Silver Plume, Colorado — population 130 — became the setting for one of the strangest unsolved disappearances in Rocky Mountain history. In the summer of 1988, sportswriter-turned-novelist Keith Reinhard rented a storefront on Main Street, began writing a fictional character based on the building's previous tenant — a reclusive man who'd walked into the mountains and never returned — and slowly lost the boundary between the story he was writing and the life he was living. On August 7th, hungover and wearing tennis shoes, Keith announced to multiple townspeople that he was going to summit 12,275-foot Pendleton Mountain alone, starting at 4:30 in the afternoon — then walked away and was never seen again. What followed was one of the largest search and rescue operations in Colorado history, a fatal plane crash, and a cold case that's now over 35 years old. Was it an accident? A suicide? A staged disappearance? This is Justin Abbatemarco here with Romy J. van Voorst and Dr. Marjo S. van der Knaap. After discussing her article, Published Neurology Consensus Base Expert Recommendation for Diagnosis and Clinical Management of Vanishing White Matter Disease. Romy, I really want to talk with you about the patient management card. What inspired you to create that in this publication, and how should patients use that? Romy J. van Voorst:  So what the main motivation was of the study was actually a previous study that we did before. And in this study, we looked at the impact of any short matter on unaffected family members. And we found out that actually many family members encountered clinicians that were unfamiliar with its disease or disease-specific management. And during interviews, we saw that there was an urgent need for moral harmonization of care and also symptom management because families felt like they are left alone with just their child and no guidance on how to go further. And we wrote these recommendations to help families better understand the diagnostic and care process so they can also participate in informed decision-making. So they can understand what kind of preventive measures they can take and whether or not this interferes, for example, with quality of life goals. So there are a lot of different recommendations families can take home with. Dr. Justin Abbatemarco:  Marjo, anything else you want to add there? Dr. Marjo S. van der Knaap:  Yeah, I think the management card also helps because they have a physical card when they go to consultation or to emergency room that they can hand over. It's an official publication. It's developed by the Finishing WebMetter Expert Consortium in combination with other experts in combination with patient advocates and representatives. And so it's really a sort of a guidance that cannot be denied. So it has some authority to it. Dr. Justin Abbatemarco:  But I think it's a theme that applies to many neurological diseases, and addressing that. You do it really practically. And I agree, giving something more tangible for patients to present, especially to non-neurologists to help them give some guidance. It's an idea that we need to think about in clinic all the time on how we're interacting and supporting caregivers and when they're interfacing with the medical community at large. So I love what you guys have done here and to make us think about this more broadly. Thanks again for all your time and your work on this topic. This is Justin Abbatemarco here with Romy J. van Voorst and Marjo S. van der Knaap. After discussing their article published in Neurology, Consensus-Based Expert Recommendation for Diagnosis and Clinical Management of Vanishing White Matter. They both work for Amsterdam University Medical Center in the Netherlands. And we're going to have a two-part episode dissecting maybe two elements of this paper. Marjo, maybe we could start here and just talking about what vanishing white matter disease is and what in the clinic and MRI findings would make us go towards a genetic testing. Dr. Marjo S. van der Knaap:  There are two things about vanishing white matter that matter most to families, and one is the stress sensitivity. So any type of physical stress, like fever, viral infection, anything may cause a rapid decline and you never know when it comes. And that brings me to the second item that's very difficult and painful for families. And that's the unpredictability. You never know when a disease is going to hit and then your child is going to go down. So you really need the support of neurologists who know about this disease and help you go through this situation. Dr. Justin Abbatemarco: Right. And this paper serves as a great resource for folks that if they have a patient in clinic like this, medications to avoid, how to manage those stress responses. And so it's a really helpful publication to have there. And then I think another message we talked a lot about on the podcast was the importance of genetic testing when patients aren't fitting a typical bucket and this specific disease has unique characteristics. I think the cystic appearance of the MRI, which you do a great job