Ina Park, MD, is back again for a focused look at her (second) favorite sexually transmitted infection: syphilis (favorite because it's fascinating). She says, “it's one of the most challenging conditions to diagnose in clinical medicine because it literally can look like anything.” Join Ina and Tammy as they explore how syphilis was almost eliminated around 2000 and then surged again to current rates of infection in the US. Ina spotlights how a rise in congenital syphilis has fueled a mobilization in public health efforts, even while she celebrates declines in rates of the most infectious types of syphilis. As national syphilis screening guidelines are adapted to increase screening, including in emergency departments, Ina is optimistic for a reduction in future syphilis rates. But here's the takeaway for healthcare providers and patients alike: keep syphilis on your radar. On this week's podcast, the National Association of County and City Health Officials' Victoria Van de Vate, Director of Government Affairs, and Lauren Mastroberardino, Government Affairs Senior Specialist, provided a Congressional update on upcoming government funding challenges. They also discussed recent letters that NACCHO sent to the Office of Disease Prevention and Health Promotion on Healthy People 2030 objectives and to the Centers for Disease Control and Prevention (CDC) Division of STD Prevention about its proposed guidelines on the use of doxycycline as post-exposure prophylaxis to prevent STI transmission.    Later in the program (4:50), Timothy McCall, NACCHO's Director of Research, hosted a discussion with co-authors Rita Burke, Associate Professor of Clinical Population and Public Health Sciences and Pediatrics at the University of Southern California Keck School of Medicine; Larissa Chiari-Keith, Chief Executive Officer of Alala Advisors; and Emma Hunter, Health Emergency Preparedness Analyst at San Mateo County Health, to talk about their recently published qualitative analysis of California public health officials' experiences of harassment during COVID-19. NACCHO's Forces of Change survey found that 60% of local health departments reported their agency, leadership, and/or staff were targeted with harassment during the COVID-19 pandemic. In fact, one study found that 20% of Americans felt that harassing public health leaders was justified early in the pandemic; this rose to 25% in 2021. Even though this harassment crisis surfaced three years ago, the impacts on individuals, communities, and the public health system persist. NACCHO is drawing attention to this urgent issue. The study aimed to collect qualitative data highlighting the harrowing stories from local health officials and to better understand their experiences of harassment. The pandemic not only exposed vulnerabilities in the public health and healthcare system, but it also subjected local health officials to psychological impacts, systemic backlash, and burnout.

Dan Savage is a sex-advice columnist and podcaster, and Ina Park is a sex positive STI researcher, physician, and author. The two come together in this episode to discuss desire, pleasure, and how to communicate about what feels good with partners and providers. Dan delves into discovering kinks, pleasure as we age, and trying new things alone and with partners. Ina reflects on her experiences as a provider, having conversations with patients around sex and pleasure as bodies, needs, and abilities change.   Dan says that gay people might be better at sex, “not because we're magic...we use the 4 magic words ‘what are you into?'” Ina explains that honest communication with a partner, a physician, a sex columnist, or a therapist knocks down barriers to explore sex and discover pleasure. We can utilize communication to set new expectations and change narratives in media about expectations of someone positive for an STI or start even sooner to get them the support that can mitigate the need for SPFPP's existence as it initially began . . . supporting people through the mental health challenges of navigating an STI diagnosis. We need to integrate these learned communication skills SPFPP's guests have shared into STD Prevention efforts.

Episode 289 of Something Positive for Positive People is the trigger warning I shouldn't have to make. If any of what I say comes as a surprise to you, I don't think you really get the gist of what my work is about. Herpes is what led us here, but getting through it is going to require some self-evaluation of identity. The psychology of the self-image is what I challenge people on. Everyone should be triggered by this podcast's existence until you get to a point of challenging what that trigger is. I believe that same intense energy that comes from our fight or flight response is the same energy that can be utilized to trace the trigger to the source of it which is the trauma. I done worked too damn hard on myself to get to this place of being able to just exist for someone to tell me something I say MIGHT be triggering to someone. Especially when this podcast is about health, and I ALWAYS speak about health, I'm supposed to shut down sharing my experience because someone might what? Be challenged and take different action? I'm willing to take the gamble that someone views me differently or chooses to pursue support from another resource. But in all honesty with the way people come and go (as they should) I don't think I'll be listening to anyone who isn't doing one of those four letter F words I mention in this podcast to me. That all aside, SPFPP is no longer paying for people to get therapy. I bring on my friend Emily who worked on this campaign with me advocating for sex-positive, identity validating health care. I brought Emily on to discuss the challenges that exist as patients going to providers because they've worked on this type of advocacy for years through theater and work of course. This episode serves as an accountability piece for myself to move forward with my business idea of bridging the gap between health care providers and patients by giving providers experience talking about sex and sexual health with patients. I want to bring this back because integrating our experiences post-diagnosis into early interventions of STD Prevention. This means that creating confidence navigating sex in the health care field will expand to our patients and expand in our education early on when it becomes more and more prevalent to our connections to others and I believe this outside in approach is an efficient pathway to the necessary self-sex education needed prior to exposure to an outcome that warrants questioning.

