Not enough clinicians are entering the workforce to replace those quitting or retiring while the US population is growing and aging. The average wait for a first‐​time doctor's appointment is 26 days. Yet, state licensing laws prevent experienced international medical graduates (IMGs) who migrate to the United States from offering their services to residents. Unlike many other developed countries, American states require IMGs to repeat their residency training in accredited US programs – convincing many IMGs to not practice medicine. Other approved states include Utah and New Jersey. (December 2023) Press play to learn about the PA Licensure Compact, what it is, who is in support of it, and how many more states need to pass legislation in order to complete the compact. In 2019 the AAPA, the Federation of State Medical Boards, the NCCPA and the Council of State Governments began working on enacting the PA Licensure Compact.  The PA Licensure Compact makes it easier for PAs to practice in multiple states without having to obtain multiple state licenses. Anderson, MD is a Family Physician and an Associate Professor.  She is the Associate Chief of Family Medicine at the George Washington School of Medicine and Health Sciences where she chairs the school's clinical curriculum committee, co-directs the Health Policy Scholarly Concentration, and directs the Transitions to Residency Fourth Year required internship readiness capstone course. Nationally, she serves on the Board of Directors of the American Board of Family Medicine and is the first African American woman to be appointed to this role in the ABFM's 50-year history.   Other national roles include serving on the Board of Directors of the Federation of State Medical Boards,  the national Management Committee of the United States Medical Licensing Examination (USMLE), and the National Advisory Council of the National Health Service Corps.  Through her career, Dr. Anderson has been active in DC health policy and medical regulation as well as teaching ethics, professionalism, and physician advocacy to medical students and residents.  She is the Chair of the DC Board of Medicine, licensing and determining regulatory policy for  approximately 15,000 DC physicians.  She is an appointee to the DC Health Scientific Advisory Committee advising ethical and equitable COVID 19 vaccine distribution in the nation's capital.  In addition to the Management Committee of the USMLE, she is a subject matter expert for national committees of the National Board of Medical Examiners (NBME), namely the Patient Characteristics Advisory Panel and the Legal/Ethical Task Force.  She sits on the FSMB National Ethics and Professionalism Committee and the Diversity, Equity, and Inclusion in Medical Regulation Task Force, creating national guidelines for state medical licensing boards. In what may be the saddest state of affairs we've seen regarding medical tyranny in months, California is pushing forth a bill that would criminalize the prescription of Ivermectin and other drugs for use against Covid-19. They want people jabbed. They want them ventilated. They want them to take Remdesivir instead.Of course, Anthony Fauci is at the center of this even if he's not associated with California Assembly Bill 2098. And it's not just his name behind the government's push for jabs and Remdesivir use. Rumors are he's been working behind the scenes with members of the California legislature to craft the wording properly.This isn't just a California problem, as I detailed in the latest episode of End Medical Tyranny. This will spread to other blue states. And if/when another pandemic pops up, whether it's a new strain of Covid, the Monkeypox, or something we haven't seen yet, they will get some red states to adopt the draconian, anti-science measure as well. As he puts it, "The system really does not want people using ivermectin, in other words, which suggests that it actually works – otherwise they would not be trying to punish physicians for helping their patients with it."California Legislators Are Trying to Criminalize Doctors Who Treat Covid Patients With Anything Other Than Remdesivir, Ventilators and VaccinesDoctors in California who advise their Wuhan coronavirus (Covid-19) patients to take ivermectin, hydroxychloroquine (HCQ), or really anything outside of the "norm" could soon face prosecution under a new proposed law.The California legislature recently introduced Assembly Bill 2098, also known as the "medical misinformation bill," which would criminalize health practitioners who deviate from the state's official position on "safe and effective" remedies for the Fauci Flu.If a doctor tells a patient to take zinc and green tea instead of deadly remdesivir, for instance – Tony Fauci profits every time a patient is given remdesivir, by the way – then he or she could lose the ability to legally practice medicine in the Golden State.Back in April 2020 near the start of the plandemic, the