highlighting, would really lead us down that road. So I think it's all really helpful and it gives us some ways to start in clinic with patients and our caregivers. So thank you. A new NPR investigation has revealed that the U.S. Department of Justice (DOJ) appears to have withheld and even removed dozens of pages from the public database of documents released under the Epstein Files Transparency Act that relate to **sexual abuse allegations involving President Donald Trump and Jeffrey Epstein. According to NPR, records tied to FBI interviews and notes from conversations with a woman who claims Trump sexually abused her as a minor are absent from the public archive, even though evidence suggests those pages were catalogued and should have been released. New Book: Lost in Time — Our Forgotten and Vanishing Knowledge | An Interview with Jack R. Bialik | An Analog Brain In A Digital Age With Marco Ciappelli There's a particular arrogance embedded in how we talk about progress. We speak about innovation as if it moves in one direction only — forward, upward, smarter, faster. But what if the line isn't straight? What if it loops, doubles back, and occasionally vanishes entirely? That's the uncomfortable question at the center of my conversation with Jack R. Bialik. His book Lost in Time: Our Forgotten and Vanishing Knowledge doesn't read like a history lesson. It reads like a case file — evidence, example by example, that the civilization we assume is the most advanced in human history is also, in some critical ways, deeply amnesiac. Take cataract surgery. We learned it in the 1700s, right? Except we didn't. Indians were performing it in 800 BC. The ancient Egyptians and Babylonians had diagrams of the procedure dating back to 2,400 BCE. The knowledge existed, worked, and then — somewhere in the chaos of collapsing empires and burning libraries — it vanished. We didn't progress past it. We forgot it, and then reinvented it from scratch, centuries later, convinced we were doing something new. Or the Baghdad Battery: clay pots, 2,000 years old, that when filled with acid can generate 1.1 volts of electricity. We don't know what they used them for. We don't know who figured it out. We just know it worked, it existed, and then it didn't anymore. This is what Bialik calls the pattern of loss — and it's not random. It follows catastrophe: the Library of Alexandria, the systematic destruction of Mayan records, the slow erosion of oral traditions as writing systems took over. Knowledge disappears when the systems that carry it collapse. And here's where the conversation gets uncomfortably relevant: we are building those systems right now, and we are not thinking about how long they'll last. The curator at the Computer History Museum told Bialik that to preserve the data from early IBM PCs and Macintosh computers, they had to print it on paper. The floppy drives had become brittle. The formats were unreadable. The digital archive was failing — and the only solution was to go analog. A vinyl record from the 1920s still plays. A CD from the 1980s may not survive another decade. I've been thinking about this since we recorded. My brain is analog — that's not just a podcast title, it's a philosophy. I grew up in Florence, surrounded by things that had survived centuries because they were made to last: stone, fresco, manuscript. Then I jumped on the digital train like everyone else, seduced by infinite libraries on my phone, music on demand, knowledge at my fingertips. But what Bialik is pointing out is that fingertips are fragile. And so are hard drives. The deeper issue isn't storage format. It's the distinction Bialik draws between knowledge and wisdom. Knowledge is the data — the cataract surgery technique, the battery design, the pyramid engineering. Wisdom is knowing why it matters, when to use it, and what the consequences might be. We've gotten extraordinarily good at accumulating knowledge. We are considerably worse at transmitting wisdom. And wisdom, Bialik argues, doesn't live in databases. It lives in the space between people — in stories, in teaching, in the slow transmission of judgment across generations. That's why oral tradition survived when everything else failed. Not because it was more sophisticated, but because it was more human. It didn't require a device to run on. I don't know how to solve the digital longevity problem. Neither does Bialik — not yet. But I think the first step is admitting we have one. That's actually one of the quietest, most powerful arguments in the book: be humble. We don't know everything. We never did. And some of the things we've lost might be exactly what we need right now. The question isn't just what we've forgotten. It's what we're forgetting today, while we're too busy scrolling to notice. Rebecca, who was introduced to me by my friend Beth Waterman of the Phoenicia Library, lives in Chichester, a hamlet of Shandaken, with her husband Brett Barry, the host of the popular Catskills' podcast “Kaatscast.” The couple recently became part owners of Catskills book publisher, the Purple Mountain Press.
