Dr. Lobos joined Purina in 2005, as Veterinary Communications Manager for Purina Pro Plan Veterinary Diets. In this role, she managed the comprehensive VIP experience for the brand, as well as Purina's robust partnership with the American College of Veterinary Sports Medicine and Rehabilitation. Throughout her Purina career, Dr. Lobos has worked as Senior Manager of Training and Senior Manager of Veterinary Communication for Purina Pro Plan Veterinary Diets. For four years, she managed the Global Scientific Programs and Events for the Purina Institute, which advances the science of nutrition, promoting global collaboration with veterinary and other scientific thought leaders to help pets live better, longer lives.  She played an integral role in the annual Companion Animal Nutrition Summit, a signature event that connects more than 150 global thought leaders to advance science and research in pet nutrition.   Dr. Lobos graduated from Louisiana State University School of Veterinary Medicine in 2003. She also became certified in canine physical rehabilitation through the Canine Rehabilitation Institute in 2010. She completed her certification as a veterinary acupuncture therapist from the Canine Rehabilitation Institute in 2021. She also is an active member of the American Veterinary Medical Association, American Academy of Veterinary Nutrition, American Association of Rehabilitation Veterinarians, and Colorado Veterinary Medical Association   Dr. Lobos currently resides in Boulder, Colorado, where she spends her evenings and weekends practicing as a relief veterinarian. EP455: A Leadership Blueprint for Measurably Better Care, With Beau Raymond, MD

A rate critical to attain better care for patients, I'm gonna say, is enlightened leadership—maybe dyad leadership—at a clinical organization. I am saying this because without enlightened leaders, it'd be harder to build from the blueprint that Beau Raymond, MD, talks about today on the show. I'd say an enlightened leader is someone—and this is my definition, but it's a term that Tom Lee, MD, brought up first in an earlier episode (EP445)—an enlightened leader really cares about providing better patient care at an affordable price. They have a manifesto to that end, if you will. They also have studied, likely, and understand how change management works because every improvement requires change. They get the bit about people, processes, and technology being intertwined and what operational excellence means. Further, they are probably doing or considering many of the things that Robert Pearl, MD, talked about in episode 412. On the opposite end of the spectrum, there's a new term floating around called administrative harm. There's a study. Admin harm refers as much to what administrators—who I refuse to call leaders at this context because I'm talking about the not good administrators, so let's be clear—but I'd say administrative harm results from what the administrators choose not to do as much as what they choose to do. It is actually a thing to be an enlightened leader, especially in these profit-driven times. It's really tough, actually, and nothing anyone should take for granted. So, maybe this whole show is kind of a shout-out to the enlightened leaders out there. Thanks for doing what you do. Okay, so this said, and it needed to be said, let's talk blueprint for better care in the conversation that follows. Dr. Beau Raymond says, step 1, right out of the gate, set clear goals. Then step 2, engage others throughout the organization to together build the framework needed to achieve said goals. Engaging frontline folks and others is really the only way that any proposed framework will actually work in the real world. Listen to the shows with Karen Root (EP381) and Ashleigh Gunter (EP447) for just one proof point after another that what I say is based in fact. Step 3 of the blueprint to better outcomes that Beau Raymond, MD, talks about today is get your data. We talk a lot about plan sponsors and the getting of data, but same thing applies to clinical organizations. For clinical organizations, the getting of data means longitudinal data. The need for longitudinal data has come up in multiple shows, most recently the one with Dan Nardi (Spotlight Episode), and this is just one example of why getting the whole bag of data really matters. Dan said on that earlier show, it's often a thing that oncologists are unaware of how many of their patients are winding up in the ER for nausea after chemo, which, by the way, is the most common cause for readmission. And the reason for this is lots of patients travel to their oncologist but go to a local ER in a different health system. The show with Brendan Keeler (EP454) about the Particle v Epic lawsuit in general dustup over who gets the data is super relevant here. That's what I was thinking when I was talking with Dr. Raymond, and maybe it just popped in your head, too. Or just continuing this topic of the importance of longitudinal data, how many specialists, in almost any specialty, see a patient and then don't know what happened to that patient subsequently? Or even primary care in transactional models? So, step 