Also check out the video version on my WeitzChiro YouTube page.]  Podcast Highlights 1:48  Dried urine testing or DUTCH testing uses a dried sample for convenience, which is a big advantage over the other leading type of urine hormone testing, which is the 24 hour urine test that requires carrying around a jug of urine that you keep in the refrigerator all day.  This is very inconvenient and you lose the the changes in those hormones levels, esp. cortisol, that vary throughout the day using a 24 hour sample.  The dried urine test you collect samples throughout the day so you can look at the cortisol pattern, which correlates with what is seen on saliva in studies that have been published.  And the urine testing of hormones allows you to see the metabolites of estrogens and androgens that you can't see on blood testing.  And there is a DUTCH complete test that includes markers in urine for melatonin and B12 and B6, etc. that allows us to see a fuller picture of the hormone story.  For example, this complete testing is helpful if you have a patient who has fatigue you can find out if it is related to testosterone or cortisol or B12. 4:57  On the other hand, dried urine testing does not allow you to measure SHBG, which can be measured through blood, such as when a woman is on birth control and this drives up her SHBG, which drives down her free hormones. Urine testing is also not a good way to measure thyroid hormones. Also, if your kidneys are not functioning well, then you are not excreting urine as well as you should and this can impact urine testing. Also, saliva has some advantages for the cortisol awakening response (CAR), so DUTCH plus includes saliva for the first two samples of cortisol, since for the CAR you need an instantaneous sample when you first wake up within the first five minutes.  DUTCH uses a cotton swab that soaks up saliva instead of having to spit into a tube like other labs use and fill it with 3 ml of saliva after waking up without rinsing your mouth and without drinking any water and for most people this takes 15-20 min, which negates the value of the CAR test.                                Mark Newman, MS is a recognized expert and international speaker in the field of hormone testing. Kim:                     Thanks for having me. Excited to get into this. Dr. Weitz:            Good. So what are some of the biggest challenges that clients have when they're trying to lose weight? Kim:                     The noise, I would say, in the rest of the world, outside of themselves. I think most people forget to listen to what's going on with their own body, and there's so much confusion with what everyone else is doing. Dr. Weitz:            But how do you know what your body's really saying? Kim:                     You got to be mindful, you got to be grounded. And most people are walking around emotionally triggered and they don't even recognize it. I mean our rational mind knows that food is fuel. Our irrational mind thinks food is comfort, joy, entertainment, the enemy, fun. And so, if you think food is anything other than fuel, it's time to check in with what's going on. Take some deep breaths and figure out where the discomfort is in your body or in your life that could be causing your mind to seek some pleasure from something that is outside of yourself. Dr. Weitz:            Now, you say that we should think of food as fuel, which is what I have always done, but a lot of people criticize me and say, "Well, you don't really enjoy your food, but yet one of your six principles is to eat what you love." Kim:                     Yeah. So, I understand what people are saying when they say that to you, and I think you do too, right?

Thanks for joining me, and let's jump into the podcast.                                 Hello Rational Wellness podcasters. Today, we will be speaking about leaky gut with Dr. Jess Armine. Leaky gut is a controversial concept in the functional medicine world. No, I'm not talking about the controversy in conventional medicine about whether leaky gut exists at all. Most of us understand that there's tons of research substantiating that leaky gut exists and that it's a problem, but there are differences about how to test for leaky gut. There's the old lactulose mannitol intestinal permeability test, but most of us today are either using a serum test for zonulin or a stool zonulin test, or a serum test for zonulin antibodies, and there's a lot of controversy whether any of these are accurate.  I personally run a stool zonulin as part of a stool test, but quite frankly, I assume that most of my patients with gut problems have leaky gut. There's also controversy about the best strategies for healing leaky gut and whether probiotics, and which ones, prebiotics, which ones, bone broth, et cetera, et cetera. How should we heal leaky gut? I'm hoping that Dr. Jess Armine can give us some clarification and guidance for how to understand leaky gut and how to treat it. Dr. Jess Armine is a doctor of chiropractic. He graduated in 1986, a year before me, and a registered nurse. He has specialized training in methylation, genetic research, neuro-endo immunology, functional medicine, nutrigenomics applied kinesiology and nutrition. Rahbar and Gurevich on SIBO and IBS: Discussion on SIBO and IBS with Dr. Sam Rahbar and Dr. Ilana Gurevich: Rational Wellness Podcast 298.  We also had a presentation by Dr. Mark Pimentel on the latest research that his group has recently published on SIBO and IBS:  SIBO: New Research Findings with Dr. Mark Pimentel: Rational Wellness Podcast 311.   I also recommend that you listen to the interview that Shivan Sarna and Dr. Allison Siebecker conducted with Dr. Ali Rezaie of Dr. Pimentel's group:  Digestive Disease Week 2023 Research Update by Dr. Ali Rezaie (June 2023)    3:42  Irritable bowel syndrome or IBS is the most common gastrointestinal disorder occurring in up to 20% of the US population.  It is a functional digestive disorder marked by the following symptoms: gas and bloating, stomach pain, diarrhea, constipation, or alternating diarrhea and constipation.  IBS is considered a functional disorder because if you do a scope or do a CAT scan, you won't see any structural problems in the intestines.  By contrast, if you have Crohn's or ulcerative colitis and you do a scope or a scan, you'll likely see damage to the lining of the intestines, possibly bleeding, and erosions.  Because it's a functional disorder, IBS was often considered to be a stress-induced disorder.  Dr. Pimentel has now proven that approximately 60% of cases of IBS are caused by Small Intestinal Bacteria Overgrowth or SIBO.  5:35  The large intestine, aka, the colon, is a large and very elastic tube and it contains the trillions of bacteria, archea, and fungi that compose our microbiome. When these bacteria in the colon ferment and produce gas, this is not a problem. But the small intestine is a smaller tube and doesn't stretch as much and doesn't contain anywhere as much bacteria as the large intestine.  The small intestine is where most nutrients are absorbed from our food, so there need to be a smaller amount of bacteria lining its mucosa or this would interfere with this.  If you consume fermentable fiber and there is an overgrowth of bacteria in the small intestine, this can lead to gas and bloating, which can cause pain and a change in bowel habits.  Hydrogen sulfide and hydrogen gas are usually associated with diarrhea, while methane gas is usually associated with constipation.    Dr. Ben Weitz is available for Functional Nutrition consultations specializing in Functional Gastrointestinal Disorders like IBS/SIBO and Reflux and also Cardiometabolic Risk Factors like elevated lipids, high blood sugar, and high blood pressure.  Dr. Weitz has also successfully helped many patients with managing their weight and improving their athletic performance, as well as sports chiropractic work by calling his Santa Monica office 310-395-3111.
