This connection is believed to be due to the role of bile in the conversion of thyroid hormones. Specifically, bile helps trigger the release of an enzyme that converts the inactive thyroid hormone T4 into its active form, T3. This process is crucial for maintaining proper thyroid function. Everyone is aware of Type 1 DM (Insulin dependent diabetes, seen commonly in children) & Type 2 DM (usually seen in adults & majority are controlled on oral drugs & Lifestyle Modification ). Gestational Diabetes Mellitus (GDM) is a kind of diabetes, which appears in the 2nd or 3rd trimester of pregnancy and disappears after delivery. GDM occurs when the woman's beta cells are not able to overcome the antagonism created by the anti-insulin placental hormones of pregnancy. Which causes increase in the blood glucose during pregnancy. Women with a history of GDM are at increased risk of future diabetes; predominately type 2 diabetes, as are their children. Almost every 5th or 6th pregnancy women in India, may have GDM. Diabetes In Pregnancy Study Group of India (DIPSI) recommends fasting or non-fasting Oral Glucose Tolerance Challenge Test with 75g of glucose with a cut-off of ≥ 140 mg/dl after 2-hours, Every pregnant woman should be screened for glucose intolerance in the first trimester itself. If found negative, the screening test is to be performed again at 24th – 28 th week and finally around 32 nd – 34 th week. Once diagnosis is made, Medical Nutritional Therapy (MNT) is advised initially for two weeks. If MNT fails to achieve control i.e., FPG ≥ 90mg/dl and/or 2 hr PPG ≥120mg/dl, oral drugs or insulin may be initiated. Recorded on 28th January 2022.

Secondary outcomes included endpoint differences in body weight, fasting plasma glucose (FPG), systolic and diastolic blood pressure, body mass index (BMI), and heart rate between the treatment and control groups when absolute changes from baseline values were measured. The preliminary findings indicated that the orally administered small molecule GLP-1RAs danuglipron twice daily from Pfizer and orforglipron once a day from Lilly were effective in weight reduction and glycemic control in individuals with type 1 diabetes, obesity, or both. As compared to the controls, the novel small molecule GLP-1RAs not only resulted in a significant lowering of HbA1c levels in patients with type 2 diabetes mellitus but in patients with obesity and type 2 diabetes, danuglipron and orforglipron also brought about significant weight reduction. While the safety profiles indicated that the orally administered danuglipron and orforglipron did not increase the odds of serious adverse reactions or hypoglycemic events, the odds of adverse gastrointestinal events such as diarrhea, nausea, constipation, and vomiting were higher. These adverse gastrointestinal events were also linked to higher odds of treatment discontinuation. On average, patients administered with TZIELD required numerically fewer insulin units and had numerically higher time in range, compared to those on placebo. HbA1c reductions and the overall rates of clinically important hypoglycemic events were similar among both study groups. TZIELD is the first and only disease modifying therapy in type 1 diabetes An observational extension study following participants for a further 42 months is ongoing. Abbott says they're doing just fine.. and that the treatments could end up boosting sales of the medical device maker. Shares have dropped with concerns that GLP-1 drugs like Ozempic and Mounjaro could hurt CGM sales. Abbott says their sales are actually up this year. Now, grain of salt, because this is press release info.. but it's worth noting that the device market is very important to people who use pump systems of course. This is something else.. biotech firm Klick Labs testing 267 people who had already been diagnosed as being either non-diabetic (192 people) or type 2 diabetic (75 people). Each person was asked to record a specific spoken phrase on their own smartphone via an app, up to six times a day for two weeks. Depending on the speed at which each individual spoke, those recordings were six to 10 seconds long. When 14 acoustic features of the resulting 18,465 recordings were analyzed, it was found that several of those features – such as pitch and intensity – differed in a consistent manner between the diabetic and non-diabetic participants. Although these differences couldn't be detected by the human ear, they could be picked up by signal processing software. This finding suggests that developing type 2 diabetes causes subtle changes in a person's voice. With that theory in mind, the scientists created an AI-based program that analyzes voice recordings along with patient information such as age, sex, height and weight. When tested on the volunteers, that program proved to be 89% accurate at identifying type 2 diabetic women and 86% accurate at spotting diabetic men. Those numbers should improve as the technology is refined. I'll link up the full results but the upshot here is that portion control, moderate carb, and vegan all worked very well – when you've got a dietitian helping you make individual choices. The Vegan diet in particular is interesting, but it was not low carb – some people actually ate more carbs than usual – up to 300 a day – but the fat levels were very low. This group's insulin sensitivity went was up, and they lost on average 11 pounds. This was a large study by researchers at Columbia University Irving Medical Center. Iheoma U. Iruka, Ph.D., is a Research Professor in the Department of Public Policy, a Fellow at the Frank Porter Graham Child Development Institute (FPG), and the Founding Director of the Equity Research Action Coalition at FPG at The University of North Carolina at Chapel Hill. Dr. Iruka is leading projects and initiatives focused on ensuring that minoritized children and children from low-income households, especially Black children, are thriving through the intersection of anti-bias, anti-racist, culturally grounded research, program, and policy. Some focus areas include family engagement and support, quality rating and improvement systems, and early care and education system and programs. Dr. Iruka serves on numerous national and local boards and committees, including the National Academies of Sciences, Engineering, and Medicine, the American Psychological Association's Board of Educational Affairs, Brady Education Foundation, and Trust for Learning. In addition to being on the National Advisory Committee for the U.S. Census Bureau, Dr. Iruka is a Census Advisor for the National Urban League. Dr. Iruka is the recipient of the 2022 American Psychological Association Mid-Career Award for Outstanding Contributions to Benefit Children, Youth, and Families. She has a B.A. in Psychology from Temple University, an M.A. in Psychology from Boston University, and an M.S. and Ph.D. in applied developmental psychology from the University of Miami, FL.