Tammy interviews Jenn Rogers, director of the National Coalition for Sexual Health (NCSH), and Bryce Furness, MD, CDC epidemiologist, about their work developing a new toolkit for primary care providers to use in order to center sexual pleasure, problems, and pride as a part of all wellness visits. The CDC encourages taking sexual history by asking about 5 Ps: partners, practices, past STI history, protection from STIs, and pregnancy intention. The NCSH recently released a video series called “A New Approach to Sexual History Taking,” along with a set of new questions for providers to ask patients in all wellness visit that add a 6th P  specifically focusing on patients' pleasure, pride, and problems.   In this episode, Jenn and Bryce discuss the 6th P, which re-centers patients' needs, enjoyment of their sexual lives, and social stigma attached to sex. As Jenn elaborates in the episode, “we really thought a satisfying pleasurable sex life is really a key element to sexual health and well-being for most people. Guest host Dana Cropper, current director of the California Prevention Training Center, sits down with two former CAPTC directors, Gail Bolan, MD, and Alice Gandelman, to discuss the founding of the CAPTC and the organization's work in supporting sexual healthcare providers over the last 33 years. We learn about the forces that shaped the sexual healthcare field. Our guests unpack how the PTC developed alongside the HIV/AIDS epidemic in the late 1980's; the surprising relationship between HIV work and STI response; strategies to meet sexual healthcare needs both within and outside of sexual health clinics; and the ever-present need for greater training for providers around testing and treatment of STIs. We learn about how behavioral interventions were centered in response to the HIV/AIDS epidemic, and the importance of centering social determinants of health and larger structural forces in prevention and treatment efforts. Beginning this project, I had a hypothesis that people who tested positive for an STI would not find the resources they obtained from health care providers useful if they've been given any resources at all. Only one response was yes to the information being useful, but with the caveat that it was very basic. Another consistency with the SPFPP 2021 herpes survey results was that people who test positive for an STI are more likely to not test positive for a second one. My reasoning behind that is because of the communication skills they develop through their own self-education about STIs leads them to vetting partners differently than they have in the past about sexual health. What you'll find over these podcast episodes is that the most useful resources in navigating sex after their diagnosis is that they didn't come from their health care provider at all. Whether trich, herpes, chlamydia, or HPV, the resources they received from their health care provider wasn't useful for them. In these podcasts you'll hear many of the guests share that their journeys led them to online resources and social media accounts talking about this very thing. This begs the question “What is it that these people who've tested positive for an STI are learning that makes 70% of them go on NOT to test positive for a second STI?”. How do we integrate their lessons into the existing CDC recommended STD Prevention efforts? I further hypothesize that while use of condoms is presented as the “best” protection against STDs, it is worth noting that my guests speak about using condoms with partners, yet still tested positive. A major difference is that they learned to communicate in their relationships about their needs and set expectations with partners intentionally rather than under assumptions. This means that they were initiating conversations about sexual health, PROPER barrier use, and STI testing routines.The CDC says to get tested and know your status, but it also says to remain abstinent and practice mutual monogamy. Many of the prevention efforts as they exist are assumed norms in sex around intercourse. Part of the communication people have expressed learning is that there needs to be an uncoupling of intercourse and sex, and then separate sex from pleasure. That last part I predict can be done in sex education. Our podcast guests from this series speak to how kink and BDSM negotiations for play between partners has nothing to do with sex most of the time. What happens in these negotiations is that language is being co-created for consent, people are identifying and asking for their needs to be met while determining if their play partner is capable of meeting their needs for the play being negotiated. Boundaries are set during these negotiations, and an established STOP or safe word is determined. During this vetting process, we identify potential red flags for abuse or green flags for a healthy interaction. Through the practice of these negotiations, people are able to move forward and have set expectations for an interaction and know that if anything goes wrong or is off, that's been pre-negotiated and they know where to go for support. Equipping people with this communication foundation could not only minimize STIs, but also rape, violence/harm, negative mental health impacts, abuse, and self-suffering in silence due to not knowing where to go for support. This podcast series has brought to the surface these needs here in Oregon which reflect much of the podcast's experiences that have been shared on SPFPP since 2017. Thank you to the team at the Oregon Health Authority for allowing me to explore these findings with the funding to put time into it and get these findings to back up the work being done through SPFPP. This sets us up to explore the need for integrating these experiences into prevention efforts.

