We discuss her book If I Betray These Words and the work being done to address moral injury in healthcare.  "Moral injury describes the plight of tough, resourceful, and resilient clinicians who feel trapped between the patient-first values of their Hippocratic oath and the business imperatives of a broken healthcare system." "Moral injury is distinct from other forms of distress or pain, including burnout. The two conditions may occur separately, or simultaneously, and may influence each other, but addressing one does not guarantee resolution of the other." Her recent article in The New England Journal of Medicine, titled Moral Injury and the Global Health Workforce Crisis, underscores the urgent need for compassionate care within healthcare systems. She is the host of the podcast Moral Matters with Dr. Simon Talbot, which explores moral injury faced by healthcare professionals with a focus on solutions and the need for change. [Digital Gonzo 2013] The 150th episode of Digital Gonzo. This was a one-off show inspired by the episode of Community 'Advanced Dungeons & Dragons'. It's a sit-down gaming session conducted over Skype with myself as Dungeon Master and my guests as a team of adventurers. For this one we were using the flexible, simple ruleset of Advanced Fighting Fantasy by Graham Bottley which itself stems from the gamebooks of Steve Jackson and Ian Livingstone (The Warlock of Firetop Mountain, Deathtrap Dungeon) and detailed in the starter book Dungeoneer. This is the first of the two adventures within that book. I wanted to bring you guys something different, in the style I have honed over the years so there's music and sound FX as well as a hefty chunk of silliness and getting into character. The lengthier fights have been edited for pacing but you're mainly missing a lot of dice rolls. The team that begins this story comprises of the following. Thrale Copperhelm: A tough-as-nails dwarf, played by Neil Taylor of GameBurst Lastern Darkfire: An elf fresh out of the forests, played by Mark Reay of The A.I. Bots Kellis Payge: A human mage from the city watch of Chalice, played by Sharon Shaw of School of Movies Harron Brondyn: A sarcastic thief played by Matt Ramsey of Do Try This at Home Focus is on character and story over action and complex systems, almost closer to an audio drama than the recording of a game. The guest soundtrack for this week is the original World of Warcraft OST by Jason Hayes, Tracy W. Bush, Derek Duke, and Glenn Stafford. Many thanks to my guests, who played brilliantly and went easy on me in my first-time role as Dungeon Master. The combat rules were still being learned by all of us, also Gonzo is an adult podcast and we use adult language in this.  And thank you all for making this one of the warmest, smartest, funniest communities on the internet for 150 episodes of Gonzo.

On each episode we chat about the most recent developments in sports medicine and dissect through all the noise so you know which literature should actually impact your practice.On today's episode we're focusing on rotator cuff tears with Dr. Anand Murthi, Chief of Shoulder and Elbow surgery at MedStar Union Memorial Hospital and Professor of Orthopedic Surgery at Georgetown University.We have some great articles for you today that contribute well to our conversation on the surgical treatment of rotator cuff injury. As always, links to all of the papers that we discuss on this show can be found on our podcast website.The first article is retrospective cohort study published this month in the Journal of Shoulder and Elbow Surgery titled “Early Repair of Traumatic Rotator Cuff Tears Improves Functional Outcomes”. Matt Ramsey and his team at Rothman reported that patients who underwent surgical repair of traumatic cuff tears within 3 weeks of injury had the best functional outcomes, as measured by ASES, SANE and VAS scores, compared to those who underwent surgery later. Furthermore, delaying surgical repair beyond 4 months was associated with a significant decline in function across all scores. The authors concluded that early MRI diagnosis and prompt orthopedic referral is imperative when a traumatic cuff injury issuspected, to avoid a delay in surgical treatment that may negatively affect clinical outcomes.Then, from the October issue of AJSM this year, we review the publication titled “Multicenter Randomized Controlled Trial Comparing Single-Row With Double-Row Fixation in Arthroscopic Rotator Cuff Repair”. Lapner and colleagues in Winnipeg and Ottawa Canada concluded that double-row fixation was associated with statistically superior WORC scores compared to single-row fixation at 10- years post-op, but that this is unlikely to be clinically significant. More importantly, double-row repair led to preserved function out to 10 years while single-row repair exhibited significant functional declines during this time period as measured by changes in the WORC and ASES scores.We are joined today by Dr. Anand Murthi, Chief of Shoulder and Elbow Surgery and Director of the Shoulder and Elbow Fellowship at MedStar Union Memorial Hospital. Dr. Murthi received his medical degree from Case Western Reserve University and completed his orthopedic residency at George Washington University. He then completed a fellowship in shoulder and elbow reconstruction at Columbia Presbyterian.Dr. Murthi is the former president and founding member of the Association of Clinical Elbow and Shoulder Surgeons. He is also an elected member of the American Shoulder and Elbow Surgeons Society and was recently elected to the Neer Circle of ASES. Dr. Murthi is passionate about research and has published and presented numerous research papers on a national and international stage. He is the current section editor for the journal “Current Orthopaedic Practice” and also sits on the editorial board of the Journal of Shoulder and Elbow Surgery and the Journal of Shoulder and Elbow Arthroplasty.

