In this episode, we sit down with Sarah Bugen, a nurse at Memorial Sloan Kettering Cancer Center, who shares her expertise on managing life after a blood or bone marrow transplant, particularly in relation to chronic graft-versus-host disease (GVHD). The conversation centers on practical advice for patients and caregivers navigating recovery and maintaining quality of life.Regarding Oral GVHD, Sarah emphasizes the importance of oral health post-transplant, recommending soft toothbrushes, alcohol-free mouthwash, and avoiding irritants like spicy or acidic foods. She suggests soft, hydrating foods and stresses the need for regular dental checkups with oncologist approval. Hydration is vital for managing dry mouth, with a goal of 2-3 liters of fluids daily. Also, sugarless candy or gum can help stimulate saliva production.For nutrition, Sarah advises increasing caloric intake with protein powders, full-fat products, and small, frequent meals. She also highlights the value of bland diets and electrolyte packets for hydration. I've touched on the topic of endotoxemia in past episodes of the Defiant Health podcast. In this episode, let's dive deeper into this topic that is absolutely crucial to understanding and managing SO many aspects of health, from subduing anxiety and panic, to depressive, to joint pain and skin rashes, to gastrointestinal conditions, even weight management. What is endotoxemia? Fecal microbes, so-called Gram-negative species because of the way these microbes take up stain for examination under a microscope, species such as E. coli and Salmonella, have something called lipopolysaccharide endotoxin in their cells walls. Other species, so-called Gram positive species such as Enterococcus, Staphylococcus, and Streptococcus, that stain in a different manner, have something called lipoteichoic acid in their cell walls. When these microbes die, both Gram-negative and Gram-positive, these toxic factors are released into the intestines. If these fecal microbes are confined to the colon, where they belong, a section of GI tract adapted to their presence, the entry of these toxic components are limited and current evidence is unclear in how importat this process is. The real trouble occurs, however, when fecal microbes have invaded the 24-feet of small intestine, a process we label small intetinal bacterial overgrowth, or SIBO, because the small intestine—the stomach, duodenum, jejunum, and ileum are not well-adapted to the invasion of fecal species. Here, they die, release their toxic components, which then enter the bloodstream, the process labeled endotoxemia. People with SIBO therefore have high blood levels, typically 200-400% higher, levels of endotoxin. This is how microbes in the GI Tract export their effects to all other parts of the body. But little kids don't sit down and study a vocabulary book. On this episode hosts Marcelene Forbus (RT, RPA) and Reece Burgoon (RT, RRA) share about some of the things they have learned over the course of starting this podcast. Learning more in-depth about various avenues our career paths we can take and some of the tips and tricks we have learned along the way. As we did this podcast we got to learn how everyone else is working but we as RA's don't really openly talk about how we do, what we do. On this episode we share some of the important things we do when performing exams of the upper GI tract. We spent some time sharing how and why we do some of the positioning we do and helping technologist gain a different perspective into setting up these exams or anticipating how to better assist in the rooms. We share on some of the tips and tricks we have learned from both our radiologist and technologist over the years, being able to take care of our patients and get the radiologist the diagnostic exam they need to make their diagnosis. These were just things we have have learned and how we personal work and are not intended as advice on how others work in their environment. This epsiode was simply to demonstrate one of the many ways and RA can assist in the fluoroscopy world and how we work within our roles. Every practice is different and each radiologist has their own protocols and expectations that us, as RA's, are expected to follow. This was just our generic take on our typical day of work.

Heather and Tracy explore the impact your diet or wellness plan may have secretly had on your digestive system. Heather shares about her autoimmune esophagus issues and related problems she's had with her GI tract. They talk about AFRID and other ways we can unknowingly create more problems for ourselves as we try to restrict foods that we think are to blame for the issues we are having. Heather shares what happened after she went for MRT testing to see what foods she was allergic to and how the further restriction may have caused even more problems. Tracy also digs into the threat response we create in our bodies when we fear eating certain foods or when past trauma connects with certain foods and we respond to them. When we talk about the microbiome, most of us think about the stomach and intestine only. We tend to miss out on the first part of the GI Tract that may affect our overall gut microbiome - the mouth!Join Amy and Nikki as they discuss the role of oral health and microbiome and their connection in keeping our gut healthy!
