Dr. Allaway explains that transrectal biopsy can lead to rectal bleeding, infections, and sepsis. He also believes that the perineal approach offers the proper trajectory to sample the prostate appropriately. Next, they discuss the equipment required for transperineal biopsies, such as probes, ultrasounds, grid steppers, and needle sheaths. They weigh the pros and cons of performing the procedure in the office versus in the clinic. Additionally, they discuss different costs and features of different probes. Next, they discuss their techniques for obtaining the transperineal biopsy, including tips for patient positioning, probe maneuvers, and local anesthetic injections. Dr. Javier-DesLoges uses a local injection of lidocaine, normal saline, and sodium bicarbonate. Dr. Allaway then shares his advice on how to deal with obstacles, such as stool burden and rectal gas. They end the episode by highlighting the importance of collaboration and learning from others' techniques. Rapp Report episode 211 How to get started with Bitcoin is the topic of this episode. Welcome to Andrew Rappaport's Rapp Report, where we provide Biblical interpretation and application! The show is a ministry of Striving for Eternity and the Christian Podcast Community. In today's episode, host Andrew Rappaport is joined by friend and Pastor Jim Osman of Kootenai Community Church in Idaho for part two of the conversation on bitcoin started in the last episode!  The hosts of the Bitcoin and the Bible podcast - Dave, Simon, and Will - join us again to discuss bitcoin, focusing today on the practical side of starting to use bitcoin.   Before turning to practical advice, though, Jim asks what will keep bitcoin from inflation or from dropping in value.  Will explains that people continue buying bitcoin because it's a superior form of money, and they will ultimately have the ability to consume all available bitcoin supply.  This dynamic, grounded in the fact that value is subjective and based on market needs, listeners can be assured that the value of bitcoin will not go to 0.  With regard to inflation, Will describes the way that bitcoin operates with a hard cap of 21 million, and how the bitcoin system is designed with a kind of self-regulation because it's in every bitcoin holder's best interest to hold others to the rules of the system.   Turning to some possible concerns and responses to them, the group first considers whether or not the bitcoin cap will pose a problem.  Even when the cap is reached, there will be plenty of bitcoin and sufficient divisibility to ensure global commerce can exist on a digital system.  And while some people are wary of bitcoin's fluctuations in market value, the current fiat system is over-engineered to avoid volatility.  Volatility, however, is a sign of life, and actually provides a reason for us to trust bitcoin!  The change in the bitcoin market is rooted in the fact that the market is free and open, and so at each moment, the dollar price of bitcoin is determined by willing buyers and sellers.  While the primary reason people put their money in banks is to be able to move it digitally, bitcoin facilitates this movement even better than banks; it allows for the transfer of money without bank control and with great speed.   As the conversation progresses, the group considers what bitcoin mining is.  At its most basic level, this mining involves grouping pending transactions into a block and finding the random number to make the block valid.  The process leaves a track of work that demonstrates the history of transactions and is done with a specially purposed computer.  The sort of computer required to mine (an ASIC miner) helps to bolster the security of the process, and some bitcoin users have also elected to study quantum computing, which is expected to protect bitcoin in the more technologically-advanced future.   Dave, Simon, and Will encourage listeners to lean into bitcoin rather than other cryptocurrencies, and the next topic of conversation is the rationale behind this selectivity.  Some cryptocurrencies are trying to be money, and others are not doing so, and in general, money wants to converge on a single best form of money.  At this point, bitcoin is the predominant form of digital money; the choice cycle pushes bitcoin value up and down, and the positioning of bitcoin at the pinnacle of the cryptocurrency world gives it a lot of security.  Other forms of cryptocurrency don't work well because the incentive behind them (the driver of crypto outcomes!) won't work, and some of them are even built of systems that allow the rich and powerful to rewrite the rules.   Now, assuming a person is convinced to give bitcoin a shot, the glaring question still remains: How can someone get set up on bitcoin?  Fortunately, there are great resources (including a Startup Guide and many others on BitcoinandtheBible.com!) to help lead the way, and though there are things to learn,

