While we may think of housing and senior care dwelling in distinct silos, a long-term care researcher says those should be torn down given compelling evidence on the influence of residential stability on long-term care needs. Nurse and researcher Marissa Bergh sat down with McKnight's Long-Term Care News Senior Editor Kimberly Marselas to discuss her team's findings, which underscore how the built environment and economic pressures often force families into making difficult nursing home placement decisions. Her review of 25 years of research suggests that for many older adults, the move to a nursing home is less a medical necessity and more a consequence of financial and physical housing failures. Bergh highlights three primary pathways through which housing impacts institutionalization: the financial asset of homeownership versus the strain of renting, the physical mismatch between declining mobility and inaccessible home designs, and the compounded inequities faced by marginalized communities. While homeownership often acts as a "financial reservoir" to pay for in-home help, renters are frequently priced out of their communities exactly when they need stability most, notes Bergh, a PhD candidate at the NYU Rory Meyers College of Nursing. She points out that waitlists for affordable senior housing in some cities stretch up to eight years — a timeline that doesn't align with the rapid health changes of an aging population. This "anemic supply" of housing often forces seniors to live in clinical settings they don't necessarily require. "When we talk to families and caregivers, this was not avoidable for them," Bergh explains. "This is sort of a last resort: They couldn't care for their family members as much as they wanted to. There just weren't the resources there." Repeated too often, that scenario fills nursing home beds that are increasingly in demand by hospitals and a growing number of seniors with complex health needs. Bergh urges clinicians to move beyond basic screenings for homelessness and begin identifying "proximal risks" like housing affordability and minor accessibility needs. Skilled nursing providers can play a role by learning about their short-term patients' housing needs and suggesting resources such as rent-freeze or home modification loan programs. Statt ein lebendiger Teil des Produkts zu sein, veraltet sie stillschweigend. Im agilen Manifest heißt es: “Funktionierende Software mehr als umfassende Dokumentation”. Diese Aussage wird oft als Aufruf verstanden, Dokumentation zu vernachlässigen oder ganz wegzulassen. Doch war das wirklich die Intention? Oder geht es vielmehr um eine neue Art von Dokumentation – zur richtigen Zeit, mit dem richtigen Fokus? The world of prehospital medicine is constantly evolving, driven by new research, technological advancements, and a shared commitment to improving patient care and provider well-being. As EMS professionals, staying informed about these developments goes beyond a professional obligation; it is an opportunity to improve our practice, champion our profession, and ultimately make a greater impact on saving lives. In this article, we will explore some of the latest research findings that are reshaping our field, from workplace culture to cutting-edge technology. The Culture of Care: Supporting EMS Providers Our work is demanding, both physically and emotionally, and the culture within our agencies plays a critical role in our well-being. A recent systematic review in the International Journal of Environmental Research and Public Health revealed that many EMS providers avoid using organizational mental health services due to stigma and a perception that these programs lack genuine care. The study emphasizes the need for person-centered support and a cultural shift that normalizes seeking help as a sign of strength (Johnston et al., 2025). This cultural component also impacts retention. Another study in the same journal found that agencies with collaborative, team-oriented "clan" cultures had significantly lower turnover rates compared to those with rigid or chaotic structures. For leaders in EMS, fostering a supportive environment is not just about morale. It is a strategic imperative for retaining skilled clinicians (Kamholz et al., 2025). Professional Recognition: Breaking Barriers Across the globe, paramedics are striving for recognition as integrated healthcare professionals. A qualitative study in BMC Health Services Research identified common barriers, including outdated legislation, inconsistent regulation, and insufficient funding. While the pandemic temporarily highlighted our capabilities, the momentum has waned. The study calls for targeted policy reforms and investments in education and leadership to solidify