I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Claudia: Hi Dr Cabral, I would like to first thank you for all of your knowledge that you teach. I have learnt so much from you and I have implemented so many healthy habits in my family too, it's actually been life-changing. I recently heard someone saying protein powders aren't a good idea for the average person because they are highly processed, spike your blood sugar and aren't good for the liver. What are your thoughts on this? I usually have the DNS, blueberries & filtered water in breakfast smoothie then in the afternoon I sometimes have a vanilla bean protein shake from Just Ingredients to help me reach my current daily goal of 100g protein as a 110 lb 31 yo female who strength trains 3 x per week and gets 8-10k steps day. Thanks in advance.     Cory: We have mold in our house that was built in 1934 and we all cough (4 adults & 2 small children [2 & 5 years old]). The 5-year-old has eczema. I am the only one in the house that trys to eat only natural & organic foods. I teach nutrition, gardening & physical activity. We use a water pitcher filter for drinking water, and we have shower-head filters. I try to tell my family the importance of eating healthy in order to help each one of us get healthier (my husband is diabetic type II). My stepdaughter loves to take the boys to fast-food restaurants. No one really listens to me, and they don't value what I have to say. They all only speak Spanish. Is there some way that I could get some of your teachings in Spanish for them? (I'm the only one that speaks English in my house.)     Sandra: Dear Dr. Cabral, I'm a big fan of your work and was hoping you could offer some advice. I'm a 40-year-old woman looking to improve my heart rate variability (HRV) as I know it's a marker of overall health. Despite consistently practicing healthy habits such as adequate sleep, breathwork, meditation, cold therapy, regular exercise, stress management, hydration, and alcohol abstinence, my HRV remains between 30 and 40, which I believe is too low. Do you have any additional suggestions to help me further increase my HRV? Thank you in advance for your time and expertise.     Anonymous: Hi there, I've been told that tinnitus and other side effects that have been possibly linked to a certain intervention ( $h0t) that a lot of people received in 2020 is irreversible. Do you believe that any ill effects from these interventions are considered “toxicity” and can be removed and restored to baseline? Is there anything you suggest besides quarterly detox/ daily detox to eliminate these specific “things”? Thanks Dr Cabral!     Stacey: Hi Dr. Cabral , I have a question about ASIA syndrome. After much research I think it may be what I have due to breast implants and hernia mesh . Knowing this is disturbing because I'm having a very hard time imagining myself without implants . I have Dysautonomia/ PoTS, Raynauds and had MS type symptoms until hydroxychoriquin. I'm looking into fat transfer to breasts when I explant but I'm afraid of that potentially causing cancer to activate and necrosis . Would the body find necrosis as a foreign object in the body as well ? I'm so confused at what to do to heal . Looks like removal of mesh and implants are extremely important. She left the Civil Service last year to recover from Long Covid, having first contracted Covid in November 2021. Ify has caring responsibilities for her elderly mother and is currently cat fostering as part of Cats Protection's Lifeline scheme. During the course of her treatment for Long Covid, Ify was diagnosed last year as being autistic. Ify has a passion for cats, the works of Jean Claude van Damme, films and music from the 1980s and 90s, and she has recently discovered the joys of crochet. Raynaud's syndrome is more common than you may think, which was a surprise t me because I first heard about it when I met Chelsea Lea MacColl around 5 years ago. Raynaud's is a condition where the person feels the cold in an intense manor. As we talked through it in this interview it became clear that there aren't many ways to explain the difference to a person without the condition of "feeling the cold a bit too much" and it making your life oftentimes a misery. The closest we got was hypothermia! In this interview Chelsea talks about the physical, emotional, mental and social effects of Raynaud's syndrome. A condition that permeates every part of her life, even in warmer temperatures. She also shares some of the things that she has found to help manage the symptoms, which for Raynaud's sufferers is currently the best level of medical intervention.

