A complex open heart surgery is an undertaking full of inherent risks, during which many things hang in the balance. But, what if some of that unforeseen risk could be mitigated?  What if the surgeon could hold your heart in their hands, study every detail, visualize every structure, even practice their operation on your heart before they ever made an actual cut, or even entered the operating room?  What if?Recent state-of-the-art developments in 3D imaging and 3D printing have been making this a reality in many hospitals, and have been changing the world of pediatric medicine - especially for children with congenital heart disease.From realistic, patient-specific heart models used in surgery planning and patient education, to medical devices specific to the smallest babies - this incredibly flexible and affordable technology has the potential to innovate care for many of the smallest patients and their families. At the same time, like many emerging technologies, the laws and regulations that govern 3D printing in the medical space have been outpaced by the speed of innovation. Dr. Jennifer Anderson joins the show for a bonus episode, where she tells her personal story of her son finding relief from his seizures through cannabis.Dr. Jen fought and endured backlash so that now she can not only provide care to her son but to more families, and so that she could influence Child Services and local lawmakers to allow children to travel with their cannabis medication.  Dr. Jen is helping lead the integration of cannabis education into the mainstream medical curriculum for Canadian medical residents. She and her colleagues are forming an initiative that will be a model for how cannabis plant medicine and Western medicine can coexist.    Dr. Jen has demonstrated not only what plant medicine can do, but also the power of a community working together to support the child and the family in their healing journey.    Key Takeaways From This Episode:  In 2015, Dr. Jen's son started having seizures, and his epilepsy got continually worse, with dangerous and alarming side effects and symptoms occurring alongside them.  She tried many forms of treatment until he was turned around by cannabis.  As Dr. Jen began to advocate for the powers of cannabis, she was met with resistance from people both personally and professionally.  Dr. Jen worked underground for the first several years and knew that in order to change the system, the had to work with the system in medicine already set in place. This included medicine, dosage, and payment plans for the families looking to use cannabis.  Politicians are people too. Dr. Jen shares an emotional story about a little boy with a brain tumor that was able to travel because she was able to explain his situation to the governor and Minister of Health of that country.  One of the biggest barriers in the United States now is that law and research aren't well informed and working collaboratively.  Dr. Jen started to make positive waves when she was able to show just how much cannabis had benefitted her son. Her county became the first in Canada to actually have cannabis in schools. Dr. Jen is now integrating cannabis into family care practice or residency for Family Care Practice because the endocannabinoid system is one most doctors do not learn about.   We are in a time where patients want more control, and they want more involvement in the care for themselves and for their loved ones.    Quotes:  “You shouldn't have to be a physician to be able to get stuff for your child.”  “I never set out to change everything. In Episode 10 of Dr. Dave On Call, we discuss with 2 pediatric specialists, Multi-system Inflammatory Syndrome in Children and its association with COVID-19. Multi-system Inflammatory Syndrome in Children (MIS-C) has emerged as a concerning syndrome during the COVID-19 pandemic. Recently, the CDC issued a health care provider alert explaining the background information on several cases of reported MIS-C associated with COVID-19 and also provided a case definition for this syndrome. We discuss MIS-C with Dr. Melissa Tesher, a pediatric rheumatology specialist and Dr. Julia Rosebush, a pediatric infectious disease specialist. Both physicians are Assistant Professors of Pediatric Medicine at the University of Chicago Medicine, Comer Children's Hospital, Chicago, Illinois. Both have treated cases of MIS-C during the past 2 months. The following background of MIS-C will be important to our discussion on how clinicians quickly recognized this association with COVID-19. Toward the end of April 2020, doctors in the UK recognized that previously healthy kids were presenting to pediatric ERs and ICUs with a severe inflammation syndrome that had features of Kawasaki disease-like features. Kawasaki's disease is an illness that was first described by Dr. Tamisaku Kawasaki of Japan in 1967, of an unknown etiology. Kawasaki's disease primarily affects children that are younger the 5-6 years of age. Clinical signs include fever, rash, swelling of the hands and feet, irritation/redness of the eyes, swollen lymph glands and the neck and irritation and inflammation of the mouth, lips and throat. Serious side effects of Kawasaki's disease include inflammation of blood vessels, specifically of the heart that can cause coronary artery dilations and aneurysms. Treatment for Kawasaki's disease is IVIG, aspirin and sometimes systemic steroids.  