In today's episode, Dr. Mitchell Posner, Dr. Sarah Shubeck, and Dr. Jelani Williams on the University of Chicago Medicine's new Comprehensive Cancer Center. Scheduled to open in 2027, the new center is a seven-floor, 575,000-square-foot building planned to have 80 private beds and 90 consultation and outpatient rooms. At the moment, the center is anticipated to see 200,000 outpatient visits and 5000 inpatient admissions annually. This would be the city of Chicago's first freestanding cancer pavilion.How will we ensure that this new center prioritizes the community's needs? In Chicago's South Side, cancer death rates are twice the national average, and cancer is also the second-leading cause of death on the South Side behind heart disease. In this conversation, you'll hear about the center's development, what patients can expect, and most importantly, the Department of Surgery's commitment to ensure the cancer center supports those who are most vulnerable. Dr. Mitchell Posner is the Thomas D. Jones Distinguished Service Professor of Surgery, Chief of the Section of General Surgery, and the Chief Clinical Officer of the University of Chicago Medicine Comprehensive Cancer Center. From clinical trials for cancer treatment to his more than 250 articles, abstracts, and book chapters, Dr. Posner is a leading authority in the management of upper gastrointestinal cancers. He is frequently voted among the country's best doctors. He is the past president of the Society of Surgical Oncology. He is deputy editor of the Annals of Surgical Oncology and section editor for the gastrointestinal cancer section of the journal Cancer. He served as chairman of the Gastrointestinal Committee of the American College of Surgeons Oncology Group (ACOSOG).Dr. Sarah Shubeck is an Assistant Professor in the Department of General Surgery. She is a Breast Surgical oncologist specializing in breast surgery, cancer, and benign disease treatment. In addition to her clinical practice, Dr. Shubeck's research has been published in many journals including Cancer, JAMA Surgery, and Annals of Surgical Oncology.Dr. Jelani Williams is a 5th-year general surgery resident at the University of Chicago. He is an aspiring surgical oncologist and attended the Eastern Virginia Medical School. However, thanks to innovations led by surgeons such as Prof. Donald Nuss of Eastern Virginia Medical School, these procedures have undergone a remarkable transformation. Such work has shifted the paradigm from radical surgery to minimally invasive solutions, changing lives and restoring confidence for countless patients. Now, a Review Article published in the African Journal of Thoracic and Critical Care Medicine, and co-authored by Prof. Donald Nuss and Dr. Ivan Schewitz of the Department of Cardiothoracic Surgery at the University of Pretoria, South Africa, charts the remarkable progress in treating pectus excavatum.

Title: Languaging in Hampton Roads Episode 9 : What It Takes to Raise Bilingual Children Hosts: Jill Winkowski and Prue Salasky Date: Oct. 2, 2024 Length: 38.08 min Publication Frequency: Fourth Friday (approx)  of each month   Co-hosts Jill Winkowski and Prue Salasky explore the topic of how best to raise bilingual children. It's an endlessly complex topic with multiple angles. First, they share some of the changes in research that show the benefits of bilingualism and the study of languages on the physical structure of the brain. They also touch on the changes in attitude toward English variations fueled by demographics and a global economy and the subsequent status accorded to those proficient in more than one language.   To determine what it takes to raise children bilingual – and what fuels the drive on the part of parents – they talk to several foreign-born residents of Hampton Roads. These include university professor of language and art history, Rosa Motta, a native bilingual speaker of Italian and Sicilian; Silvina Bocca, a Spanish speaker from Argentina, an MD/PhD, an expert in infertility and in-vitro fertilization; Brenda Musto, a Spanish speaker from Argentina whose parents were native Italian speakers. She now runs a medical residency program at Eastern Virginia Medical School. They also talk to her daughter Bernie, a high school senior and dual bilingual in Spanish and English; to Yuliya Dobrednova and her husband Boris Dobrednov from Moscow, a microbiologist/biotechnologist and physicist respectively; and to Russian speakers Elena Chambers from Belarus and Natalya Peretyatko of Kazakhstan, founders of the Russian school in Newport News.   