Welcome to this special edition of Diabetes Dialogue: Technology, Therapeutics, and Real-World Perspectives, hosted by Diana Isaacs, PharmD, an endocrine clinical pharmacist, director of Education and Training in Diabetes Technology, and codirector of Endocrine Disorders in Pregnancy at the Cleveland Clinic, and Natalie Bellini, DNP, program director of Diabetes Technology at University Hospitals Diabetes and Metabolic Care Center. Included as part of the HCPLive This Year in Medicine series, this episode recaps headlines and practice updates related to continuous glucose monitoring (CGM) from the past year, with a focus on regulatory news from Medtronic, Dexcom, Abbott, and Senseonics. In recognition of the 25th anniversary of the first US Food and Drug Administration (FDA) approval of a CGM device in 1999, hosts also provide historic context for each company spotlighted during the episode. Medtronic To begin the episode, hosts start where the journey began: the FDA approval of the MiniMed in June 1999. In the 25 years since, MiniMed was acquired by Medtronic but has continued to remain among the leaders in CGM technology. During their conversation, hosts spend time discussing early experiences with CGM as well as newer advances from Medtronic, including the 2024 approval of the Simplera CGM. Now available for iPhone users (using Dexcom G6), the update allows features like preset meal settings, a valuable tool for patients who use flexible carb counting approaches. XX Insulet Corporation announces the full market release of the Omnipod 5 App for iPhone® in the U.S. App allows users to fully control their Omnipod 5 Automated Insulin Delivery System (Omnipod 5) from their compatible iPhone1. For example, users can bolus for a meal, change a Pod, and adjust settings. Importantly, this also means that Omnipod 5 users who have a compatible iPhone no longer need to carry a separate Controller to help manage their insulin delivery requirements. Insulet says that the App for iPhone has been the number one feature requested by Omnipod 5 users. The Omnipod 5 App for iPhone is currently compatible with the Dexcom G6 Continuous Glucose Monitoring (CGM) System and is expected to be compatible with the Dexcom G7 CGM in the U.S. in 2025. Abbott said it makes its systems the first and only patient-applied CGM sensors approved for these screenings. Imaging procedures often come as part of diabetes care, Abbott said, especially as diabetes can cause a number of medical complications. The company rigorously tested its Libre 2 and 3 sensors to ensure they remain effective after radiologic procedures. Tidepool+ provides intuitive tracking and visualization of diabetes data to make the data informative and actionable for clinicians, people with diabetes and caregivers. https://www.drugdeliverybusiness.com/tidepool-abbott-data-integration-freestyle-libre/ XX Beta Bionics announced today that it launched the integration of the iLet bionic pancreas with the Abbott (NYSE:ABT) FreeStyle Libre 3 Plus. The companies announced their plan to combine the FreeStyle Libre 3 Plus continuous glucose monitor (CGM) with iLet last month. According to Beta Bionics, this makes iLet the first available automated insulin delivery (AID) system to integrate with Libre 3 Plus in the U.S. iLet users can now update their app and software to gain a choice of integrated CGMs. They can choose between Libre 3 Plus and Dexcom CGMs.   Abbott, meanwhile, continues to push forward on the automated insulin delivery integration front. The company recently paired its FreeStyle Libre 2 Plus with Insulet's Omnipod 5 in Europe. It also announced compatibility with the Tandem Diabetes Care t:slim X2 system in January.   Notably, the company also struck a deal with Medtronic in August. This is designed to address nighttime glucose control for insulin pen users. It's meant for use only during sleep - Luna system was developed as a wearable insulin pump and alternative to insulin pen to automate the insulin delivery process. The device works with continuous glucose monitors (CGM) to calculate and deliver rapid-acting insulin doses during sleep. Both glucagon-like peptide-1 receptor agonists (GLP-1RA) and sodium-glucose cotransporter 2 (SGLT2) inhibitors have shown significant benefits for cardiovascular and renal health in other populations, particularly in patients with type 2 diabetes.   SGLT2 inhibitors carry a significant risk of euglycemic diabetic ketoacidosis, a dangerous condition most common in T1D individuals where toxic levels of blood acidification can occur. Due to this risk, SGLT2 inhibitors were removed for T1D use in Europe, and the U.S. Food and Drug Administration (FDA) has not approved them for T1D.   