Uansett er det å foreslå bussing eller bosetting i nord for å bøte på en vidåpen grense, med de konsekvensene den har for borgernes sikkerhet, kultur og felleskap, nå i 2023 intellektuelt tilbakestående. I likhet med andre europeiske folkeslag får også nordmenn kjenne dette både på kroppen og lommeboka, for å ikke nevne sosialt: Fremmede, fiendtlige kulturer får utfolde seg fritt på bekostning av deres egen. Visit your primary care provider for additional medical advice.Introduction:Obesity is one of the most common pediatric chronic diseases affecting 14.4 million children and adolescents (about twice the population of New Jersey).A recent simulation study from the US found that by 2030, a staggering 55–60% of today's children will be obese.1 in 4 children in California have obesity.Research shows that the ages between 0 and 5 years is a critical period in the development of overweight and obesity. Obesity has complex genetic, physiologic, socioeconomic, and environmental contributors. Pediatricians and other primary care physicians need to be aware of the risk factors for pediatric obesity to provide early anticipatory guidance for prevention, close monitoring, and early intervention when the weight trajectory increases.We will discuss the risk factors for children and adolescents to develop overweight and obesity, we will be diving deep into general, environmental, and familial factors. This is based off the AAP (American Academy of Pediatrics) “Clinical Practice Guidelines for the Evaluation and Treatment of Children and Adolescents with Obesity.” This guideline was published in February 2023, it is available online for free, and this is the first edition.A. General Factors- Socioeconomic StatusA longitudinal analysis of predominantly non-Hispanic white children in the United States found that low socioeconomic status before 2 years of age was associated with higher obesity risk by adolescence in both boys and girls.Poverty is associated with toxic stress, limited access to healthy foods, and low physical activity.-Children in Families That Have Immigrated to the USRecently arrived immigrants tend to be healthier than their US-born counterparts. However, as immigrants try to adjust to a new culture, they may adopt Americanized foodways, which are high in fat, sugar, and salt.Second-generation Hispanic immigrants are 55% more likely to have obesity than nonimmigrant white children, whereas first-generation Asian immigrants had a 63% lower risk of having obesity.Larger body sizes may be an indication of health and wealth in some cultures. This cultural factor may make it more difficult for parents to understand the gravity of their children's obesity.Comment: This is a common concern among Hispanic families that bring their children to the clinic to get “vitamins” to gain weight because they look “sick,” but their BMIs are normal. PCPs should be prepared to address that concern in the clinic.B. Neighborhood and Community Environments-School EnvironmentThe presence of fast foods, vending machines, and/or sweetened beverages in schools may negatively influence children's food choices, this effect is larger in younger grades.One day I went to have lunch with Devin, I liked that they had to go through the salad bar before they went to get other foods. They had the choice between vegetables or fruits.-Lack of Fresh Food AccessNeighborhood food environment has been shown to have a mixed association with children's BMI.Children and families in these settings may be unable to access fresh fruits and vegetables and safe physical activity spaces. There may be limitations in transportation, cost, affordability, and availability.-Fast food proximityLow-priced, calorie-rich fast foods with elevated levels of saturated fat, simple carbs, sugar, and sodium are commonly sold in fast food restaurants. Because they are easily available, they taste good, and they are strategically marketed, fast foods tend to be popular among children and adolescents.Some studies, not all, have shown an association between fast food locations near schools and obesity in children; a stronger association is seen in populations with lower socioeconomic status.-Access to safe physical activityGreater exposure to green space has been shown to be associated with higher levels of physical activity and a lower risk of obesity.That is something we have to recommend during our well-child visits. We are seeing a lot of aversion to going outside among the new generations. Going out seems to be torture when they find so much fun inside their houses (countless amounts of videos, video games, air conditioning/heater, etc...). A strategy for parents can be recommended 1 hour of playing outside before allowing screen time.-Environmental Health Exposure to environmental hazards during the prenatal period, infancy, and childhood can have impacts on the health and well-being of children.Exposure to endocrine-disrupting chemicals may occur through breastfeeding, inhalation, ingestion, or absorption through the skin. We are basically surrounded by hazardous chemicals used in cleaning agents, food packaging, pesticides, fabrics, upholstery, etc. Exposure during early childhood can affect the programming of several systems, including endocrine and metabolic systems, which may affect BMI, cardiovascular, and metabolic outcomes later in life. C. Family and home environment factors-Parenting feeding styleFour types of parent feeding styles have been described:AuthoritativeAuthoritarianPermissive or indulgentNegligentAuthoritative feeding is considered protective against excessive weight gain. Children tend to eat more healthy foods, be more physically active, and have healthier BMI compared with children raised in homes with authoritarian, permissive or indulgent, or negligent parenting styles.