Getting your ducks in a row is easier said than done. Still it's a task that can be done on your own all the way up to the actual filling. In our experience the more complex your business structure the more you should lean on your lawyers. Most mistakes can be fixed later on down the road, but some could kill the business before it even starts. Used good judgment.How to be cost effective and efficient.Lawyers get paid based on their time, i.e. the more they spend working on your account the more you end up paying. That is a lesson we have learned the hard way, which is why we recommend laying out exactly what you need to have accomplished, very clearly. This way they know exactly what is expected and they can knock it out quickly and correctly. Where to splurge and where to take ownershipThere are several different documents that have to be complied and filled out during the incorporation process. Some of these need to be owned by the lawyers; bylaws, articles of incorporation etc. Let them handle that. On the other hand, there are forms that you can find online that you can have filled out and ready to rock on your own. Entrepreneur Writes Startup Guide After Hearing Crazy Startup Ideas, Financial Confusion

Dave Parker is an entrepreneur's Superman, ready to save them from disaster that could strike after being swept away by the passion of an idea.  So there's some dramatic exaggeration there, but it's true that many entrepreneurs are blinded by an idea and dreams of success, without exploring financial realities or even whether their idea is viable.  Dave is more than qualified to advise startups. He has founded multiple companies over the past 20 years, sold some, and served as a CEO and board member for others. These days, Dave spends 80 percent of his time helping founders actually land the deal that they started and 20 percent of his time involved with community development and community building work around startups.  He's been involved in a leadership role at Startup Weekend, and it was in part that experience that sowed the seed for Dave's book Trajectory: Startup—Ideation to Product/Market Fit, released on April 13, 2021.   Dave noted people would leave the Startup Weekend overwhelmed with passion for their business idea. They'd be saying things like “I've met my startup soulmate!” and were ready to quit their day-jobs with only an untested idea and no sense of the financials.   Killing Bad Ideas Fast Is A Great Option  Says Dave: “There's this phrase and startup land that the idea doesn't matter, only execution matters, which assumes by default, that it is a good idea.  “If it's a bad idea, you're just going to fail faster. And I think failing fast is a really bad idea. I grew up a son of a single mom on food stamps. So failure wasn't an option. I think killing bad ideas fast is a great option. It allows you to take the framework of learning and understanding what makes a great idea. And then applying that to your next idea if this isn't the only one.”     However, not everyone gets carried away by their idea. Some people don't have ideas but want to become founders. Dave met a woman at a seminar who was such a case, and she didn't know how to think about ideas. So his book includes a chapter on the 11 frameworks for ideas.  When it comes to financials, many startups don't focus enough time on them, or have the wrong financial model for their industry. This prompted Dave to investigate and he discovered that there are basically 14 revenue models in tech, including things like services, subscription services, market places, and new media.   Says Dave: “Hopefully, your startup idea is unique. But how you make money is never unique. And there have been very little changes with those financial models over the last six plus years.”  See the link below for the free excerpt from Dave's book available on DKParker.com where you can access the 14 revenue models detailed in the book.  A Book From Blog Posts  The second thing that prompted Dave to write Trajectory was that he fielded lots of calls or emails from entrepreneurs about their startups. The good thing about this was that their questions often led to blog post ideas; the bad, he often had to repeat himself, or direct an enquiry to his blog.  “After a while, I looked at my 150 blog posts and went: ‘Ha! That actually might be a book. So I started compiling them into the pieces.”  Some of the blog posts had added components from other authors, because Dave wanted to make sure he gave credit to them. Dave realized that he needed to include the 50 books that he believes are the most important 50 books in startups.   “You'll find that throughout the book, I've gone to great deliberate effort to really give credit where it's due to the standing on the shoulders of giants, if you will, the people who've written some amazing things.”  A Diverse Range Of Book Blurbs  Angel investor and venture capitalist Dave Berkus has written a blurb for Trajectory's cover, and describes Dave's book as “Awesome . . . this will be the handbook for startup founders.”  Dave was strategic in his request for blurbs for the book's cover, which are a powerful component of a book's marketing. They're not turning into potential clients, they're not getting business from it, and so they're flustered.