Today I talk with Dr. Michael Hughes, he is a consultant rheumatologist at Salford Royal Hospital, which is a national UK referral center for SSc, and Honorary Senior Clinical Lecturer at The University of Manchester. He has emerged as a leading national and international investigator in SSc and Raynaud's phenomenon, with a focus on novel clinical assessment of vascular biology and function, including the development of new novel approaches to treatment. His doctoral PhD research fellowship (2013-2016) at The University of Manchester investigated the outcome measures of treatment efficacy, pathophysiology, and local treatments for SSc-digital ulcers. Dr.Hughes has a strong research interest in non-invasive imaging and the development of patient reported outcome measures, and works closely with patient-led organizations. He was awarded the Edith Busch Young Investigator Award in recognition of his significant contributions during the World Scleroderma Congress in 2022. Under the auspices of the World Scleroderma Foundation he leads the Digital Ulcer Working Group, and co-leads the Gastrointestinal disease Working Group. Dr. Hughes has published extensively in both clinical and basic research and has contributed to the development of SSc treatment guidelines and recommendations. He contributes to many international initiatives and collaborations including as a Fellow of the OMERACT Scleroderma Vascular Disease Working Group. He has presented and chaired (including invited oral presentations) at prestigious national and international conferences and is on the editorial board of several international rheumatology journals. Furthermore, he has published several studies in high-impact journals as lead author utilizing data from the EUSTAR cohort. Our topic today is 3 things that affect me. Zone 2 cardio [16:27] Tracey says: Thank you for all of the hard work you do spreading the good word! I have been a fan for many years and i just can't get enough of your content. You are both so smart and down to earth at the same time. While I have burnt out and cycled through many other podcasts and nutrition/fitness trends, you are tried and true. I am writing because I am interested in zone 2 cardio. I heard Peter Attia discussing it and then Robb brought it up recently as well. I would love to learn more and any guidance on where to start, as the internet is generally overwhelming, would be much appreciated. I am hoping to take up this cardio fitness endeavor by swimming laps in an indoor pool. I have a small amount of experience with freestyle swimming. Some probably irrelevant background: I am 33 and in good health. I did CrossFit for about 5 years, then gravitated to competitive weightlifting for 4 years and the last 3 have been a combination of barbell strength and hypertrophy. Cardio has been nearly non existent and I am feeling it! I'm 20 weeks pregnant with my first so longevity is weighing on my mind, and I am a little more out of breath at the gym than usual. I have been feeling great and very strong though! It is generally not in my nature to “take it easy” at the gym but I know I need to start thinking about backing off at this point in pregnancy. Starting a new endeavor may seem counter to that, but I am very cautious and know when and when not to push myself physically. Changing gears with my fitness will help me to both stay motivated and remain cautious. I am in it for the long haul and take a lot of joy in seeing long journeys (like this one would be) through. Slow and steady as they say. And advice or words of wisdom you may have as far as zone 2, swimming, or even where I can look for pregnancy/baby resources would be so incredibly appreciated. Check out today's questions:    Tommy: Hi Dr C you incredible human being. I'm a level 2 IHP. For years Iv been battling self diagnosed lupus, my face goes embarrassingly red. Iv been working on this and have it more under control than ever. But since the summer has come, Iv really noticed how bad heat affects it. I thought it was just food such as histamines and sensitivity. I know heat can trigger histamines. Iv run the OAT and I'm high on 3 yeast markers including oxalates. By time this gets read out I should be some way through the CBO protocol, however I have done this before, but I didn't stick to the meal plan. Iv also purchased DAO and Iv tried many supplements including quercitin. I know there still so much more info you would need but do you have anything you can suggest.   