Picture this, you wake up one morning with nausea, body aches, abdominal pain, and despite sleeping all night, you are exhausted. You then find yourself running to the restroom with watery and somewhat explosive diarrhea. You think to yourself – what could this be? Well, if the year was 2023, in the summer and you happen to live in Texas, chances were somewhat high that you had probably had cyclosporiasis.  And today, we are going to talk about this particular parasite and focus in on what our options are for detecting it given that it is not routinely picked up on O&P exams. And while there are molecular panels out there that include Cyclospora as a target, as our speakers will share, there is an easier and cheaper approach we can consider to quickly ID this pathogen. Piše Katarina Mahnič, bereta Eva Longyka Marušič in Igor Velše. Pretežna oblika pisateljevanja filozofa, profesorja, glasbenika, pisca Boštjana Narata je bila do zdaj esej, tako da je z romanom Bend stopil na novo ozemlje, pravzaprav je zelo suvereno zakorakal vanj. Kako tudi ne bi, roman govori o glasbenikih in glasbi, Narat pa ima za seboj trideset let igranja v bendih in dobro pozna njihovo dinamiko. Zgodba je dokaj preprosta. Študentka Zoja gre na avdicijo za pevko v bendu, ki je pred časom nepojasnjeno izgubil svojega pevca, Gorana. Kar izginil je. Zoja s svojim glasom in besedili navduši člane skupine Hektorja, Domna in TJ-ja in takoj jo sprejmejo medse. Vendar pa to ni edini bend v romanu; skoraj enakovredno vlogo ima Zojin družinski bend. Foter Damjan, nekoč znani muzičar s super po krivici pozabljeno glasbeno skupino, se je potegnil vase, hodi po hiši kot duh in izdeluje ptičje hišice; mama Ksenija je izstopila iz literarne scene in zdaj piše uspešne priročnike za življenje, vodi delavnice, predavanja, terapije … in s tem odlično služi; starejši brat, nakladaški Tejo, pa zagrenjenost zaradi vrnitve pod domači krov, ker je tako pač najbolj udobno, kompenzira s filozofiranjem. Prav Tejo izpostavi to, kar je poleg glasbe rdeča nit pripovedi: ali je vredno delati tisto, kar res ljubiš in v kar res verjameš, in zaradi tega životariti, ali pa je bolj praktično in razumno privoliti v kompromise ne tako zadovoljujoče službe, ki ti prinaša dober denar? In ker seveda tudi na tem področju ni vse črno-belo – ali si se z odločitvijo za udobnejše preživetje tudi prodal, izdal svoje ideale? Ali pa je ravno tako prav, kot vzkipi Domen, da hočeš od tistega, kar rad delaš, tudi dostojno živeti in zato ni nespodobno sprejeti kakšnega kompromisa. Da je muzika tvoja služba, tvoj poklic: “Zoja, kaj pa je narobe s kariero? Malo serješ s polno ritjo. Lahko je bit pameten v maminem fensi hotelu.” Ali ko skuša Ksenija prenapetima sinu in hčeri pojasniti, kako je že zdavnaj spoznala, da so uniforme povsod: “Seveda so različnih barv in pomenijo različne stvari, ampak vse to govorjenje o samosvojosti in neukalupljenosti … vse to je rahlo precenjeno. Pa malo brez veze.” Poleg razpravljanja o preživetju in seveda o glasbi Narat tankočutno in v gledališki maniri dialogov in monologov v Zojini glavi vpleta tudi spomine, navezanost, strahove, upanje, razočaranje, ljubezen …, stvari, ki so vsem pomembne in domače. Več je zamolčanosti kot odprtega, neposrednega uvida; takšne so povezave z Goranovim izginotjem, Zojino osuplo spregledanje tesnega, še vedno “zacopanega” odnosa njenih staršev, pa njen impulzivni vzgib za edino ljubezensko srečanje v romanu, ki bi ga nepozoren bralec mimogrede spregledal. Taki občutljivi, dragoceni detajli. Kratki, preprosti stavki, ki imajo svojo težo. Veliko je naštevanja, seznamov, sploh zajtrkov. Z enim se roman tudi začne in nevsiljivo razkrije družbeni status Zojine družine. Zelo dobro delujejo tudi naslovi pesmi in imena izvajalcev, ki se vrtijo v kafiču, kjer se pred vajo ali po njej dobivajo člani benda, in se pojavljajo skozi ves roman. Pravzaprav tako super, da sem si ob branju včasih nataknila slušalke in se potopila v omenjene komade, čeprav nisem kakšna glasbofilka in so mi zakulisne strukture in delovanje bendov španska vas. Vendar je ta sicer kratkočasni roman zbudil v meni nostalgijo in hrepenje; ne po glasbenem bendu kot takem, ampak po popolni pripadnosti določenim ljudem, določeni zadevi. Po popolni predanosti, da bi prišel stvari do dna. Jezik, ki ga med seboj govorijo člani benda, se zdi glasbenemu nepoznavalcu na trenutke zelo specifičen in prav zato posrečeno vznemiri, kot na primer naštevanje glasbenih rekvizitov na odru – zame zares kos čudovite poezije (z znanstvenofantastičnim pridihom). Prostor je majhen, no, ne tako majhen, bolj zabasan. Dva marshalla ob desni steni, dvakrat po štiri desetke, dve glavi, ena JCM 900, druga neka zgonjena reč neznanega izvora. Dve stojali za kitaro, stol, zraven štirje kitarski kovčki, nasproti bobni – “A to so ludwigi?” – dva prehodna, kotel, sner, vse po jusu, standard, ob kotlu zložene torbe. Levo in desno dva visokotonca, miza je v desnem kotu, šestnajst kanalov, kabli v glavnem povezani in natančno razpredeni po prostoru, neuporabljeni so lepo zviti in obešeni nad mešalko. Ob steni nasproti vhoda vufer, klaviature na štenderju in rhodesi v kovčku, zraven en vox in dva fenderja, manjši nič