3 here is get your data and also, as part of that, figure out how to make sure everybody understands the data and also understands that it is fair. Eric Gallagher (EP405), Dr. Raymond's dyad counterpart over at Ochsner, talked about this some in that episode. So did Kenny Cole, MD (EP431), interestingly, also from Ochsner. Amy Scanlan, MD (EP402) mentions it as well. Step 4 in the blueprint to measurably better outcomes that I discuss with Dr. Beau Raymond, data collection and data management probably need to be system-wide because … yeah, longitudinal and etc. But the “What are you gonna do now with the insights that you derived from the data?” is pretty local. The obstacles and enablers are going to be different depending on the geography. For example, an area with a large Vietnamese population and a big variation in colorectal screening rates as a priority, just logically, is gonna have a program that is in no way suited to roll out in an area with, say, a large Black or African American population with high hypertension rates. Priorities and programs are just different depending on the geography. So, step 4 here is, ask each region, based on the data, what fixes they're going to own. What will they take ownership on and commit to improving? What I thought was interesting in this interview is kind of the way that equity comes up between ethnic groups or between genders. In and of itself, obviously, striving for equity is critical. But also, if you're trying to improve quality across the board and you see disparities in care, figuring out what is going on with the group experiencing the worse outcomes is also just operational excellence. You don't want to be a solution looking around for a problem, after all; so, you need to figure out the actual problems for the actual people experiencing the problems to avoid that. Those are the only solutions that are actually gonna work. Step 5 is to learn from each other. Maybe not a whole program is flat-out transferable from one geography to another, but that doesn't mean that nothing is transferable either. As usual, it's about being thoughtful and nuanced and finding that productive middle. At Ochsner, they do these cool weekly primary care huddles to share learnings and goings-on that Dr. Raymond explains in the show that follows. Throughout all of these steps in this blueprint, there is obviously a need to align how the practice or system is getting paid for the time and capital expenditures, of course. And Dr. Raymond addresses this and interestingly says something similar to what Dr. Tom Lee (EP445) and Scott Conard, MD (EP391) have said on earlier shows: that a lot of times compensation for improving care, if you do it in an operationally excellent way, can be revenue positive for systems with a combination of both FFS (fee for service) and value-based reimbursement. Underline, however, the part about having an enlightened leader who cares about clinical quality for that to work out. Dr. Nathan Bryan earned his undergraduate Bachelor of Science degree in Biochemistry from the University of Texas at Austin and his doctoral degree from Louisiana State University School of Medicine in Shreveport where he was the recipient of the Dean's Award for Excellence in Research. Physical fitness can also be an important aspect of mental health and well-being. A mystery dog illness that spread across the U.S. last year and can lead to serious or fatal respiratory problems is still under investigation, but cases are starting to slow, experts say. The illness, which started to spread more widely in the summer and fall of 2023, causes coughing, sneezing and fatigue, among other symptoms, and can progress quickly to pneumonia. The dogs suspected to have it test negative for all of the typical causes of respiratory symptoms and often don't respond to treatment. In a statement to TODAY.com, the American Veterinary Medical Association says that based on "conversations with various sources ... the number of cases are declining," adding that an "ebb and flow" of respiratory illness in dogs over the course of the year is common, similar to cold and flu season in humans. The statement also explains that the U.S. Department of Agriculture has so far not seen that the rise in respiratory illness in dogs is linked to a new virus or bacteria. A statement from the Animal and Plant Inspection Service, part of the USDA, confirms to TODAY.com that the state-led testing that the agency is helping coordinate "has not indicated the presence of a novel pathogen or single infectious cause among these cases." Numerous veterinary diagnostic labs across the country are investigating the outbreak, from Oregon to Kansas to New Hampshire. The APHIS spokesperson adds that the illness is not regulated by APHIS and therefore the branch doesn't have data on case numbers. The Oregon Veterinary Medical Association also shared in an Feb. 14, 2024, update that "cases seem to be waning, if not back to normal," citing veterinarian Dr. Scott Weese's Worms and Germs blog. At last count, at least 19 states had reported cases of the illness, according to various expert groups. The AVMA explained in