ABOUT THE EPISODE:It probably doesn't surprise you that the majority of Hopestream Community members are moms. About 70% of therapists are women, as well, and women are significantly more likely than men to seek out their services.Last year, we launched The Woods, our private community for dads, stepdads, and male co-parents. And as much as we would love to help more dads who have kids struggling with substances, they don't come flocking to our services. I wanted to dive into this imbalance and asked Steve Andrews to join me for a conversation. As host of The Woods, Steve brings a lot of harrowing personal experience to the table. When his son first began misusing DXM, Kratom, alcohol, and marijuana in middle school, he had no idea how bad things would get. (To hear his whole story, check out his first appearance on the show at the link below.)Now, with his son in full recovery, Steve is a practicing therapist. He's seen this gender dynamic play out within himself, in The Woods, and the world of therapeutic practice. Today, he gives us his best take on why men seem to have a natural aversion to approaches like The Invitation to Change and CRAFT and how we can reframe those ideas in a more accessible way.EPISODE RESOURCES:Steve's Story, Hopestream Episode 183Never Split The Difference, book by Chris VossSteve's websiteThis podcast is part of a nonprofit called Hopestream CommunityLearn about The Stream, our private online community for momsLearn about The Woods, our private online community for dadsFind us on Instagram: @hopestreamcommunityDownload a free e-book, Worried Sick: A Compassionate Guide For Parents When Your Teen or Young Adult Child Misuses Drugs and AlcoholHopestream Community is a registered 501(c)3 nonprofit organization and an Amazon Associate. They warned their kids of the alcoholism and mental health issues on both sides of the family, but when one of them began misusing DXM, Kratom, alcohol, and marijuana in middle school, the couple found themselves in an increasingly volatile situation.Meanwhile, Steve was also trying to maintain a stressful and demanding work life at the office – with me! M., Oorschot, V., Becher, I., Neumann, B., Muley, T., Winter, H., Duerr, J., Mall, M., Savitski, M., Pepperkok, R. Abstract: Excessive deposition of fibrillar collagen in the interstitial extracellular matrix (ECM) of human lung tissue causes fibrosis, which can ultimately lead to organ failure. Despite our understanding of the molecular mechanisms underlying the disease, a cure for pulmonary fibrosis has not yet been found. In this study, we screened an FDA-approved drug library containing 712 drugs and found that Dextromethorphan (DXM), a cough expectorant, significantly reduces the amount of excess fibrillar collagen deposited in the ECM in in-vitro cultured primary human lung fibroblasts (NHLF) and ex-vivo cultured human precision-cut lung slice (hPCLS) models of lung fibrosis. Reduced extracellular fibrillar collagen levels in the ECM upon DXM treatment are due to a reversible trafficking inhibition of collagen type I (COL1) in the endoplasmic reticulum (ER) in TANGO1 and HSP47 positive structures. Mass spectrometric analysis shows that DXM causes hyper-hydroxylation of proline and lysine residues on Collagen (COL1, COL3, COL4, COL5, COL7, COL12) and Latent-transforming growth factor beta-binding protein (LTBP1 and LTBP2) peptides coinciding with their secretion block. In addition, thermal proteome profiling of cells treated with DXM shows increased thermal stability of prolyl- hydroxylases such as P3H2, P3H3, P3H4, P4HA1 and P4HA2, suggesting a change in activity. Transcriptome analysis of pro-fibrotic stimulated NHLFs and hPCLS upon DXM treatment showed activation of an anti-fibrotic program via regulation of pathways such as those involved in the MMP-ADAMTS axis, WNT, and fibroblast-to-myofibroblast differentiation. Taken together, the data obtained from both in-vitro and ex-vivo models of fibrogenesis show that Dextromethorphan has potent anti-fibrotic activity by efficient inhibition of COL1 membrane trafficking in the ER. It's fall! This means a few things. Not only are the leaves changing color and nearly everything is flavored with pumpkin spice, but cold and flu season is also officially in full swing. If you find that you've got an aching head, a runny nose, or a scratchy throat, you're not alone. You're going to need something to make you feel better, but the cold and flu aisle can be so intimidating. What are all these pills, syrups, and herbal elixirs? Which one will effectively alleviate your symptoms? Before you grab the first thing you recognize from a TV commercial and end up feeling dissatisfied with the results or suffering from some undesirable side effects, it might be useful to draw on the expertise of a Board Certified Pharmacotherapy Specialist like Greg Castelli. Today, we ask Greg, a real-life clinical pharmacist, which cold medicine is best. He shares his take on intranasal sprays, cough syrups, home remedies, and more. We also discuss whether or not brand-name drugs are better, what contraindications to look out for, and how to go about caring for your kids when they've got a case of the sniffles. Hopefully, by the end of this episode, you'll have a much better idea of which remedy to reach for the next time you're feeling under the weather! Topics discussed in this episode: Letting your symptoms (and your pharmacist) lead you Nasal sprays and how to use them Brand names vs. generic drugs When you should and shouldn't take Sudafed Antihistamines for nasal congestion Why you should only take the products that you need Your guide to cough medications The importance of managing your expectations of recovery Symptoms of cough medicine and DXM (dextromethorphan) abuse Common antipyretics used to treat fever and inflammation Recommendations for treating your kids when they are sick Home remedies: from honey to humidifiers Tips for topical treatments like Vicks VapoRub A look at the pros and cons of herbal remedies and vitamins How pregnant women can feel better without harming the baby Greg's Halloween plans, weird questions he gets as a pharmacist, and more! Mike used to pound entire strawberry shortcakes right before bed. Trump made fun of one of the kids from the Meijer clan known for their popular department stores and we wanted to share Trump's doofus audio with you. There are many Hollywoods outside of the California version. Luke can't do any impression either. None of us can do a quality Trump impression. Scott went to Cedar Point last week and we get into our favorite coasters and early Cedar Point memories. Did you know the Millenium Force has kick-ass techno music from the turn of the century? Mike first went to Cedar Point to ride the Magnum XL-200 and bond with his once removed stepdad. Why does anybody need to add substances to roller coaster adventures? There's a lot of Million Dollar Man laughs in this show. Just sayin'. Mike wore a dumb hat in high school that everybody hated. Ravishing Rick Rude brings a flow of memories back for John Scott. What was the first thing you ever bought on the internet? Was it a shirt? A mug? drugs? Luke used to love Eastbay catalogs. He also bought bootlegged Seinfeld. Scott was hardcore into Fat-Wreck stuff online. Mike bought Andro and DXM with his brand new credit card that he didn't know how to use as a freshman at college. We got your emails covered! We start with Danny who finally reveals the meaning behind squids! What director is must-see for you when their movie comes out? Also, Jon Bernthal is a positive bad-ass. Luke and Mike get pissed about people immediately dismissing a Heat sequel, but will rush out to see every Marvel movie right away. Former guest host Jason emails the show and wants to know if RDJ will get canceled again. Mr. Pink sneaks a message in at the end of the show, but we will give his message more time next week. David Rickard serves as Vice President at Everest Group. He's based in the UK and manages the customer experience management practice and sourcing and vendor management on a global scale. Before Everest, David spent a couple of years consulting and was at Microsoft for 18 years overseeing their contact center outsourcing business.    Despite the pandemic, the BPO industry has bounced back considerably and more quickly than initially anticipated. David explained that the current state of the outsourced customer experience management (CXM) market is at approximately $89 to $91B in terms of spend. That number is expected to continue increasing over the coming years.     CXM Growth by Vertical    Everest has found that there has been growth across all industries. Still, the most significant and fastest-growing verticals are in technology, including retail and online retail brands, due to increased demand. People are buying online more than ever. There's also been a growth in government outsourcing. This growth has been consistent across all regions and geographies, except the Middle East and Africa regions, since they are experiencing significant growth as they introduce more outsourcing to these areas.    Major Trends in the CXM Sector of the BPO Industry    The Everest Group found that there has been a significant push towards digital customer experience management (DXM). They consider DXM a combination of technology such as speech analytics or robotic process automation and the servicing around DXM. This includes customer journey mapping, design thinking, and consulting. Everest has seen DXM increase by $17B over the past year, driven by COVID and companies needing to accelerate their digital transformation to ensure business continuity.     COVID wasn't the only driver, however. Everest did a survey and found that there were four main reasons companies were accelerating their digital transformation. They found that 47 percent of respondents said it was due to their customers' expectations; 57 percent said it was to reduce costs; 55 percent said it was due to their desire for market differentiation; 49 percent said it was because they were trying to achieve a 360-degree view of the customer.     Where are CXM BPOs Providers Investing Heavily?   BPOs invest in the technology and capabilities to bring in more DXM solutions and provide more than contact center labor. For example, providing Contact Center as a Service (CCaaS) solutions or an end-to-end solution while bringing in technology analytics and intelligent virtual agents.     