For GLP-1RAs, there are concerns about substantial weight loss potentially leading to ketoacidosis or worsening hypoglycemia. While these medications can be beneficial in managing weight, the extent of that weight loss can create new problems in people with T1D as their insulin needs may quickly change as a result. Prescribing in People With Type 1 Diabetes," published in JAMA, the authors stress the critical need for more research to confidently allow the off-label usage of potentially dangerous secondary treatments. The data suggests clinicians are turning to these treatments to manage cardiovascular and renal complications in T1D patients, even though the drugs are not explicitly approved for this condition.   Conversely, those newly prescribed GLP-1RAs had higher rates of obesity (69.4% vs. 45.7%). This data indicates that clinicians are prescribing GLP-1RAs to help manage obesity in T1D patients, as weight management is a critical component of diabetes care.   The percentage of the T1D population prescribed GLP-1RAs increased significantly, from 0.3% in 2010 to 6.6% by 2023. The percentage prescribed SGLT2 inhibitors rose from 0.1% in 2013 to 2.4% by 2023. Overall, the percentage of T1D patients prescribed either of these medications increased from 0.7% to 8.3% during this period.     Results of the study suggest that despite regulatory concerns, off-label use of GLP-1RAs and SGLT2 inhibitors in T1D continues to grow primarily due to their cardiorenal and weight management benefits.   "Prospective studies on the efficacy and safety of GLP-1RAs or SGLT2 inhibitors in the T1D population are needed," the Research Letter concludes, "Before such evidence becomes available, caution should be exercised when prescribing these treatments to individuals with T1D."   https://medicalxpress.com/news/2024-10-glucose-lowering-drugs-diabetes-patients.html XX More good islet transplant news.. from Eledon Pharmaceuticals and University of Chicago. These are potentially the first human cases of insulin independence achieved using a transplant of insulin-producing islet cells in combination with its experimental immunosuppressant drug. The first two subjects achieved insulin independence and normal hemoglobin A1C (HbA1c) levels, a measure of average blood glucose, post-transplant. The third subject, who recently received an islet transplant, decreased insulin use by more than 60% three days following the procedure and continues on an insulin independence trajectory. This effect was observed consistently across various subgroups, including differences in obesity status, gender, and age. Semaglutide, which acts on glucagon-like peptide receptors (GLP-1R) to curb hunger and regulate blood sugar in T2D, is also the active component in the diabetes and weight-loss drugs Wegovy and Ozempic. But this sounds like just software.. no watch or hardware. According to the report, Apple doesn't currently have plans to release the app, but may integrate the technology into its future health products. The app could reportedly show consumers how certain foods impact their blood sugar levels, based on measurements taken by existing blood sugar monitoring devices. One report says apple is exploring uses for blood sugar data and what tools they could create for consumers as a result. Nature is an incredible source of inspiration for scientists and R&D experts, leading to the development of biomimicry: a new branch of science that seeks to imitate living things. Through biomimicry, researchers are able to develop new technologies by studying and imitating the designs created in nature by Allah. This approach has been applied in the world of technology, particularly in nanotechnology, robot technology, artificial intelligence, medicine, and the military. By drawing from these natural systems as models, humans are able to create more sustainable technologies than those in use today. Biomimicry has surprised scientists and revealed incomparable structures and systems that have the potential to enrich technology in a wide range of fields. Nature is a source of inspiration for scientists, and biomimicry is a way of using the extraordinary designs in living things in the service of humanity. Scientists are making important gains with regard to time and labor, and using less material resources by imitating nature. There are many scientific papers that discuss these topics, such as "Science is Imitating Nature," "Life's Lessons in Design," "Biomimicry: Secrets Hiding in Plain Sight," and more. In the 19th century, nature was only imitated aesthetically, but in the 20th century, scientists began studying natural mechanisms at the molecular level and learned from living things, as revealed in the Qur'an 1,400 years ago. On today's episode, we are going to talk about designing open source medical software.  