her intervention, called People Need People (PnP), along with the link between obesity and binge eating disorder. She will also discuss the relationship of obesity and binge eating disorder with stigma, shame and our attachment style, and why it is crucial to address those issues in order to help someone recover. Dr. Eik-Nes is an Associate Professor at the Department of Neuroscience and Behavioral Medicine at the Norwegian University of Science and Technology (NTNU). She completed her MSc at NTNU and has her PhD in public health. We spoke about this a little in episode 219. Let's start with real world examples of when and how this starts. I just saw this video but it has been around for quite a while. This video is of a 4-year-old getting her hair done and she tells the hairdresser “I'm So Ugly.” It is not uncommon for body dysmorphia to start this way. The hairdresser tries to tell the girl how beautiful the girl is. The young girl has so much energy tied up in the message that she is not pretty or enough. Body dysmorphia can easily start this early. In fact, this young girl had most likely thought this more times before saying this. The crazy thing about body dysmorphia is that there is no reality to it. Ariyonna, this young girl 4 years old already has preconceived ideas about her beauty. Her ideas and her looks don't match. Where does the evidence come from, behind these kinds of thoughts? The truth is there is no evidence. The focus is on a “perceived” defect or flaw in one's body. But when you have body dysmorphic disorder you intensely focus on parts of your appearance and image. The people suffering from this take it to the extreme. Think about how many times you look at yourself in the mirror each day? Are you seeking out cosmetic fixes or procedures to overcome perceived flaws? Do you look for reassurance from others about how you look? Do you fixate on your face, nose, wrinkles, hair, skin breast size, muscle tone or genitalia. But all these dysmorphic ideas have no evidence in reality. As these ideas continue to grow, they can start to appear in experiences such as eating disorders. Eating disorders show up as anorexia, Binge Eating Disorder (BED), and four other feeding and eating disorders. Dysmorphia also can play a part in depression, suicidal thoughts, anxiety, obsessive-compulsive disorder, substance abuse, behavioral health issues, and disfigurement due to repeated surgical interventions. I'm sure you can think of people in the news that have suffered from at least one of these. It is not hard to see how one can go from being preoccupied with the apparent flaws your body has to being on this list. These problems are very common in fact there are more than 3 million cases per year in the US alone. Body dysmorphia is based on the thought of how others view them. It is about fears that you have. You fear the things you believe others will see in you. It is a good practice to include journaling, breathing, and meditation. Your ideas once written down will be observed and you will more easily be able to move past them. Once written down you might be able to see how the view you have accepted is just perceived and not as real as you originally gave it credit. Rid yourself of things that cause you to have low self-esteem. If clothing doesn't fit you well donate it. If friends or relatives bring you feelings of not being good enough, put some distance between you and them. You may be comparing yourself constantly with others. You may set unrealistic goals for yourself. Instead of trying to set up outcome related goals make better choices on what you eat. Eat less processed foods and more fruits and vegetables. Is bingeing the same thing as Binge Eating Disorder? (No, not exactly.) Does Binge Eating Disorder require restriction to treat? (NO.) Is bingeing a response to physical and/or mental restriction? (YES.) Can trauma/emotions/anxiety cause Binge Eating Disorder without the presence of restriction?? (Yes! However it is extremely rare in our culture for restriction/guilt to not ALSO enter the picture after the fact, and therein lies the importance of addressing restriction, even if and when it isn’t the cause.) The line between disordered eating (bingeing) and Binge Eating Disorder is blurred, just like it is for all disordered eating/eating disorders. People binge in response to restriction, and they binge in response to trauma, anxiety, or difficult emotions. The important thing to remember though, is even if restriction isn’t the main cause of your bingeing, it is probably a big piece of it, just because of the culture we live in. Addressing restriction is extremely important, along with any other emotional/trauma-based treatment. Anyone with an active eating disorder should be getting treatment, and The F*ck It Diet is just a supplement. (Note: This particle post is associated with an episode that I recorded on August 1st, 2020, responding to a disgruntled instagram follower who was angry that I was “ignoring binge eating disorder” in my instagram posts, and saying that I imply that all binge eating disorder is a response to restriction. To get the full drama, (anonymous) angry comments and DMs, the accusations of being unethical and predatory, and my full response, listen to the episode above. For the purposes of this post, I am going to distill it down to discussing the nuance of bingeing vs. BED.) The first and most important thing I want people to know about bingeing, is that when it is in response to calorie restriction, it is the body's attempt to protect you against past and future famine. In a very basic sense: it is protective. And if you've tried to diet a lot, whether you were "successful" or not it is also going to be a response to diet thoughts. That's an example of what people mean when they say: "mental" restriction. Restriction doesn't have to