State of California Department of Consumer Affairs, the California State Board of Pharmacy and the Medical Board of California all issued a statement regarding the "improper prescribing of medications related to treatment of Novel Coronavirus." One of the treatments mentioned was HCQ, the prescription of which was described as "inappropriate" and "unprofessional."On June 29, 2021, the Federation of State Medical Boards also issued a warning about how "Physicians who generate and spread COVID-19 vaccine misinformation or disinformation are risking disciplinary action by state medical boards, including the suspension or revocation of their medical license."A few months later, Fauci himself came out to claim that there is no evidence that ivermectin works against the Chinese Virus, even though Fauci said the exact opposite back in the early 2000s. Should doctors who spread disinformation lose their medical licenses? Phony cures and/or resistance to public health messaging is nothing new, and during the COVID-19 pandemic, we've seen it all -- from patients suing their doctors or hospital systems to allow for use of the deworming medication ivermectin to treat COVID-19, to healthcare providers echoing conspiracy theories that the virus is a hoax. The Federation of State Medical Boards put out a statement in early August saying that they support disciplinary action, including suspension or revocation of medical licenses, for those who "generate and spread COVID-19 vaccine misinformation or disinformation." When we inquired if any such actions had been taken and what the numbers looked like, we were told they are currently surveying their member boards to find out just how widespread of an issue this has become. On this week's episode, Eve Bloomgarden, MD, of Northwestern University Feinberg School of Medicine in Chicago, who is a member of No License for Disinformation, a non-partisan grassroots coalition taking on disinformation by healthcare providers, joins us to explain how they plan to push for action against those who spread false information.

Omar Sultan Haque, MD, PhD, talks with Lorenzo Norris, MD, about the need for medical schools to become responsive to physicians, medical students, and residents with mental disabilities. Dr. Haque is a physician, social scientist, and philosopher who is affiliated with the department of global health and social medicine at Harvard Medical School, Boston. He disclosed founding Dignity Brain Health, a clinic that seeks to provide clinical care for patients struggling with major depressive disorder. Dr. Haque also serves as medical director of Dignity Brain Health. Dr. Norris is associate dean of student affairs and administration at George Washington University, Washington. He has no disclosures. Take-home points Dr. Haque and colleagues recently published a perspective piece in the New England Journal of Medicine about the “double stigma” against mental disabilities, which the authors define as “psychiatric, psychological, learning, and developmental disorders that impair functioning,” including common diagnoses, such as attention deficit disorder and major depressive disorder. Physicians and physicians-in-training, such as students and residents, face major challenges in disclosing mental disabilities, from fear of discrimination during the admissions process to stigma throughout training and licensure. Medical leave is often the only suggested solution to an exacerbation of a disability, and this response is likely to instill fear in trainees, because taking leave will require future disclosure and worsen the double stigma. Reasonable accommodations could improve functioning and allow trainees to remain enrolled and on their desired academic path. Dr. Haque recommends that medical schools and training programs have trained disability service providers (DSP) with specialized understanding of medical education and curricula who do not have conflicts of interest – as sometimes happens when they participate in other roles, such as serving as deans or professors within a medical school. A continued challenge to disability disclosures are questions on medical licensing applications and renewals about past or current diagnoses or treatment for mental disabilities. Dr. Haque reminds listeners that, according to the American Disabilities Act, these questions about past and current diagnoses are illegal if the answers to those questions do not affect physicians’ current functioning. Summary   Dr. Haque’s article offers several recommendations for medical schools, training programs, and licensing boards aimed at addressing the burden of the double stigma against mental disabilities within the culture of medical training and practice. Medical schools should clearly communicate that applicants with disabilities are welcome as part of a larger commitment to diversity, and individuals with mental disabilities should be admitted and allowed to complete training. Universities