I want to ask if resting for three weeks, complete bed rest and I can afford to do this because I am a student, and it is summer holiday then I can walk to my college to do daily normal tasks. I do not do any physical training or running and my college is about a kilometer away.” Will three weeks of bed rest heal a stress fracture?  Well, that is a great question and that is what we're talking about today on the Doc On The Run Podcast.

The ACOG defines Fetal Growth Restriction (FGR) as fetuses with an estimated fetal weight or abdominal circumference that is less than the 10th percentile for gestational age. Currently, the ACOG, SMFM, FIGO, and the ISUOG do not recommend maternal bed rest for this condition. However, a recent publication from the AJOG (May 2024) concluded that strict maternal bed rest could in fact reverse FGR in a short two-week interval! Is this possible? In this episode, we will dissect this controversial, retrospective study and give it proper perspective with real world clinical implications.

In this week's episode we're hearing from Leonora just 8 weeks after the birth of her daughter. She shares about her conception journey including the difficulty of her 4 early losses and the anxiety that came with her subsequent pregnancy; midway through which Leonora was diagnosed with an incompetent cervix. She underwent a cervical cerclage to prevent her going into preterm labour and was put on bed rest, and then house rest, which she understandably found very isolating. Leonora chose to birth her baby via c-section which she describes as the best experience of her life! Professor Ben Bikman discusses sarcopenic obesity, a condition involving obesity and muscle loss. This condition results from factors like sedentary lifestyles, aging, and metabolic disturbances. While obesity is common, sarcopenia typically affects the elderly, diseased, or very sedentary individuals. The combination of excessive fat and muscle loss makes sarcopenic obesity particularly challenging.Dr. Bikman explains the crucial role of muscle in blood glucose regulation. Muscle mass reduction impairs glucose control, leading to higher blood sugar levels and increased insulin resistance. Even a short period of bedrest can significantly reduce muscle mass and insulin sensitivity. Inflammation from enlarged fat cells also contributes to muscle loss and insulin resistance, creating a vicious cycle.Insulin resistance and sarcopenic obesity can both cause and result from each other. Insulin resistance impairs muscle protein synthesis and promotes fat cell growth, leading to further insulin resistance. Reduced muscle mass and increased fat cell size negatively impact metabolic health. Bikman stresses the importance of diet in managing sarcopenic obesity, advocating for a low-insulin diet by controlling carbs, prioritizing protein, and not fearing fats.To combat sarcopenic obesity, Dr. Bikman recommends proper nutrition and resistance training. Reducing insulin levels helps preserve muscle mass and promote fat loss. Resistance exercise is more effective than aerobic exercise for improving metabolic health. She wanted to share this story to bring light to the world of being a NICU parent. Jes is a wife and mother of two children, postpartum doula and pediatric sleep consultant, and an passionate plant and pug lover. SummaryJes shares her birth stories, focusing on her second pregnancy and birth. Her first birth was a natural, birth center birth with midwifery care. For her second pregnancy, she experienced complications with high blood pressure and had to switch to an OB. Her pregnancy was considered normal until 20 weeks when a routine scan showed her cervix was shortening, exacerbated by the pressure of excess amniotic fluid. The following day a cervix stitch was inserted and Maria spent the next 10 weeks in hospital. She talks about the power of meditation and the practises she embraced to stay present and positive. Her boys were born at 33 weeks and are now healthy, happy four-year-olds who speak three languages. Ashley requested bedrest at 38 weeks due to the stressful work she does as a social worker. She was induced at 39 weeks, and was in labor for over 40 hours. Her epidural failed twice, so she felt it all. She credits her amazing playlist to get her through!Ashley's postpartum experience in the hospital was very negative, and 4 years later, she now knows she is one and done. Darüber muss natürlich in all seiner Breite und Ausführlichkeit diskutiert werden, weshalb sich Tino und Schröck zwei frische und längst vertraute Gäste auf die Sofas geladen haben: Lucas Barwenczik und Fynn Benkert. Der eine hat schon einen Film gedreht, der auch schon in KINO+ Thema war, der andere kämpft mit Daniel jedes Jahr aufs neue im CUTS-Podcast um die besten Filme des Jahres. Und darüber hinaus haben die beiden noch jede Menge andere Eisen im Feuer, was anhand dieser doch so eloquenten wie schwafeligen und dann eben auch umfangreichen Ausgabe bewiesen werden soll. Na ja und Tino schafft es dann natürlich auch noch, ein bisschen Werbung für seine kommenden Screenings zu ADIPURUSH zu machen. Abgerundet durch das, was die Vier zuletzt gesehen haben. Zum Beispiel WINTERS BONE oder PAST LIVES und THE MOUNTAIN, komplettiert durch Lucas' Empfehlungen für das Japan FilmFest, dass dieses Wochenende in Hamburg stattfindet. Laut Lucas sollte man sich dort NOT BEER, TEN ROOMS und HOWLING nicht entgehen lassen, beziehungsweise waren dies 3 Highlights, die er dort als Jury-Mitglied mitgenommen hat. Ein geordnetes Chaos, das natürlich mal wieder ganz anders geplant war, aber wie so oft als „es ist was es ist” geendet hat. In episode 342 Laura details her experience with gender disappointment, a confronting experience that she never anticipated but one that she shares without reservation. After being diagnosed with placenta previa, a scan at 32 weeks revealed that she had a shortened cervix. Bed rest and progesterone pessaries definitely helped and she was induced at 37 weeks and experienced a quick and positive vaginal birth. She fell pregnant with her daughter two years later and regular scans showed that her cervix was shortening quite quickly so she went on strict bed rest at 23 weeks. Today's show features a guest that can give us some perspective on the current climate around two important topics: abortion and postpartum depression. She's a psychiatric nurse practitioner, who experienced an abortion in the 1970s and peripartum and postpartum depression in the 1990s. She's written about her experience. In a piece, titled “No Stranger”. Here are some excerpts from her writing. First, she writes:“How do you know?” the patient might ask.