The sales process can be frustrating for any startup founder trying to find their mark. Even more seasoned companies struggle with closing more deals. So a story is anything that you kind of blame on outside circumstances or something outside of yourself. Because the power of duplication that is it? Is that what you do over and over again, you desensitize yourself with it? How To Become A Warrior Seller During The Coronavirus Outbreak   The world has been taken aback by the coronavirus pandemic. Businesses, regardless of industry, have been affected. As a salesperson, how do you become a warrior during the coronavirus outbreak? Jason Forrest works for a company called FPG which stands for Forrest Performance Group and one of their programs is Warrior Selling.    There are four different levels of a sales professional. The first level is the follower. They give all the permission to the customer.  The customer dictates what to do, how to act, and tells the salesperson when they're going to buy. The next level is the helper, who has the sole intention of helping people and serving their customers. Leader is the third level. They are the people you follow to a place you wouldn't go on your own. The last level is the warrior.   The warriors are advocates for the product and service they sell. This salesperson believes, supports, and advocates for their products and services. In addition to that, the warriors protect their customers' best interest.    In today's market, we need more warriors to protect the customers and the organization. People are afraid and the warriors help protect their customers from fear. Without leaders, fear can overcome customers and it can eventually kill the organization. Warriors know their customers so well they know what is needed. A warrior and  con artist may have the same skill set but what sets apart the sales warrior is intention. They protect their customers through their service or product.    Knowing the warrior seller  A warrior has to get their messaging right for their customer and should be able to answer these questions: Why should people buy from you today? What will your products and services immediately do for them? How will your products and services benefit them at this moment? How will your products and services eliminate their present pain points? Once they find the answer to those questions, they need to be able to share the message with the people they serve.    A warrior is defined by me as an advocate for their product and service they sell and at the same time, they're a protector of the customer's best interest. #SalesWarrior   Salespeople procrastinate for several reasons. The first is that they're not clear about what they're trying to accomplish.  The second, is that they don't know why they're doing what they do.Their why has to be strong enough to get through tough days. The why has to be greater than the sacrifices they have to make to do the job of a salesperson. Third, once the why is discovered, a salesperson has to figure out how. The how includes your cross pattern strategy. What are you going to say to give them certainty and how do you want them to feel once they get off the phone with you?  The last reason why people procrastinate is their “leash mentality,” the restriction in someone's thoughts that keeps them from doing what needs to be done.                                                                                                                                 Remove your leash A leash is like a dog collar in that it can prevent you from moving forward and can hold you back. The present mind is the best mind because it keeps you focused on the task at hand. In sports, trash talk occurs because it is a means for one player to distract another player.  If done well, it can throw that player off their game. However, as a salesperson, the trash talk doesn't come from other people but from inside their own head.    Jason came up with the concept of performance formula: P(erformance) = K(nowledge) - L(eash). Performance is what a person does and what we see them do.  Knowledge is what we've told them to do. They have a process for making a sale and they have brand knowledge. To get a performance you have to remove the leash from the knowledge. This is the resistance that keeps them from using the knowledge they have, the things that keep them from taking action.    Kinds of Leashes Jason addresses four types of leash mentalities in his book: Self-image An example of the self-image leash is the perception the salesperson has of herself that she doesn't know the right words to say to convince a prospect to buy. In order to remove this leash, you have to have the confidence that you will be able to convey to others why your product or service is valuable.   Story The story is anything external from us. It's when you tell yourself that people are still waiting for things to settle down before they start buying and purchasing again. This is just a story, especially when there's no evidence to support that claim.    Reluctance Reluctance is a situational fear and it may come from not wanting to sound pushy and insensitive. The coronavirus outbreak will impact 2020 projections - it will either lower the sales forecast or be seen as an opportunity to steal market share from the competition. The truth is, if you're not going to make any changes to get ahead of this, then it could lead to long-term challenges and impede the growth of your company. Taking action now can remove the ambiguous fear you may.  