This week we review a recent report of 4 patients with serious complications related to either perforation or external compression from the distal tines of the Alterra prestent when used for transcatheter pulmonary valve replacement. We speak with Dr. R. Allen Ligon, interventional cardiologist at Children's Healthcare of Atlanta/Emory University about a recent multicenter report of rare complications seen with this prosthesis. How common are perforations seen with this device and how often are these clinically silent? What should be done for clinically silent prestent perforations? What sort of surveillance of all patients with this device is warranted after successful placement? These are amongst the questions posed to Dr. Ligon and discussed this week. Jeremy Ligon was brought on just in time for the 2021 harvest. Barrel Oak was purchased from its' previous owners and the new ownership wanted to up the quality of the wines from their Vineyard. In my opinion, Jeremy was a significant step up and his wines reflect that. In 2014 Jeremy was recognized as one of the new upcoming winemakers in Virginia as someone to keep an eye on. He has lived up to that from what I have tasted of his new wines from Barrel Oak.There is an intriguing story he relays as to why the previous ownership had planted their Norton vines in the vineyard and why. It turns out it has a connection to Dr. Daniel Norton, who propagated the Norton seedling in the early 1820's.Jeremy got his introduction to winegrowing in his early teens when his family planted a small vineyard in Southern Virginia. In the following years, he completed a senior fellowship in pediatric immunotherapy at the National Cancer Institute and another in pediatric sarcoma at Johns Hopkins University. Dr. Ligon is board-certified in both general pediatrics and pediatric hematology and oncology by the American Board of Pediatrics. In addition to his numerous original research publications and editorial review appointments, he is a member of various professional societies, such as the Children's Oncology Group and the American Association for Cancer Research. Dr. Ligon has been honored with a variety of awards for his research skills and academic excellence. His research interests include immunotherapy, tumors and bloodstream infections.

Bert has published four books: Jazz Theory Resources Vol. 1 & 2, Connecting Chords with Linear Harmony, and Comprehensive Technique for the Jazz Musician. Additionally, he has published several original compositions and arrangements for big bands, jazz orchestras, and steel drum ensembles. He composed extensively in the Radio/TV/film industry receiving many awards including national PBS awards and an EMMY nomination. Bert is an accomplished pianist and guitarist - he also has a deep love for baseball - he plays a little 3rd base and pitches which we spend a little time discussing as well.
