In this podcast, Enhancing Surgical Outcomes: The Vital Work of RNFAs, we investigate an area of nursing that you may not know about: the registered nurse first assistant, or RNFA. You will find this practitioner in the operating room assisting the surgeon. Anna Gray, who serves in this role, is here to explain what it involves. This is a position that requires years of training and passing a difficult examination. It's an expanded set of nursing skills that involves both knowledge of the dynamics of an OR and a deep understanding of surgical processes. And that's not all. Communication skills assume paramount importance in this position. The RNFA must coordinate the flow of work and appropriately delegate tasks so that surgeons have everything they need to successfully perform the operation. It might be considered a high-level version of directing traffic in order to keep traffic flowing smoothly. Having RNFAs working in an OR is frankly more cost-effective than having another surgeon as first assist, and both surgeons and hospitals have embraced having these highly-qualified nurses in the OR. For LNCs, though, the RNFA's role is problematic, since it's not required to be documented. This will prove frustrating to LNCs, as they often need to figure out, when an operation goes wrong, why it happened. OR notes may not even record that the RNFA was present. Anna says she relies on the operating room nurse to record her presence, but LNCs may also want to dig down to discover if an RFNA was working in the OR. Learn how the European Surgical Outcomes Study (EUSOS), presented at a previous Evidence Based Perioperative Medicine (EBPOM) conference, helped to inspire the African Surgical Outcomes Study ASOS which then went on to inspire us all; after an international gathering in Prato. A truly global community - the perioperative medical community - has now been galvanised into action. Nicole Rich and Sharif Ellozy talk with Dr. Misty Humphries and Dr. Gregory Magee about what they did, or wish they had done, during training to help them establish their practices as young vascular surgeons. Dr. Misty Humphries is an Associate Professor in the Division of Vascular Surgery at the University of California at Davis Medical Center, where she serves as the Vascular Surgery Residency Director as well as the Vice Chair of Surgical Outcomes. She completed her general surgery residency at UC Davis, and her Vascular Surgery Fellowship at the University of Utah. Her current research interests include studying disparities in care for patients with critical limb ischemia and in 2019, with grant funding from the National Institute of Health, she began investigating the role of telemedicine programs to reduce amputation rates in rural populations. She serves as an examiner for the Board of Vascular Surgery and a reviewer for the Journal of Vascular Surgery, Annals of Vascular Surgery, and other surgical journals. She participates in multiple national committees as a part of the Society for Vascular Surgery and the Vascular and Endovascular Surgical Society. In 2017 she was awarded a Women's Leadership Grant from the Society for Vascular Surgery for her work as a mentor and scholar. Dr. Gregory Magee is an Assistant Professor of Clinical Surgery at the University of Southern California, and the Director of Research for the Division of Vascular & Endovascular Surgery. He completed his general surgery residency at Stanford University, a trauma & critical care fellowship at the University of Southern California, and his Vascular Surgery fellowship at the University of Colorado. His research interests include flow dynamics modeling in aortic dissection, medical device development, and novel techniques for complex endovascular aortic repair with fenestrated/branched grafts. He has provided mentorship and teaching to numerous medical students and surgical trainees, serves as a peer reviewer for the Journal of Vascular Surgery among other journals, and participates in multiple hospital-level and national committees dedicated to improving care for vascular surgery patients. In this health care podcast, I talk with Karl Bilimoria, MD. Dr. Bilimoria is a surgical oncologist and a VP of quality over at Northwestern Medicine. Plus, he is also a John B. Murphy professor of surgery. The second I heard that Dr. Bilimoria and his colleagues had worked on an initiative to “rate the raters” of hospital and physician quality, I reached out to get him on the show. I had just had about four conversations with various people about the difficulties of judging quality. And I had also had a confounding personal experience visiting a patient at a hospital judged a top hospital by a well-known national rating scale. And this “top” hospital had some readily apparent issues, and I am no expert. That got me wondering about the validity of some of these quality raters. Given the importance and the need for health care quality transparency, Dr. Bilimoria and his colleagues set out to fill this gap by undertaking a (as mentioned) Rating the Raters process to evaluate and compare probably the major publicly reported hospital quality rating systems in the United States. These include the CMS (Centers for Medicare and Medicaid) Hospital Compare Overall Star Ratings, Healthgrades Top Hospitals, Leapfrog Safety Grade and Top Hospitals, and the U.S. News & World Report Best Hospitals. Interestingly, that “top” hospital I was in was scored a top hospital by one of the lowest-rated raters. Karl Bilimoria, MD, is a surgical oncologist and a health services, quality improvement, and health policy researcher at Northwestern University’s Feinberg School of Medicine. He is the vice president for quality for the Northwestern Medicine system. He is also the vice chair for quality in the Department of Surgery and the John B. Murphy professor of surgery. His clinical practice is focused on melanoma and sarcoma. Dr. Bilimoria is the director of the Surgical Outcomes and Quality Improvement Center of Northwestern University (SOQIC), a center of 50 faculty and staff focused on national, regional, and local quality improvement research and practical initiatives. He is also the director of the 56-hospital Illinois Surgical Quality Improvement Collaborative (ISQIC). Dr. Bilimoria has published more than 350 scientific articles, including numerous publications in JAMA and the New England Journal of Medicine. His research is funded by the National Institutes of Health, the Agency for Healthcare Research and Quality, Health Care Services Corporation, and numerous others.