We learned how years of English study in their home countries prepared these parents to become bilingual but it wasn't until they came to the U.S. and were immersed in an English-speaking environment that they reached fluency. They talk about their backgrounds and the intense efforts they put into raising their children as heritage speakers – and how success isn't guaranteed. There's ‘no recipe' for success. The parents talk about the challenges and rewards of passing on their culture through language.   We end the show with reference to the huge growth of formal bilingual/foreign language education in Hampton Roads in the past decade: These include a Russian daycare in Virginia Beach; multiple preschools with  Spanish language options; dual immersion Spanish/English programs in the public schools in Virginia Beach and Newport News (the latter serves 600 students and has a waiting list);  weekend language enrichment programs in multiple languages; and the featured Russian school in Newport News. Meet Dr. Jessica Mendelsohn, owner of New South Family Medicine and MedSpa located in Fort Mill, SC.Dr. Mendelsohn is originally from Virginia Beach, VA and went to Virginia Tech for her undergraduate studies.  After graduating from Eastern Virginia Medical School in 2003, Dr. Mendelsohn moved to Charlotte, NC to begin her residency training in Family Medicine. Upon completion in 2006, she practiced in Charlotte for 8 years at a large hospital practice, and then in hospice care for 5 years. In 2019, Dr. Mendelsohn opened New South Family Medicine as a solopreneur.  New South has since grown into both a booming direct primary care (DPC) practice and a full service medspa.Dr. Mendelsohn now employs 15 teammates, including her husband, Dr. Dave Mendelsohn, who joined her primary care practice in 2022. Episode 165: John Edwards on ketamine treatment for depression and suicide prevention

Today we have Dr. Johnathan Edwards, an anesthesiologist and medical practitioner who specializes in human health and optimization. He is perhaps best known for treating mental health conditions with ketamine,  a dissociative anesthetic that is used for general anesthesia, pain relief, depression and epilepsy. John also uses ketamine to help adolescents overcome depression and suicidal ideation. In today's interview, we talk about his new book, “The Revolutionary Ketamine: The Safe Drug That Effectively Treats Depression and Prevents Suicide.” More Americans have died from suicide than all the wars since Vietnam. The suicide rate among 10- to-24-year-olds in this country increased 62 percent from 2007 through 2021. As John points out in today's discussion, most people are not aware that American children between the ages of 10 and 14 are twice as likely to die from suicide than homicide. Show notes: [00:02:39] Dawn explains that suicide is a pressing problem in America, with more Americans dying of suicide than from all the wars since Vietnam. She also points out that police and firefighters are more likely to die from suicide than in the line of duty. John then gives an overview of ketamine and its ability to help treat depression and suicidal thoughts. [00:06:56] Dawn pivots to mention the dark side of ketamine, including ketamine misuse and overdose.  Recent studies have reported a worldwide increase in ketamine misuse and overdoses. Back in October, Mathew Perry, one of the stars of the popular sit-com “Friends,” died from what the Los Angeles cororner described as the acute effects of ketamine. Because this was such a high-profile case, Dawn asks John to discuss the potential adverse effects of ketamine. [00:13:03] Ken mentions a 2022 study in the American Journal of Drug and Alcohol Abuse that looked at ketamine overdoses and deaths. The study found no cases of overdose or death from ketamine used in a clinical setting as therapy for depression. Ken asks if there is anything John would like to add about ketamine and safety. [00:13:059] Dawn shifts to talk about John's background, mentioning that he grew up riding dirt bikes and eventually raced bikes professionally. [00:15:18] Ken asks John to share his story of how a junior college professor sparked his interest in science. [00:17:49] Dawn mentions that John jumped around from Eastern Virginia Medical School, to the University of Reno, to the University of Utah, at which point it looked as though he was heading for a career in internal medicine. Dawn asks why John changed his mind and decided not to pursue that career path. [00:19:40] Dawn asks John what led him to the University of South Florida. [00:20:18] Ken