Maya Friedman is a digital product design lead and art director who designs solutions for unmet needs in the health tech and femtech spaces. She currently leads the user experience, visual and sonic design for Tidepool's FDA-cleared, automated insulin dosing mobile application, Tidepool Loop. She is also the founder of The Period Project, which supports the creation of an ecosystem of software solutions, research initiatives, and education to address the unmet needs of women+ with diabetes, through the different phases of a woman's life cycle including menstruation and pregnancy. Prior to joining Tidepool, Maya was a creative lead for Allbodies Health, designing education to improve sex, mind and body literacy. She also worked as a creative at Made Music Studios, driving the design and strategy for digital-audio experiences, most notably leading a partnership with the Cooper Hewitt Design Museum to develop an audio-first design exhibition around alarm fatigue in hospital settings. She thrives leading a team of product designers, illustrators, user researchers and sound designers to design solutions for topics such as health and body literacy, menstrual health and alarm fatigue. Maya has her MFA in Media Design practices with a focus on femtech and sextech design from ArtCenter College of Design. Kelly Watson is VP, Product & User Experience for Tidepool, a digital health nonprofit building solutions for people with diabetes. Kelly has worked to ensure the patient, the clinician and care partners are at the center of the product design process, leading Tidepool to achieve rapid market adoption growing its global ecosystem to over 450,000 people with diabetes and 12,000 clinicians. At Tidepool, she has led partnerships with many of the world's largest diabetes medical device companies and technology companies. Previously Kelly worked in bench research at the Center for Biologics Evaluation and Research, FDA (CBER-OVRR DVP) and the National Institute of Mental Health prior to building out a product design consultancy. She is an angel investor and advisor for emerging digital health startups. Kelly advocates for patient ownership of health data, interoperability and open healthcare standards. This is Dr. Travelbest's Guide In this episode: FAQ is: Who sets the standards for airline seats? Today's Destination is: Point Loma, San Diego Today's Lesson Learned- I forgot the tent poles. Travel Advice:  Join the US STEP program if you plan to travel out of the country in the next year.   FAQ: Rebecca asks, Do the airline seats seem to be getting smaller these days? Am I the only one who sees or feels this happening? I would like to know. Also, please give me information on how much space should be on a plane for my seat. I'm struggling with this issue now. Who sets the standards for airline seats?   Answer: Dear Rebecca, You are being heard. I'm so glad you asked. I've wondered the same thing. In the USA, The Federal Aviation Administration is looking at a rule for minimum amounts for the width of airplane seats and the distance between the front of one seat and the back of the seat ahead.  They are gathering public comments on the issue now. Their concern is safety. If seats are too small, it can hurt evacuations in an emergency. How is your legroom? We want to hear from you now, so contact the FAA to give them your concerns. Today's destination: Point Loma, San Diego   Point Loma is a land mass that sticks out in the Pacific Ocean, a large peninsula; on the water, you can't go from Downtown San Diego without bumping into this point. So it was one of the first places to land for settlers in the land.    Why visit Point Loma? The National Park, Cabrillo National Monument, and the lighthouse are the most famous places to visit as a traveler. This is one best viewpoints for the entire city and region. You will want to spend a half day exploring nature and the visitor center. You can see inside the lighthouse and climb the stairs to see what it was like to live there hundreds of years ago. I highly recommend this destination.   You can see Mexico, downtown, and a lot of ocean from here. Tonight, I met a couple from Lexington, Kentucky, and they visited the lighthouse today. They raved about the experience of being in the home that was a lighthouse and being able to see afar.   The tidepools are also worth seeing. If you have never been to a tidepool, it's when the ocean tides recede, and you can see the small sea creatures like starfish and crabs among the nooks and crannies of the rocks on the beach, walking carefully not to disturb wildlife.    My favorite place to eat fresh seafood locally is Point Loma Seafoods. My favorite Coffee shop is Peet's. My favorite YMCA is Ryan Family Y. My Favorite thrift store is Goodwill San Diego. Point Loma has Sunset Cliffs Blvd, which has great views and sunsets. You can swim here, too, if you can navigate the hike safely. However, it's not meant for the faint of heart, as swimming here is not as safe. It's better to head north to the public beaches. You'll also see beaches just for dogs, and dozens of them running around the sandy beach.   You'll be in Ocean Beach, which is a community next door, almost connected.    My first visit was in 1982, as I was relocating from Chicago. Little did I know how this part of the region would impact my life when I prayed in my car that day. I said out loud, “Lord, if it is meant to be, find my subsequent work and life in this city. So I prayed in my car, parked on Cornish Drive, a block from where I lived a few months later. Yes, the same street, about a block from where this prayer happened. And it turned out great for the past several, well, 40 years, it's been. Today's Mistake- Forgot the tent poles   I was camping in Nevada along a beautiful river. I brought my tent but forgot to bring the tent poles. So, I slept under the stars. It was decent weather that night, so pretty good. Don't forget your tent poles as I did.    