Leaving home is never easy—especially when discrimination, xenophobia, and cultural barriers await you in your new destination. In this episode, Zero Hour™ speaks with two leaders who are working to not only ease the transition for migrants and displaced people, but to also connect them with vital HIV services and resources. Our guests are: Denis Onyango, a grassroots activist who organizes HIV testing, prevention, and peer support services through his role as Programs Director for the Africa Advocacy Foundation. He reflects on how losing his brother and three sisters to AIDS spurred his focus on HIV activism, as well as the harsh realization that the care they went without was widely available in other parts of the world. Isjed Hussain, founder of the Prisma Group Foundation, which supports bicultural and Muslim LGBTQ people in the Netherlands. They explain how Prisma helps refugees to navigate the asylum process and how they assist refugees living with HIV. This podcast was created and fully funded by Gilead Sciences, Inc. You do it. Your parents did it. Even the Puritans did it…so why are we so puritanical when it comes to talking about sex? In this episode, we're bringing it all out of the bedroom and discussing how openly talking about sex is a necessary first step to talking about HIV. Sex positivity, health exploration, eliminating stigmas and taboos—they all start here, with the birds and the bees.  Our guests are:  Nathan Hale Williams, an actor, film and television producer, and entertainment producer known for writing and co-directing the film 90 Days, as well as directing the 2021 short film adaptation of All Boys Aren't Blue. He discusses the social challenges that limit critical conversations about HIV and his tactics for authentically portraying the experiences of people living with HIV in his work.  Dr. Cedric Pulliam, the Director of Global Public Policy at Prevention Access Campaign, where he works to advance global LGBTQ+ and human rights policies. He explains the impact U=U can have on international HIV strategy and the intrinsic relationship between sexual health and health equity.  Goody Howard, a world-renowned sexologist who normalizes sex-positivity through sexual skill workshops and professional development programs. Back in our March 2020 episode, This is Hope, we spoke with Dr. Larissa May about HIV screening in the ED. Screening programs, along with education and other resources, is making a difference, but there are still tens of thousands of new HIV cases in the U.S. each year, and probably 1-2 million new infections annually worldwide. There are clinical trials in progress studying potential HIV vaccines, which is exciting, but a widely available vaccine is still many years away. Luckily, we DO have a safe and effective way to help prevent new infections and transmission - pre-exposure prophylaxis (PrEP). Dr. Michael Casner joins us to share his expertise in all things PrEP, so we can help our patients stay safe and healthy.  Do you talk to your patients about PrEP? Both HIV Infection and Conditional Tat Expression Decrease Prepulse Inhibition with Further Impairment by Methamphetamine History in Humans

Link to bioRxiv paper: http://biorxiv.org/cgi/content/short/2020.06.11.130666v1?rss=1 Authors: Walter, T., Young, J., Milienne-Petiot, M., Deben, D. S., Heaton, R., Letendre, S., Grelotti, D., Perry, W., Grant, I., Minassian, A. Abstract: HIV infection and methamphetamine (METH) use are highly comorbid and represent a significant public health problem. Both conditions are known to negatively impact a variety of brain functions. One brain function that may be affected by HIV and METH use is sensorimotor gating, an automatic, pre-conscious filtering of sensory information that is thought to contribute to higher order cognitive processes. Sensorimotor gating is often measured using prepulse inhibition (PPI), a paradigm that can be conducted in both humans and animals, thereby enabling cross-species translational studies. While previous studies suggest HIV and METH may individually impair PPI, little research has been conducted on the effects of combined HIV and METH on PPI. The goal of this cross-species study was to determine the effects of METH on PPI in the inducible Tat (iTat) mouse model of HIV and in people with HIV. PPI was measured in the iTat mouse model before, during, and after chronic METH treatment and after Tat induction. Chronic METH treatment decreased PPI in male but not female mice. PPI normalized with cessation of METH. Inducing Tat expression decreased PPI in male but not in female mice. No interactions between chronic METH treatment and Tat expression were observed in mice. In humans, HIV was associated with decreased PPI in both men and women. Furthermore, PPI was lowest in people with HIV who also had a history of METH dependence. Overall, these results suggest HIV and METH may additively impair early information processing in humans, potentially affecting downstream cognitive function. Copy rights belong to original authors. Virtually eliminating the spread of HIV? Ten years ago ,the idea that we could end new cases of HIV spread through sexual contact would have been seen as a pipe dream. But, thanks to pharmaceutical advances, a new drug in the United States has the potential to make those dreams a reality. Pre-exposure prophylaxis, more commonly known as PrEP, is a pill that, if taken daily, can reduce the risk of contracting HIV from sexual contact by up to 99%. In 2014, the Centers for Disease Control and Prevention reported that 68% of new HIV cases can be attributed to men who have sex with other men. PrEP is meant only for people who are currently HIV negative and is targeted towards men who have sex with men as well as those that that have sex with men engaged in same sex activity. PrEP was among methods listed when the United States Department of Health and Human Services released a plan in 2019 with the goal of slowing the spread of HIV. While the drug is relatively new to the market, awareness

Host: Mark Nolan Hill, MD Guest: Jeffrey Klausner, MD, MPH Detection and treatment of HIV in the United States has improved dramatically since the peak of the epidemic here in the 1980s, but there is far more work to be done. HIV transmission is one area of heightened concern, particularly those cases spread by newly infected persons who remain undiagnosed. Though tests are available to diagnose HIV in the acute phase, many experts believe we must improve these tests to really make headway in reducing the incidence of HIV in the United States. Dr. Jeffrey Klausner, a deputy health officer and director of the STD Prevention and Control Services Section at the San Francisco Department of Public Health, joins host Dr. Mark Nolan Hill to lay out the path toward progress in detecting acute cases of HIV infection. Newer, more effective tests are available, but must be adopted around the United States, and as Dr. Klausner looks ahead, a point-of-care rapid exam, though still in development, could eventually change the paradigm for early-stage HIV detection.