Vassar has a virtual livestream series and Circle All Access TV show Phil Vassar is one of country music's most prominent singer/songwriters and was racking up hits on the radio before he began his own recording career. Vassar's reign of signature songs include 10 No. 1 singles and 26 Top 40 hits, including "Carlene," "Just Another Day in Paradise," "Six-Pack Summer," "When I Love You," and many more. Vassar's songwriting career blossomed in the mid-'90s when he landed a publishing contract with EMI and penned hits for Collin Raye ("Little Red Rodeo"), Alan Jackson ("Right on the Money"), Tim McGraw ("For a Little While"), Jo Dee Messina ("Bye Bye," "Alright"), and Blackhawk ("Postmarked Birmingham"). Since he can't tour due to the ongoing pandemic, the award-winning singer/songwriter created a virtual series that will make fans feel like they're hanging out at a retro piano bar, much like the venues where Vassar got his start. The livestream series airs every other Tuesday at 8 pm ET and features Vassar covering well-known songs by Billy Joel, Elton John, James Taylor, Jackson Brown, and Kenny Rogers, as well as requests from fans via social media. This year marks 20 years since "Just Another Day In Paradise" hit the top of the charts, marking his first-ever No. 1 as a recording artist. Earlier this year, he released his long-awaited tenth studio album, Stripped Down, featuring songs he's written but never recorded in the past 20 years, and new music, including a duet with his daughter Haley. In addition to the new album, Vassar's TV series Phil Vassar's Songs from the Cellar launched on the Circle Network. The weekly series, produced by NashPhil Cellars and The Nice Kids, featured all-star celebrity guests including Mike Tyson, Cam, Carrot Top, Jessie James Decker and more. Vassar and I discuss the honor of being nominated -- although it wasn't until early November that others were chosen for this induction this year -- his career, both livestream and TV series' and his daughter's foray into the music industry. A couple of weeks after our interview, it was announced that Vassar would be headlining Christmas 4 Kids (C4K) fundraising livestream concert on November 23rd. This episode dives into the world of vibrato. Many people have different ideas on how vibrato is made. Tara B explains healthy ways of producing vibrato, which can be a lovely addition and facet to our unique voices. Find out how to create a vibrato that lets you feel free in the process.   (3:22) The 3 parameters of vibrato (from Richard Miller's The Structure of Singing) Fluctuation of Pitch A Variation of intensity Number of undulations per second   (4:54) There are 6.5 undulations per second for an average vibrato. A super fast vibrato is called a tremolo. A super slow one is called la voce oscilla—“the unfavorable wobble”   (6:33)  Vibrato pitch is smaller than a semi-tone, a half step.   (7:36) Unhealthy vibrato can be produced through the base of the tongue, causing unnecessary tension.   (8.58) If the laryngeal structure isn't supported by breath flow and the right pressure, the back of the tongue may become tense. It may shake, then causing an unhealthy vibrato.   (10:16) Unhealthy vibrato can also be produced by stomach vibrato.   (13:01) Everyone can potentially have vibrato, according to Thomas Shipp.   (13:40) Healthy vibrato starts in the brain. But as it activates, it relaxes the vocal folds.   (14:35) Karyn O'Conner says in Singwise “Vibrato is a result of balance between muscle systems in antagonistic relation to each other during phonation.  When this balance occurs, the antagonistic muscles develop an alternating pulse that is a reflection of the continued energy level required of them to maintain equilibrium.”   (17:26) “Oscillations that occur in vibrato are the body's response to mounting tension.”   (18:28) Richard Miller says that a wobbles is where the natural structural support is removed from the larynx. It takes more energy to oscillate pitch.   (20:03) Onset exercise:   1.  Ha, hee      (Sing on the same pitch)   ha-2-3-4    hee-2-3-4   (21:09) 2. Sing “ga” on the same pitch. [Digital Gonzo 2013] This was the most requested Gonzo show in history. For a project of this magnitude we needed a Dynamic Analysis, so the first hour is a collection of audio articles. The second half focuses on the rest of it. There are definitely still issues with combat and structure that affect the gameplay experience regardless of how you felt about its close, but there is also a deep and rich adventure to be had finding everyone and everything in the last hours before the galaxy takes on the Reapers. There were so many people who wanted to talk about this game that I couldn't not confine them to a single, epic round-table, so instead we have three. Appropriately you'll meet many old faces and some new. Everyone has something to say. At the close we spend a good 48 minutes talking about the Citadel DLC with full plot spoilers for that as well as Leviathan and Omega. Everyone