Today's Travel Advice-Join the US STEP program if you plan to travel out of the country in the next year.   I joined the US STEP program in 2023, and they will inform me when I travel abroad and see a problem in a country I visit. Also, they will have my information if I need to contact them, making it easier to find and help me. They help people with problems. I hope I don't need to contact them, but better safe than sorry.

For Adam Summers, natural history and the practice of biomechanics starts with those moments where you witness an animal or plant doing something that you can't quite understand.  How did that eagle fly upside down? How did that fly land on the ceiling? How did that suckerfish stick to that rock?  I find Adam to be one of the most captivating communicators of science and enthusiastic curiosity I've ever encountered, I think you will too. He now runs the comparative biomechanics and biomaterials lab at the University of Washington's Friday Harbor Labs. Current research topics include the evolution and mechanical properties of cartilage and tendon, swimming mechanics of sharks, respiratory patterns of sharks and rays and solid-solid interactions in aquatic organisms. This work has led to more than 90 publications and two patents. He has consulted on films (including one of my personal favorite movies, Finding Nemo), television and for 8 years his monthly column in Natural History Magazine – ‘Biomechanics', brought comparative biomechanics to a wider audience.

When Tidepool Loop approval was announced last month, it came without the original pump partners, Omnipod and Medtronic. CEO Howard Look said they do have a pump partner but that they're not ready to announced yet. I spoke to Tandem for next week's episode and they say its' not them. Quote “That's not part of our roadmap, it's not an integration we're working on.” XX Insulet announces it will discontinue the original Omnipod system. In a letter to healthcare professionals, the company says the system, referred to as Eros or Omnipod Classic will be discontinued at the end of this year. It's no longer available to new patients and existing customers will be transitioned to Dash or Omnipod 5. I'll link up the letter in the show notes. “Dear healthcare professionals, We have made the decision to discontinue the original omnipod insulin management system in the us after December 31 2023. This system is often referred to omnipod eros or omnipod classic. Insulet will continue to support the omnipod system throughout this year to allow existing patients ample time to transition to newer omnipod technology such as Omnipod dash or Omnipod 5. please note this product is no longer available for new patients in the US. After December 31, 2023 Insulet will not be able to guarantee the availability of the omnipod system supplies. “ https://www.facebook.com/photo/?fbid=10225188520677483&set=a.1253748061199 XX Civica continues to move ahead with more affordable insulin, announcing that Ypsomed will make and supply their pens. Civica plans to produce three insulin biosimilars priced at no more than $30 per vial and no more than $55 for a box of five pen cartridges. Contingent on FDA approval, Civica anticipates that its insulins will be available for purchase beginning as soon as 2024. XX New study shows screening kids at risk for type 1 diabetes only once – at age ten – is very effective in detecting the condition by the age of 18. These researchers also say “Almost no one” who is islet autoantibody negative at age 10 years despite a high familial risk for type 1 diabetes will develop clinical diabetes by the age of 18. The Washington, D.C.-based group has filed a complaint with federal bodies alleging that the CBS "60 Minutes" segment that aired on New Year's Day breached the FDA's “fair balance” rules for drug ads. The Committee said in a release that the feature failed to talk about alternatives to the drug or about other weight-loss methods; that only experts “paid by Novo” were used in the program; and that the piece used overly promotional language. This has caused Wegovy supply issues for Novo which, coupled with manufacturing problems, only deepened bumps in production last year for the drug and led to an official shortage of the drug in 2022. A spokesperson for Novo Nordisk told Fierce Pharma Marketing: “Novo Nordisk did not provide any payment or sponsorship to CBS 60 Minutes for their reporting on obesity as part of a news segment that aired on January 1, 2023, and we did not control any of the content or have any role in identifying or selecting the doctors and patients featured in the news