asks John about another career shift that came about as the result of a suggestion from one of John's professors. [00:21:42] Dawn asks about John's motivation to move to Las Vegas to be close to his father. [00:22:59] Dawn asks John to explain what motivated him and his wife to move to France after their daughter turned five. [00:24:55] Dawn asks John to talk about the transition of ketamine from anesthetic to antidepressant. [00:28:16] In his book on ketamine, John writes about how the benefits of supervised psychedelic therapy can be broken down into four effects. Ken asks John to briefly explain each of these effects. [00:31:39] Dawn asks John to explain how ketamine manipulates the function of brain receptors as an antagonist and agonist. [00:33:40] Dawn mentions that some people do not believe that ketamine functions as a classic psychedelic like psylocibin or LSD. She asks John if he agrees. [00:35:54] Ken mentions a recent STEM-Talk interview with Mark Mattson discussing glutamate. In Mark's book, “Sculptor and Destroyer: Tales of Glutamate,” he points out that ketamine's highest interactions are with glutamate, and this affinity has been shown to alleviate depression and schizophrenia.

We are all the product of a reproductive process, yet reproductive biology, or the study of the processes and mechanisms involved in reproduction, is not well understood. Deepening our understanding of reproductive biology is crucial to advancing assistive reproductive technologies (ART) and advancing our collective comprehension of inheritance and evolution. Our guests for this episode are a couple, and we mean a literal married couple, of reproductive biology experts. Dr. Pavla Brachova and Dr. Nehemiah Alvarez, both working in the Eastern Virginia Medical School's Department of Physiological Sciences. In their collaborative work they aim to better understand and characterize the role of RNA and cellular events that impact ovarian function in women. The recent shortages underline why we need to inspire the next generation to pursue these critical specialties. The conversation becomes a call to action, emphasizing the urgency of addressing shortages in pediatric subspecialties. Dr. Carmody discusses potential solutions, including inspiring and supporting you to choose these specialties and adapting care models to ensure continued excellence in pediatric care. Why Residency Matching is Becoming More Competitive  The residency matching process is becoming more competitive by the year. The USMLE has undergone what Dr. Carmody refers to as "scope creep." This term highlights the expansion and intensification of exam expectations over the years. In the early 1990s, the USMLE mean score stood at 200. Fast forward to the present day, and the median score has surged to approximately 250. This substantial increase of 50 points is not something to be ignored. To put this into perspective, the worst test-takers in today's environment outperform the average test-taker from the 1990s. Even the very best test-takers from that era are average in today's ultra-competitive landscape. So what is driving this upward trend, and is this trend good or bad? Despite the perception of increased competitiveness, the number of available residency positions relative to the number of graduating US MD students is more favorable than in recent memory. In the most recent cycle, there were approximately 1.9 residency positions available for every graduating US MD student. When factoring in US DO students, this number remains favorable at 1.38 residency positions per student. This proves that the match process is not all doom and gloom after all. Dr. Paul Marik is the former Distinguished Professor of Internal Medicine and Chair of Pulmonary and Critical Care Medicine at Eastern Virginia Medical School in Norfolk, Virginia. Dr. Marik holds fellowships in the Canadian Royal College of Physicians and Surgeons, the American College of Physicians and the American Board of Physician Nutrition Specialists. He has authored over 400 peer reviewed journal papers, 50 book chapters and four books on critical care. GUEST BIO: Nicole is a Physician Assistant residing in Virginia. She obtained a BS at James Madison University in 2000 and then went on to complete the Master of Physician Assistant program at Eastern Virginia Medical School in 2004. She practiced in an Urgent Care and Occupational Medicine setting until severe illness from benzodiazepine withdrawal syndrome left her unable to work.In 2005, she was prescribed Xanax for “work-related stress.” Over the course