our role in the broader healthcare system (Feerick et al., 2025). Physical Demands and Injury Prevention The physical toll of our work is undeniable. A scoping review in Applied Ergonomics confirmed that musculoskeletal injuries, particularly to the back, are rampant in EMS. Tasks like handling stretchers and patient extractions are among the most strenuous. The review also highlighted fitness disparities, with male paramedics generally showing more strength but less flexibility than their female counterparts. These findings underscore the need for targeted injury prevention programs and realistic physical standards to keep us safe throughout our careers (Marsh et al., 2025). Advancements in Cardiac Arrest Care When it comes to cardiac arrest, every second counts. A study in Resuscitation reinforced the value of bystander CPR, showing that dispatcher-assisted CPR significantly improves outcomes for untrained bystanders. For those with prior CPR training, acting independently yielded even better results. This highlights the importance of public CPR education alongside dispatcher support (Tagami et al., 2025). On the scene, our interventions matter immensely. Research in The Journal of Emergency Medicine found that for traumatic cardiac arrest patients, aggressive interventions like prehospital thoracostomy can be lifesaving (McWilliam et al., 2025). Meanwhile, a study in Critical Care Medicine revealed that extracorporeal CPR (ECPR) significantly improves outcomes for patients with refractory ventricular fibrillation, emphasizing the need for early transport to specialized centers. The Role of Technology in EMS Technology is poised to revolutionize EMS, from dispatch to diagnosis. A study in The American Journal of Emergency Medicine demonstrated that large language models (LLMs) like ChatGPT could prioritize ambulance requests with remarkable accuracy, aligning with expert paramedic decisions over 76 percent of the time. This proof of concept suggests that AI could one day enhance resource allocation in dispatch centers (Shekhar et al., 2025). On the diagnostic front, machine learning is opening new possibilities. For example, a study in Bioengineering showed that analyzing photoplethysmography waveforms could estimate blood loss in trauma patients, offering a non-invasive way to guide resuscitation (Gonzalez et al., 2025). Similarly, research in Medical Engineering & Physics explored using multidimensional data to differentiate ischemic from hemorrhagic strokes in the field, potentially enabling more targeted prehospital care (Alshehri et al., 2025). Addressing Disparities in Care Equity in EMS is a cornerstone of our profession, yet recent studies highlight troubling disparities. Research in JAMA Network Open found that ambulance offload times were significantly longer in communities with higher proportions of Black residents (Zhou et al., 2025). Another study in JAMA Surgery revealed that Black and Asian trauma patients were less likely to receive helicopter transport compared to White patients. These findings are a call to action for all of us to examine our systems and biases to ensure equitable care for every patient (Mpody et al., 2025). Looking Ahead The research discussed here represents just a fraction of the advancements shaping EMS today. From improving workplace culture and injury prevention to leveraging AI and addressing systemic inequities, these findings have real-world implications for our protocols, training, and advocacy efforts. As EMS professionals, we have a responsibility to stay informed and apply these insights to our practice. Andrea Evers was een Duits-Nederlandse wetenschapper, ze bekwaamde zich in de gezondheidspsychologie en was hoogleraar aan de Universiteit Leiden. Zij kreeg in 2019 een hoge wetenschappelijke onderscheiding, de Stevinpremie, voor haar baanbrekend onderzoek naar het placebo-effect. Ze was gezaghebbend van op het gebied van psychologische factoren die een rol spelen in het verloop en herstel van medische aandoeningen. En toen werd ze zelf ziek. Sie hat tiefe Wurzeln in der alpinen Geschichte geschlagen. Was 1875 mit wenigen bergbegeisterten Hanseaten begann, ist heute eine der größten Sektionen im Deutschen Alpenverein – mit mehr als 23.000 Mitgliedern, drei hochalpinen Hütten und einem ausgeprägten Bildungs-, Sport- und Umweltengagement. Ein Symbol für die Verbindung zwischen Hamburg und den Alpen ist das Ramolhaus im Tiroler Ötztal. Auf gut 3.000 Metern Höhe thront die Schutzhütte über den Gletschern und Gipfeln der Region – und gilt als Hamburgs höchstgelegener „Außenposten“. Wirtin Lenka Mazurkova hat inzwischen ihre vierte Saison hinter sich. „Ich liebe diesen Platz“, sagt sie. „Nächstes Jahr hoffentlich wieder.