an earlier statement that the mystery dog illness is difficult to track because there's no national surveillance system. In November 2023, veterinarians in critical care settings told TODAY.com that they'd seen dozens of cases since the fall. Late last year, multiple groups — including the Colorado and Oregon departments of agriculture, the AVMA, the Rhode Island Department of Environmental Management and the Los Angeles County Department of Public Health — issued warnings about the mystery illness in dogs. “We don't know what is causing this situation, where dogs are presenting at veterinarians with what private veterinarians would call kennel cough,” Rhode Island's state veterinarian Dr. Scott Marshall told NBC affiliate WJAR. “What's different about this situation is that dogs are presenting with little bit more severe signs, not responding to usual treatments, and unfortunately had a small number, still a number, of dogs that have succumbed to them.” “It seems to happen very, very quickly — to go from this cough that's just won't go away ... and then all of a sudden they develop this pneumonia,” Dr. Lindsey Ganzer, veterinarian and CEO at North Springs Veterinary Referral Center in Colorado Springs, Colorado, told TODAY.com. She said she treated over a dozen dogs with what she believes is the condition between October and November 2023. If your dog develops a cough, do not panic, the AVMA advises. But stay vigilant about its progression and your pet's overall health, and contact your vet right away if anything seems off. What is the mystery dog illness? Dogs with this mystery illness usually have coughing, sneezing, eye or nose discharge, are abnormally tired, and do not test positive for any common causes of canine respiratory illness, the Oregon Department of Agriculture noted in a Nov. 9 press release. Typically, dogs with respiratory illnesses have a cough for seven to 10 days, but some vets saw an uptick in dogs with coughs lasting weeks to months that don't respond to treatment, the Colorado Department of Agriculture said in a Nov. 22 statement. One of the distinguishing characteristics of this mystery illness outbreak was the high number of dogs who developed pneumonia. One Colorado vet, Dr. Michael Lappin, director of the Center for Companion Animal Studies at the Colorado State University School of Veterinary Medicine, told NBC News the number of canine pneumonia cases in the state rose by 50% between September and November 2023 compared to 2022. Marshall estimated that Rhode Island saw at least 35 cases of the mystery respiratory illness, but it's hard to know exactly how many because not all cases are reported. Dogs are most likely to contract it by being in close contact with numerous other dogs — so places like doggy day care, dog parks, groomers or boarding kennels, Ganzer said. The illness seems to affect dogs regardless of age, size or breed, though dogs with snort snouts, like bulldogs and pugs, may be at higher risk. Between mid-August and mid-November, the Oregon Department of Agriculture received reports of over 200 cases of the illness from veterinarians in the state but has had no additional cases to report since. A Dec. 1 statement from the Washington Department of Agriculture said the state has received 16 reports of unusual canine respiratory disease. Two cases were confirmed as such. The Colorado Department of Agriculture also shared in November that veterinarians in the state were seeing "double the number of cases than what is typically seen during a canine infectious respiratory disease outbreak."   The Wisconsin State Journal reported that between late October and December, clinics around the state saw six to 12 cases each, according to Dr. Keith Poulsen, director of the Wisconsin Veterinary Diagnostic Laboratory at University of Wisconin-Madison. Dr. Amanda Cavanagh, head of urgent care services at Colorado State University James L. Voss Veterinary Teaching Hospital, in Fort Collins, Colorado, told TODAY.com that this past summer she started seeing cases of dogs with coughs lasting several weeks or longer, and the trend continued into the fall. She estimated her caseload of coughing dogs doubled from October to November. While dogs with a contagious cough are common in veterinary settings, Cavanagh noted that she normally sees cases decrease in the fall as temperatures drop and fewer dogs are gathering at parks. "But this year, the spike has stayed high," she explains. In its original press release, the Oregon Department of Agriculture noted that the illness can progress in three ways: Mild to moderate cough for six to eight weeks or longer that either doesn't respond to antibiotics or only responds a little Chronic pneumonia that doesn't respond to antibiotics Severe pneumonia that "often leads to poor outcomes in as little as 24 to 36 hours" Cavanagh witnessed all three scenarios play out at her hospital. Last fall, she said she saw more dogs than usual with a long-lasting upper respiratory infection who then developed pneumonia from a secondary bacterial infection. Of the dogs she treated, she said most who developed pneumonia responded to antibiotics, and