posebnega, ampak ima vintage šminko – tista rumena varianta – večji je bolj resen, hot red deville, ni še klasika, ampak enkrat bo, zraven dva bas feršta, ampeg in mesa boogie. V kotu štiri stojala za mikrofone, eno notno in bobnarska žirafa, ki je očitno zašla. Nekje v romanu Narat zapiše, da je “improvizacija velika beseda za preprosto stvar, kot je preprosta muzika – samo pravo noto moraš zaigrat ob pravem času.” Mojster je tudi v pisanju, zato je škoda, da je v tem igrivem in dinamičnem romančku za nekaj not, pravzaprav za kar nekaj glasbenih sklopov, izgubil skladateljski “filing” in dovolil, da iz njega spregovorita esejist in filozof. Dolgi filozofski traktati, ki jih besediči Tejo, zame najšibkejši člen romana, in razne literarne razprave, ki jih na avtobusu prebira ali v slušalkah posluša Zoja, so popolnoma odveč. Sicer gladko tekoče, jedrnato besedilo pretirano zavrejo, mu odvzamejo nekaj “žmohta” in poskočnosti. Nekaj dialoških momentov je tudi predolgih, saj so – glede na ostale izbrušene, žive dialoge – malo za lase privlečeni. Vse to bi bilo mogoče izpustiti, ali v manjših dozah in verodostojneje položiti v usta glavnih junakov; potem bi bil roman res izčiščen, skorajda idealno pripravljen za odrsko ali filmsko upodobitev, po katerih glasno kliče. Ampak tudi pomanjkljivostim navkljub je Bend domišljena, duhovita in pretanjena pripoved za vse bralce, ne le za glasbeno navdahnjene. Ko ga prebereš, se ti zgodi, kot se je zgodilo Zoji, ko sta šla s Hektorjem prvič skupaj na kavo – nekaj lepega.

We managed to make it through 2023 without a major epidemic or pandemic, and as we get into the holiday spirit and look forward to the new year, some of us took a look back and picked out one of our favorite JCM paper published in 2023, which I will tell you, was no easy task as there were many of good ones to choose from! And personally, after looking through all the issues over the last year, it never ceases to amaze me the variety of topics that we publish on in JCM – including unique things like the evaluation of a molecular assay for diagnosis of Buruli ulcers, to development of an EIA for detection of Taenia coproantigen, and then of course we have the more bread and butter things like evaluation of new assays for detection of TB resistance, to multiple studies this year on women's health diagnostics, and evaluation of new molecular assays for congenital CMV detection, and then I'm also told that there were some awesome AST-focused papers too, which is all just great. Gonorrhea remains one of the more common sexually transmitted infections. In North America, the number of reported cases has generally risen for over ten years, with interruptions in the early days of the COVID-19 pandemic, followed by returns to rising rates of infection. Despite the many advances in diagnostic testing for infectious diseases, detection of Coccidoides infections continues to rely on serologic assessment for anti-fungal antibodies, and what is perhaps more astonishing is that the serologic methods we use today, such as complement fixation and immunodiffusion, were first developed about a century ago. These assays are technically challenging to maintain and perform, and as a result few labs offer this testing, and even if performed on-site, testing can take up to 3 days complete, so there is definitely room for improvement. So today, we are talking about a study that aimed to modernize and improve our current options and diagnostic approach for Valley Fever, using a simpler and definitely faster lateral flow immunoassay. And as an added bonus, the study was not homo sapien-centric. Susceptibility Testing for Aztreonam with Ceftazidime-Avibactam

The combination of aztreonam with ceftazidime-avibactam is increasingly used for treatment of antimicrobial-resistant Gram-negative bacilli. Clinical laboratories are asked to perform susceptibility testing using this combination, but many laboratory directors have been unsure how to approach this. Today, we'll discuss a recent paper in the Journal of Clinical Microbiology in which the investigators evaluated a promising method for testing this combination of two antimicrobials and a beta-lactamase inhibitor. On this episode of the NHRA Insider Podcast, Tony Schumacher and Bob Tasca III join host Brian Lohnes to talk about off-season moves, upcoming testing, and the reasons why both think they are in a position to succeed during the 2023 season.    For Tony Schumacher, 2022 was not a return year that was arranged in a relatively hasty fashion. It's easy to hear in his words and tone that the Seattle winning team wasn't exactly firing on all eight cylinders all year long, nor were they in a luxurious position with their equipment. With the addition of Mike Neff and John Shafer as the leaders of the team, new crew members throughout, and a load of fresh parts, the SCAG Power Equipment top fuel dragster is ready to roll. The two also chat about the addition of Tim Wilkerson to the JCM team. A fun chat.    Bob Tasca has grouped Aaron Brooks and Todd Okuhara together to lead his Motorcraft Ford Mustang team in 2023. He's also added new people to the crew and the team has some really lofty goals for the season.