-Sugar-sweetened beverages within the homeA systematic review of 20 prospective cohort studies and randomized controlled trials from 2013 to 2015 found that sugar-sweetened beverages (SSBs) were positively associated with obesity in children in all but 1 study.Comment: Sugary drinks are an easy way to get calories in your body. People tend to think that drinks don't count, but they can be loaded with carbs. Orange juice can have up to 25 grams of sugar in a cup, some sodas may have double, and both are high in sugar.-Portion sizes and snacking behavior Positive association but need long-term studies-Dining out and family mealsEating outside of the home—irrespective of the type of restaurant establishment visited—is associated with a higher risk of weight or BMI gain.Conversely, 2 meta-analyses found that an increased frequency of eating family meals was associated with a lower risk of childhood obesity.-Screen time – This is one of the major struggles we have as parents. It's a daily fight.A recent meta-analysis reported a 42% greater risk of being overweight or obese with more than 2 hours per day of television (TV) compared with 2 or fewer hours.Meta-analysis showed that even short exposure to unhealthy food and beverage marketing targeted at children resulted in increased dietary intake and behavior during and after the exposure.Marketing occurs via television, websites, online games, supermarkets, and outside schools.Male children and adolescents tend to spend more time on media screen devices and other Internet technology than female children and adolescents do.There is no conclusive guideline about it, but in general, we can recommend sleep hygiene to avoid insomnia or abnormal sleep patterns, such as avoiding “screens” 1 hour before bed.-Sedentary behaviorNo association. However, many confounding factors include physical activity, screen time, and unhealthy food intake.-Sleep durationChildren 13 years and younger with short sleep duration (∼10 hours) had a 76% increased risk of being overweight or obese compared with their counterparts with longer sleep duration (12.2 hours).Sleep restriction may be associated with increased calorie consumption, fatigue, and decreased physical activity.-Environmental smoke exposureChildren under 8 years old exposed to environmental tobacco smoke (ETS) have been found to have higher BMI compared with their nonexposed counterparts.I wonder If this is a direct or indirect effect of smoke.-Psychosocial stressA meta-analysis showed that prenatal psychological stress was associated with a higher risk of childhood and adolescent obesity.Psychosocial and emotional issues may lead to weight gain through maladaptive coping mechanisms, including eating in the absence of hunger to suppress negative emotions, appetite up-regulation, low-grade inflammation, decrease in physical activity, increase in sedentary behavior, and sleep disturbance. Depression has been shown to be a risk factor in both pediatric and adult obesity.-Adverse childhood experiencesACEs include a history of physical, emotional, or sexual abuse; exposure to domestic violence; household dysfunction from parental divorce or substance abuse; economic insecurity; mental illness; and/or loss of a parent because of death or incarceration.A study found that having many ACEs increased two times the risk of children having overweight or obese compared with children with no history of ACEs. Stress may result in abnormal coping strategies—such as binge eating, eating in the absence of hunger, impulsive eating, and poor sleep hygiene—which may result in further weight gain.Summary: The consequences of childhood obesity are far-reaching and extend beyond physical health issues. Children with obesity are more likely to experience a range of health complications, including type 2 diabetes, high blood pressure, sleep apnea, joint problems, and psychological issues such as low self-esteem and depression. Moreover, children with obesity are at a higher risk of carrying their weight-related problems into adulthood, increasing their susceptibility to chronic conditions such as cardiovascular disease, certain types of cancer, and premature mortality. As medical providers, we all need to keep general, environmental, and familial factors in mind when discussing weight changes among our pediatric patients. Having knowledge of such influences will help us intervene and prevent further progression. ______________________________Conclusion: Now we conclude episode number 144, “Risk Factors for Pediatric Overweight and Obesity.” Future Dr. LaL reminded us that childhood obesity is a disease linked to multiple risk factors, including but not limited to: low socioeconomic status, lack of access to safe spaces for exercise, parenting feeding styles, sleep disturbances, and adverse childhood events. Frøya har hatt rask befolkningsvekst og stor byggeaktivitet, i Åfjord etableres en stor park for vindkraft og Sigdal er en stor hyttekommune, mens Flakstad i Lofoten har vedtatt arealnøytralitet. Forskerne har intervjuet politikere og administrativt ansatte på lokalt, regionalt og nasjonalt nivå, og representanter fra næringsliv. Hovedspørsmålet har vært: Hva er de viktigste faktorene som former bærekraftig arealbruk i disse kommunene? I denne episoden forteller Anders Tønnensen om hvilke svar de har fått. Ever since the YouTube video of Dr. Lustig and the evils of fructose went viral in 2010, many have advocated that fructose is the single most common cause of obesity in the United