Breelyn Vanleeuwen is a creator of one of the BEST sunscreens ever, and she's a PA, mom of 4, and phenomenally resilient human. She's a Utah Native and still lives there. She hoped to go into dermatology but ended up in urologic surgery, which turned out to be a pretty great gig. She was the only female Urology provider for 60 miles and she quickly became the go-to provider for female urology. Three years ago she was asked to help start Utah Valley Univeristy's PA Program and was on full-time faculty until recently when she left to focus full-time on her suncreen business, Daily Shade. She is an advocate for patient empowerment and education. This lady! I'd never met Venee, but I saw a FB reel she'd made about her unicorn job; underneath her words, I sensed she was longing for something more, so I invited her on the podcast to talk about it. She's open and honest about why a perfect job might not be perfect for everyone and how she's discerning her next right step and giving herself permission to honor her gut over her guilt. This is a powerful episode CHOCK FULL of delicious pearls of wisdom, I wrote down 22 key points, but here are the top 10! Just because a job feels like a unicorn for one person doesn't mean it'll be a unicorn job for every person, and just because a job starts out wonderful doesn't mean it'll always be a perfect fit, we change, our needs change, and our jobs change. Pay attention to the pull toward something different and practice tuning out guilt and fear of missing out. If you don't listen to your inner guidance, you might always wonder “what if” about risks, leaps, and opportunities. When you're frustrated or unhappy, look first at yourself and your own narrative. What story are you telling yourself? What are you feeling? When you feel a sense of unease, get quiet, and get yourself away from the noise of everyday life. It's when you get alone you can hear what's really going on. Ask yourself what you're feeling. If you don't sit in it, you'll blame your discontent on other people and things external to you. Validation comes from within, Only you can put that stamp of validation on you. Burnout can look like a candle dimming; everything can look great on paper, but that inner spark is slowly dimming. Every time you ignore that gut instinct, you're ignoring your inner wisdom. What is “Christmas happy” for you? How can you incorporate that into your everyday life? Identify what aligns with your passions. The stories we tell ourselves determine how we move through life, so pay attention to how you script your experience. Challenge it. Look at the opposite. What if you COULD? You can't change the past; just look at what you'll do to better yourself for tomorrow. Focus on this, and know that you don't have all the answers. It's all a learning process, there's no finish line. Mirela and I met years ago when I was involved with PAs for Global Health, and our paths crossed again when I read an article where she was quoted about burnout. I had no idea she'd taken a job with NCCPA, our national PA certification program (I haven't met anyone who works with them) and I was very curious about how she landed the job and what she does there. Mirela also balances being a mom, devoting a lot of time to International PA Educators and PAs for Global Health as well as teaching (she has her doctorate!) and still working (how the heck?!) in family medicine. Listen to our conversation about how to create a life that has all the flavors of the things you most love and avoid burning out. Take home points: Start noticing coincidences—they're happening all the time, but you have to be open and curious to see them. Doing a career 360 isn't easy, but sometimes a challenge is exactly what you need to grow into the next version of yourself. Let your interests guide you with how you spend your time and energy outside your job. Instead of asking, “What's in it for me?” try asking, “How can I help?” It shifts everything. When something rare or unexpected shows up, take it! You don't have to have all the answers right away. Ask yourself how you can craft a job with Pieces of your heart—what you love and what you're good at— Happiness, not fame or status, should be the goal; identify what truly brings you joy and move toward it. Burnout often stems from systemic issues; while institutional change is slow, personal empowerment can start today. We must teach PA students and early-career clinicians to recognize burnout, seek help, and reject the “just push through” mentality. Balance is not a permanent state—it's a daily practice, and even the most seasoned professionals struggle at times. Raquelle, "Rocky" Akavan did something amazing, but it probably didn't feel all that big at the time. She craved a group of fellow PA moms to talk to, so she created a small Facebook group for PA moms. Little did she know it would eventually grow to 19,000 members only 8 years later and become one of the largest special interest groups of AAPA (the national PA organization). The group is like a professional home filled with friends I talk to and share advice, as well as tips and tricks for surviving motherhood, marriage, and my career. It's become a way of empowering and advocating for our profession by a shared vision and a group of moms who can get $hit done! I wanted to highlight Rocky and let her tell her story of how it all started and what she went through along the way. As she says, it takes a village (and gives credit to her amazing team of PA Moms who work 24/7 to keep the group going). Take home points: If there's something lacking in your and it's not already available, take the brave step and create it! Spend time figuring out your core values, and reassess them periodically, then use them to make any decision. Set aside time with no phones or screens. Everything else can wait. Good leaders have to implement boundaries, which in turn, gives everyone on the team permission to set boundaries. Burnout is multifactorial, for some it's all about their work, but relationships and health also contribute. You are not alone. If you start feeling overwhelmed or burned out, use this as a sign to step back and reevaluate; ask yourself what needs to shift? You're never really stuck; even when it feels like it. There are so many more choices. Stay true to yourself by really spending time getting to know yourself and what's important to you. How many of you have had the thought, "Someone