In this episode, we're talking with Nixon Arauz, who is a PhD Student in Social and Behavioral Sciences at the Virginia Commonwealth University School of Medicine. Nixon's personal experiences have shown him how important things like housing, education, and local politics are for health and wellbeing and has prompted him to become a passionate advocate for improving social determinants of health through his research. Nixon got his bachelor's degree from Cornell, and a Master's degree from Columbia. After this he worked at the Office of Health Equity within the Division of STD Prevention at the Center for Disease Control and Prevention. I possess a deep love for Minnesota, but two winters there was enough for me.  I settled back in California, where I completed a fellowship in Sexually Transmitted Diseases at the UC San Francisco School of Medicine (UCSF).  After all this training it was time to get a real job.  I'm now an Associate Professor in the Department of Family and Community Medicine at UCSF.  I also serve as the Medical Director of the California Prevention Training Center and a Medical Consultant for the Division of STD Prevention at the Centers for Disease Control and Prevention.  A few years ago, I decided that my time on this earth would be best spent making people feel better about their sex lives, reducing stigma around STIs, conducting good science and sharing it with the world as best I can.  So I decided to try my hand at writing a book about STIs, Strange Bedfellows, and someone (who is not related to me) thought it was good enough to publish.  Writing a book is similar to pregnancy and childbirth; it's a hell of a lot harder than it looks, and when it's over you need a few years to forget how bad it was before you can think of doing it again.  I live in Berkeley with my husband and two sons.  If I had more time, I would plant vegetables, pickle them, knit and brew bone broth.  I don't do any of those things.  I do practice yoga, and feebly attempt to meditate from time to time, but most of my waking hours are spent parenting and thinking about syphilis.   If you'd like me to come and speak to your group about my book or generally about the topic of sex and STIs or sexual health, please contact me here Buy Strange Bedfellows ------------------------------------------------------------- 1:22 At LOG OFF, we are passionate about lowering social media's impact on mental health while teaching teenage users and their parents about how to navigate the vast inner -workings of life on social media. Is it really true that having a greater number of sexual partners necessarily means you have a greater risk for sexually transmitted infections (STDs)? Nope! It turns out that a lot of the things we think we know about STDs are just plain wrong, which is why this episode of the Sex and Psychology Podcast is all about the surprising secrets of STDs. I interviewed Dr. Ina Park, an associate professor at the University of California San Francisco School of Medicine. She is also a Medical Consultant at the Division of STD Prevention at the CDC, and Medical Director of the California Prevention Training Center. She is author of the new book Strange Bedfellows: Adventures in the Science, History, and Surprising Secrets of STDs. Some of the topics we explore in this episode include: The conventional wisdom is that STDs are caused by promiscuity—but is it really the case that having more partners necessarily translates to greater risk? Is everyone equally vulnerable to STDs? Do some people have more natural immunity? How does STD contact tracing work, and what kinds of reactions do people tend to have when a contact tracer informs them that they might have an STD? Potential contestants on The Bachelor are screened for STDs before coming on the show, and a positive result is the most common reason for disqualification; however, a lot of people receive false positives because certain tests have low accuracy rates. So is this a good idea or a bad idea? Can an STD really ‘highjack' your brain and change your sexual behavior to facilitate viral transmission? I interview a woman I met almost 7 years ago on a dating site. She didn't know I had herpes at the time, but eventually I shared with her that I had it and not long afterward, she found out she did as well. This is actually our first time meeting even though we've grown to become very close over the years. This is someone I connect with on a soul level and she's actually a person who heavily contributed to my spirituality/emotional intelligence spark of interest. Please bear with me through my sporadic coughs in this and the next maybe 6 episodes. I had something going on at the time and did my best to conceal. The harm caused by herpes simplex viruses (HSV) 1 & 2 often arises not from the pathology of the viruses themselves but rather from the stigma and shame associated with a positive diagnosis. In this episode, we attempt to lay a clear foundation for understanding not only how these viruses work but also what occurred to change the perception of them from “innocuous infection” to “dreaded disease”. Starting us off with his firsthand account is the incredible Courtney Brame, founder and host of Something Positive for Positive People, a non-profit organization and podcast that aims to provide community support, healing resources, and educational discussions around positive HSV and other STI diagnoses as well as larger issues in sexuality and physical and mental health. We then dive into the meat of the episode, tackling such questions as “how do these viruses hide out in your body?”, “what kind of treatment is available?”, “where did these viruses even come from?” and “why is there such a huge amount of stigma and what can we do about it?”