States. The video had metabolic pathways that had the hypothesis that most fructose either becomes fat in the liver, or that it might go down a pathway to cause joint issues, leaky gut, and inflammation causing obesity. Here are the two chemical structures of glucose, also known as the "good sugar" and fructose, or the bad sugar. I cannot help but thinking of the good witch and the bad witch on the Wizard of Oz. Dr. Lustig's hypothesis was based on studies done in mice and rats. In those studies, published two years before the viral video, found that high fructose diets in mice lead to increased "gut leak" and led to liver damage (1). This was even confirmed with a small study of 8 men (2) who had their complex carbohydrates replaced by fructose and fed normal caloric diets - meaning, these eight men had increased liver fat, more de novo fat formation (de novo lipogenesis) . This all makes that logical sense since sugar-sweetened beverages are associated with chronic inflammation and these days everything to do with chronic inflammation increases the risk of obesity, diabetes, heart disease, and aging. Then came a double-blinded, randomized, crossover study where a group of people were fed a standard diet but drank 25% of their calories as either fructose (the evil sugar), or glucose (the good one) or a high fructose corn syrup sweetened beverage. Oddly, there was no difference in any of those groups with regard to markers of inflammation, intestinal permeability, or inflammation in the fat tissue. To quote from the study: "Excessive amounts of fructose, HFCS, and glucose from SSBs consumed over 8 d did not differentially affect low-grade chronic systemic inflammation in normal-weight to obese adults." SSB = sugar sweetened beverages. There have been a number of human trials looking at sugar-sweetened beverages and inflammation, and the results are not consistent. Even finding that people who drink large amounts of either glucose of fructose didn't find changes in inflammation or visceral fat. Mice are not men, and the link between gut permeability, leading to systemic and chronic inflammation, occurs in mice, but not men. And while in mice fructose can lead to inflammation and liver issues, this doesn't happen in human beings. (4) The key may not be fructose itself, but increased caloric intake. If you eat more, you will increase in weight, but it is not fructose alone. It is, in fact, high caloric intake combined with sugars. Meaning, that great tasting snack with the high levels of sugars and fats act in concert with the increased calories consumed to give you that portly look. To be fair, all of the studies were short term, less than a couple of weeks. Fructose, over the long term, may still be a bad actor. What about fruit? It is exceedingly difficult to eat enough whole fruits to make this a problem. To eat the excess amount in some of these studies you would have to eat several pounds of apples, and most people simply cannot do that in a day. And whole fruit consumption, as a part of the Mediterranean diet reduces the burden of cardiovascular disease, diabetes, and even erectile dysfunction.(5) In fact less than 10% of most Western dieters have adequate levels of fruit (6) leading to a serious threat to human health. Today, Finance Minister Nirmala Sitharaman announced a second set of measures as part of the 'Atma-nirbhar Bharat Mission' economic package. The second set of announcements focus on migrant workers, street vendors, small farmers, self-employed individuals. First she spoke about 3 steps for migrants workers 1. Free food grain supply to all migrant workers, even non-card holders, for the next two months — 5 kg wheat/rice/1 kg chana. And for this Rs 3,500 crore will be spent over the next 2 months. State govts will be implementing agency for this. 80 million migrant workers to benefit by the initiative 2. Then she announced National portability of ration cards.'One nation, one ration card' to be implemented and 100% coverage will be done by March 2021. In 23 states, 67 million will be benefited from this. (83% of all PDS beneficiaries) already covered 3. Affordable rental accommodation for migrant workers/urban poor. This is a special scheme for rental housing under Pradhan Mantri Aawas Yojana. this is incentivising manufacturing units to build affordable rental housing complexes. This will also convert govt-funded houses into rental housing complexes. And the scheme will be implemented on PPP model via concessionaire arrangements. Finance Minister Nirmala Sitharaman said Rs 10,000 crore worth of steps were taken so far this year for generating work for migrant labourers in 187,000 gram panchayats 4. Next is about the steps taken under Mudra Shishu Loans (Rs 50,000 or less) Interest subvention support of 2% for 12 months at the cost of Rs 1,500 crore. So far, Rs 1,62,000 crore given out under this type of loan. 30 million people to benefit by this 5. Fifth measure is for 50 lakh street vendors Each Street vendors will be able to avail Rs 10,000 loan from Rs 5,000 crore special credit facility once the lockdown is lifted. And those making repayments through the digital mode to get additional benefit 6. Next are the few steps announced for Housing Credit-linked affordable housing subsidy scheme provided for the middle income group (that is the the lowest strata... with Rs 6 lakh to Rs 18 lakh annual income). Well, this was started in 2017 and now extended to March 31, 2021. This is a Rs 70,000-crore scheme to spur job creations and demand for construction material is expected to improve with this. 330,000 families have benefitted from this scheme so far and now another 250,000 