should invent ___________!" Erica had that thought and followed through. Hear how she went from idea to execution in this week's episode. Here are the show notes: Start noticing when there's a gap in the marketplace, this might be your next Shark Tank idea. Surround yourself with supportive people when you're about to embark on something new, and avoid the naysayers. If you're working with a partner, pay attention to your strengths and try to stay in your own lane. Medicine is the long game. Keep your eye on the prize. You might have to work your way toward your dream job by taking a circuitous route. Switching jobs or specialties might be a way to mix things up and avoid monotony. Lots of opportunities as a PA, and you have the ability to remake yourself. Be in the moment. Go to your kids' games, and put down the phone and the computer Use CME for family vacations. Take breaks. Go to conferences. Build yourself up, no one else is going to do that. You have to ask for raises and learn to negotiate, esp if you're not getting what you're worth. Katie Bean is a PA who's passionate about helping other clinicians escape the clinic (or add in a side hustle). She worked in ER, and in academia before getting her doctorate. After reaching her professional peak, she realized she'd hit a ceiling and missed out on everything meaningful while she was busy hustling. A tragic loss awakened her to her life's purpose. She now helps other clinicians start businesses and has her own podcast, Escape the Clinic. Show notes: Consider starting a business, it can give you massive freedom and allow you to have a bigger impact and serve others in a new way. Burnout can cause you to miss out on your life and the people in it if you're focused on the hustle and grind. Before you're driven to an identity crisis, start exploring who you are outside your role as a PA. What gives you worth besides your job title? It's only a myth that you need time and experience to start a business. Plenty of business models don't require burning the candle at both ends. By empathizing with those around you, you can help identify and solve their problem; voila, that's a business. Starting a business allows you to use a different part of your brain, and that excitement over a new pursuit will often shift your feelings of burnout. Do life on your terms now; don't wait until you retire. You're allowed to try something and pivot; you're allowed to do it messy; the point is to take action and see what aligns Obstacles teach you to ask different questions and think outside the box Get crystal clear on what you want from your life. What's not aligned? Where are you unhappy and discontent? Imagine you had a blank canvas for your life That provides the vision and the direction. Then, just take one brave step a day. If you let other people's problems and emotions derail you or distract you, this episode is for you. As healthcare givers, we often give too much and too often. This can leave us depleted, bitter, and resentful--AND it's unfair of us to off-load other people of their problems because it robs them of the ability to deal with it on their own. Rob's been around medicine for long enough to know what works and what doesn't, and he was generous to share his wisdom. Hear his take on what makes a PA career sustainable in the long run, how to discern what's really important, and how work stress can take a toll on home life. Take home points: Burnout bleeds over into your relationships If you're experiencing traumatic patient cases, consider how this might be affecting you and leading to unhealthy coping mechanisms. Be careful saying yes to overnights or shift work that causes you to lose sleep, you won't be able to think clearly and this will take a toll on your mental health, too. Even if you make a terrible decision and regret it, use it as a lens to see your situation through new eyes. Don't let the small stuff bother you. If you're that miserable at work, make a choice to change something. Make the choice to organize your life in a way that works for you. Traffic, health, stress, where you live, your hours. Think in terms of sustainability over short term rewards in pay. Even if you're paid more to take call, this might not be worth it in the long run. You can't get time back Look at your values and make sure your time and energy reflects that. Find a way to leave work at work rather than bringing in emotional baggage from your stressful day. Use your time off. Take vacations, Simplify your life as much as you can. Lyndsey uses the word "propelled" to describe how she got two bachelor's degrees, two master's degrees AND held down a part-time job while being involved in global health. She got her MPH and worked part-time while in PA school! Lyndsey is very involved in global health and worked for a year in South Africa teaching PA students (called Clinical Associates there). After years of working 6 days a week she finally hit a wall, becoming frustrated with patients, a clue for her that she was experiencing burnout. Figure out what kind of framework you want for your life in 5 years and ask yourself what decisions you can make right now that'll set you up for that Notice if you're getting frustrated with patients, this is often an early sign of burnout. Can you find a way to deal with that frustration that empowers both you and your patients like educating them about their condition? Ask “who takes care of me?” Are your needs getting met? How can you show up for yourself and meet those needs? Boundaries: Just because you can doesn't mean you should. Ask yourself why you're saying yes when you really mean no. Are you saying yes because you feel obligated? Will saying yes sacrifice your needs or your ability to provide good patient care? Ask if the money is worth it? What are you giving up for the money? Think of your time in terms of a scale. Sometimes adding in things that you're passionate about helps tip the scale away from burnout, but it's a conscious choice and requires stepping back and looking at how you're spending your time. This episode stemmed from a discussion (aka argument) with my husband around values. How we spend our time, what's most important to us, etc. My son also pointed out one of my big distractions/addictions later that week and it made me think about how each of us has a safety mechanism in place when we're burned out or super stressed. These aren't bad per se, but they can distract us from the real issue at hand and keep us living in Stuckville.
