Our conversation today welcomes Sara Jenkins and Mandy Underwood from the AIDS Response Effort (ARE). Our discussion sheds light on the organization's mission, services, and the ongoing battle against HIV and STIs in the Lord Fairfax Community Health District. AIDS Response Effort, Inc. (ARE for short) is a local grassroots nonprofit that has been serving the Northern Shenandoah Valley for more than 30 years. You may not have heard of ARE, but its staff and volunteers work tirelessly to ensure everyone has access to HIV care and prevention resources. While HIV may not be a top news story, it is still around and spreading, along with sexually transmitted infections (or STIs), in our region. ARE offers free testing services and prevention resources to everyone in the community, as well as free medical case management for those diagnosed with HIV. Mandy explains her dynamic role as a Prevention and Testing Specialist, emphasizing the challenges and rewards of their outreach efforts. She shares insights into the reasons behind the reluctance to get tested, notably stigma and lack of education, while noting the rise in STI cases and the critical need for regular testing. Sara, a Medical Case Manager, brings attention to the comprehensive support provided to those diagnosed with HIV, from immediate medical care to long-term case management. She shares touching success stories, narrating how the organization helps clients achieve undetectable viral loads, thereby transforming their lives. Both guests underline the importance of community support, funding, and volunteers in continuing their work. They highlight the organization's various community engagement efforts, including the recently opened LGBTQ+ Center, and discuss future fundraising initiatives. In this episode, Rodney McCoy, a Black queer man with over four decades of experience in HIV prevention and education, shares his journey as a Leatherman of color and discusses the intersection of BDSM, kink, and HIV prevention. From his entry into the kink community to becoming a titleholder in the American Leatherman competition, Rodney emphasizes how the kink community provided a safe space for self-discovery and empowerment. The episode explores the link between pleasure, power dynamics, and HIV prevention, highlighting the importance of honest conversations, sex positivity training, and the destigmatization of "risky behavior."   Rodney, a health educator, program director, adjunct professor, and researcher, emphasizes the partnership between healthcare professionals and patients in promoting sexual health. Rodney says, “I am about encouraging people to embrace all pleasure: sexual pleasure, pleasure that comes from good mental health, from good physical health and from good social connections. But, for me – it's also more exciting. And, having this conversation, and talking about sex can bring us joy; it can make us happier and more fulfilled people; it can enhance our relationships and our mental health; and it can literally save lives. My guest today – Dr. Joshua Rosenberger – is an expert on this subject. He is an associate professor of Biobehavioral Health at Penn State and the Director of the Survey Research Center. This was done with an awareness of the personal and contextual factors that increase sexual and gender diverse individuals' vulnerability for HIV/STI infection and poor care-related outcomes. His subsequent HIV/STI prevention work, which includes community education in both urban and rural areas, serving as the national spokesperson for HIV testing campaigns, and workforce development with about 1400 nurses across three continents, has been inspired by that study. Serving in numerous leadership positions for the Association of Nurses in AIDS Care is only one example of the man's career-long dedication to nursing leadership and HIV/AIDS care. BBC Media Action has launched a campaign to improve social acceptance and draw recognition to the work done by Bengaluru's waste pickers in an effort to improve their social standing. The campaign has been launched by BBC Media Action India, which is part of a larger collective called Saamuhika Shakti, funded by H&M Foundation, with the objective of improving the lives of informal waste pickers in Bengaluru. Partners of this collaborative include, CARE India, Hasiru Dala, LabourNet, Save the Children, Social Alpha and WaterAid along with The Nudge Foundation, serving as the backbone organisation for the intervention. BBC Media Action's campaign focuses on invisibles or waste pickers, as people of Bengaluru don't recognise the ‘humans behind waste'. And over the course of the project, turn them into #Invaluables. The overall objective of #Invaluables, which is an online community of Bengalureans, is to reveal categories of waste pickers and lift the shroud of invisibility surrounding them. With this initiative, Bengaluru will come closer to the waste picker community through content that will unpack the value and interconnectedness between the two. Radharani Mitra is Global Creative Advisor at BBC Media Action, the BBC's international NGO that uses media and communication for development. She mines insights, shapes strategies and tells