Mirela and I met years ago when I was involved with PAs for Global Health, and our paths crossed again when I read an article where she was quoted about burnout. I had no idea she'd taken a job with NCCPA, our national PA certification program (I haven't met anyone who works with them) and I was very curious about how she landed the job and what she does there. Mirela also balances being a mom, devoting a lot of time to International PA Educators and PAs for Global Health as well as teaching (she has her doctorate!) and still working (how the heck?!) in family medicine. Listen to our conversation about how to create a life that has all the flavors of the things you most love and avoid burning out. Take home points: Start noticing coincidences—they're happening all the time, but you have to be open and curious to see them. Doing a career 360 isn't easy, but sometimes a challenge is exactly what you need to grow into the next version of yourself. Let your interests guide you with how you spend your time and energy outside your job.  Instead of asking, “What's in it for me?” try asking, “How can I help?” It shifts everything. When something rare or unexpected shows up, take it! You don't have to have all the answers right away. Ask yourself how you can craft a job with Pieces of your heart—what you love and what you're good at— Happiness, not fame or status, should be the goal; identify what truly brings you joy and move toward it. Burnout often stems from systemic issues; while institutional change is slow, personal empowerment can start today. We must teach PA students and early-career clinicians to recognize burnout, seek help, and reject the “just push through” mentality. Balance is not a permanent state—it's a daily practice, and even the most seasoned professionals struggle at times. Other approved states include Utah and New Jersey. (December 2023) Press play to learn about the PA Licensure Compact, what it is, who is in support of it, and how many more states need to pass legislation in order to complete the compact. In 2019 the AAPA, the Federation of State Medical Boards, the NCCPA and the Council of State Governments began working on enacting the PA Licensure Compact.  The PA Licensure Compact makes it easier for PAs to practice in multiple states without having to obtain multiple state licenses. This has far-reaching implications when it comes to telemedicine practices and improving PA portability and patient access to care. Melissa Wilson, PA-C, MPAS from UPMC Hillman Cancer Center, and Vernon Sondak, MD, Chair, Department of Cutaneous Oncology, Moffitt Cancer Center and Research Institute in Tampa, Florida, join host Raymond Liu, MD, for a discussion about the future of melanoma in 2023 and beyond. Treatments combining immunotherapies with targeted therapies, which disable the carcinogenic products of mutated cancer cells, have further increased treatment efficacy and durability. Toxicity and resistance, however, remain critical challenges to the field. During this podcast, our guests review past treatments and novel therapeutic interventions and discuss the term "de-escalation of care" and future directions as well as how to communicate with your healthcare team. About Our Guests Dr. Sondak is Chair of the Department of Cutaneous Oncology at the H. Lee Moffitt Cancer Center and Research Institute in Tampa, Florida. He holds the Richard M. Schulze Family Foundation Distinguished Endowed Chair in Cutaneous Oncology, and is also a Professor in the Departments of Oncologic Sciences and Surgery at the University of South Florida Morsani College of Medicine. Since 2004, when he came to Tampa, the Cutaneous Oncology Clinic at Moffitt has grown into one of the largest multidisciplinary treatment centers for pediatric melanoma and related conditions in the world. Dr. Sondak has been a leader in studies of surgical treatment of melanoma and other cutaneous malignancies, particularly in the application of sentinel lymph node biopsy and lymph node dissection to the staging and treatment of melanoma, and has been instrumental in training many surgical oncologists in these techniques over the years. Dr. Sondak also has a strong research background: he served as Principal Investigator of the Moffitt Skin SPORE, a major NCI-funded “team science” grant conducting translational research in melanoma and other cutaneous malignancies. His research interests include surgical treatment of melanoma in adults and children; adjuvant (postoperative) and neoadjuvant (preoperative) therapy of melanoma; and evaluation of new therapies for patients with localized or disseminated melanoma. Melissa Wilson, PA-C, MPAS, has a wealth of knowledge and experience helping melanoma patients and their families. Melissa is up-to-date about the latest melanoma treatments and research. A natural trailblazer, Melissa is proud of her collaborative work as an NCCPA-certified physician assistant senior since 2005 with the distinguished melanoma specialist Dr. John Kirkwood at the University of Pittsburgh in the Cancer Institute Division of Hematology/Oncology, Department of Medicine. Their treatment and research initiatives were so successful that they have been used as a template for other cancer programs throughout the country. Despite increasing evidence that bipolar disorders often emerge in adolescence, only about half of adolescents with a bipolar disorder receive mental health care. Research has shown that an early onset of a bipolar disorder, concomitant with a poor diagnosis and lack of treatment, can have lifelong devastating effects for the patient. Adults and younger people who initially exhibit symptoms only of depression may later develop bipolar disorder. The risk of depression turning into bipolar disorder is estimated at 10 percent or less in adults but appears to be as high as 20–40 percent in children and adolescents. In this episode, Dr. Neera Ghaziuddin answers many questions regarding how to better diagnosis and treat pediatric bipolar disorder, and depression with mixed features in the pediatric population. Opioid addiction, abuse, and overdose have all grown to epidemic proportions in the United States as well as worldwide in recent years—such information is no longer up for debate. With an estimated 400,000 persons using illicit opiates each month, and another 4 million reporting use of prescription pain relievers for nonmedical purposes, in the United States alone, the debate now surrounds how best to treat those individuals suffering from opioid use disorder (OUD). In the past few years, there has been growing evidence for the efficacy of medication assisted therapy (MAT) for the treatment of OUD. However, there are still many challenges. Questions arise such as: What sort of training does a prescriber need to undertake in order to be able to prescribe medications for the treatment of OUD and how is this a barrier to treatment? What are some healthy coping strategies for parents to model at home, to protect their children’s mental health during this pandemic? Methamphetamine is an extremely serious problem in the United States, and it remains one of the most commonly misused stimulant drugs in the world. There are severe psychological, and medical consequences from methamphetamine misuse and abuse. This poses a particular challenge for clinicians treating patients for mental health conditions that are comorbid with substance use disorder (SUD). This is especially challenging when there are also interactions that can alter or influence psychiatric medications used to treat these patients. Questions such as the following arise: Given the high comorbidity of SUD with ADHD, how often should you check drug screens, and what is the threshold for discontinuing the stimulants if tests come back positive? In this podcast episode, Dr. William Sauve’ addresses these concerns, and much more. He also employs strategies to improve communication with patients about being transparent when it comes to using substances.