many with the long-lasting cough recovered with time and never got pneumonia. But of those who developed the severe pneumonia, some died or had to be euthanized. "That really bad pneumonia, historically, is very, very rare. Maybe I would see one case a year," but last fall alone, Cavanagh saw a "handful," she said. How many dogs have died from the mystery illness? Because most states are not tracking case numbers, it's not known exactly how many dogs have died from the illness. The Oregon Department of Agriculture tells TODAY.com that it does not know how many dogs have died from the illness in the state. However, it is confirmed that some dogs in the U.S. have died from the mystery illness. The Colorado Department of Agriculture said in a statement that "in rare cases, the canine patients progress quickly from pneumonia to death." Colorado State University College of Veterinary Medicine and Biological Sciences also said the illness has resulted in "some fatalities." Ganzer and Cavanagh both have had several canine patients die from what they believe to be the condition. Rhode Island Veterinary Medical Association President Shelly Pancoast told WJAR that she's seen five to 10 dogs die from the mystery illness. “We still don't have a great handle on how exactly we should be treating these dogs,” Pancoast said. “A vast majority of them are making full recovery, it's just unlike anything we've seen in previous years with kennel cough." What states have the mystery dog illness? According to various expert groups, cases that match the description of the mystery dog illness have been reported, officially or anecdotally, in: Colorado California Florida Georgia Idaho Illinois Indiana Maine Maryland Massachusetts New Hampshire Nevada Oregon Pennsylvania Rhode Island Tennessee Vermont Washington Wisconsin This above list of states comes from the AVMA, the Tennessee Veterinary Medical Association, the Louisiana State University School of Veterinary Medicine and the Wisconsin Veterinary Diagnostic Laboratory at University of Wisconsin-Madison. Mystery dog illness 2023 symptoms Symptoms of the mystery dog illness include: Coughing that doesn't get better on its own after a week or so Sneezing Nasal or eye discharge Red eyes Lethargy Trouble breathing, especially from the stomach Blue or purple gums (due to not getting enough oxygen) Related: What does coughing in a dog sound like? Vet explains in viral video Signs of coughing in dogs For many dog owners, the signs of coughing can be difficult to recognize, Dr. Michele Forbes, Dr. Michele Forbes, owner of Compassionate Care Hospital in Ann Arbor, Michigan, says. "Rarely do (dog owners) identify a cough until it becomes an overt problem and it's clearly coming from the chest,” Forbes explained in a now-viral TikTok. As a result of owners not noticing coughing or confusing it for something else, like throat-clearing or gagging, some dogs are arriving at vet facilities in more advanced stages of the condition, which can make it more difficult to treat, Forbes told TODAY.com. Some tips to help identify signs of coughing in dogs: The sound dogs make when they're choking can be confused for coughing. Some dogs sound like they're honking, kind of like a goose. Coughing can also sound like the dog is gagging or clearing its throat. The chest may heave or the abdomen may move while the dog is coughing. For some dogs, it looks like they're trying to cough up a hair ball and they may produce some liquid. A wet cough may produce a gargling sound. If a dog is coughing a lot, they may end up vomiting, which dog owners often confuse for a gastrointestinal issue. Reverse-sneezing can be confused for coughing but it's not usually a reason to call the vet unless it's paired with actual coughing, nasal discharge or any other concerning symptoms. When to see the vet for the mystery dog illness Other signs that your dog should see the vet, per the AVMA, include: Lingering cough Weakness Loss of appetite Difficulty breathing Worsening of illness Cough that is sufficiently severe that it causes the dog to vomit or makes it hard for the animal to breathe Tips to prevent mystery dog illness from vets Dogs are most likely to contract the illness when in close contact with other dogs, so previous guidance was to keep dogs away from other dogs. But case rates have essentially returned to normal, Weese noted in his blog. To keep your dog safe and healthy overall, the AVMA recommends keeping up to date with vaccinations. "While the existing vaccines may not specifically target this unknown infection, maintaining overall health through routine vaccinations can help support a dog's immune system in combating various infections," it said. The AVMA stressed the following vaccines: Bordetella, Adenovirus type 2, and parainfluenza combined with the injectable influenza H3N2 vaccine. Be sure to give your dog two weeks after vaccination before interacting with other dogs so they can build up immunity. If your dog is sick, consult a vet as soon as possible, as early testing can help with treatment, and keep the dog away from other dogs to avoid spreading the illness, experts advise. While it's