stories using new and legacy media to bring about social and behaviour change – from 30 second adverts to social media campaigns, from graphic novels to mobile games, from radio shows to rural outreach activities. She has masterminded several short format ‘fast fiction' radio drama series in different parts of the world - on Ebola in West Africa, on sanitation in India and on Covid in Afghanistan and Somalia. She has also coneptualised two full-fledged TV dramas – AdhaFULL, a whodunit series on gender and Navrangi Re! on urban sanitation. Radharani has played a key role in designing BBC Media Action's mHealth innovations – Mobile Kunji, Mobile Academy and Kilkari, of which the latter two have been scaled up nationally by the Government of India. She has worked extensively across Asia and Africa country programmes on a variety of themes. Her work has won several Indian and international awards. Ragini Pasricha is the Director for Content Strategy at BBC Media Action. Combine domain knowledge of adolescent health, maternal and child health, family planning, HIV/STI prevention and nutrition with an understanding of behaviour change communication and communications research. Experience in advocacy with government to scale up RMNCH+A best practices and social and behaviour change communication. Sanjay is a sexual health & HIV activist who works mostly with gay black men to teach them about to bring awareness to HIV, STI and sexual health. He grew up in the Deep South and has a powerful story about his experience with becoming positive and being criminalized for it. When we originally connected, I had no idea about his story and what he had been through, being HIV positive and criminalized because of it. Again, we know the audio quality on this episode is less than our usual. It should be back to easy listening next week. Thanks for your patience.  We have a MAN on the show! Our first man on the FFF podcast is Sophia’s husband, Louie. In this episode he shares the hilarious and honest details of sperm collection! NOTE: The audio on this isn’t the best, but we PROMISE you it’s worth it for how funny and informative this episode is! Louie opens by sharing with us WHY he went to the sperm cryopreservation bank in the first place. Since he is active duty military and deploying to combat zones, our local San Diego cryobank does a great service to active duty families by discounting their cryopreservation services! Unfortunately, military members tend to be guinea pigs, and sometimes years and years down the line we find out that the meds/exposures/chemicals/etc actually harm fertility. Military members are exposed to toxins, radiation, and dangerous situations that can damage your ability to reproduce. Sophia and Louie decided to take advantage of this wonderful program and store some of Louie’s goodness before he left on deployment. For them, it was like an extra layer of insurance making sure that there would be healthy sperm available set aside in the event that something catastrophic occured to Louie while in the line of duty. First thing first was to run some tests on Louie to make sure his reproductive system was healthy. Cryobanks want current info on HIV/STI tests, as well as other things. Louie HATES blood draws, and he recommends doing he blood draw a few days in advance to reduce added stress on the day that you make your sample(s). For him, the stress of a blood draw really impacted his ability to climax and produce a sample. Of course he pushed through and made it happen in spite of the looming needle-poke, but it wasn’t easy! Listeners, we CANNOT STRESS THIS ENOUGH: test your semen early! Things happen, and it’s not always possible to make a quality sample on the first go! Sophia and Louie had a looming deployment date and didn’t discover the cryobank benefits until just before Louie had to leave. So, they were on a tight deadline that put added stress on Louie. If that is NOT your situation, go as early as possible in your TTC journey to ensure plenty of time to spread things out and get the best possible sample! It’s important to note that before you go in to make your sample, you need to abstain from sex or anything that could result in ejaculation for 2-5 days before you go to the cryobank. You also need to avoid any hot baths/jacuzzis/sauna/etc. As well as trying to avoid stressful situations. Stress greatly impacts your semen quality and volume. This can put an extra pressure on men! When you’re TTC, it’s all about whether or not the man can orgasm. Women can get pregnant from sex whether or not they climax, but if the man doesn’t orgasm and ejaculate; no joy. The clinic we went to was a fully functional urology office! Turns out this clinic is also the urologist for the local men’s penitentiary! Louie was busily focused on his paperwork while a parade of shackled convicts were streaming through the lobby. Sophia was VERY uncomfortable, but thankfully Louie never looked up from his paperwork and was totally oblivious. The sample collection room was clean… but it made you feel dirty! There was a creepy recliner that was made of some kind of material that could be easily wiped. Also, a stale collection of pornographic magazines from the late 90’s. Important to keep in mind: the skin of the penis must be sterilized with wipes, and absolutely no lubricant, lotion, salvia, or vaginal secretions can be present. Though Sophia initially went into the sample room with Louie, this ended up not working out when Sophia had to run down to the truck and get Louie’s headphones.  