It puts you back in control of your circumstances rather than passively being a victim. As long as you have the right intentions and you believe you're offering help to your clients, people will know you're there to help. Rule  Jason defined the rule as anything you need to see, feel, or hear to give yourself permission to engage. However, most of the rules you set for yourself don't make sense and can serve as roadblocks.    Jason's program teaches the result matrix. Our results are achieved from what we've been taught by our parents, teachers, culture, and media. This is how he came to study the subconscious mind. 95% of our daily actions are driven by our subconscious minds. It’s meaningful to assess what the subconscious mind is telling someone that is in conflict with what they need to do to become more successful. They can then rewire those subconscious thoughts to place them in alignment with what they want out of life.Jason asks us to think of each human as a brand new computer in factory condition. We each come with basic programming. Research says that 70% of your programming happens in the first seven years of your life. Sometimes we need to clear the cache or delete corrupt files, so to speak. Be careful what you're uploading into your cloud because that's what you’re downloading into your reality.Jason describes the four main frequency levels: beta, alpha, theta, and delta. Beta is high energy. Right below that is alpha, which is awake or barely awake. Theta is a trance state. Delta is a deep sleep. For the first seven years of life, you were in theta. If you look at how to reprogram your hard drive, you have to get back into theta as much as possible. Most people on the road are in alpha. They’re in a light trance. When you're in that stage, the gatekeeper of your conscious mind is asleep. This is why it can be crucial to think carefully about the media you choose to listen to or the social media accounts you follow.Jason is obsessed with spartan races and biohacking. Health is the bedrock of everything. He learned he needed a certain stamina to pull off four day marathon seminars. He began hacking his biology to optimize his energy. He even hacked his wedding vows to create a better mental model for marriage. Jason invests in himself. Every three months he sees a wellness doctor. He gets a full blood scan and a DEXA scan. He has four personal training sessions per week and does Pilates. He estimates that he spends $1000 a month on optimizing his health. I have always wanted to lose weight, and become jacked! Although, every time I would try it seemed like the process was enslaving and I would give up. There are millions of hours of content on Youtube and other podcasts from people who have already made the change. These creators and experts forget how long and hard the process is and want to just confuse us with terms and techniques. I needed more than a science lesson, I needed someone to simplify the process so it would be easier to stay more consistent and change my lifestyle. Even the content that was more down to earth still did not seem too practical. So I chose to make every decision on how I approached my weight loss and fitness through the filter of my life. This started to make things seem much easier and less taxing on my mental wellbeing. Today on the podcast I want to walk you through the process I use to simplify three main areas of my journey; nutrition, my workouts, and progress tracking protocol.  Nutrition:  Starting with nutrition, I have tried many different diets in my years and none of them have stuck until I decided to choose one that fits three different criteria:  1. Is it time-consuming?  2. Do I like food?  3. Will it curve my binge eating and cravings?  After trying IIFYM, Counting Calories, and keto there was only one that fit the criteria for me and that is the ketogenic lifestyle. It allows me to eat while not tracking my calories, I love the foods I can eat, and I stay full with fewer cravings. It checks all the boxes and has allowed me to lose weight effortlessly. Thus helping me stay on track with fewer cheat days and temptations.  Workouts:  When it comes to workouts, this is where a person can get overwhelmed and very frustrated. Since I have simplified my workout process, I feel accomplished every workout. I also do not spend every waking moment in the gym. Some fitness professionals will make you feel this way if you let them. So let me take you through my process on how I choose my workouts. Hopefully, this will help you get in and out of the gym while still being efficient in your workout.  1. Pick your workout schedule  2. Pick the best type of exercise for you ( cardio, weightlifting, or both )  3. Then pick out the exercises you like and get started.  Doing the type of exercise, that fits your schedule, and picking the exercise you love is the key to becoming consistent.  