When Hans Rueffert learned he had gastric stomach cancer at age 33, he had to have half of his stomach and half of his esophagus removed, followed by chemotherapy and radiation treatment. However, because gastric juices were leaking into his lungs, he subsequently had to undergo surgery removing both his stomach and esophagus in full. It was a battle, but Hans achieved survivorship. He has to watch what he eats and how much he eats, but Hans has resumed his life as a chef, and is an active participant in competitive pickle ball.
Digestion is a complex process that involves breaking down food into nutrients that the body can absorb and use. For a healthy person, the time it takes to digest meals and snacks can vary based on the type and composition of the food consumed. Keep in mind the digestive process varies significantly between healthy individuals and those with digestive disorders. While healthy individuals typically digest meals within a predictable timeframe, digestive disorders can cause delays or speed up this process. Keep in mind that GLP-1 agonists like semaglutide (Wegovy) and tirzepatide (Zepbound) influence digestion by slowing gastric emptying and regulating appetite, which can be beneficial for weight loss and diabetes management but may pose challenges for those with certain digestive conditions. In this podcast, we'll discuss how digestion is impacted by digestive orders and how GLP-1 agonists like semaglutide (Wegovy) and tirzepatide (Zepbound) affect digestion. The Digestive Process Explained Starting in the Mouth: The digestive process begins in the mouth when you chew food. Chewing breaks down food into smaller particles, and saliva, which contains an enzyme that starts the digestion of starches, moistens the food, making it easier to move along the gastrointestinal (GI) tract. Moving to the Esophagus: When you swallow, food travels down the esophagus. Here, peristalsis—rhythmic contractions—automatically propel the food forward. At the lower end of the esophagus, a circular muscle called the lower esophageal sphincter (LES) relaxes to allow food to pass into the stomach and then closes to prevent stomach acid from flowing back into the esophagus. Stomach Function: Once food reaches the stomach, muscles in the stomach wall mix it with stomach acid and enzymes, creating digestive juices that break the food down into a liquid mixture called chyme. The Small Intestine: In the small intestine, food mixes with digestive juices from the pancreas, liver, and gallbladder. The walls of the small intestine then absorb nutrients into the bloodstream before peristalsis moves the remaining mixture forward. Large Intestine Function: Undigested food, fluids, and damaged cells from the GI tract's lining enter the large intestine or colon. Here, water is absorbed, transforming the waste material from liquid to stool. Peristalsis then moves the stool into the rectum, where it is stored until it is expelled during a bowel movement. Typical Digestion Times: Liquids: Water and other clear fluids pass through the stomach quickly, usually within 20-30 minutes. Simple Carbohydrates: Foods like fruit, white bread, and sugary snacks are typically digested within 30-60 minutes. Complex Carbohydrates: Whole grains, quinoa, vegetables, and legumes take longer, around 2-3 hours. Proteins: Meat, dairy, and other high-protein foods generally take 3-4 hours to digest. Fats: Fatty foods such as nuts, cheese, and fried foods are the slowest to digest, often taking up to 6 hours or more. On average, the entire digestive process, from ingestion to elimination, can take about 24-72 hours in healthy individuals. Digestion in Individuals with Digestive Disorders Digestive disorders can significantly alter the time it takes for food to move through the digestive system. Conditions such as gastroparesis, irritable bowel syndrome (IBS), and inflammatory bowel disease (IBD) can disrupt normal digestive function. Gastroparesis: This condition, often caused by diabetes or other underlying issues, slows stomach emptying. Symptoms include nausea, vomiting, and bloating. In gastroparesis, digestion can be severely delayed, with food remaining in the stomach for prolonged periods, sometimes up to several hours longer than normal. Irritable Bowel Syndrome (IBS): IBS can cause both accelerated and delayed digestion, depending on whether diarrhea-predominant (IBS-D) or constipation-predominant (IBS-C) symptoms are present. IBS-D can lead to faster transit times, while IBS-C can slow digestion, causing significant delays in the passage of food. Inflammatory Bowel Disease (IBD): Conditions such as Crohn's disease and ulcerative colitis involve chronic inflammation of the gastrointestinal tract, which can disrupt normal digestion. Depending on the severity and location of inflammation, digestion can be faster or delayed. What is the Impact of GLP-1 Agonists on Digestion? GLP-1 (glucagon-like peptide-1) agonists, such as semaglutide (Wegovy) and tirzepatide (Zepbound), are medications primarily used for managing type 2 diabetes and obesity. These drugs mimic