more to benefit 7. Seventh sets of measures are for employment generation Rs 6,000-crore CAMPA funds to be used for afforestation and similar projects. These Proposals have come from states, FM said. This aims to generate employment for tribal and rural people 8. Next She announced steps for farmers Rs 30,000-cr working capital facility is provided to them, in addition to the Rs 90,000 crore already provided by Nabard. States will be given this fund via SSBs, RRBs. FM said that these Funds will be released immediately for post-rabi harvest work and kharif preparation of small and marginal farmers. Well, 30 million farmers to benefit by this 9. Last measure is about credit for farmers Rs 2 trillion worth of concessional credit to be extended to 25 million farmers. This will be provided through Kisan credit cards. Animal husbandry and fisheries will also be eligible for this scheme.
Today's guest, Dr. Hunt Allcott, had two recent papers with colleagues Benjamin Lockwood and Dmitry Taubinsky, on whether soda taxes are effective, and how an optimal soda tax might be established. They were published by the National Bureau of Economic Research. These are important papers and an important time, given all the activity around the world on soda taxes. I'm Kelly Brownell, director of the World Food Policy Center at Duke University, and professor of public policy at Duke. Welcome to The Leading Voices in Food. Hunt Allcott is a principal researcher at Microsoft Research, an associate professor of economics at New York University, a research associate at the National Bureau of Economic Research, and a co-editor of the Journal of Public Economics. Hunt, I'm so pleased to have you join us, and would like to begin with a key question based on your work. What are the key ingredients for calculating the economically optimal soda tax? That's a great question, and thank you so much for having me on the program. I'm a big fan of the program and I'm a big fan of the work that you're doing. The key ingredients for calculating the economically optimal soda tax, I would say, are actually the key ingredients for thinking about the optimal policy in any number of domains. And so, there's a subfield called public economics that has a framework for thinking about this, and it is a utilitarian framework. It starts with the idea that everybody is maximizing their happiness, possibly making mistakes in doing so, but they're trying to maximize their happiness or utility, and there is a benevolent policy maker or a government that's trying to do well by its citizens, but it's trying to maximize this sum of happiness across all people [inaudible 00:01:56], and you might call that social welfare. So the way that we think about lots of economic policy problems, setting optimal taxes on sugary drinks, but also tobacco taxes or income taxes or disability insurance, is from this perspective of a benevolent policy maker that's trying to maximize social welfare. And so, what does that framework specifically then say, or what are the key ingredients for an optimal soda tax? I think that the two key ingredients are, really, a share of health costs that are caused by soda consumption, which are those costs that are born by other people, and then, what you might call internality, is are we making some mistake? Are we failing to maximize our own happiness by drinking too much soda, possibly because we don't know how bad the stuff is for us, or because we have self-control problems? Building on those two key ingredients, you, in general, want impose a corrective tax on tobacco or sugary drinks or carbon pollution or anything else, if and only if the consumption generates externalities or internalities. We, in our paper, calculate what we think is the economically optimal tax rate, and for a nationwide tax, our calculation is that somewhere between one and two cents per ounce actually it would be the optimal nationwide tax rate. And so, there's a sense in which the cities are actually pretty close to the optimal tax that we would set nationwide. And these calculations are speculative. There's a whole industry of economists and public health folks that are thinking about what would be the optimal tax on carbon pollution or the optimal tax on tobacco, and this is an early attempt at setting that optimal tax in the context of soda. And so we've done our best, and I think there's more work to be done there to try to figure out what the optimal amount is. You may have noticed that I was a little bit careful in distinguishing between the optimal nationwide tax versus the taxes that are being set in different cities, and that's actually a key distinction. Most of the taxes, in fact, all of the explicit soda taxes that we have are at the city or sometimes the county level in the US, and when that's been done, there are some papers that document what economists call leakage, in other words, that when you tax soda in Philadelphia, people, when they're shopping outside of town, start buying more soda outside of town. Similar things tend to be happening in Berkeley and I imagine they're happening in other cities that have soda taxes. This leakage reduces the effectiveness of the tax. You know, it's basically, you're taxing one good, which is soda in Philadelphia, and demand for another harmful good, soda outside of Philadelphia, rises. And that means that the optimal level of the taxes lower, and the effectiveness of a tax, at any given rate, is lower. And so, it does mean that the optimal city level tax, taking into account leakage, is certainly lower than the optimal nationwide tax that you would set. And what are the ways in which soda tax design