Ann: Due to my increased risk of breast cancer Im on a medicine to eliminate estrogen in my body ( I have also had my ovaries removed). The 5 year limit to take the medicine is nearing, so Im wondering what I can do, from your perspective, to reduce estrogen in my body? I want to ask if there's a way to remove painful periods? I was born with two uteruses and I have extremely painful periods on the first day and sometimes the second. I'm in pain as if I was being stabbed in my uterus 1000 times. Ever since I got my period when I was 13 years old, I've been taking at least one advil a month to relieve myself of the period pain. And when I turned 23 I started to develop itchy scalp, vaginal itch, and itchy ears. Listening to your podcast helped me discover the OATS and CBO protocol. I've taken CBO twice and the itch has reduced but not completely gone. Vaginal itch is still there a bit as well as the ears and scalp. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Eve: Hi Dr Cabral. Thanks for answering my question. I have always struggled with belly fat. The rest of me is thin but I have lower belly fat and love handles. No matter what I do I can't get rid of it. I eat incredibly well, alternate between strength and cardio training daily and fast for approx 16 hours every day. I definitely do not overeat, perhaps sometimes under eat when I am having busy days. My only thought is that it could be hormonal (I've just taken out my IUD that I have had in for 6 years) or something to do with stress levels? Any advice you have would be greatly appreciated as I have been trying to find an answer since I was about 10 years old.   Angie: Hi Dr Cabral. I have had HPylori and am healing gastritis due to that. I used to drink one cup of coffee in the morning, but haven't done that in over two years. I started drinking bio coffee which ingredients are: Instant Arabica coffee powder, non dairy creamer (glucose syrup from tapioca starch and vegetable fat from palm kernel), oligosaccharide (from chicory root) and wheatgrass powder. It's suppose to be alkaline and it's the only “coffee” that doesn't hurt my throat and stomach. I actually feel better throughout the day if I drink it. Ideally, it would be best if I never drank coffee or processed foods, but I am curious to hear your review of the product.   Anonymous: Hi Dr. Cabral, I am a fairly new follower but want to thank you for all that you do. I listen to your podcast every day and enjoy learning about the wide range of topics that you talk about. My question is on behalf of my partner. He is an extremely heavy sweater at night, to the point where I feel like I need to wash the sheets every day. He is 29 years old, vata body type. He is not over weight but is working on implementing less red meat and more vegetables into his diet. He has a pretty active job but does weight work outs 2-3 times a week and runs a few times a week as well. The one thing he does enjoy is beer, and drinks around 3-4 daily. Other then that, he is a pretty healthy guy. I have tried to look for what could cause sweating at night but mostly find information on menopausal women. I was wondering if you had any suggestions for things to look into or tests to run to start trying to figure out the cause of his heavy nighttime sweating. Thank you so much.   Anonymous: Hi Dr. Cabral, I had a question about niacin. Growing up my mom had told me that getting too much niacin, specially as a female, was not good for your health. However, if I take the DNS powder and activated vitamin B complex each day I would be consuming well over the recommended daily intake of niacin. My mom doesn't specifically remember why she thought too much niacin was bad, so I figured I'd ask you. In my research I did find that pregnant women need to be careful with niacin intake, but couldn't find any information on any health risks for non-pregnant individuals. Any insight would be appreciated, thank you.   Stacey: Hello Dr. Cabral You have become a beacon of light & hope to me. I have been suffering for 15 years with extremely strange symptoms. My first symptom was SVT on my daughters 1st BDay my HR was 240 when the ambulance arrived and took about 10 min of baring down to get it under control or they would have to shock me . This continued until my catheter ablation to my heart a year after in 2007 . Soon after new symptoms popped up .. numbness in my chin , left leg , weakness in legs , brain fog and the very worse is and was the sensory overload. I have not been able to go to a movie , family gathering , b day party , any event without alcohol to calm my nervous system down . If I don't drink ( and it's shameful & no I'm not an alcoholic) I will actually throw up from the sensory overload and a feeling of what I'd think a seizure feels like comes me . It's unbearable to say the least . ( p.s. zero family history of this) MS has been ruled out , my Drs have not been able to diagnose me with anything except SVT , Raynauds, MCAS & now POTS / dysautonomia in 2018 . These last 4 years have been my absolute worst ! I'm afraid I'm going to die . I'm now on xanax and plaquniel. Do you think your program can help? I have breast implants since 2006 , hernia mesh and amalgams.   