segment.” https://www.fiercepharma.com/marketing/health-group-lambasts-novo-nordisk-60-minutes-paid-news-program-weight-loss-med-wegovy XX Some researchers have pulled out of a trial investigating the use of an old tuberculosis vaccine to treat children with Type 1 diabetes only months after they began enrolling participants. This is part of the Faustman study of the BCG vaccine. Some recent research suggests that the vaccine, first administered in 1921, also may protect against Covid-19 and respiratory diseases because of its broad effects on the immune system. The lead investigators of the pediatric trial are proceeding with the study, but N.Y.U. Langone's abrupt withdrawal could potentially jeopardize its viability if they are unable to collect data on the children at the N.Y.U. site. An earlier study by the scientists at Massachusetts General found that two doses of B.C.G. vaccine reduced blood sugar levels to near normal in a very small group of adults with Type 1 diabetes. Other studies of B.C.G., using different versions of the vaccine and following Type 1 diabetes subjects for varying periods of time, have yielded mixed results. The lead investigator of the trial, Dr. Denise Faustman, director of the immunobiology laboratory at Massachusetts General Hospital, said that N.Y.U.'s withdrawal was a serious deviation from trial protocol that may call into question the final results. https://www.nytimes.com/2023/01/20/health/diabetes-bcg-nyu.html XX A new smart insulin pen sensor can also be used to track dosing of GLP-1 medications. Brand names for GLP-1 medications include Trulicity and Ozempic. This device, called Mallya, attaches to the injector and connects it to a mobile app. It's already cleared in Europe and should be available in the US in the next few months. The device is compatible with most popular insulin pens, including Lilly's Kwikpen, Sanofi's Solostar, and the Novo Nordisk FlexPen. Biocorp also announced that it has partnered with Novo Nordisk, Sanofi, and Roche Diabetes Care to improve Mallya's current design moving forward. According to the Mallya website, the device's lifespan is two years before a replacement is needed, and in no way does it change the way that users operate their insulin pen. The device can be charged via USB and, with average use of three to four injections per day, requires charging once a month. Also, for those who have different pens for multiple types of insulin, the app can connect multiple Mallya devices, one for basal insulin and another for bolus, for instance. https://diatribe.org/fda-clears-smart-injector-pen-device-mallya XX Tempramed/ VIVI Cap XX New features popped up this week in the latest version of The Sims – after an update, players can opt for their characters to wear a CGM – Libre or Dexcom Freestyle Libre in the Sims 4 game. Search for CGM or Libre XX The Jonas Brothers officially have a star on the Hollywood Walk of Fame. On Monday, Nick, Joe and Kevin Jonas were honored during a ceremony on Hollywood Boulevard where their careers as musicians were celebrated. Their star was dedicated in the category of Recording and the trio was introduced by Monte and Avery Lipman of Republic Records, Ryan Tedder and writer and producer Jon Bellion. While the brothers still maintain their own separate projects -- Nick Jonas is set to produce and star in the upcoming film "Foreign Relations," Joe Jonas recently starred in the film "Devotion," and Kevin Jonas hosts the ABC series "Claim to Fame" -- they continue to make music together and are slated to release new songs this year. The group closed out Monday's ceremony by announcing big news about their upcoming new album, which will be released in May. They also said they'll be going on tour again. A Pacific Northwest camper, hiker, fisher, and ocean swimmer. That's the growing-up bio of Aiyesha Christian. And being the super outdoors person, Aiyesha also was obviously a big consumer of trail mix to keep her nutrition gauge from going empty. But the trail mix she found was just too dull; either too much salt or too much sugar, always the same ingredients and definitely not enough flavor. Aiyesha loved her outdoor life and wanted all the diversity, flavors, freshness and spirit to be reflected in the food she put into her body, especially when she was on the outdoor trails – being a nomad of course. She had the food background to begin her venture, having worked in restaurants and having tended bars. She began the grand experiments in her kitchen and her friends and family ooed and ahhed over her creations. Some of them also steered her to the Oregon State University's Food Innovation Center in Portland, Oregon and straight to the Director, our show host Sarah Masoni. Sarah was duly impressed with the great taste of the trail snacks and immediately encouraged Aiyesha to continue with the recipes and learn how to commercialize her products so that more people could share in her discovery and the Nomad company began to take shape. One of her first big wins was being part of the Food Innovation Center's holiday markets, where she became introduced