present agreed that Citadel was downright essential if you love the characters of Mass Effect so if nothing else, play that before you listen to the last third of this show. [Digital Gonzo 2013] The 150th episode of Digital Gonzo. This was a one-off show inspired by the episode of Community 'Advanced Dungeons & Dragons'. It's a sit-down gaming session conducted over Skype with myself as Dungeon Master and my guests as a team of adventurers. For this one we were using the flexible, simple ruleset of Advanced Fighting Fantasy by Graham Bottley which itself stems from the gamebooks of Steve Jackson and Ian Livingstone (The Warlock of Firetop Mountain, Deathtrap Dungeon) and detailed in the starter book Dungeoneer. This is the first of the two adventures within that book. I wanted to bring you guys something different, in the style I have honed over the years so there's music and sound FX as well as a hefty chunk of silliness and getting into character. The lengthier fights have been edited for pacing but you're mainly missing a lot of dice rolls. The team that begins this story comprises of the following. Thrale Copperhelm: A tough-as-nails dwarf, played by Neil Taylor of GameBurst Lastern Darkfire: An elf fresh out of the forests, played by Mark Reay of The A.I. Bots Kellis Payge: A human mage from the city watch of Chalice, played by Sharon Shaw of School of Movies Harron Brondyn: A sarcastic thief played by Matt Ramsey of Do Try This at Home Focus is on character and story over action and complex systems, almost closer to an audio drama than the recording of a game. The guest soundtrack for this week is the original World of Warcraft OST by Jason Hayes, Tracy W. Bush, Derek Duke, and Glenn Stafford. Many thanks to my guests, who played brilliantly and went easy on me in my first-time role as Dungeon Master. The combat rules were still being learned by all of us, also Gonzo is an adult podcast and we use adult language in this.  And thank you all for making this one of the warmest, smartest, funniest communities on the internet for 150 episodes of Gonzo.

[Digital Gonzo 2013] We chose Green lantern to kick this off because he's the first proper DC hero to get a big screen movie since superheroes became a serious business who wasn't the Kryptonian or the Bat. Also because we just finished watching the now-canceled Green Lantern: The Animated Series and defying all expectation it turned out to be my second-favourite animated show after Avatar. I do not mince words here, this is a must-see series, criminally cut off in its prime. [Digital Gonzo 2013] This episode focuses on the 2013 reboot of the long-established Lara Croft franchise. My guests and I go into great detail as to the strengths of this game, it's alarmingly serious tone, it's abandonment of the franchise staples of the Mary-Sueperwoman and enforced fiendish puzzling, the new narrative thrust, the frantic combat, the potential for exploration and most of all the completely transformed central character herself. We also outline clearly why it's neither an Uncharted clone nor torture porn, how it's already raising the bar on third-person action and how tricky it's going to be to write a sequel that does this initial outing justice.  With me on this nightmarish trip to the island are Sharon Shaw, Matt Ramsey, David Merrett, Paul Gibson and Jerome McIntosh. [Digital Gonzo 2012] After many months of teasing Gonzo finally covers one of the most beloved Sci Fi shows of all time. If you're one of the people that saw this when it was originally aired then please let the other six know because they will probably adore this episode.  Cut off some way before its prime, this exists as a cautionary tale never to trust a Fox but also as a wistful glimpse of what might have been one of the (empirically speaking) best TV shows of all time. Fourteen episodes, broadcast completely out of order with the beginning at the end and three left unaired, followed up by a cinematic outing that made one bag of popcorn less than its budget, Firefly is the perennial underdog. Perhaps this is why Browncoats, the die hard fans have one thing in common; the passionate, romantic inability to look facts in the face and accept defeat. On the show; six Browncoats. [Digital Gonzo 2012] This is a two part epic investigation into the making of Batman: Breakdown.  