Megan: Hi Stephen I have two questions for you: 1. to what extent does diet outweigh exercise i.e. if I only ate super clean and my exercise regime consisted of a light walk and gently yoga every day, would I lose muscle tone or would I retain light tone? 2. in your opinion what should a typical eating day be like for a vegetarian? Episode 95: Dawn Veselka's Story of Raising a Child With Mixed Connective Tissue Disease In this episode, guest Dawn Veselka discusses her experience raising a daughter with a mixed connective tissue disorder. Her mix is juvenile arthritis, scleroderma, Raynaud's and dermatomyositis. Sadie's triggers are exposure to viruses, sunlight, and certain foods. They determined her triggers through trial and error. Her daughter, Sadie, started showing signs of her condition at 18 months. She started showing trunk rashes, stopped being able to walk, not eating much due to jaw pain, wouldn't hold a spoon or pencil, and  ate with her fingers. Dawn tried to find a diagnosis for her, but the doctors didn't think she needed to get testing for Sadie. Eventually, she couldn't move or get off the floor. She was told that she was just a worried first time mom, was told that she was putting her child through horrible tests for herself and not her child. She had to research things on the internet to find a proper doctor, and then Sadie was properly tested to find her actual diagnoses. Dawn recommends listening to your parenting instincts.   To help Sadie, they tried a lot of different treatments, including medications, shots, IVIG, infusions, anti-inflammatory diets, chiropractic, and acupuncture. It took a lot of time and treatments for Sadie to be able to be more independent with activities, physically. Due to discrimination from others, Dawn and Sadie decided to do homeschooling, which has worked very well for Sadie. Dawn heard so many stories of teacher's discrimination against children with chronic illnesses. Sadie did a lot of co-ops to help with school. Although Dawn  didn't want Sadie to have to use a wheelchair, Sadie greatly increased her independence and abilities when using a wheelchair, so she started using one.   Dawn recommends making family a priority. Sadie prioritizes her energy and plans in down time or recovery time for afterward. Dawn has advocated for Sadie by standing up for her, getting involved in foundations, teaching Sadie to follow her internal voice, and giving  up expectations others put on her. Sadie has found kind and empathetic friends who are really compassionate. Dawn  taught Sadie how to stand up for herself. They use video chats to help Sadie be social when she doesn't feel well enough to get together in person. Sadie is finally in a place where her condition is very stable.   Dawn uses social supports by talking with other medical moms and  running a charity. Dawn tried to find a diagnosis for her, but the doctors didn't think she needed to get testing for Sadie. Eventually, she couldn't move or get off the floor. She was told that she was just a worried first time mom, was told that she was putting her child through horrible tests for herself and not her child. She had to research things on the internet to find a proper doctor, and then Sadie was properly tested to find her actual diagnoses. Dawn recommends listening to your parenting instincts.   To help Sadie, they tried a lot of different treatments, including medications, shots, IVIG, infusions, anti-inflammatory diets, chiropractic, and acupuncture. It took a lot of time and treatments for Sadie to be able to be more independent with activities, physically. Due to discrimination from others, Dawn and Sadie decided to do homeschooling, which has worked very well for Sadie. Dawn heard so many stories of teacher's discrimination against children with chronic illnesses. Sadie did a lot of co-ops to help with school. Although Dawn  didn't want Sadie to have to use a wheelchair, Sadie greatly increased her independence and abilities when using a wheelchair, so she started using one.   Dawn recommends making family a priority. Sadie prioritizes her energy and plans in down time or recovery time for afterward. Dawn has advocated for Sadie by standing up for her, getting involved in foundations, teaching Sadie to follow her internal voice, and giving  up expectations others put on her. Sadie has found kind and empathetic friends who are really compassionate. Dawn  taught Sadie how to stand up for herself. They use video chats to help Sadie be social when she doesn't feel well enough to get together in person. Sadie is finally in a place where her condition is very stable.   