What does proper midlife care actually look like, and who keeps getting left out of it?In this episode, Laura sits down with Dr Caoimhe Hartley - founder of Menopause Health in Dalkey, clinical lead of the Complex Menopause Clinic in the Rotunda, and clinical lead for women's medicine at the new BlackRock Health Women's Health Centre. They talk about the women being told no - those over 60 who feel they have missed the boat on HRT, and those who have come through breast cancer and are still struggling with symptoms. Caoimhe explains where the evidence actually sits on bones, blood vessels and brain, why so much of what gets repeated still comes from the Women's Health Initiative, and how modern HRT differs from the older oral preparations. The conversation moves through midlife weight gain and changing body shape, the rise in adult ADHD diagnoses, and the link between oestrogen and dopamine. They also dig into osteoporosis screening in Ireland, why there is no national DEXA programme, dietary calcium, and the histamine flare some women notice on HRT.Throughout, Caoimhe keeps coming back to the same point - care should be personal, joined up and built around the woman in front of you.

Farmers in Wales and Scotland are asking what last week's elections in Scotland and Wales will mean for agriculture. Neither Plaid Cymru in Wales nor the SNP in Scotland have ended up with an outright majority. In Scotland, Mairi Gougeon, the Rural Affairs Secretary, didn't stand for re-election, so a new appointment will have to be made. In Wales, Plaid Cymru have won 43 out of the 96 seats in an expanded Welsh parliament so are short of an outright majority. It means the Welsh pro-independence party will need the support of others to pass laws and a budget in future. So what does it all mean for agriculture and the environment - both policy areas that are largely devolved? The agriculture show season begins with Balmoral Show just outside Belfast this week. Farming Today will be reporting from the show, and all week, we'll be looking at different aspects of farming across Northern Ireland. Unique to the United Kingdom, Northern Ireland's Soil Nutrient Health Scheme is the largest baseline soil sampling programme ever undertaken. The £37 million government-funded scheme is managed by the Agri-Food and Bio-Science Institute (AFBI) and has taken four years to complete.A new centre to identify and address plant diseases is being set up with government funding of £3 million. The National Centre for Environmental Horticulture Plant Health will be virtual, operated by staff at the government's Animal and Plant Health Agency and the charity, the Royal Horticultural Society. It's hoped that commercial plant growers and gardeners too will send in evidence of pests and diseases to help stop their spread.Presenter: Anna Hill Producer: Rebecca Rooney

Roy L Hales/Cortes Currents -The Quadra, Cortes, and Surge Narrows communities, as well as the Klahoose and We Wai Kai First Nations, have sent the BC Ministry of Health an application for funding to set up a Discovery Island Community Health Centre. “It offers stability for practitioners. It offers leadership stability through a joint board and a hired executive director. It offers some economies of scale in terms of practitioners being shared and supplies being shared. It offers a shared medical record between the communities. Potentially, one of the things is using video conferencing as opposed to an in-person visit, if that's appropriate. It's community driven, it doesn't involve Victoria telling remote communities what works for them, because that generally doesn't work for them,” explained Bernice McGowan, a member of the planning committee for the proposed Health Centre.

Send us a textOn this episode of the CMAJ Podcast, Dr. Mojola Omole and Dr. Blair Bigham explore the growing challenge of antimicrobial resistance and how shorter courses of antibiotics are reshaping prescribing practices. The conversation builds on insights from the CMAJ practice article “Five things to know about shorter courses of antibiotics” co-authored by Dr. Maria Ivankovic, an emergency physician at St. Joseph's Health Centre in Toronto.Dr. Ivankovic explains why shorter courses of antibiotics are as effective as longer ones for many common infections and how this approach can reduce the risk of antimicrobial resistance and adverse effects for patients. She highlights key conditions with strong evidence for shorter durations and discusses practical strategies for implementing these changes in practice.Dr. Lynora Saxinger, an infectious diseases specialist and professor of medicine at the University of Alberta, broadens the discussion by examining the current state of antimicrobial resistance in Canada and globally. She explores the drivers of resistance, including unnecessary and inappropriate antibiotic use. Dr. Saxinger introduces the concept of the antibiotic footprint as an analogue to the carbon footprint, encouraging physicians to consider the collective impact of their prescribing decisions. This episode provides actionable advice for physicians looking to balance patient care with stewardship practices, helping to preserve the effectiveness of antibiotics for future generations. To learn more about exciting physician careers from our sponsor, Vancouver Coastal Health, visit vch.ca/jobyouwantJoin us as we explore medical solutions that address the urgent need to change healthcare. A 24-hour fully-serviced drop-in mental health centre is being proposed for Shannon. The suggestion comes from Independent local election hopeful Pat Barry who's described the ongoing mental health crisis in Clare as well as nationwide as "appalling". Shannon's Hope Café played host to its first mental health conference in January where Azim Khamisa, who's been celebrated by the Dalai Lama, Al Gore and Bill Clinton, as well as renowned psychotherapist Robert Carley, were guest speakers. Long-time Shannon resident Barry, who was one of the event's organisers, insists the town needs a full-time accessible mental health service.