“ Neben dem Betrieb von Hütten setzt die Alpenvereinssektion auf Naturschutz, Klimabewusstsein und moderne Infrastruktur: Photovoltaikanlagen auf Berghütten, Kletterhallen in Hamburg, Ausbildungskurse und Jugendarbeit – vieles davon getragen von Ehrenamtlichen. As fossil fuel use and greenhouse gas emissions continue to rise, there is renewed interest in what can be done to capture carbon dioxide. Until now, most of the investment in carbon capture has gone into projects to take those emissions and store them underground forever. But what if we could make use of that captured carbon? To find out what role carbon capture and utilization, or CCU, could play in tackling climate change, host Ed Crooks is joined by three experts in the sector. He is joined by Sarah Lamaison, who is the CEO and co-founder of CCU start-up Dioxycle, Tim van den Bergh, the climate tech innovation lead at the World Economic Forum, and John Ferrier, a senior research analyst at Wood Mackenzie. Together they unpack what CCU actually is (and isn't), and where it can deliver the biggest punch; for example in the chemical industry, which is a sector in large part built on carbon.Sarah explains how Dioxycle's carbon electrolysis can turn carbon dioxide and carbon monoxide into high-value molecules such ethylene using electricity and water. It is effectively “dual” decarbonization: it uses captured carbon instead of fossil feedstock, and also avoids process emissions.But despite those compelling advantages, CCU faces some steep challenges. The gang examines the policy landscape, and the economics that can make or break CCU projects. John outlines why support has historically skewed toward carbon storage rather than utilization: it offers measurable, near-term reductions and simpler business models. To accelerate the growth of CCU, it needs clearer incentives, and standardized lifecycle assessment of carbon emitted and avoided. Sarah compares Europe's current framework, which can disadvantage CCU, with more supportive tax credits that are available in the US. She explains that the choice of product to be made using CCU really matters. For fuels, conventional feedstocks such as crude oil and natural gas are hard to beat on cost. For complex chemical pathways, there is room for CCU to undercut incumbents as efficiency improves. Tim looks at the system level, calling for global, aligned policies, early markets in cost-competitive niches and “patient capital” to bridge the valley of death that innovative companies face as they scale up.There's a strong case that can be made for CCU, if policy, finance, and industry can travel in the same direction. Gleich mehrere Wanderwege durchziehen das hügelige Hinterland der „Île de Beauté“, der „Insel der Schönheit“, wie Korsika auf französisch gerne genannt wird. Die Routen führen über steile Berghänge, duftende Wiesen und durch verwunschene Wälder. Wer hier wandern will, sollte Höhen- und Hitzeerprobt sein, aber immerhin für Letzteres gibt es Abhilfe: Der nächste, eiskalte Wasserfall oder See ist garantiert nicht weit. In der Abgeschiedenheit der Berge haben sich Eigenheiten in Sprache, Musik und Lebensweise bewahrt. In den Bergen, so sagen die Einheimischen, ist die wahre korsische Seele zu finden. Lange Zeit spielte sich hier, und nicht an der heute beliebten Küste, das Leben ab: Denn am Wasser wütete die Malaria, es gab Attacken feindlicher Invasoren und kein Weideland für die Schafe und Ziege, die auf Korsika bis heute allgegenwärtig sind. Im Sommer zogen die Hirten mit ihren Tieren in die hohen Berge, um Hitze und Mücken zu entfliehen. Mancherorts wird die Wanderviehwirtschaft, die Transhumanz, noch heute praktiziert. Verena Carola Mayer ist den alten Hirtenwegen gefolgt: Sie war zu Fuß unterwegs auf dem „Sentier de la Transhumance“, der auf fünf Tagesetappen und knapp 80 Kilometern vom Landesinneren an die nordwestliche Küste führt. Anderer Wanderer traf sie unterwegs kaum – dafür Hüttenwirte, Sänger, Hirtinnen und Schafherden.

This is the second episode of a series focused on the findings of the Horizon Europe project CAPABLE (ClimAte Policy AcceptaBiLity Economic framework). The aim of this podcast series is to provide an overview of the CAPABLE project and draw attention to some particularly relevant findings. In this second episode we host Jeroen van den Bergh, full professor of environmental and climate economics at the Autonomous University of Barcelona and at the Vrije Universiteit Amsterdam. This episode with Jeroen will be focused on the need for clear guidelines for climate policy that all climate and policy scientists should agree about. The climate policy expert Jeroen will tell us about how to provide these clear guidelines through the 10 Commandments for climate policy design.