Dawn uses social supports by talking with other medical moms and  running a charity. Raynaud’s Phenomenon is an interesting condition in which spasms of small blood vessels cause changes in the skin color of the hands and feet. Raynaud’s phenomenon can also cause pain in the feet. Foot pain is also very common in runners.  Raynauds is one of those conditions that isn’t typically diagnosed with a blood test or and x-ray. The diagnosis is typically made “clinically” meaning the doctor hears your story, listens to your history and decides you probably have the condition, because nothing else fits. The question is whether or not a runner who gets an MRI because of foot pain, might have something show up on the MRI images that can indicate Raynaud's is contributing to the runner’s trouble. Much appreciated, Naama Marie: Hello! Do you believe that liposomal vitamin c is more beneficial than „regular“ vit c? If so, do you have any brand recommendations? Greetings from the UK! Jordan: Hi dr Cabral thank you for your podcasts I love listening to them! I wanted to ask you a question that no doctor has been able to help me with. I have had vagina thrush for longer than I can remember. The symptoms come and go same with being positive on any tests. I even tested positive for candida glabrata at one point. I did take quite a lot of antibiotics at one point for strep throat when I was around 17. I am now 22 and have taken every possible candida prescription out there for this. I haven’t taken any antibiotics for over a year now and refuse to go to a M.D. as they just seem to prescribe more items that won’t work. I came off birth control 2 years ago and live a pretty healthy lifestyle. I don’t take any other medication and am generally healthy otherwise- although I did have a thyroid that was slightly slow according to my naturopathic doctor. Last time we checked he did say my numbers were better due to eating and taking vitamins to balance that out. We have not been able to solve my thrush condition tho and nothing has been able to solve it. Please please please let me know if you have any suggestions, natural remedies, or what to buy! My guest on the podcast this week, Gilly, was someone I had never met before. Yet I felt so familiar with some of the swims she had done earlier this year, due to the amazing photographs her friend, James Kirby, took of her and submitted to Outdoor Swimmer magazine. I looked at the snowy landscape and frozen lakes with envy and admiration, and knew that I wanted to speak to her to understand what that had felt like. Gilly had been doing an alternative version of the R.E.D. challenge - which stands for 'run every day' and had committed to swimming outdoors every day in January. I've got to admit, I was reassured to hear that it wasn't easy every day. That Gilly can procrastinate a day away. That it was mentally quite hard. That she is human like the rest of us. It can be too easy for us to assume that other people manage with ease the things we know we would find really difficult. It was also lovely to hear about the people who she helped take their first step into winter swimming as part of the process, and what different people's reaction was to achieving something they might have thought was impossible for them. She talks about winter swimming as sailing close to the edge of what is possible, and peeping over the wall of the edge of comfort and glimpsing what is beyond. It is about pushing ourselves and testing and finding out what we are capable of. Gilly also talks about what can happen to our lives when particularly difficult or tragic things happen. We can lock it away, close in on ourselves and have our world become smaller. Or we can look at it, process it, and allow the possibility that we will experience joy again. At some point. In the future. She says we have to push hard to swim up to the light again. To reach that flow state. Check out today's questions: Anonymous: Hi Dr Cabral, I'm a long time listener and have used some of your tests and products. I truly appreciate everything you do. My question for you is about my recent food sensitivity test results. Out of the three levels of reactivity (high, moderate, mild), I was moderately high for dairy and moderate for 22 other foods, such as nuts, gluten, green beans, peas, soy, coconut, and eggs. This seems like a lot of foods to have a sensitivity to so I'm wondering if I should do your SIBO/Candida protocol a 2nd time (did it earlier this year) in case I have SIBO/Candida issues again or if I have to just omit these foods for a while to give my body a rest. As always, appreciate any insights you can