to Market of Choice, one of our show sponsors, and got signed on to be displayed on their shelves. Each flavor of Nomad mix has its own special story, Tide Pool to represent the coast, Forest for the woods and so forth. But Aiyesha has a guideline for each product and flavor. Each bag contains at least one type of seed, nut, fruit, and vegetable. It is gluten free, paleo, and almost vegan except for locally sourced honey as the only added sweetener. All the snacks are great to share with your pet, but Aiyesha is contemplating a pet line of snacks as well. So why this foyer into the saturated market of trail snacks, a market that is old, dominated by big food companies that bought up all the small ones years ago and have all the shelf space already taken? In one of her Masters of Business classes, Aiyesha was introduced to the book, Blue Ocean Strategies, that talked about finding new opportunities in old, stodgy markets. This gave her the insight to find her new opportunity and she is making the most of it. Her packaging is exquisite and truly reflects the mission of her company, and the website is impressive. Rather, their sights are set on a merchant named Sergio and the treasure he's reportedly trying to trade. ------------------------------------------------ To my dearest Captain Hano, My thorned rose, if the whispers that dance to me on the waves of the sea speak truth, then you and your crew have indeed survived. If that is the case then I send up every song in my heart to the Goddess that this letter reaches you safely. My mind reels imagining what feats of swashbuckling daring-do you managed to pull off allowing your escape from those blasted Bluecoats. What I would give to one-day bear witness to such skill and panache. Though I fear with my constitution in the state that it is, the mere sight of blood would see me laid out upon the deck like the many fools who meet the business end of your trident. Forgive me, for I seem to have found myself rambling. I know how your days and nights are filled with endless adventures. So, I will neither waste words nor ink and find myself to the point swiftly. There is a deal taking place in Paraiso Encontrado on the 13th of Blooming Tides. A merchant by the name of Sergio Diez De Laton has been making waves around the Shark's Tidepools recently. He's Petite Espenoran nobility, known for his ever-growing collection of "Pirate History". He apparently considers himself an academic of a sort. Recently he's been searching for a buyer for one of many objects in his prized personal collection, an unusual circumstance.  Not sure what the item is specifically, but word has it that it once belonged to one of the old Pirate Lords. For context, Vertex is the company that got all the headlines last fall – big article in the New York Times for a stem cell therapy that brought one person with type 1 off insulin completely. The FDA put them on hold but lifted that just last week. Vertex previously acquired Dr. Doug Melton's Semma. Viactye is the company featured in the documentary "The Human Trial" - they're using CRISPR technology to keep the body from rejecting their stem cells. Both companies technology does NOT require immunosuppressive drugs and both say they are working toward a functional cure. Some experts are saying this will speed up that effort while critics worry that it eliminates the competitive aspect of the race. Sernova is still one separate company that has shown stem cell therapy can work.. These researchers looked at screening for the type 1 autoantibodies at ages 2 and 6. This included nearly 25-thousand children from Europe and US. One issue in this study, funded by JDRF, all the participants in the combined dataset had genetic risk factors for the disease or a relative with type 1 diabetes, in whom performance is expected to be higher. It also included almost exclusively kids of northern European ancestry. https://www.medscape.com/viewarticle/976992 XX Tandem bolus by phone is slowly rolling out. This week, the T:Connect app was updated in the The t:connect app was updated and emails started arriving for those notified they were part of the early release groups. I did see some reports of people not yet getting the email but seeing the update in their portal so be sure and check. Mobile Bolus is reportedly available for those using Control IQ and Basal IQ. It's worth noting that for those outside the US there really is no timeline, but Tandem has previously said they're going to replace t:connect with a new system that will debut outside the US first. By the way, I got my email this morning, but Benny is still at camp for three more weeks so it'll be a bit before we do the update! XX Interesting look at teens and young adults with type 1.. the early morning psychosocial state may explain a proportion of their daily variance in time in range. The study included 88 participants aged between 14 and 26 years, 56-percent of whom were women. This is tough to quantify but they had the participants complete an engagement prediction survey