can be improved? The most common structure of soda taxes in the US is that you would tax it one cent or two cents per ounce of sugary drinks. Now of course, sugar drinks vary in the amount of sugar that they contain. Soda pop has more sugar per ounce than some other types of SSBs. Sometimes ice tea, for example, have less sugar per ounce, but it really is that sugar that's causing the harm to our health. And so, we would advocate, and some countries have started to do this, that you would set a soda tax that scales with the amount of sugar in the drink, not the amount of liquid that comes with the sugar. And in fact, our team is working on a paper to sort of quantify that point, and other people have made that point as well. So that's one example, and there are others. And given this work, do you think soda taxes are effective public health policy? Yes, absolutely. Absolutely. So one is that it would be cause consumers to, even if they like some kind of sugary drink, it causes consumers to substitute the less sugary drink, so that's good for their health. And the other benefit is, as you stay on the production side, it will induce faster changes in the product. But it certainly seems like the benefits of a tax are not just that price goes up so demand goes down, but indeed what people infer, what information people start to sense that they're getting from their government about the types of things that are being packed. And as an example of this, you may know that there's a paper by Alex Rees-Jones and Kyle Rozema that is circulating recently, that looks at cigarette taxes, and they look at cities that debated, but in the end, did not pass cigarette taxes. So there was no impact in these places on cigarette prices, yet there was a lot of coverage about the health harms from cigarettes, and there seems to be some evidence that people's cigarette consumption in these areas actually went down in response to these debates. And so I think that's clear evidence that the benefits of taxation are not just through the price channel, but they're also through the sort of public debate and information channel. You also have another line of work on food deserts, and have published impressive work on that topic. How much effort do you believe we should put into combating food deserts by subsidizing grocery stores in underserved neighborhoods? It's a great question, and yes it is a something that we've been interested in. We've just had a paper accepted at the Quarterly Journal of Economics that touches on this issue. And we got interested in this because we've been reading about food deserts and reading about nutritional inequality, just the idea that low income folks aren't able to eat as healthfully as high income folk, and then also the fact that low income neighborhoods often have more convenience stores and fewer healthy grocery stores with fresh produce. This sort of correlation, this connection between lack of healthy supply and lack of demand for healthy groceries might've been sort of a causal thing, where the food desert is causing the lack of demand for healthy groceries. And so, we were interested to try to find out if that was actually true. When new grocery stores open up, does that actually impact, does that actually cause people nearby to buy healthier groceries? And so, we looked at this, we gathered Nielsen Homescan data, and we combined that with data on all the grocery stores that have entered in the United States over the last 15 years or so, and it turns out that when grocery stores enter in the food deserts, they don't cause people to eat healthier food. So what does happen is that people shift their shopping towards the new supermarket, but they don't buy healthier foods. And this is, I think, an important fact, and when we think about why we'd want to subsidize grocery stores in underserved areas, I think that this is an important fact to take into account. It doesn't seem to have much of an impact on healthy eating.

Identify the socio-economic correlates of sugar sweetened beverage (SSB) consumption among pregnant women and analyze to what extent SSB consumption is associated with diet quality and total energy intake. Additionally, we aim to predict how diet quality scores and totally energy intakes would change if SSB consumption was artificially set to 0. Design: Repeated Cross Sectional Study. Setting: United States. Subjects: SSB consumption was estimated from 1–2 24-hour dietary recalls from 1,154 pregnant women who participated in the 1999–2006 National Health and Nutrition Examination Survey. Methods: Linear regression models were used to identify socioeconomic and demographic factors associated with SSB consumption and to assess the associations between SSB consumption and diet quality and total energy intake. Diet quality was measured with the Alternate Healthy Eating Index modified for Pregnancy (AHEI-P). Results: The mean SSB intake was 1.3 servings per day (sd 1.5). Having a household income ≤100% of the Federal Poverty Level, being born in the United States, and not being married or living with a partner were positively associated with SSB consumption. Every 12 oz. of SSBs consumed was associated with a 2.3 lower AHEI-P score (95% CI: 1.6, 2.9) and the consumption of 124 more calories (95% CI: 85, 163), after adjusting for age, country of birth, race/ethnicity, educational attainment, marital status, household income, survey year and day/s of the week the recall/s were collected. Our predictive models indicated that average AHEI-P would be 6.4 (5.4, 7.6) higher and average total energy intakes would be 203.5 calories (122.2, 284.8) lower if SSB intake was set to 0.