Other recommendation: bring headphones! The walls are THIN in these cryobanks, and it’s hard to reach climax when you can hear doors opening and closing, clinicians talking, and other distractions. This is all mixed with the fact that you’re told again and again to “try and relax” because samples are greatly affected by stress. Miraculously, Louie was able to make a sample while Sophia was running down to get the headphones! After the sample was produced, the clinician analyzed the quality and told Sophia and Louie about it in the most AWKWARD way possible! Lo and behold, Louie’s sperm were healthy and his semen quality was stellar! Praise the Lord! Next, you have to decide what you want to do with the sample. For instance, samples that will be used for at home insemination or IUI are processed differently than samples destined for IVF. Because Sophia and Louie didn’t have time for extra samples, they decided to preserve the sample for IVF, since the sample was only enough for 1 IUI vs. 4 IVF cycles. After ALL of this, Sophia and Louie found out they could have done a home collection! Facepalm. If you’re like them, and you prefer the comforts of your own home, do the dang sample at home! As long as you can get it to the clinic within an hour, your sample can be successfully collected at home and used. Something important to note is the legal side of cryopreservation. If you are using the semen right away, or simply analyzing it to see if babies can be made from that sperm; it’s not an issue. If you’re freezing it for future use, a lot of other legal factors can arise. It could be worthwhile to sit down with your lawyer and go over the paperwork prior to the sample to sort out any issues that could arise down the line. Again, that’s only if you’re planning to freeze the sample for future use like Louie and Sophia. In this episode especially, we take plenty of time to laugh and make silly comments about fertility, sperm, and masterbation. We do this because fertility struggles are HARD! We believe that laughter is medicine, so though we fully embrace the sorrow and anguish that can come along with struggling to conceive, we want to make the process more enjoyable by finding ways to laugh.

Dr.Anderson's research targets the relationship between patient-provider communication practices and clinical and quality of life outcomes among women and adolescents of color. She studies the ways in which patients and medical care providers share power and responsibility to achieve patient health goals in sexual health and chronic disease management. She has been the co-investigator for two federally funded studies to examine patient-provider sexual communication and sexual quality of life among women with early-stage, hormone receptor-positive breast cancer. She has also conducted research to explore how adolescents use social media for health information seeking and sexual risk reduction. Dr. Anderson's research program also includes HIV/STI prevention interventions for people of color,specifically Black women, adolescents, and sexual minorities. On PopHealth Week series on health equity, the social determinants of health and associated disparities exacerbated by the Covid-19 pandemic Dr. Flores discusses the impact on the LBGTQ community. Dr. Dennis Flores is an Assistant Professor at the University of Pennsylvania School of Nursing, an affiliated faculty at the Gender, Sexuality, and Women’s Program, and is a Visiting Professor at the Center for Research on AIDS at Yale University’s School of Public Health. He has led several studies that investigates the role of parents in the sexual health education of their adolescent sons who identify as LGBTQ. Through interventions around inclusive parent- child sex communication, he believes that the early provision of sexual health information attuned to the attractions, behavior, and identities of LGBTQ adolescents can reduce the formation of risky sexual behavior and this population’s risks for HIV and STI infection. With an awareness of the personal and contextual factors that increase sexual minority populations’ vulnerability for HIV/STI infection and poor care-related outcomes, the first study Dr. Flores led investigated the conditions that contributed to the recent HIV infection of young gay men in Atlanta. That study has fueled his subsequent HIV prevention work that includes both urban and rural community education, being a national spokesperson for HIV testing campaigns, and workforce development with around 1200 nurses across three continents. His commitment to HIV/AIDS care and leadership throughout his career includes serving in various leadership roles for the Association of Nurses in AIDS Care. Dr. Flores earned his PhD from Duke University, a Masters in Public Health Nursing Leadership at Emory University, and a Bachelors in Nursing from Kennesaw State University.

