Monk Fruit Extract: Zero-Calorie Sweetener, Surprising Benefits? Ep. 1241 MAY 2025A recent PRISMA-guided systematic review published in Nutrients synthesized findings from randomized controlled trials (RCTs) to assess the impact of monk fruit extract (MFE) on metabolic health, lipid profiles, inflammation, and regulatory considerations. The review included a number of RCTs (note: while some search results indicated 10, others mentioned 5 included studies; refer to the final published article for the definitive number). The findings indicate that MFE may help reduce postprandial glucose levels (by 10–18% in included studies) and insulin responses (by 12–22%). Some studies also reported reductions in inflammatory markers such as IL-6 and TNF-α. No severe adverse effects were observed in the reviewed trials. The review also notes that MFE is approved for use in the United States and China, with its status under review in the European Union. The authors suggest MFE shows potential as a functional food ingredient for metabolic health, though they highlight the need for long-term clinical trials and a harmonized regulatory framework to confirm its long-term safety and efficacy within sustainable health strategies. Let's talk about alternative sweeteners. Trying alternative sweeteners when starting the ketogenic diet can make the transition much easier. Notice I said alternative sweeteners and not artificial sweeteners. While alternative sweeteners aren't completely natural, they are less artificial and less harmful than artificial sweeteners. Keto snacks and desserts often have different kinds of alternative or artificial sweeteners. Trying keto snacks and keto desserts isn't a bad thing, but it's crucial to check the ingredients. Just because it's labeled “keto” doesn't make it healthy. Out of all of the sweeteners, erythritol and xylitol* are two of the best. But it's still better to only consume small amounts. Monk fruit and stevia are also great options. However, with stevia, it's important to make sure that the product doesn't also contain maltodextrin. Maltodextrin is higher on the glycemic index than sugar. Maltitol, agave nectar, and functional fibers should also be avoided. Including too much of these sugar alcohols (even the good ones) in your diet can cause bloating, fluid retention, diarrhea, and can upset the gut microbiome. They may also prevent you from getting rid of cravings. I think sugar alternatives are great to help a person transition off of consuming sugar and for an occasional treat—but they shouldn't be consumed on a regular basis. *Pet owners should keep in mind that certain alternative sweeteners, like xylitol, can be highly toxic to animals. Sugar alternatives can be found in a wide range of products and may go by different names. It's best to keep these types of products far out of reach from your pets. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Natalie: I am interested to know Dr Cabral's thoughts on the safety of sweeteners like Stevia and Monk Fruit? Monk fruit especially is increasingly showing up in food and supplements and it's very difficult to find information online about it. Stacey: Hello, I was recently diagnosed with SIBO and I am wondering what protocol you recommend. I am also breastfeeding Thank you Stacey Hannah: My question is relating to an ongoing histamine intolerance. I've done the 21 day detox, CBO, 2 months CBO finisher, heavy metal & mold detoxes, two more 7 day liver detoxes, and the parasite protocol. Each protocol was helpful but the parasite protocol (I completed 8-wks) is where I saw the most help with my histamine intolerance. It is significantly better than before that protocol in particular, but I can only handle small amounts of high histamine or histamine liberating foods. I get muscle twitches, sour stomach after eating highly acidic fruits such as oranges/berries, my right foot feels weak/like I'm limping, my right ankle swells up especially with beer, and I get stuck burps/trapped air in between and during meals. I also have a persistent scalloped tongue. Any advice? Anonymous: Hello Dr. Cabral, I am an IHP Level 2 and RN and appreciate all the work you do and the course you made. I'm slowly chipping away on protocols as it seems I have symptoms of everything. That being said I have a history of recurrent incessant pericarditis resulting in a Pericardiectomy in 2021. My surgeon said this should have gotten rid of the pain - it has prevented me from being hospitalized again but I do still experience pain when stressed and sometimes around the heart when taking deep breaths. Additionally I struggle with a lot of jaw /neck pain. I'm looking for any advice or expertise around addressing pericarditis at the root cause as I know that these symptoms are probably still related to the pericarditis that was only addressed by surgery and not at the root. New World Health Organization (WHO) guidance advises against the use of sugar substitutes for weight loss purposes. In this episode of the Xtalks Food Podcast, Sydney talks about the WHO's review, which suggests that while a mild reduction in body weight may occur in the short term, it is not sustainable over time. The review also highlighted potential negative effects associated with long-term use of sugar substitutes, including a slightly increased risk of type 2 diabetes and cardiovascular diseases. The review incorporated 283 studies, including randomized controlled trials and observational studies. Both the Calorie Control Council, an international association representing the low-calorie food and beverage industry, and the International Sweeteners Association, an industry association, expressed disappointment with the WHO's guidance. The team is not surprised with the WHO's guidance but wonders how much of an impact it will have on the broader food industry.Also in this episode, Sydney talks about Pairwise, a startup based in Durham, North Carolina, that launched its first CRISPR-developed product in the US: Conscious Greens. Touted as a mix of superfood leafy greens, the product claims to offer twice the nutrition of traditional romaine lettuce and comes with an appealing fresh flavor. This product is notable because the company claims it's the first food product in the US that was developed using CRISPR technology. CRISPR, or Clustered Regularly Interspaced Short Palindromic Repeats, is a gene-editing technique that can be used to alter the DNA of cells to enhance certain characteristics or reduce less desirable ones. In this case, Pairwise used CRISPR to address the issue that most lettuces lack nutritional value, and many other greens are too bitter or hard to eat. Sydney also mentions other companies that are using CRISPR in agriculture, including Inari Agriculture, Monsanto and DuPont. The team wonders whether food companies will have to include a label that indicates whether a food product was developed with CRISPR. There are all sorts of “natural sweeteners” on the market, but we've got questions… How natural are they? Are they a healthy substitute to other sweeteners?First off, our preferred natural sweeteners are very minimally processed; honey, maple syrup, and coconut sugar. Things like agave, cane sugar, rice syrup, etc. tend to be much more processed and therefore “empty calories.” Then there are the real garbage products like corn syrup and beat sugar (which are GMO, toxic foods) as well as synthetic sweeteners like aspartame, surculose, saccharin, etc. which can be toxic to the nervous system.Today we wanted to unpack a couple of other products that get slapped with the “natural sweetener” label - those derived from Stevia and Monk Fruit.