unlikely a humans can get sick with the respiratory illness, because the cause is still unknown, the AVMA suggests thoroughly washing hands after handling any dogs. There are two ways the body normally makes NO. One is from the amino acid L-arginine. The enzyme that converts L-arginine into nitric oxide becomes dysfunctional. This is due to many factors including aging, oxidative stress, poor diet, lack of physical exercise, smoking, diabetes, high sugar diet, etc. The other pathway is from nitrate and nitrite found naturally in some foods. Each pathway contributes about 50% of the total NO production and one can compensate for the other. Optimize Nitric Oxide in Your Body  -       Nitric Oxide is the Holy Grail of anti-aging medicine in longevity. -       Everything we've discovered about the onset and progression of chronic disease over the last 30 years has been linked to a decrease in nitric oxide production. -        Nitric oxide is the glue for protein to bind glucose to the membrane. -       We can practically overcome all of the metabolic characteristics of type 2 diabetes if we restore nitric oxide production and insulin signaling. -        The improper diet, lifestyle, and standard pharmacotherapy all lead to disruption in nitric oxide. But if we have a drug that inhibits and dodges cholesterol production, which is what a statin does, then our body can't respond. -        Physicians and drugs don't heal patients; patients heal themselves. [29:39] How Vegetables Increase Nitric Oxide Inside Your Body -       Vegetables contain nitrate. Green leafy vegetables are high in what's called inorganic nitrate. -       Nitrate is inert in humans. But when we consume these vegetables, 90 minutes after we consume them, the nitrate is taken up in the good and concentrated in our salivary glands.  -       We have a mechanism that allows you to eat a plant-based diet, restore nitric oxide production, and reverse cardiovascular disease. However, not all patients respond to this mechanism. Some got better, and some didn't. -       There's high variability in the nitrate content of vegetables. So, there's no way to predict how much you will get from eating vegetables. It depends on where they're grown, how they're grown, and when you consume them. -       The other issue is you have to have the right oral bacteria. So, if you're using mouthwash, you won't get the nitric oxide benefits of that plant-based diet.  [33:53] How Big Pharma Brain Washes The World -       75% of all the advertisements on TV are from big pharma. [1:10:01] Nitric Oxide vs. Methylene Blue: Learn the Truth Behind The Effect Of Methylene Blue  -       Methylene blue eliminates the signaling aspects of nitric oxide, and bad things happen if you don't get enough nitric oxide. -       The ability to create nitric oxide inhibits the majority of people. -       People out there are saying bad things about nitric oxide because it's in the literature that methylene blue inhibits its production. He completed a one-year fellowship in oral surgery in 1984 at Naval Dental Center Great Lakes, IL. Dr. Ohmer served his country in the Navy in many assignments worldwide until his military retirement in 2016 as a Captain. Dr. Ohmer owned Dental Doctors of Florida for 6 years until merging his practice with Harbour Dental Care in March of 2022. In his spare time, Dr. Ohmer enjoys spending his free time outdoors; at the beach, hiking, and playing tennis. This week our  Melanated Docs Spotlight is on  Dr. Aniika Jackson-Smith. Dr. Aniika Jackson-Smith is a native of New Orleans, LA.  She received a Bachelor of Science Degree from Xavier University of Louisiana in 1999.  In the years following graduation, she taught 7th grade science at Thurgood Marshall Middle School in New Orleans.  In the fall of 2002 Dr. Jackson matriculated at Creighton University Medical Center School of Dentistry in Omaha, NE.  She completed her studies at Creighton in 2006 earning a Doctor of Dental Surgery Degree.  Upon graduation, she entered into a General Practice Residency at Louisiana State University School of Dentistry.  Dr. Jackson was among some of the first healthcare providers to return to New Orleans after Hurricane Katrina.  Thus, providing a much needed service to the citizens returning to rebuild the city.   In 2008 Dr. Jackson-Smith relocated to Kansas City.  She enjoys treating patients and doing community outreach.  She is committed to providing quality care to underserved, underinsured and uninsured individuals.  As such, the majority of her 15 year career has been spent working in Federally Qualified Health Centers or similar institutions.  Over the years, she has mentored countless aspiring dental and medical professionals. You are also very likely to find her in school classrooms introducing students students to the dental profession and promoting HBCU's. She loves talking to youth about her profession and her beloved XU! She currently serves at Deputy Dental Director at one of the largest health centers in Missouri. She is an International Board Certified Lactation Consultant who is passionate about improving the health outcomes of African-American infants and moms through breastfeeding advocacy and support. In her spare time she enjoys spending time with her husband and 2 daughters, crafting and online “window” shopping.  She is the owner of Nia Rae's Fine New Orleans Pecan Candy. EP343: What Provider Leadership Teams Need to Know to Operationalize Value-Based Care, With David Carmouche, MD