of five years, she developed many classic symptoms of benzodiazepine tolerance withdrawal, which multiple psychiatrists misdiagnosed as mental illness. This resulted in prescribed polypharmacy to “treat” the troubling symptoms of tolerance, including two benzodiazepines prescribed simultaneously, a Z-drug, an antidepressant, and an antipsychotic. In late 2010, after discovering a magazine article authored by a journalist experiencing similar symptoms from his prescribed benzodiazepine, Nicole was prompted to research further and made the connection between her own troubling symptoms and the medication. This was followed by her immediate decision to withdraw. Unfortunately, lacking the proper guidance or information at the time regarding the absolute need for a slow taper, she was negligently cold-turkeyed in a detox center. This ultimately resulted in a severe and protracted withdrawal syndrome that persists to date.When symptoms allow, Nicole writes about benzodiazepines and their potential to cause severe and/or protracted withdrawal syndromes and volunteers her time helping with ongoing benzodiazepine awareness initiatives including Benzodiazepine Information Coalition and the Benzodiazepine Action Work Group of the Colorado Consortium. Nicole also co-founded The Withdrawal Project and she does marketing, distribution and outreach for Medicating Normal-The Film. She is an associate at the International Institute for Psychiatric Drug Withdrawal. She hopes to continue to use her lived experience to advocate for more education and awareness around benzodiazepine risks and harms as well as for changes in prescribing and withdrawal practices. She completed her undergraduate studies at Virginia Tech (Go Hokies!) and attended Eastern Virginia Medical School in Norfolk, Virginia, where she earned her M.D. She moved to Charlotte, NC to complete her residency in Family Medicine. Upon residency completion, she was Board Certified in Family Medicine and practiced full-time in Charlotte until 2014. While practicing in Charlotte, Dr. Mendelsohn was very involved in physician leadership, both as medical director of her practice and throughout the hospital system, serving on various committees. She graduated from the McColl School of Business Physician Leadership Institute in 2012 with 30 of her physician colleagues. Also in 2012, she was voted by her peers as one of Charlotte's “Top Doctors” in the May edition of Charlotte Magazine. After the birth of her first son,Charlie in 2013, Dr. Mendelsohn decided to pursue hospice work with Hospice and Community Care in Rock Hill, SC while continuing to assist family medicine practices throughout the Charlotte area. Her hospice experience led her to become medical director of an advance care planning initiative, helping doctors and the communities they serve to better understand the importance of discussing future healthcare wishes and goals of care. Dr. Mendelsohn had her second son, Henry, in 2017. A year later, she started recognizing the need for improved primary care in her community and decided to re-enter the practice of family medicine in a new, independent way. God's Timing and ProvisionWhy would a man happily immersed in a great family life, fulfilling ministry, and a job he loved make a major move and uproot all this blessing? Because the Giver of all that blessing had called, and His timing and provision can be trusted.1999 alumnus Jason Grahame is Director of Cedarville's up-and-coming physician assistant (PA) program, which is scheduled to begin in 2023, pending external approvals. Before his new role, he was a PA in private practice and also Associate Program Director for the Physician Assistant Program at Eastern Virginia Medical School.But when he spoke to Cedarville President Dr. Thomas white in 2019, and found out that his alma mater was thinking about a PA program, the wheels were set in motion for him and his wife, Charity, and their seven children, ages 2½ to 17, to make a major life change.Even though COVID-19 impacted their transition to Cedarville, the Grahame family has been welcomed by long-time friends to the campus community. Even with the PA program, Jason has learned that the Lord is sovereign over the process, and he has learned, just as he did with God's initial call, to trust the Lord's provision and timing, not his own.Jason's wife, Charity, is a constant source of encouragement and prayer support, reminding him of inspiring verses such as Isaiah 40:31.Jason and the new PA program are part of Cedarville's new School of Allied Health, established this summer.
