Het is donker op Cuba. Grote delen van de dag, en van de nacht, zitten mensen op het eiland zonder stroom. De oorzaak: een groot tekort aan olie. Het verouderde energienetwerk draait daar op. Cuba haalde veel olie uit Venezuela, maar de Amerikaanse president Trump blokkeert dat nu. De olietekorten komen bovenop andere problemen, en maken die ook groter. In de winkels ligt minder eten, het openbaar vervoer rijdt niet meer, en de straten liggen vol met vuilnis dat niet meer wordt opgehaald. The management of complex regional pain syndrome (CRPS) continues to challenge clinicians—especially when conservative and interventional strategies fall short. This episode of RAPM Focus, hosted by RAPM Editor and Social Media Editor Ryan D'Souza, MD, explores these challenges in managing CRPS with Daniël van der Spek, MD, and Frank Huygen, MD, PhD, FIPP, FFPMCAI, following the August 2025 publication of their original research paper, “Long-term outcomes of amputation in patients with complex regional pain syndrome (CRPS): a mixed-methods study.” In rare and severe cases, amputation has been proposed as a last resort intervention, but the topic remains completely controversial. How does one weigh the potential relief of unrelenting pain against the risk of further complications and disability from an amputation? This episode explores a fascinating, mixed-method study into the long-term outcomes of patients with CPRS undergoing limb amputation in hopes of pain relief, despite risks of residual pain or phantom limb pain. Dr. Daniël van der Spek is a medical doctor and PhD candidate at the Erasmus University Medical Center under the supervision of Professor Huygen his research focus is on CPRS with a particular emphasis on interventional pain management. Dr. Frank Huygen is an anesthesiologist and pain physician serving as the head of the Department of Pain Medicine at Erasmus University Medical Center, and has been a full professor since 2009. His expertise focuses on CPRS and neuromodulation. He leads multiple national and international research projects, and maintains close collaborations with CRPS patient organizations, in both the Netherlands and the United States. *The purpose of this podcast is to educate and to inform. In this episode of Plan Sea, hosts Anna Madlener and Wil Burns sit down with Dr. Phil Renforth and Dr. Mijndert Van der Spek of Heriot-Watt University to unpack their newly published, harmonized framework for evaluating the viability of ocean alkalinity enhancement (OAE) pathways. Moving beyond lab-scale assumptions, their approach integrates techno-economic analysis (TEA) and lifecycle assessment  (LCA) to allow the exploration of 54 known OAE variations and how they perform under future, decarbonized energy scenarios. The conversation highlights why real-world data, a cleaner energy grid, and feasibility assessments are important for determining which OAE pathways will deliver results in global scale carbon removal. With any emerging solution, both feasibility and cost must be effectively evaluated. Renforth and Van der Spek combine two essential lenses – techno-economic analysis (TEA) and lifecycle assessment (LCA) – to build a comprehensive picture of OAE's real potential. TEA determines if a pathway is economically viable and scalable, while LCA screens for its full environmental impacts, not only if it is net-negative, but also whether it engages in “burden shifting,” or solving one problem while creating another. Operating far beyond “carbon balancing,” LCA works across a range of categories, from greenhouse gas emissions to terrestrial and marine acidification, resource use, and pollution. Together, the two tools are meant to provide a level of quantification for decision-makers investigating the viability of any CDR approach. In looking at the framework, Dr. Renforth and Dr. Van der Spek began by introducing  the framework's structure through a case study of BPMD and its functionality as OAE technology. Rather than offering predictions, the framework helps to show how different technologies perform under current assumptions. This means the framework should not be viewed as forecasting long-term outcomes on its own, but instead as a tool to see how each pathway changes. While these tools are powerful, they are incomplete and alongside rigorous research into the broader social, regulatory, and ethical implications of each potential pathway. For example, LCA aims to measure global stressors by normalizing impacts, but it does not have the ability to detect localized effects. This highlights that any comparison drawn from the framework must be paired with site-specific environmental assessments. Jerome Powell heeft nog ruim elf maanden te gaan als Fed-voorzitter. Maar die maanden duren Donald Trump net wat te lang. Daar weet Trump wel raad mee. Als hij nu al een opvolger aanwijst, dan heeft hij het alsnog praktisch voor het zeggen. Want zo richt hij een soort 'schaduw-Fed' op, waar de markten meer aandacht aan zullen hechten dan aan Powell. Maar zet Trump 'zijn' economie en 'zijn' dollar op het spel als hij zijn eigen pion neerzet bij de Federal Reserve? Met die vraag mogen beleggers wereldwijd gaan worstelen de komende tijd. Verder zegt de Amerikaanse president een deal te hebben bereikt met China. Nou ja: een tussendeal. Een 'raamwerk' dat de handelsoorlog verder pauzeert voordat een andere deal de oorlog écht beëindigt. En er zit nog een haakje aan, want zowel Trump zelf als Xi Jinping van China moeten er nog een handtekening onder zetten. Autoproducenten in de VS en Europa staan te juichen, want de zeldzame aardmetalen stromen China weer uit. Chinese studenten mogen in ruil daarvoor verder studeren in de VS. Ook Elon Musk maakt een deal, maar dan met zijn eigen geweten. Hij biedt excuses aan voor het gebekvecht met Donald Trump van vorige week. Teslabeleggers zijn uitzinnig: het aandeel staat alweer hoger dan het voor de ruzie stond. Misschien is de nieuwe proef met robotaxi's die op 22 juni van start gaat wel de kers op hun taart. Tot slot draait Jensen Huang van Nvidia even 180 graden bij. In januari zei hij nog dat kwantumcomputers pas over 20 jaar nuttig zouden worden, waarna kwantum-aandelen zoals Rigetti en IonQ van de trap vielen. Rigetti verloor zelfs 70 procent van zijn waarde. Maar vandaag zei Huang opeens dat de kwantumrevolutie een keerpunt heeft bereikt en dat de kwantumtoekomst binnen handbereik ligt. Mensen die maandenlang hadden gewacht op de felbegeerde afspraak zagen ineens op hun telefoon dat hun reis van soms duizenden kilometers voor niks was geweest. Tijdens omstreden landhervomingen moesten zij, samen met duizenden andere witte boeren, hun land afstaan om koloniale ongelijkheden recht te zetten. Maar nu compenseert het Afrikaans land de boeren. Een groot bedrijf wil er goud gaan winnen, wat leidt tot massale houtkap. Klimaatactivisten vinden dat dit moet stoppen en voeren actie – met muziek als wapen. Redacteur Cevahir Varan sprak met hen. De stoepen van de Mexicaanse grensstad Tijuana liggen vol met drugsverslaafden. De afgelopen jaren raken daar steeds meer mensen verslaafd aan fentanyl. De zombiedrug is de boosdoener van een grote drugscrisis in de VS. Fentanyl is doodsoorzaak nummer 1 onder jonge mensen. De bovenburen proberen die crisis aan te pakken en daarom richten de drugskartels zich op een nieuwe markt: Mexico.  Correspondent Boris van der Spek vertelt in podcast de Dag wat hij aantrof in Tijuana, een stad die altijd al gebukt gaat onder drugssmokkel. De verslaafden zijn veelal uitgezette migranten uit de VS die in de grensstad blijven hangen en aan de fentanyl raken.  Teun Voeten is fotograaf en legt al tientallen jaren verslaafden vast. Hij zag hoe fentanyl in Amerika opkwam en hoe het levens verwoestte. Volgens Voeten is de markt in de VS verzadigd en dus zoeken kartels nieuwe afzetmarkten voor de super verslavende drug.