Impact on Consumers Who Need Support   Digital channels have a positive impact on consumers who want more choices for service. At iQor, we use our omnichannel customer care services to provide customer service to consumers where they want, when they want. David mentioned that the best customer service companies in the world, however, remember that not all consumers want to be digitally served. Some still want to chat on the phone with an agent. David mentions his mother, for example, who doesn't have internet access. His mother still relies on a live agent on the phone line when she needs customer service.      Most consumers, especially younger demographics, like the options of digital channels, but the best outsourced customer care providers keep in mind the inbound solution because not every customer wants to be served through one of those digital channels. And for certain types of calls, it's better to talk to someone on the phone. With the availability of all of these options, the consumer gets a better choice and a better experience over time.   CXM Sector Growth    David expects that the CXM sector will continue to grow at a rapid rate. What will change, though, is the mix. The BPO providers with CXM solutions will grow quicker than traditional voice-centric contact centers. David quickly pointed out that traditional contact center needs will never disappear, but they will decline slowly as digital CXM offerings capture more and more of the market share.     What David Does for Fun   David likes to take his boxer dogs on walks with his wife for fun. He also just started playing golf again. He used to play regularly with his three friends but took a 20-year hiatus due to raising children but is now picking it up again.  To learn more about the Everest Group, go to everestgrp.com.  Watch the video here. R. Abstract: Epigenetic changes, such as aberrant DNA methylation, contribute to cancer clonal expansion and disease progression. However, identifying subpopulation-level changes in a heterogeneous sample remains challenging. Thus, we have developed a computational approach, DXM, to deconvolve the methylation profiles of major allelic subpopulations from the bisulfite sequencing data of a heterogeneous sample. DXM does not require prior knowledge of the number of subpopulations or types of cells to expect. We benchmark DXM's performance and demonstrate improvement over existing methods. We further experimentally validate DXM predicted allelic subpopulation-methylation profiles in four Diffuse Large B-Cell Lymphomas (DLBCLs). Lastly, as proof-of-concept, we apply DXM to a cohort of 31 DLBCLs and relate allelic subpopulation methylation profiles to relapse. We thus demonstrate that DXM can robustly find allelic subpopulation methylation profiles that may contribute to disease progression using bisulfite sequencing data of any heterogeneous sample. We apologize to our listeners for our show being late this week. We are coming to you directly from the Exploring Psychedelics Conference at Southern Oregon University, and preparations and travel have slowed down the release of this week's episode. On Today's Episode of the podcast, Jonathan talks with Jon Bridge. Jon is  a 34 year old father of two. from Canada. The two met through a Facebook Group, The Terence McKenna Experience, thanks to Jon's willingness to share his inspiring story of hope and healing. Jonathan felt called to respond almost immediately after the posting appeared on his feed, and the two connected to share Jon's story. Jon's experience of the failure of standard  psychiatricpharmaceutical drugs  and his journey back to health with the help of psilocybin mushrooms and a clear understanding that the conditions that ailed him for years did not have to be permanent, or simply "managed" with ill-understood collections of molecules. Jon [I was on] daily doses of high strength, high dose pharmaceutical garbage. I was supposed to be a customer for life. Although I have no credentials behind my name I have a story of healing and hope. I had been a long time sufferer of mental illness but thanks to psychedelic medicines I have discovered a new, better self. My journey into psychedelic healing has just begun and I hope to be able to inspire someone to help themselves. Jon's "before" picture, on the let, is dated March 20, 2010, exactly 6 years before the date of his Facebook post..Looking in the eyes, it feels possible to almost see the changed interior landscape behind them. This is a story that our listeners will not want to miss. (None of Jon or Jonathan's remarks should be interpreted as psychiatric advice. Neither is a licensed therapist. Changes in medicines should be done with the partnership of a qualified professional.) At recommended doses dextromethorphan, a common ingredient in most cough medicines like Sucrets®, Robitussin®, and NyQuil®, produces little or no central nervous system effect. At recreational doses, positive effects may include acute euphoria, elevated mood, dissociation of mind from body, creative dream-like experiences, and increased perceptual awareness. Other effects include disorientation, confusion, pupillary dilation, and altered time perception, visual and auditory hallucinations, and decreased sexual functioning. Recreational doses impair judgment, memory, language, and other mental performances however the drug experience varies greatly depending on the amount of DXM ingested. Due to the high dosages needed for pleasurable effects, the potential for immediate and long term negative health consequences, and unpredictable nature at different dose levels, we strongly recommend that DXM be avoided.