. To help us address this last question is our other fantastic guest, Dr. Ina Park, Associate Professor, University of California San Francisco School of Medicine and Medical Consultant, Division of STD Prevention, Centers for Disease Control and Prevention. Today’s podcast focuses on contact tracing, a longtime tool used by public health officials. During this pandemic, contact tracers identify people infected with the coronavirus and then contact others they’ve interacted with recently. Contact tracers then help people get testing and offer support for self-isolating. While every state receives some funding from the federal government to support contact tracing, states have the flexibility to manage their contact tracing plans differently. At least 17 states and the District of Columbia have introduced legislation related to contact tracing, with at least and 11 states and D.C. having enacted these measures so far. Our guest is Dr. Kyle Bernstein, chief of the Epidemiology and Statistics Branch in the Division of STD Prevention at the CDC and an expert in contact tracing. This project is supported by the Centers for Disease Control and Prevention of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $120,000 with 100 percent funded by CDC/HHS. He is the former Chair of the Nashville Community Health and Wellness Team and the current Topic Network Chair for Health Promotion for the Society for the Study of Emerging Adulthood. He has studied human physiology intensively for over 30 years and has developed a profound understanding of human functioning from a biochemical perspective. Dr. Malone has brought these physiological insights to the area of relationships and sexual health. Relatedly, he has had the honor of working with youth and young adults on their overall well-being for over 30 years as well. Perry N Halkitis, Ph.D., M.S. is Professor of Applied Psychology, Public Health, and Medicine, Director of the Center for Health, Identity, Behavior & Prevention Studies, and Associate Dean at New York University. He is also an affiliate of the Center for AIDS Research and Center for Drug Use and HIV Research also at NYU. Dr. Halkitis is highly recognized for his work examining the intersection between the HIV, drug abuse, and mental health in LGBT populations, and is well known as one of the nation?s leading experts on substance use and HIV behavioral research. He is lead editor of two volumes: HIV + Sex: The Psychological and Interpersonal Dynamics of HIV-seropositive Gay and Bisexual Men's Relationships. (American Psychological Association, 2005), and Barebacking: Psychosocial and Public Health Perspectives (2006, Haworth Press). His book, Methamphetamine Addiction: Biological Foundations, Psychological Factors, And Social Consequences was published in 2009, and he is currently working on a new manuscript examining the life experiences of gay men who are long-term survivors of HIV/AIDS, The AIDS Generation (Oxford University Press). Author of over 130 peer-reviewed academic manuscripts, Dr. Halkitis? research has been funded by the National Institutes of Health, and Centers for Disease Control and Prevention, among others. Dr. Halkitis serves on the Committee on Psychology and AIDS of the American Psychological Association (APA), is a member of the Advisory Committee on HIV and STD Prevention and Treatment of the Centers for Disease Control and Prevention (CDC) and Health Services Research Administration (HRSA), as well as the National Institutes of Health (NIH) College of CSR Reviewers. He is the recipient of numerous awards from both professional and community-based organizations, and is an elected a fellow of The New York Academy of Medicine, The Society of Behavioral Medicine, and the American Psychological Association. Dr. Halkitis received his PhD in 1995 from the Graduate Center of the City University of New York, and in 2012 will earn his MPH in the field of epidemiology.

Host: Mark Nolan Hill, MD Guest: Jeffrey Klausner, MD, MPH Detection and treatment of HIV in the United States has improved dramatically since the peak of the epidemic here in the 1980s, but there is far more work to be done. HIV transmission is one area of heightened concern, particularly those cases spread by newly infected persons who remain undiagnosed. Though tests are available to diagnose HIV in the acute phase, many experts believe we must improve these tests to really make headway in reducing the incidence of HIV in the United States. Dr. Jeffrey Klausner, a deputy health officer and director of the STD Prevention and Control Services Section at the San Francisco Department of Public Health, joins host Dr. Mark Nolan Hill to lay out the path toward progress in detecting acute cases of HIV infection. Newer, more effective tests are available, but must be adopted around the United States, and as Dr. Klausner looks ahead, a point-of-care rapid exam, though still in development, could eventually change the paradigm for early-stage HIV detection.

Gardasil is the first cervical cancer vaccine ever developed. After it was approved in 2006, controversy has surrounded this vaccine. Its safety and effectiveness have been questioned, and the requirement that immigrant women take the vaccine has come to the forefront of both immigrant rights and reproductive justice organizing. These groups have also put this new vaccination requirement into a historical context. Advocates argue that this vaccine is a new chapter in a history of reproductive oppression that targets women of color and immigrant women.On this edition, we look at the intersection of reproductive justice and immigrant rights. And we hear from activists, doctors, attorneys and women most affected by the new vaccination. This program is made possible in part by The Reproductive Justice Fund at the Tides Foundation.