Why doesn't mold affect everyone the same way? Why can it destroy one person's health while causing no noticeable problems for someone else? Well, about 75% of people can make antibodies to the mold toxins (mycotoxins) that help bind to the toxins so the body can remove them. But the other 25% or people don't have this ability, so they're relying heavily on their liver and other systems of elimination like lymphatics, lungs, skin, kidneys, and GI Tract. If these detoxification systems aren't working properly because they're literally poisoned with toxins and chemicals, then they can't do their jobs. This means toxins of all types, including mold, continue to accumulate. Once this vicious cycle begins it can be very hard to unwind it. We need these systems to be working to remove toxins, but when they're overloaded and can't work, more and more toxins will build up which keeps people trapped. And extremely sick! This is also why so many chronically ill people feel worse when they try treatment, but I'll cover that in another post! After exploring this covering in detail, we'll progress systematically, organ by organ down along the GI tract, stopping at each structure to discuss its function and histology. Sandy tells a hilarious story about they guy she is dating and asking him if he was pooping and how he got so uncomfortable being asked. There is a double standard of this human thing that men can joke about and its unlady-like for girls and women to talk about it. As a health professional you need to talk about your poop as a part of your health. A lot of the time the belly pooch or not being able to get rid of the baby belly can be an issue of your gut, not just the muscles and fat. Talking about poop with the kids gets you a little more comfortable with the topic. It's so important to get to know your poop. Precision Nutrition is the curriculum that Sandy uses for Nutrition Counseling with her clients and they have a graphic showing the 7 types of poop. There are some real red flags that you can find if you see a change in your bowel. It can help with early intervention with serious illness. You want smoothe sausage shaped poop and not overly hard or runny. Some of the detoxes or cleanses may cause the watery poop which is so bad for you. Be aware of the crazy diets really impact you and can make things not work properly. Fiber is part of the equation, helping to go down your GI Tract. So is proper hydration and exercise. Drinking water, consuming my water rich foods like fruits and vegetables. Over working or being stagnant can have serious negative consequences. The other thing is that your poop should not smell terrible and it should not take forever to use the bathroom. If either of those things are happening you should see a nutritionist. Men seem to have gotten the green light to spend so long in the bathroom but being an efficient pooper means that everything is actually functioning properly. Stress and exercise can also have a huge impact. It's all one big system. Overtraining or undertraining can drastically affect your bowel and may change your gut health. There are a lot of irritants to be aware of and consider. Another poop that many women experience is the first one postpartum. James Sapirstein, President and CEO of AzurRX BioPharma, Inc., a developer of targeted therapies for gastrointestinal (GI) diseases, discusses the continuing need for therapeutics to treat COVID-19, how SARS-CoV-2 can remain trapped in the GI system, the "long-haul" phenomenon,and FW-1022, the drug they are developing for COVID GI infections. They anticipate a Phase 2 trial in the first half of 2021. Mr. Sapirstein, who most recently served as chief executive officer and board member for ContraVir Pharmaceuticals, Inc. (now known as Hepion Pharmaceuticals, Inc. (NASADQ: HEPA)), has nearly 36 years of pharmaceutical industry experience spanning areas such as drug development and commercialization. In particular, Mr. Sapirstein has participated in 23 product launches, six of which were global launches that he led. He is also credited with leading numerous business development transactions over the course of his career. Prior to ContraVir, Mr. Sapirstein was chief executive officer of Alliqua Therapeutics, served as the founding chief executive officer of Tobira Therapeutics, and as executive vice president, metabolic and endocrinology for Serono Laboratories. Mr. Sapirstein's earlier career included a number of senior level positions in the area of marketing and commercialization, including global marketing team lead for Viread (tenofovir) while at Gilead Sciences and director of international marketing of the infectious disease division at Bristol Myers Squibb. Mr. Sapirstein serves on the emerging companies and health section boards of Biotechnology Innovation Organization (BIO) and is currently the Chairman Emeritus of BioNJ. One thing for certain is CHANGE. Change is a constant law of creation and a law of the Universe. Your human body is in a constant flux at all times. The body is in a constant state of flux just as the Universe is. Sometimes in life, we don't embrace change well.