and set a diabetes management goal for the day when they woke up, and also completed an evening survey. These questions were about mood, motivation, control beliefs, social support, stress, general health, self-esteem, and perceived need for assistance. One Drop plans to enter what they're calling the continuous health sensing market with a daily disposable, minimally invasive biosensor in development. They say this is designed to provide greater flexibility and insights for people living with diabetes compared to blood glucose monitor (BGM) fingersticks alone. The availability of our continuous health sensor is subject to receipt of clearance or approval from the U.S. Food and Drug Administration. XX The Food and Drug Administration received more than a thousand comments—mainly from diabetes patients and their family members—in response to draft cybersecurity guidance. These are for staff to use when processing submissions from medical-device manufacturers. “Please do not let medical device manufacturers use cybersecurity as a pretense to prevent me from accessing my OWN devices,” reads one entry from a sample of the comments FDA posted to the docket on the guidance. Howard Look, CEO of Tidepool is quoted in some of the article around this. Tidepool provides software that allows patients to see their data and better manage the disease. And supporters are working to make Tidepool Loop the first FDA approved app for more convenient automated insulin delivery. “Following best practices for cybersecurity does not need to imply blocking patient users from accessing their own data or controlling their own devices,” the comments read. “Tidepool asserts there is a risk that the FDA guidance will be interpreted or misinterpreted to suggest restriction of access by the patient user is appropriate or encouraged. In this episode, Susan talks about the molecule of life, the matrix of the world, the cosmic juice - water.  As Barbara Kingsolver writes, “It is the gold standard of biological currency.” She says, “Water is life, it's the briny broth of our origins, the pounding circulatory system of the world, a precarious molecular edge on which we survive. It makes up two-thirds of our bodies, just like the map of the world; our vital fluids are saline, like the ocean. The apple doesn't fall far from the tree.” This episode is part tribute, part meditation on the journey of water from the stars to the tide pool. John Steinbeck wrote in The Log from the Sea of Cortez about how all things are one thing and that one thing is all things — plankton, a shimmering phosphorescence on the sea and the spinning planets and an expanding universe, all bound together by the elastic string of time. He advises us to look from the tide pool to the stars and then back to the tide pool again. For the remainder of this podcast season, we will cover various dimensions of water — from history to folklore, to the impact of big dams, to the challenges of providing drinking water and sanitation, and more. This episode is also an introduction to the remainder of the season and how we can speak of our current crises in view of the space of water as an urgent territory of engagement. Today I'd like to introduce you to Ben West. Ben was diagnosed with Diabetes Type 1, at 21 years of age, and if you or any of your friends or loved ones have a similar diagnosis, you are definitely going to learn so much from Ben.Ben is currently a Staff Product Engineer at Dexcom, a global leader in diabetes care transformation and management, where they design, and manufacture continuous glucose monitoring devices (CGM), and you'll hear a lot more about that technology in today's talk. He spends his time today at Dexcom helping to prioritize where investments in innovation are spent, and which ideas to accelerate into the concept funnel that will yield the most impact for the organization and its customers.Ben spent the last 5 years, as part of his personal mission to reduce the burden of living with Diabetes Type 1, reverse engineering the open loop system of the insulin pump and has done a tremendous amount of work there, and is also a co-founder of the NightScout Project, an open source cloud based system that provides the availability of CGM information to web browsers, smartphones, and other devices, to help people with the diseases, manage the condition in real time and even predictively based on their bio metrics, activity, and glucose levels. Ben has also spent some time doing engineering work with Tidepool.org, which is a software that is helping to take the DIY capability of addressing both the data on the insulin pump and the data on the CGM, merging this data into a closed loop system, and providing prescriptive suggestions on ongoing therapy. Currently the software is up for approval with the FDA, and is looking to be democratized and provided within app stores for easy consumption. Ben also spent some time as a Cloud Networking Engineer at Cisco, and you'll see some of his experience in these capacities play out in the talk.