There's so much detail, so many hidden references and so much forethought behind the writing and production that this ended up needing two shows to contain it. I am joined by Daniel Floyd of Extra Credits who played Robin, Neil Taylor of KDS 2.0 who played Nightwing and Sharon Shaw of Gonzo Planet who played Doctor Jennifer Whitman. There is also an extensive discussion between myself and story editor David Hartrick of the wildly popular football website In Bed with Maradona. Between us we deconstruct the many complex, continuity-accurate and psychologically sound aspects of this dramatic, emotional, disturbing story of The Dark Knight. I've spent a long time layering in audio to give context to the dialogue and make this an entertaining, enlightening document of what might be the best single piece of work I've ever put out there. Also outtakes. Those are great too. In the second half we deconstruct the story elements and discuss how said elements were constructed to make this feel like a real part of the Batman story. Discussed here are the later scenes and the climax. At the very end there is a special treat in the shape of EPIC OUTTAKES. As well as this the tantalizing prospect of further installments to this new continuity. [Digital Gonzo 2012] Director of two of the Resident Evil movies and producer of the rest, Paul WS Anderson helmed the first PG-13 foray for these licenses, combining them into one family-friendly Jurassic Park-style romp. If you've not seen it the plot could be adequately described on the back of a matchbox or indeed with crude, crayon diagrams. Humans go somewhere they shouldn't, looking for stuff. They find ALIENS and then PREDATORs show up. The humans go squish and the creatures fight. It's a right palava. [Digital Gonzo 2011] In this show I'm joined by Neil Taylor and James Batchelor of GameBurst along with regular Gonzo guest Matt Ramsey. Part of the inspiration for this show came from reading Ebert's Bigger Little Movie Glossary: A Greatly Expanded and Much Improved Compendium of Movie Cliches, Stereotypes, Obligatory Scenes, Hackneyed, Shopworn Conventions and Outdated Archetypes (available from all good book shops or indeed on Amazon) It is a compilation of multiple submissions from Ebert's own fanbase edited into an alphabetical compendium. For the show today I formed the best of these into four sections; Horror/Action/Rom-Com and Old Hollywood. My guest's job is to listen to the first part of the cliche's I read them and fill in the rest themselves, citing examples and rare exceptions. It's pretty goddamn hilarious.

[Digital Gonzo 2011] The first instalment of our pop quiz show episodes based on the winning format of long-running and superb British music quiz; Never mind the Buzzcocks.. This one was centred around 80s cartoons, others include commercials, music, toys and a special episode on the 90s. If you used to watch Transformers, Thundercats and the Ninja Turtles (or if you still do) you will love this episode. If, however you remember the obscure ones like Jayce and the Wheeled Warriors and Captain N: The Game Master then this will evoke so many memories you may go completely 2D! If you're too young to have been there first time, be prepared for confusion, and learning a thing or two about the old timers. This was one of the most fun podcasts I've ever had the pleasure to record. I was joined by Michael Fox of Joypod, Neil Taylor of GameBurst and DC forumites Matt Ramsey and Duncan O Sullivan. [Digital Gonzo 2011] The book, not the rubbish movie. This time we tackle 'World War Z', an oral history of the Zombie apocalypse that claimed the lives of the majority of mankind. The show was originally broadcast in early 2011. The most important word here is "World" and this truly is a globe-spanning, culturally sensitive, intelligent and fearless piece of work. It unexpectedly drops the zombies into the background of the social upheaval and focuses on the people involved in the conflict and the various decisions from individuals and nations that lead to events both catastrophic and life-saving. Once again, my guests are Neil Taylor of GameBurst and Matt Ramsey, then of the DC forums. Our biggest hope in all of this is that we will encourage as many of you as possible to pick up the superb book and audio book for yourselves. They are written in a factual style, so we have taken an equally factual approach to discussing their contents. This is effectively a 90-minute primer of need-to-know information in case of an emergency situation (however unlikely it may seem) that involves the walking dead. Find out what happened thousands of years ago, hear the deep-sea sequel idea conceived and marvel at the inappropriate attempted vocal characterization of the PREDATORS. 53m: This was the last ALIEN film produced leaving the once acclaimed series in the gutter of creature features filled with and made by talentless nobodies. The lack of gore in the Paul WS Anderson original caused the pendulum to swing back violently, the result being that this film is ONLY about the blood and guts. Its dingy, nasty, grotty, witless, artless garbage, unfit for the term “film” and the seven assembled reviewers get their critical teeth in with savage acuity. The PREDATOR series fared better after this as the 2010 film PREDATORS was deliberately set apart from these crossovers and pitched itself as a follow-up to the 1987 original. As the spiritual equivalent of Cameron's ALIENS it gave hope for continuation of that series. With Scott's plans for the PROMETHEUS story skewing further away from the Alien franchise it could be a while before we see them emerge, dripping from the shadows once again.