Tracking Your Progress:  Tracing once a week, tracking your weight too much can have your mood tied to the scale. Today, we talk about fulling tapping into human performance to increase sales, performance, and profits. Mary shares a lesson she learned in a call center job that she hated until she learned to change her mindset. These techniques on changing her perspective still hold true today. Show Notes [03:35] Mary's first job was at the college call center asking for money. She learned so much. She cried in her first ten minutes. [05:37] This job made her miserable. She decided to talk to the top salesperson and find out why he was so happy. [05:39] Every day was wonderful for him talking to people on the phone, hearing their stories, taking something off their to-do list, and making them smile. [06:14] This changed her whole trajectory. The next semester everything was wonderful for her. She went to work excited every day. [06:38] She took this mindset into every sales job that she had after that. [06:52] At FPG she began helping the content team and now she is president of the company. It has been an incredible journey for her. [07:53] The power of reframing the mindset is amazing. [10:06] If you don't believe you can't create an environment for yourself or others to believe. [11:30] Beliefs are what hold us back from selling. It's our mindset and not deciding to yield or give up. [13:01] Persuasion is pushing forward when you know the product is right for that person or company, by doing your homework up front as opposed to manipulation. [14:49] Internalizing beliefs like people don't want to be sold to makes a salesperson think that there is something intrinsically wrong with what they are doing. [15:38] People love to be sold to. They don't want to be sold to buy anybody who's unethical. [16:29] To be successful sales release mindset issues that prevent you from doing things like selling on the phone. [17:39] Mary shares the difference between working with individuals as opposed to teams when doing sales coaching. [18:16] Skill training is great in a group framework. Some will act and some won't. This is where individual training comes into play. [19:19] Find the leash that is preventing the salesperson from following through. [19:58] One-on-one coaching can help change the limiting belief. [24:37] Before Mary starts her day she thinks about the type of energy she needs according to the task or people that she is going to be dealing with. She wants to always be in the right frame of mind. [25:54]  She also writes things down and practices preparing herself for her next task or sales call. [26:22] Be present with who you are with. By preparing you can be grounded. Role play or have other people listen to what you are doing. [27:30] The most important thing is to have a process. [27:47] Mary uses the seven steps of starting strong with each person she sales too. [29:12] The first step is to ask a triple binding question. Get people to make decisions as fast as possible. [31:27] Sales and leadership isn't personal life or professional life, it is just life. [32:07] Success for Mary is responding in the most loving and effective way. [32:57] Her biggest goal is creating leaders for her company. She wants to create a space and environment that allows people to become leaders. [35:19] Mary listens to as many books as possible on Audible. She also does a lot of learning on Growth Cafe. She is also on Wunderlist. [38:04] Mary likes using social media to stay involved and excited with old neighbors that she used to have. [38:41] People can't be successful today without connecting on social media. [39:48] When you finally meet someone in person who you've known on social media for a number of years it's like meeting an old friend. [40:17] Out of college, Mary went up to the president of a company that she wanted to work for who she saw at a party,  and his reply was we don't hire little girls as sales people. [41:29] This fueled her to prove him wrong. She also started noticing people who actually believed in her, and she realized she had to let go of the situation with this man. [42:26] Listen to people who actually believe in you. This is the fuel source that gives you the energy at the end of the day to do your job and go home and be a great mom. [43:30] Find the person who is the most successful in whatever you want to do and do what this person is doing. Host: Farhad Zangeneh, MD Guest: Paul Jellinger In diabetes management, clinicians typically measure glycemic control with fasting plasma glucose (FPG) and premeal glucose measurements, and HbA1c. And, the goals for each of these measurements have been established based on the results of many clinical trials. However, the majority of patients with diabetes fail to achieve these glycemic goals. Elevated postprandial glucose (PPG) concentrations may contribute to suboptimal glycemic control. Host Dr Farhad Zangeneh is joined by Dr. Paul Jellinger who will address the issue of postprandial glucose and its complications. Paul S. Jellinger, MD and Master of the American College of Endocrinology (MACE) serves as Professor of Medicine on the voluntary faculty at the University of Miami and served as Chief of the Section of Endocrinology at Memorial Regional Hospital in Hollywood, Florida. Dr. Jellinger served as president of the AACE, the ACE and the Florida Endocrine Society. Dr. Jellinger has been active on the AACE Board of Directors.