share on how best to move forward. Zoe: hi Dr Cabral, another question for you to kindly answer - i wondered if you had any thoughts how to manage a combination of rosacea and rayneuds (as, they appear to me, to be equal and opposites) - ie what is good for rayneuds - like saunas, keeping warm, spices - is bad for rosacea. Can you tell me what body type this is and recommend what diet I can benefit from the best? I am training for a marathon as well so I want to feel and look my best. I had blood work done and found I am low on Iron but I've been taking it for a while now, with little improvement. I try to drink coffee to give me an alert boost but it doesn't help much. As much as I love coffee, it doesn't love me, as It never fails that I breakout after drinking it for more than once a week. Why is this, and what could all of these symptoms be a sign of? IS there a test I could run to determine what is going on? Many thanks, Jenett   Kelly: Hello Dr. Cabral. I love your podcast and listen every day! I am a 31 year old female that has suffered from on-again/off-again bulimia since I was 15. The root of my bulimia has always been my desire to lose weight. I am VERY pear-shaped (medium sized on the top, very large in the hips, thighs, and butt) and have always been overweight despite infinite futile attempts, which always leads me back to bulimia. Two questions- FIRST QUESTION: Can you speak to why some women have such large bottom halfs? I have been taking DIM, Calcium D-Glucarate, adrenal support herbs, extremely clean vegan diet, as well as progesterone cream on days 14-28 of my cycle to try to balance out my hormones after blood tests revealed almost non-existent progesterone levels and average to above-average estradiol levels. SECOND QUESTION: What are the long term effects of bulimia? I have always suspected that my bulimia habit is working against my weight loss efforts due to an insulin rollercoaster. I have a friend with a nagging achilles injury that just got back from a sports therapy clinic. They are using nitro patches on the inflammed site, then will be doing concentric stretches/strengthening of the area, all to hopefully get more blood into the site to promote healing. Have you heard of this? What would be potential side effects? Do you think it would be effective for just this type of injury, or might it potentially be useful for other injury site (namely ITBS on the knee). Mike Schrankel asks: What do you think of Low Volume/High Intensity weight training? One apparently reputable website (http://www.exrx.net/WeightTraining/LowVolumeTraining.html) goes to great lengths explaining that performing extra sets beyond one light warm-up and one heavy set of 8-12 reps yields only 0-5% more muscle growth. But most pros, including yourself recommend 3-5 sets. I've checked out your "Get Fit in 30 minutes or Less" (12/5/10) post, in which you recommend 5 sets. I like the short time commitment, but I'd like to do a wider variety of exercises with less sets if I can get decent results (i.e., more lean muscle, not "big and brawny") Lorna asks: why does it matter whether I burn fat or glycogen during my workout? I thought that if I am in deficit at the end of the day - fat will be used for energy - giving me a body fat loss. Whether I burn glycogen or fat - a deficit will mean fat stores reducing when it all balances out- will it not? Laurel asks: I have two questions.  1-Are there homeopathic or home remedies for Raynauds phenomena? 2- You tout magnesium, what do you think of epsom salts as a source? Jeff has a call-in question about soreness when getting started with weight training. In my response to Jeff, I recommend he listens to the first 5 episodes of the Get-Fit Guy's Quick & Dirty Tips Podcast Ralph Sanabria asks: Ben, I have a hard time running (more breathing). I've been told that I have athletic induced asthma. I've been told this because i have a hard time breathing during. and it sounds like wheezing. Is there anything that i can do to help with this? In my response to Ralph, I recommend oil of oregano. Cat asks: Do you know of any potentially harmful effects of chewing sugar-free gum? I am a student and find that chewing gum helps me to stay focused while I work, but it seems like I've developed sort of an addiction to it. Are there properties in sugarfree gum that could make it addictive and harmful to health? In my response to Cat, I mention "Spry" gum. Ryan asks: I recently read this article http://www.consumeraffairs.com/news04/2011/02/california-deaths-after-lap-band-surgery-lead-to-lawsuits.html  about complications that are occurring with lap band surgeries. My sister is considering a lap band surgery and this article really concerns me!