the action of the GLP-1 hormone, which plays an important role in regulating appetite, insulin secretion, and gastric motility. Slowed Gastric Emptying: GLP-1 agonists slow the rate at which food leaves the stomach. This effect can help increase feelings of fullness and reduce overall food intake, contributing to weight loss. In healthy individuals, this slowed gastric emptying can extend digestion times slightly but is generally well-tolerated. Appetite Regulation: By acting on receptors in the brain, GLP-1 agonists reduce appetite and promote satiety. Meal Composition: Mixed meals containing fats, proteins, and carbohydrates will generally take longer to digest, especially when combined with the effects of GLP-1 agonists. Side Effects and Considerations: While GLP-1 agonists are effective in managing weight and blood glucose levels, they can also cause gastrointestinal side effects such as nausea, constipation, belching, and diarrhea, particularly during the initial stages of treatment or when the dose is increased. These side effects are typically temporary and go away on their own when your body gets used to the medication. The use of GLP-1 agonists requires careful consideration and monitoring for individuals with digestive disorders. The slowed gastric emptying effect might exacerbate symptoms in conditions like gastroparesis. This episode is not meant to be a long one, but as I tried to keep it as short as possible, I kept adding a little here, and a little there. I wanted to touch on Epsom salts, SIBO - Small intestinal bacterial overgrowth, IBS - Irritable Bowel Syndrome, Fasting, and the migrating motor complex. Some people may not be aware but our body has this built-in mechanism called the Migrating Motor Complex that is only triggered when you are in a fasted state for around 4 hours. This mechanism has 4 phases and some people get theirs activated a little earlier, some a little later, and others very infrequently (Think IBS and SIBO sufferers). You need this migrating motor complex to keep your body functioning optimally. It's just one of many processes that need to function to keep your machine well-oiled. Once it starts to fall apart, you can expect lots of downstream problems as well. I believe that a lot of autoimmune diseases that affect the gut also affect the MMC and therefore fixing this is one of the keys to returning your health. What is the Migrating Motor Complex? Think of it like the cleaner in the small intestine. The main purpose is to clean the bacteria out of the small intestine. Moving them into the large intestine and eventually to the ileocecal valve and then the colon. The MMC only happens during fasting. This is very important. It is different from peristalsis. Peristalsis is what we imagine when we think about food being pushed through our bodies. Often both terms are used interchangeably. Peristalsis is more about mixing and slightly pushing down food. The MMC is what is actually the cleaner and pusher. Without it, you're really going to struggle with cleaning the small intestine from bacterial overgrowth. Think of it like washing the dishes after a meal. It's called the digestive system house cleaner wave for a reason. How to improve the MMC for SIBO AND IBS? FASTING BETWEEN MEALS ACTIVATE THE VAGUS NERVE PROKINETIC SUPPORT TOLERANCE OF HIGH FODMAP PRODUCTS FIXING YOUR DIURNAL RHYTHM Finally, Why you should be fasting for IBS and SIBO? Nothing applies to everybody, that's definitely a solid rule to keep in mind. Getting this out of the way, you already fast for 7-8 hours when you sleep, so stop being so scared of it. The Migrating muscle complex gets activated with a 4-hour fast. So let's get to it. You are already a bit of a faster, since you fast while you sleep. Start with waiting for breakfast for an hour or two extra, don't eat a few hours before bed and there you have it, you've started your fasting and healing journey. The benefits of fasting are fantastic. Once you start healing your migrating motor complex, your gut, and your digestive system, you will start to realize that fasting was always there for you, you just didn't know it. If you have IBS or SIBO and you have never tried to fast, I am trying to convince you that this is the time to try it. You know that when you don't eat, so probably in the morning, you feel your best. Extend that feeling. Let your body guide you. Another thing to consider with fasting is that your fat cells have all the nutrients and vitamins you need. Work on replenishing your fat cells during your refeeds with good healthy foods, and you will exponentially feel better on future fasts as well. Once you are comfortable with intermittent fasting you can work your way up to your first 24 hours fast and longer. This will introduce you to a new world where you can start to wave goodbye to both SIBO and IBS. Improving Gut function TOP to BOTTOM - Organs Involved & Gut-brain connection- Dr. Jason Jones Elizabeth City NC, Chiropractor The food we eat moves through the gastrointestinal tract (a long, connected, hollow tube that starts from the mouth and