Most people who have been in the healthcare industry for a while have heard by now the metaphor about the two canoes. Provider organizations or health systems with some of their payments coming from a fee-for-service (FFS) payment model and some of them coming from value-based arrangements have the challenge of one foot in the FFS canoe and one foot in the value-based canoe. They're probably going through a lot of metaphorical pants is the main takeaway that often comes to mind for me. But wardrobe malfunctions aside, this is a really difficult organizational challenge. That's what I'm talking about in this healthcare podcast with Dr. David Carmouche: how to deal with the operational challenges, the cultural challenges, maybe even (very arguably) the generational challenges here. Top line (very top line), to succeed in value-based care, you gotta have three things aligned: The payment model, the construct of the contract. No kidding, you have to have value-based contracts to succeed in value-based care. The big problem here—which is not to be underestimated—is that there are some areas of the country where it's really tough to find somebody, or enough somebodies, willing to offer a capitated, prospective value-based contract. That would be really frustrating to want to go forward (if you're a provider) in a value-based way but to not have a willing payer partner and/or employer partner to do so. So please step up, payers, policy makers, and employers in those areas of the country. But the construct of the value-based contracts can also not be overlooked. Toward the end of this interview, Dr. Carmouche gets into the different results that were achieved between two patient populations: one served by a Medicare Advantage (MA) plan and one in an MSSP (Medicare Shared Savings Program) model. So, the same provider network, the same environment, same geography, same number of lives, different payment model. Stick around for that part of the conversation. It's pretty eye-opening. The second of the three things to be aligned to be successful in value-based care are physician/administrative incentives and the employment models. Seriously, who is thinking that anyone's gonna succeed managing downstream risk when the physicians making the decisions about downstream services used are bonused by how much downstream costs they can drive and everyone is eating what they kill? If culture eats strategy for breakfast, incentives eat culture for lunch, as they say. Leadership skills. Leaders who are going to succeed in a world moving from FFS to VBC have to be mission driven toward that cause. They have to be strategic enough in their approach to take potential short-term revenue hits in pursuit of the longer-term goal—even the medium-term goal, honestly, if you think about the whole context of what's going on here. Leaders also need the skill and aptitude to pull off the change management and adjustments to the organizational culture that are needed. Staffs and teams really need systematic support. Value-based care is a team sport, and teams require leadership. Here's one example of where not having great leadership trickles down to bad results: If nurses or social workers or, in general, people of color or women in an organization feel demeaned or not valued by a critical mass of those in power—and maybe here I mean physicians or other physicians that they work with—then patient safety scores diminish and quality goes down. There's enough studies on the impact of having and not having psychological safety that it's getting harder to dispute what I just said. And if this environment becomes as toxic as the stories that you read about often enough, that's on the C-suite to fix. If the C-suite has value-based aspirations, that C-suite really might want to reprioritize their to-do lists. So, think about stuff like this because toxic environments make consistently delivering high-value care and satisfied patients difficult at best for many reasons. Here's a timely side note: I heard someone say the other day that in light of the pandemic and the FFS inpatient and outpatient volume fluctuations that plummeted and rose at various points during the pandemic, compounded with Medicare FFS rates that some institutions claim are not profitable or profitable enough … someone said that, given these factors, the best way to de-risk is to take on more risk. That's interesting to think about on a number of levels. In this healthcare podcast, as I mentioned, I'm talking about all this and more with Dr. David Carmouche. Dr. Carmouche was recently the executive vice president of value-based care and network operations at Ochsner, which is a very big integrated delivery network in Louisiana. You heard it here first, folks, but Dr. Carmouche will take on a new role in November 2021. He will oversee Walmart's expanding clinical care offerings and operations, including Walmart Health MeMD and its social determinants of health line of business. She went on to earn her medical degree from Louisiana State University School of Medicine in New Orleans, LA. After medical school, she finished a residency program in Pathology at New York-Presbyterian Hospital, Weill-Cornell College of Medicine, and a fellowship in Transfusion Medicine at the University of California, San Francisco.  