I can't believe I fell for the oldest trick in the book. I actually believed what the voice in my head told me, which was "You suck." This was related to pursuing my passion, ie, writing this book I've been working on for the last year. For two months I wallowed, I stalled, I whined. I believed it and allowed it to almost sabotage my efforts in getting this book out to those who need it. Luckily, I know a few amazing coaches. Show notes: Challenge Self-Imposed Rules: Often, what we perceive as facts are just self-imposed rules, especially those involving "I can't" or "I have to." Recognizing this can help you break free from unnecessary constraints. Embrace Discomfort for Growth: Stepping out of your comfort zone and experiencing discomfort can lead to greater fulfillment through growth, learning, and self-challenge. Prioritize the Important: Let go of minor issues and give yourself a break. This allows you to be more present with your family and enjoy quality time. Increase Your Value as a PA: Enhance your value within your current practice by learning a new skill or focusing on a niche. This not only increases your negotiation power but can also lead to greater job satisfaction. Recognize Burnout Signs: Be aware of burnout symptoms like irritability, hopelessness, and a lack of enjoyment in things you used to love. These are key indicators that you need to take action. You're Not Alone: Remember, burnout is common, and there's nothing wrong with you. Many others share your experience, and support is available. Create Quiet Time: High achievers often try to do it all, but if you're burned out, it's crucial to carve out quiet time for yourself. Even a few minutes of journaling can make a big difference. Aggressively Pay Off Student Loans: If you still have student loans, paying them off aggressively can be a favor to your future self, essentially buying yourself freedom later on. Empower Your Language: Notice the words you choose. "I choose to" is empowering, while "I have to" isn't. This shift in language can change your mindset and actions. Believe in Yourself and Let Go of Perfectionism: Believe that you can make changes and take responsibility for your actions. Stop fearing failure by asking, “What's the worst thing that can happen?” Remember, you can always go back if needed. There are things I've done (and I see PAs do) that are not only unhelpful, they sabotage our happiness and create a deeper pit of despair and stuckness than is necessary. Some of these are obvious, but that's the problem. They're so obvious and common (because we all do them) that we're not even aware we're doing them. In this eye-opening interview with Amy Story, a PA who's transitioned out of clinical medicine into a very satisfying career with various healthcare technology companies, you'll learn how to reframe what you currently do as a PA into skills you can use in almost any job. I had no idea there were PAs doing what Amy's done, and she generously shared so many valuable pearls. I had a page of notes! Key points: Ask yourself if you can see yourself doing this in ten years? If you can't, go on a journey of “what's out there?” You know yourself better than anyone. Are you numb? Itching to grow and change? Have you lost your sense of self? Does your internal temperament match with what you're doing day to day? You can stay stuck or figure out a way out. Acknowledging that you have agency is empowering and will give you a different mindset When you're considering what to do next, look at the things that drive you crazy at work and that you think you'd be well suited to handle. (processes, patient experience, communications etc) Build your network, which is really just a community of people you talk to. When you read a job description, spend time translating all the skills listed into what you already do as a clinician. Once you make a career transition, know that you'll have to change how you approach this job. Give yourself grace to ease into a new role and a new mindset. Know yourself. Don't accommodate yourself to fit into a mold. What's in your best interest. When you first start out you might have to take something that's not the perfect role, but allows you to utilize your skills you're good at and enjoy, then you can grow from there Think of your career less linearly and more like a jungle gym. When I started my podcast, the idea was to share stories from women who'd done brave things, some had crazy career pivots, some had clawed their way back from the edge of disaster, others had made scary leaps into the unknown. What I didn't realize was that each woman, while she shared her words of wisdom, was mentoring me in the process. The podcast helped give me the courage to step outside my comfort zone and to gather enough strength and courage to make it through burnout. Tajuana is one of the easiest people to talk to, she made it seem like we'd known each other forever. If you're feeling stuck, her zen vibe and can-do attitude will pull you out of your rut on this episode. Tijuana's FB link and her email Key points: Give when you're able, inspire when you can. Serving others can give you a sense of purpose. Burnout can look like being Irritable, not wanting to meet the day, not being fully present in your life, and physical symptoms like being achy and inflamed. Try mixing iti up with doing lots of different things both for your career and otherwise. . Burnout is lower in clinicians who work more than one job. Add in creativity, it engages you in a new way. Listen to those inner pulls for each season of life. Allow yourself to be gung ho and then phase out, Even if your new endeavors don't resonate with those around you, allow yourself to be a unicorn and do things that light you up. Have fun, figure out what feels rewarding. Then try it out and learn about yourself in the process. You can't control circumstances in life but you can add in intentional practices like setting a timer twice a day to take a deep breath, stretching, yoga, or walks outside. Have grace for yourself, sometimes you'll make decisions that feel like mistakes. No one has all the answers, forgive yourself for the missteps and move forward, don't be afraid to seek out help or take steps in a new direction. You don't have to just have one lane in life, you can have lots of identities, in fact, it will probably be more fulfilling.