ends in the anus), and there are lots of organs involved in this process. Along the way, the body absorbs the beneficial part of the food after digestion, giving your nutrients and energy. The remaining part is given out as a waste product. A series of muscles work together to coordinate the moment of food and cells that produce enzymes and hormones involved in the process of digestion. Other “accessory” organs, including the liver, gallbladder, and the pancreas also involved in this process. What organs are involved in the process of digestion? The major organs that are part of the digestive system includes: Mouth Esophagus Stomach small intestine large intestine rectum These organs work hand-in-hand to convert foods into nutrients and energy that is needed for survival. In the end, it packages the solid waste or stool and gets rid of them through the bowel movement. Mouth This is the starting part of the gastrointestinal tract. It starts working even before you ingest food. The moment you see and smell that warm bread or pasta dish, your salivary glands gets active. After ingesting your food and chewing into smaller pieces, your saliva mixes with the food, breaking it into an absorbable form that your body can use. The tongue helps to move the food to the next organ Esophagus The esophagus is a long tube close to your trachea (windpipe). It receives food from your mouth, and move it to your stomach through a series of muscular contractions called peristalsis. But for this food to move down, the ring-like muscle called lower esophageal sphincter has to relax to allow passage into the stomach. After that, the muscle contracts to prevent backward flow into the esophagus, which may cause acid reflux or heartburn. Stomach The stomach holds food and serves as the mixer and grinder of food. The cells in the lining of this sac-like organ secrete a strong acid and enzymes that are involved in the breakdown of food into a usable form. After proper processing of the food in the stomach, changing it to a consistency of paste or liquid, the content is then moved to the small intestine. Small intestine This organ is a muscular tube 22-foot long, having three segments- the duodenum, jejunum, and ileum. This organ is the “workhouse” of digestion, and most nutrients are absorbed here. Foods are mixed with digestive enzymes released by the pancreas and bile produced from the liver. Peristalsis is also in action in this organ, moving food through as it is being mixed with digestive juices. The duodenum is the first segment and it is largely responsible for continuing the breakdown process. The jejunum and ileum are responsible for the absorption of nutrients into the bloodstream. Once the nutrients have been absorbed, the leftover (the waste) passes into the large intestine or colon. Large Intestine (Colon) The large intestine is a muscular tube that is 6-foot long, and it links the small intestine directly to the rectum. It has segments, including: Cecum Ascending (right) colon Transverse (across) colon, Descending (left) colon, Sigmoid colon This organ is highly specialized and it is responsible for processing waste to ease emptying of the bowels. The stool or waste left over is sent to the colon through peristalsis, and any remaining water is absorbed. The stool is stored in the sigmoid colon until a mass movement pushes it down into the rectum to begin the process of elimination through the anus. Accessory Digestive Organs and functions Liver The liver secretes bile and process the nutrient-rich blood coming from the small intestine. It purifies this blood of impurities before allowing it to travel to the rest of the body. Pancreas The pancreas is the main factory for digestive enzymes that helps to break down carbohydrates, proteins, and fats. These enzymes are secreted into the duodenum segment of the small intestine. Gallbladder This is a sac for the storage of excess bile made in the liver. This bile is released via the bile duct into the small intestine, where it helps to break down fat and move waste out of the liver. Gut-brain connection The brain has a strong connection to the gastrointestinal tract, especially the stomach and small intestine. For example, the thoughts of food alone can trigger the release of the stomach’s juice even before eating. The walls of the digestive system are lined with more than 100 million nerves, starting from the esophagus to the rectum. That’s called the brain in your gut, otherwise known as the enteric nervous system (ENS). So those moments when you’re nervous and feel “butterflies in your stomach,” the ENS is at work. This brain in your gut mainly controls digestion, from swallowing to enzyme release and food breakdown and absorption. Although this ENS is not capable of thought as we know, it has a direct link with our big brain. More so, the ENS can trigger big emotional shifts, especially in people with irritable bowel syndrome (IBS) and bowel problems like diarrhea, constipation, bloating, stomach upset, and pain. Little wonder people with these issues tend to develop depression and anxiety.