On the eve of Transgender Day of Remembrance, Teddy Cook, the Manager of Trans & Gender Diverse Equity at ACON, talks us through the latest developments in peer-led policy, research and law reform. In a context where trans women of colour and sex workers experience the greatest risk of violence, the conservative right is pushing for ‘religious freedom’ to discriminate, trans kids have born the brunt of backlash against marriage equality and our Prime Minister has opposed human rights guidelines for trans inclusion in sport, we discuss the role of grief, the need to end cis heteronormativity, toxic masculinity and white fragility, and strategies for mobilisation and justice. Teddy introduces new findings from the largest Australian sexual health survey for trans and gender diverse people and explains what a peer-led partnership approach looks like in methodology, design and process. He explains recent law reform achievements (and next steps) in changing birth certificates and accessing gender affirming care as well as new initiatives, including a specialised trans and gender diverse health service that offers peer-run, free, rapid, HIV/STI testing and hormone level testing. Whilst advocating for appropriate questions in data collection so we can ensure culturally competent care and tailored health programming, Teddy explores how we can shift away from deficit-based, pathologizing gatekeeper models and why we should be talking about gender euphoria instead of gender dysphoria.

Since then I’ve been an organizer, writer, educator, speaker and activist in the LGBT, leather/kink, polyamory and HIV/STI prevention realms. On January 18, 2018, we had the privilege of hosting Jamila Perritt MD MPH FACOG for our annual Roe v. Wade celebration. She spoke about her experience as an abortion provider, shared her story along with stories of the women she takes care of and highlighted why reproductive justice is what we all need to be working towards. Jamila Perritt MD MPH FACOG is a board-certified Obstetrician and Gynecologist who specializes in Family Planning and Reproductive Health. Dr. Perritt is a graduate of Howard University College of Medicine and completed her residency training in Obstetrics and Gynecology at Pennsylvania Hospital in Philadelphia, PA. She completed a Family Planning Fellowship at The Johns Hopkins University in Baltimore, MD. During her fellowship training, Dr. Perritt received a Masters Degree in Public Health from The Johns Hopkins Bloomberg School of Public Health. After completion of her fellowship, she returned to her hometown of Washington, DC where she served as the Medical Director at Planned Parenthood of Metropolitan Washington DC, Inc. from 2011-2016. Dr. Perritt works as a clinical provider in Washington, DC, Maryland and Virginia. Additionally, she serves as a reproductive health care consultant where she develops, organizes and facilitates health education workshops and outreach events to diverse communities and community partners on topics including but not limited to HIV/STI treatment and prevention, reproductive justice and access to care, adolescent health, contraception and family planning. She collaborates with various organizations to provide ongoing support and subject matter expertise on sexual and reproductive health, family planning and reproductive justice.

Joining “CAN WE TALK FOR REAL”, cohosts Teresa Jackson and Michelle E. Brown on Wednesday, February 3, 2016 to address the Asian/Pacific HIV/AIDS awareness strategy and [Prep] will be Jens Tamang from the Minnesota Aids Project who before joining MAP he provided care coordination for individuals living with HIV in adult foster care and served as a Projects Administrator for several LGBTQ youth programs funded by the Minnesota Regional Arts Council. Joining him will be also be Kayden Hoang Bui he will discuss his vision on how to expand the MAP’s work into hard-to-reach populations to promote HIV/STI education, awareness, and testing through increased outreach and media presence.In recognition of Feb.7th National Black HIV/AIDS Awareness Day, joining CWT4R, will be Pastor Anthony Sullivan Jr. a Pastor-Leader, Preacher, Challenger, Trainer, Teacher, Resource, Initiator, Counselor, Guide and a justice advocate committed to the liberation of persons and groups who suffer from all forms of oppression. He is a graduate of the Samuel DeWitt Proctor School of Theology at Virginia Union University having earned the Master of Divinity degree. He is especially concerned about those impacted and affected by the pandemic of HIV/AIDS.  He will talk about, “What does faith has to do with the mobilization initiative aimed at Blacks to help reduce the HIV/AIDS spread and to bring more awareness”.

"Implementation of pre-exposure prophylaxis (PrEP) and other HIV prevention initiatives in Rhode Island" The presentation will focus on recent advances in HIV prevention including post-exposure prophylaxis (PEP) and pre-exposure prophylaxis (PrEP). In Rhode Island, we have implemented several new initiatives to address HIV and other sexually transmitted infections (STIs) including a new PEP/PrEP program and free/accessible HIV/STI testing. Our PrEP program is among the first in the country to offer the medication on a clinical basis to at-risk individuals. One of the goals of the presentation will be to identify areas for collaboration between Rhode Island and Connecticut.