should hire medical school–specific disability service providers who understand medical education and are committed to parity for individuals with physical and mental disabilities. Policies related to mental disabilities should be clearly publicized so that students and trainees know what to expect if they disclose a disability, and should create reasonable accommodations for those with mental disabilities instead of promoting medical leave as the only option. Faculty members and administrators could publicly describe their own protected time for therapy and highlight the professional successes of people who were able to disclose their condition and get reasonable accommodations. The Federation of State Medical Boards should enforce the ADA-based legal standard that questions about mental disabilities should be asked and answered only if they address current functional impairments that affect a physician’s ability to practice medicine safely. Other organizations have weighed in on the subject including the Association of American Medical Colleges, the American College of Physicians, and the Federation of State Medical Boards.  I highly recommend this episode for medical school applicants, current residents as well as physicians and healthcare professionals looking to establish and enhance their online, social media presence.  It is my hope that generating discussions on Clinical Medical Ethics and Culturally Competent Care will have a positive impact on the lives of patients that look like me. In October 2014, Wright was elected as the Chair of Panel A, one of the disciplinary and hearing panels for the Medical Board of California. In 2015, Wright was appointed to the Board of the Federation of State Medical Boards. Wright was reappointed in 2018, and she was most recently appointed to the Marijuana Task Force where she is one of three Board members who drafts regulations for physicians prescribing cannabis. Wright currently serves as the Secretary to the Board of the Southern Christian Leadership Conference, Southern California and Chairs the Emerging Leaders Committee which is comprised of young adults ages 18 to 35 who are engaged in combating social injustice in communities of color. Additionally, Wright was the former Chairperson of the Board of Directors for the Inglewood Airport Area Chamber of Commerce. Wright is also a volunteer lawyer for a collective of minority cannabis growers in South Los Angeles. Wright is a recipient of the LA's most influential under 40 award and the Legacy Award from the Southern Christian Leadership Conference (SCLC) and the 2nd Supervisorial District. Dr. Humayun “Hank” J. Chaudhry is President and Chief Executive Officer of the Federation of State Medical Boards (FSMB) of the United States. The FSMB is the umbrella agency for all the state medical licensing boards. Dr. Chaudhry, graduated from New York University, NYITCOM, and Harvard School of Public Health. He is board certified in internal medicine and completed his residency training at Winthrop-University Hospital. In addition, he served as health commissioner for Suffolk county and was flight surgeon with the 732nd Airlift Squadron and as the medical operations flight commander.  He is the author of multiple books and textbooks, and was listed by Modern Healthcare magazine as one of the 50 Most Influential Physician Executives and Leaders. Dr. Benjamin served simultaneously as Surgeon General and as the first chair of the National Prevention, Health Promotion, and Public Health Council (National Prevention Council) - 17 cabinet-level Federal agencies that developed the road map for the Nation's health – The National Prevention Strategy. Before becoming “America's Doctor,” she served her patients at the rural health clinic she founded in tiny, Bayou La Batre, Alabama, keeping the clinic in operation despite damage and destruction inflicted by Hurricanes Georges (1998) and Katrina (2005) and a devastating fire (2006). She has a B.S. in chemistry from Xavier University of Louisiana, an M.D. degree from the University of Alabama at Birmingham and an M.B.A. from Tulane University. She attended Morehouse School of Medicine and completed her family medicine residency in Macon, Georgia. Dr. Benjamin is the recipient of 22 honorary degrees. She is former associate dean for Rural Health at the University of South Alabama College of Medicine in Mobile and past chair of the Federation of State Medical Boards of the United States. In 1995, she was the first physician under age 40 and the first African-American woman to be elected to the American Medical Association Board of Trustees. She served as president of the American Medical Association Education and Research Foundation and chair of the AMA Council on Ethical and Judicial Affairs. In 2002, she became the first African-American female president of a state medical society in the United States when she assumed leadership of the Medical Association State of Alabama.