In her current role at the University of Minnesota, she practices Clinical Pathology, oversees the blood bank, and teaches medical students.  Dr. Cohn’s research interests include platelet transfusions, alternative platelet storage solutions, HLA antibodies, and their role in solid organ transplants and platelet refractoriness. She focuses her efforts in two areas: patient blood management and platelet storage and utilization. Dr. Cohn was named in 2020 chief medical officer (CMO) for the AABB organization where she works to strategize and implement ways that achieve AABB’s mission through the development of policies and by engaging the communities it serves.

When I was talking with Dr. David Carmouche from Ochsner in EP316 about the importance of collaboration amongst anybody trying to actually pull off value-based care, we took a little detour, which I wound up cutting out, into the potential and challenges for health systems to collaborate and do value-based contracts with pharmaceutical manufacturers. It’s a really interesting sidebar, though, that I wanted to share with you—especially on the heels of the recent interview with Troy Larsgard from Johns Hopkins (EP318) on how Pharma can better meet the needs of their health system customers.  Here’s an interesting point that Dr. Carmouche makes in the sidebar that I thought was worth highlighting. Chalk this up as one of the challenges when trying to create some kind of risk-share agreement with a pharma company to get the manufacturer to put their money where their mouth is when they say that downstream costs will be saved or complications avoided or better outcomes attained. The challenge comes in assembling enough patients to make it worth everyone’s while. As we’re considering the assemblage of patients, we have to first consider who’s paying for the drugs. It’s a payer, usually, who contracts with a PBM (pharmacy benefit manager). So, any given health system is going to have to have enough patients not only on that one particular payer in its payer mix but also on that one payer with a plan design that uses that one PBM. Palliative care is compassionate comfort care that provides relief from the symptoms and physical and mental stress of a serious or life-limiting illness. Palliative care can be pursued at diagnosis, during curative treatment and follow-up, and at the end of life. Marie-Carmelle Elie, M.D., has been named chair of the Department of Emergency Medicine at the University of Alabama at Birmingham School of Medicine. Carmelle obtained her undergraduate degree from Columbia University and her medical degree from the State University of New York in Brooklyn with a distinction in research. Since then, she has continued to identify herself as a scholar and clinical investigator. Following her emergency medicine residency at Mount Sinai Medical Center in New York, she completed a Critical Care/Trauma Fellowship at the R. Adam Cowley Shock Trauma Center at the University of Maryland. She is triple board-certified in emergency medicine, critical care, as well as hospice and palliative care medicine. Dr. Ashley Shreves earned a Bachelor of Arts degree in Biology and Chemistry from Washington and Lee University in Lexington, VA. She went on to receive a doctor of Medicine degree from Louisiana State University School of Medicine located in New Orleans, LA.  Ashley completed a residency in Emergency Medicine with St. Luke's-Roosevelt Hospital in New York City.  She then completed a Palliative Medicine Fellowship at Mount Sinai School of Medicine. I don’t know what I thought we were going to talk about during my interview with David Carmouche, MD; but I’m glad it turned out exactly as it did. Lately, we’ve had a number of guests on Relentless Health Value talking from the point of view of the employer: what a self-insured employer wants and needs from the large, and small, providers in their network. In this episode, we’re flipping the script and talking about what a large provider organization wants and needs from the commercial side of its payer mix. If value-based care or risk shares are to be a thing, we can’t have, as Troy Larsgard has put it, all risk and no share. In this health care podcast, I had the honor and pleasure of speaking with Dr. David Carmouche. Dr. Carmouche started out as a physician in a multi-specialty group. He practiced there for about 15 years before leaving to become chief medical officer at BCBS (Blue Cross Blue Shield) of Louisiana. Five years ago, Dr. Carmouche transitioned to Ochsner Health, where he is currently executive vice president of value-based care and network operations. At Ochsner, Dr. Carmouche helps lead the value-based