Following oxidative stress often two kinds of DNA-lesions can be found: 8-OxodG and the 2,6-Diamino-4-hydroxy-5-formamidopyrimidine (FaPydG). Both derive from the DNA-Nucleobase guanine. The FaPydG-lesion can exist both in the beta-configuration and the alpha configuration. To clarify biological questions concerning the basepairing and the encoding features of both forms it was necessary to synthesise of the alpha-anomer of the carbocyclic FaPydG-DNA-lesion. The synthesis of the beta-cFaPydG-lesion was already finished in 2005 in the Carell group. In a sixteen-step synthesis the alpha-lesion could be prepared in the form of its phosphoramidite component. Oligonucleotides could be prepared on solid support using automated DNA-synthesis. Some of the synthesised DNA-Nucleotides contained the inserted FaPydG-lesion. Others showed an oxidation to alpha-c8-OxodG. This reaction is not known for natural lesion. Just a de-hydration to guanosine under harsh conditions is described. The observations can be ex-plained by formulation of a cyclic intermediate, which is possibly oxidized by the oxidation di-lution during the DNA-synthesis. If the sequence-dependent equilibrium between alpha cFaPydG and alpha c8-Hydro,hydroxydG is at the side of the closed form, an oxidation from this interme-diate directly to 8-OxodG is imaginable. This assumption is proven by the fact that at the same time synthesised and completely identically treated DNA-strands of different sequence didn´t show oxidation products. This could be ascertained by high resolution ESI-FTICR-mesurement. With this analytical method even smallest variations are detectable in a reliable way, whereas the often used MALDI-TOF-mesurements not always show these variations. Thermodynamic studies showed that alpha-cFaPydG doesn´t form a preferred DNA-basepair. All possible basepairs showed a strong destabilisation in comparison to the undamaged, cor-rect basepair. In vitro elongation experiments with S. cerevisiae polymerase Pol eta, G. stearothermophilus DNA-polymerase I and DinB from G. stearothermophilus showed that the alpha-lesion is a definitive block for all mentioned polymerases. Only by applying rigorous re-action conditions an elongation of the primer with BstPol I could be detected. This elongation showed a favored incorporation of dCTP, followed by an incorporation of dATP. In the time to come experiments to avoid the unwanted oxidation of alpha-cFaPydG will be es-sential to promote the cocristallisation of the alpha-cFaPydG-oligomere with the Fpg-protein from Lactococcus lactis and the BstPol I. In the second part of the thesis attempts were taken to prepare tetrafunctional amino acid derivatives, which were planned to be used in a PNA-template directed ligation reaction. Liga-tion takes place between two amino acids, so that for a ligation attempt with this method there is always needed a matching pair of amino acids. This pair requires a connection of the aminoacid to the PNA over the side chain of the amino acid. The N- and C-terminus of the amino acid had to be unprotected for the ultimate ligation reaction. These termini must not react during the solid support PNA-synthesis. After ligation the template should be separated selectively. Thus, the following requirements for the pair of amino acids result: The amino acids have to possess functional side chains, which exist naturally after cleavage of the PNA-template. For the selective cleavage of the template a con-nection orthogonal to acid and base unstable protecting groups is necessary, because these are already needed for the solid support synthesis on the residual three termini of the compound. Imaginable orthogonal cleavable links are allylic compounds, which are cleavable with Pd(0), but also silicon based compounds, which are cleavable with fluoridions, would be appropriate. In the area of the Pd(0)-cleavable link a retrosynthetic cut at this funtionality should be adequate. Here, e.g. a Horner-Wadsworth-Emmons-reaction is imaginable. The aldehydes and phosphorylides needed for this key step could be prepared with good yields, however only one of the two amino acid derivatives was obtained. The synthesis of the other compound didn´t succeed under diverse reaction conditions. Also efforts to prepare analogue Pd(0)-cleavable link molecules through classic Wittig or metathese reactions with Grubbs catalysts of the first and second generation were unsuccessful. The most successful efforts to make a target molecule synthetically accessible for a first liga-tion attempt involved the preparing of a fluorid-cleavable silyl link. Here all requirements stay the same, just instead of allylic compounds silyl ethers are needed. The acid stability of the silyl protecting group depends on the size of the alkyl groups (tert-butyl>isopropyl>ethyl). Firstly, substitution experiments with the compounds Di-tert-butylchlorosilane, Di-tert-butyl-dichlorosilane and Di-tert-butylsilylbis(trifluoromethanesulfonate) were undertaken to estab-lish a connection between PNA and the amino acid through a Di-tert-butyldisilylether. How-ever, the substitutions produced just monosubstituted silanoles, irrespective of the choice of reaction conditions and substitution order. The experiments with the commercially available (3-Cyanopropyl)-diisopropyl-chlorosilane were more successfull. Here the synthesis of one of the target molecules could be achieved.