As UK doctors recognized the increasing cases of healthy children who with severe inflammation with Kawasaki disease-like features, these children also tested positive for COVID-19 active or recent disease. Their symptoms included low blood pressure, multiorgan dysfunction and elevated inflammatory markers, but not one child had respiratory symptoms. The UK described eight cases, one of which died and all tested positive for COVID-19. In early May 2020 in New York City, there were multiple reports of MIS-C in children and as of May 12, there were a reported 102 cases of MIS-C, many of whom tested positive for COVID-19. According to the CDC, the case definition for MIS-C is as follows: Less than 21 years of age, presenting with: a) fever b) lab evidence of inflammation c) clinically severe illness affecting 2 or more major organ systems AND no other plausible cause AND current or recent diagnosis of COVID-19 within 4 weeks of presentation. We know that MIS-C is a rare condition and that pediatric patients who are diagnosed early and treated aggressively do very well. Currently, we are still assessing the association of COVID-19 and MIS-C. This disease may be caused by a post-inflammatory process initiated by COVID-19, and more research will need to be conducted in the future to determine if the association between COVID-19 and MIS-C is strong or weak. Through the guidance of experienced clinicians like Dr. Tesher and Dr. Rosebush, they can provide helpful information to public health entities so that parents can be prepared for particular warning signs of MIS-C, as early diagnosis is paramount. Just four years ago, pediatrician Dr. Orit Stolar was dead set against using cannabis to treat the autistic children under her care. “I would say, ‘you're off the wall — this is a dangerous drug, it's illegal.'”Today, she is running one of the only clinical trials in the world looking at how cannabis can help autistic children, and seeing results in her clinic.So what changed?“One kid,” Dr. Stolar explained on The Cannabis Enigma podcast. One of her patients came for a periodic visit vastly improved. “I was very sure it's not something I did, so I asked the mother, ‘what did you do?' And she quietly said, ‘you know, I'm giving him cannabis.'Dr. Stolar stayed up all night that night looking for medical research on cannabis and autism. She couldn't find any, so she set out to create it herself.One of the biggest problems she faces in using cannabis as a treatment, she explained, is that it's extremely difficult to know if the plant, or oil extract in her case, is staying consistent over time.“That's what's happening in my clinic. A family starts and says, wow, this really, really helping my child. And then the next month they say, oh my God, it's gotten really, really bad. Daniel Brewer, D.O. joined the Active Medical Staff at Summit Healthcare in 2017. He graduated from A.T. Still University in 2014, followed by a Pediatrics residency at Texas Tech University 2014-2017. Daniel has always been fascinated by the human body but in his senior year of high school he watched his best friend die of cancer, and Daniel decided to become a doctor. Daniel has a passion for working with kids with disabilities and providing optimal care to those who need it most. When he is not working, he enjoys art, hiking, fishing, wood working, and spending time with his children. Dr. Shan Hussain is a General Practitioner working in partnership at a large family practice near his hometown of Nottingham, England. He graduated from Imperial College School of Medicine, London, in 1999. He has worked in the fields of Orthopaedics, General Surgery, Care of the Elderly, Emergency Medicine, Obstetrics/Gynaecology, Pediatric Medicine and Palliative Care. Since completing his postgraduate training in General Practice and joining his partnership, he has developed a keen interest in the promotion of health and well-being. He has authored the bestselling book The Big Prescription. He has studied neuro-linguistic programming, coaching, healing and is currently working towards a PhD in Psychoneurology. He is a passionate believer that the vast majority of illnesses are preventable by taking simple and natural approaches to diet, activity, mindfulness and interactions with our environment. The combined state of our physical, mental and social well-being are by definition the single greatest marker of health. Since becoming CEO in 2011, he has fostered a culture committed to advancing care and research by putting patients first and actively championed innovations in research, operations, and clinical care. He has forged creative and productive partnerships with other health systems in the region, with government and community entities and with industry, at the local, national, and global level. In addition, Dr. Newman serves on the Board of the Economic Club of Washington, Greater Washington Board of Trade, Federal City Council, DC Chamber of Commerce, and Fight for Children. Previously, he served as a Board Member of Commissioners of The Joint Commission, a member of the Board of Governors of the American Pediatric Surgical Association, and as Chair of the Surgery Section of the American Academy of Pediatrics. Dr. Newman is a Professor of Surgery and Pediatrics at the George Washington University School of Medicine and Public Health. He is a graduate of the University of North Carolina at Chapel Hill and received his medical education at Duke University. He completed his surgical residency at the Brigham and Women's Hospital and also served as the Arthur Tracy Cabot Fellow at Harvard Medical School.