be literal and physical (ie, less calories), it can just be mental guilt, rules, or diet ideation. A LOT of our collective bingeing is because of our collective dieting, and out collective diet culture. The question then becomes, is that all? Do we binge for other reasons? To make sure I've covered my bases, I consulted Casey Bonano, CEDRD-S to weigh in on bingeing vs Binge Eating Disorder (BED). She is a Dietitian specializing in Eating Disorders, an anti-diet approach, HAES, & Intuitive Eating. You can follow her on instagram here, and find her Food Freedom Guide here. Here is Casey’s breakdown of the difference between bingeing and Binge Eating Disorder: “Bingeing is usually based on restriction, and with incorporating all foods, some self reflection, and general coping skills it can be resolved. BED is more complex and will take a deeper dive into the psychological or emotional components, usually requiring mental health professions such as a therapist and a dietitian, but restriction (whether physical or mental) is often a component of BED as well and must first be addressed or ruled out before the emotional work can be done.” There are people who believe that avoiding binge trigger foods is the way to treat Binge Eating Disorder, along with addressing the emotional/mental components of BED. I do not consider this a full recovery, as it uses restriction as a treatment. This is treating one eating disorder with another eating disorder. This will tend to backfire longer term, or just be manifested as restrictive disordered eating. When people claim that avoiding binge foods is how they “recovered” – I would never tell them that their lived experience isn’t valid, people should do what they feel is working for them,

While I think the majority of these roles are positive, there are also some people who feel trapped by food. Chevese is the woman who almost 9 years ago founded the Binge Eating Disorder Association (BEDA) in June 2008, recognizing the need for an organization to advocate on behalf of individuals affected by Binge Eating Disorder (BED), as well as the providers who treat them. Turner’s well-rounded career in the health care, pharmaceutical, political and non-profit arenas, as well as her many leadership roles within these fields, prepared her to launch BEDA, the only organization specifically suited to support individuals and treatment professionals managing BED. Today, you’ll hear Chevese talk about: – How old she was when she lost the innocent, joyful, and non-judgmental attitude towards her own body – What drove her to start searching for, stealing and hoarding food – What every type of binging behavior stems from – How her mother related to food and weight – Why denial about unhealed past wounds always leads to us passing along the same pain (we can not outrun our own shadow) – Why those old painful memories are often the ones that leave the deepest imprint on our soul – Why we often treat ourselves the way we have been treated as children – Why food (restricting/dieting, or overeating) is so commonly used to try to escape difficult emotions – How we can start to make sense of our own behavior around food – Why there is no need to be ashamed for recognizing those behaviors for what they are – How shame and weight-bias affect a person’s physical and mental health – Why all the advice she got actually only perpetuated her eating disorder – Why she never really knew what she liked/what her hobbies were – Why she immediately regretted having lap band surgery – What is commonly misunderstood with Binge Eating Disorder (BED) – Why, at the root, anorexia and BED are much more alike as they outwardly seem – Why our society is too quick to categorize people, instead of listening to the person in question first – How terribly effective diet-culture and its constant messages and images really is, and why it is important to be aware of how all of that seeps into our subconscious and colors our own judgment – What she wants us all to know about our bodies – Why bariatric surgery is often done solely for profit, and why there’s so little information about the dangers and the long-term effects of it – The medical ramifications of our society’s belief that "healthy" looks a certain way – How she reacted when she first heard that her Binge Eating Disorder was only to be treated effectively by nourishment – How many years she wasted by being stuck in the diet-cycle, and having little mental space for anything that would have really sparked her interest – Why so many body-obsessed women have no idea who they really are outside of dieting – What led her to found BEDA, and how wonderfully that organization developed – What BEDA is about and what they are actively working on – Why it is important for all of us to resist any sort of weight-stigmatization, and to speak up against it... ... Key Points: Hoarding, bingeing, and other related behaviors are the most common experiences from childhood dieting/restriction. I am sorry on behalf of all health and medical professionals for the oppression. You don't have to choose health OR healing. Working on healing your relationship with food will help you promote health long term. Healing is not passive or giving up or weakness. Healing is an active process never passive. Fat on the body didn't CAUSE the problems. Eating sugar didn't CAUSE the problems. Being shamed in your body and not teaching you how to tolerate feelings caused the problems.  Healing from Binge Eating Disorder (BED) takes on average 7 to 14 years. Restricting nutrients or calories may show favorable outcomes in short term yet worse in the long term especially if affected by BED. Keep the data! You need proof that you're bingeing less as time goes on. Diabetes is a chronic progressive disease. It is always changing and slowly getting worse even if you do everything you can.