care agenda—that’s everything from managing strategic partnerships with payers, as well as managing risk in value-based contracts for Ochsner and affiliated network partners across their ACO (accountable care organization) and CIN (clinically integrated network). Highlighting one point that Dr. Carmouche makes early in our chat, there’s four things that have to come together for meaningful value creation for providers: (1) willingness of providers and provider leadership to think and do things different than they have historically; (2) they have to be able to affect payment for those things; (3) they have to have data and be able to access it; and then (4) some control over steering patients. This kind of sets the stage, actually, for our fast dive, in this conversation, right into employer and commercial collaborations. Three of the four things on that list—affecting payment, data, steering patients—are right in the wheelhouse of forward-thinking employers, or commercial payers/TPAs (third-party administrators) trying hard to compete for or serve employers. Just a quick heads-up here: Coming soon, we’re going to release a second episode with Dr. Carmouche giving some great advice for the leadership of provider organizations who are trying to figure out their transition away from FFS (fee for service) to a more risk-based, value-based model. One quick point that I thought was also relevant to the show here: It was super interesting to me how quickly Dr. Carmouche got from “transition to value” to “knows how to collaborate with other organizations.” Here’s the pretty obvious inference: You can’t transition to value if you don’t know how to play well with others to co-create value and share the rewards of such an endeavor. There might be a broader lesson in here for whoever you are in the health care ecosystem. Dr. Mustapha is a board-certified interventional cardiologist who specializes in endovascular revascularization of PVD, specifically CLI. He is co-founder and CEO of Advanced Cardiac and Vascular Amputation Prevention Centers in Grand Rapids, Michigan. He graduated from Wayne State University and St. George University School of Medicine and has completed Fellowships in both Cardiology and Interventional Cardiology at Louisiana State University School of Medicine. Dr. Mustapha is a Fellow of the American College of Cardiology and Fellow of the Society of Cardiac Angiography and Intervention. He serves as Clinical Associate Professor of Medicine at Michigan State University College of Human Medicine. Dr. Mustapha speaks internationally on the topic of CLI, alternative access, atherectomy, vessel morphology, calcification and drug eluting technology.  He has multiple publications on CLI and limb salvage. Brad Walker is one of the most active and sought-after performers in the city of New Orleans. In addition to freelance work around the world with such luminaries as Bernard Purdie, George Porter, Jr., Leo Nocentelli, Marcia Ball, Valerie June, Rickie Lee Jones, and countless others, Brad has anchored the horn section for local stars Eric Lindell, Colin Lake, Johnny Sketch and the Dirty Notes, and Honey Island Swamp Band, to name a few. He has performed as sideman and leader many times at the New Orleans Jazz and Heritage Festival, South by Southwest (SXSW), Farm Aid, French Quarter Festival, and other national showcases. In the fall of 2015 Brad was Guest Artist with the LSU Jazz Ensemble, and has joined the LSU Jazz Faculty on numerous occasions on their acclaimed ‘Hot Summer Nights and Cool Jazz’ summer concert series. In 2016 Brad was hired to lead the horn section for Nashville mega-star Sturgill Simpson, who was embarking on a world tour in support of his eventual Grammy- winning album ‘A Sailor’s Guide to Earth’. In addition to extensive domestic and international touring, Brad performed with Simpson on The Late Show with Stephen Colbert, Conan O’Brien, The Daily Show, Late Night with Seth Meyers, and was a featured soloist on The Tonight Show, Saturday Night Live, and the 2017 Grammys telecast. Rolling Stone, NPR, Relix, and Variety all took note of his “soaring,” “energetic,” and “tasteful” soloing. As a solo artist Brad has released 5 independent records, three featuring his improvisation-only alter ego redrawblak, and two his more composition-oriented Quintet. These records have been reviewed in local publications OffBeat Magazine, Gambit, Dig!, and others, and was included in a feature on notable emerging artists in DownBeat Magazine, considered the premier jazz publication in the world. ‘Quintet’ received a 4-**** review from DownBeat. She has won over one hundred lighting design awards for her work, including a 2018 International Dark Sky Association Lighting Design and Technology Award and the 2017 Lighting Designer of the Year at the Lighting Design Awards. Suzan Tillotson is one of the finest lighting designers in the world right now and I am honored to have met her already 9 years ago. Now I am fortunate to get to know and share the story behind her success, challenges, highlights and trends she observes in the lighting design industry that are needed or are about to happen.
