Welcome! Today, we're discussing my concerns with a single dose of doxycycline for a tick bite. Case Study: Ineffective Prophylactic Treatment I had a patient who took a single 200 mg dose of doxycycline within 24 hours of removing an engorged tick, thinking it would prevent Lyme disease. Unfortunately, seven months later, he was chronically ill with Lyme disease. He never developed a rash, and his lab tests for Lyme disease were negative. He eventually resolved his chronic illness with doxycycline followed by a combination of atovaquone and azithromycin. Co-Infections and Their Impact Ticks can harbor multiple infections, not just Lyme disease. In the Northeast, common co-infections include Ehrlichia, Anaplasmosis, Babesia, and Bartonella. These can lead to acute and chronic illnesses such as Lyme arthritis, Lyme carditis, chronic neurologic Lyme, Postural Orthostatic Tachycardia Syndrome (POTS), and Pediatric Acute-onset Neuropsychiatric Syndrome (PANS). The Need for Effective PreventionEffective prevention following a tick bite remains a challenge. One study found that a ten-day antibiotic course failed to prevent Lyme disease. Another study suggested that a single 200 mg dose of doxycycline could be effective if taken within 72 hours of a deer tick bite in endemic areas.However, a single dose of doxycycline hasn't been shown to prevent common Lyme disease manifestations like Lyme arthritis, Lyme carditis, Lyme meningitis, POTS, or PANS. It also doesn't prevent co-infections such as Babesia. Concerns with Single Dose ProphylaxisA single dose of doxycycline raises several concerns. It may prevent the erythema migrans rash, which is crucial for early diagnosis and treatment. It might also prevent a positive test result needed to confirm Lyme disease early.Discussing Treatment Options I discuss with my patients the risks of prophylactically treating with a single dose of doxycycline versus not treating at all. I've seen patients return with acute and chronic neurological symptoms after a single dose of doxycycline.I generally follow two approaches. Some patients are monitored closely without treatment and followed up after a month to check for any signs of Lyme disease or co-infections. Blood tests can be helpful in these cases. Other patients are treated for 3 to 4 weeks with a follow-up to assess the outcome. Patients and their families unfamiliar with Lyme disease are more likely to opt for monitoring. Those with prior experience with Lyme disease often agree to antibiotic treatment with follow-up. Conclusion We need a preventive measure that effectively prevents both acute and chronic manifestations of Lyme disease and its co-infections. Shuailin Li, a PhD Student at the Jenner Institute in the group of Prof Helen McShane, studies the link between cytomegalovirus (CMV) infection and M.tuberculosis infection or tuberculosis(TB) disease. Infections rarely, if ever, happen in isolation: humans are infected with 10 chronic viruses on average and this imprint shapes our response to other pathogens. On this episode, find out how data from a vaccine study cohort uncovered a detrimental association between CMV infection and TB, the likely immunological mechanisms for this effect and the possible implications of these findings in endemic regions.
Why are chronic, persistent infections like Lyme, Bartonella, Babesia and others like long-COVID and Epstein-Barr Virus so difficult to diagnose and treat? Infectious Disease specialist, Dr Bela Chheda, MD, walks us step by step through testing options including Elisa, Western Blot, PCR, Immunoblot (Igenix, Vibrant America), FISH testing for Babesia (Igenix), and some of the uncommonly tested parts of the immune system like the T-cells (InfectoLab). She also reviews how these bacteria can hide deep within tissue compartments and intracellularly, and that they can change themselves to hide from our immune system as well as changing our immune system itself. This flows right into the discussion about treatments with a focus on antibiotics: when to start them, when to stop them, how long they may be used for and what they can or can't do for recovering from chronic persistent infections. Dr Chheda explains why the CDC-criteria for Lyme disease set in the 1990's to target surveillance of population health lacks the sensitivity to find the cases of Lyme disease out there that can be responsive to treatment and discusses the critical importance of interpreting all test results in the clinical context of the individual patient. This one-hour show cracks the nut on how to think about testing and treating of these tricky vector-borne diseases that can trigger a chronic persistent infection that impedes optimal health.

In this week's podcast, Sarah speaks with Dr. Thomas Moorcroft, a physician from Connecticut who specializes in Lyme disease and Lyme-related infections. He explains what co-infections are and differentiates between infections that are contracted from a tick bite, and concurrent infections that people with Lyme disease may experience. During the 2020 International Lyme and Associated Diseases Society (ILADS) conference, he presented several cases of known tick bites in which individuals tested positive for 3-5 different pathogens. He explains the implications of having more than one infection on disease expression and test accuracy. Dr. Moorcroft talks about Bartonella, a co-infection of Lyme disease that can be transmitted not only from ticks, but also from fleas, lice and bites or scratches from cats - hence its other name “Cat scratch disease”. He explains some of the symptoms of Bartonella, including sudden behaviour changes in groups of children after a lice infection has gone through their classroom. Beyond some of the unusual symptoms of Bartonella infection such as foot pain and muscle fasciculations, Dr. Moorcroft points out the often debilitating cognitive effects of the disease. He gives some pointers on clinical signs helpful in the diagnosis of Bartonella, including characteristics of skin changes that occur due to the infection.Finally, Dr. Moorcroft explains a mentorship program through ILADS, in which physicians, often in Lyme endemic areas, share their clinical knowledge and time with physicians who want to learn more about how to treat Lyme disease and co-infections. Gas, bloating, diarrhoea, constipation, reflux. Modern allopathic medicine still does more harm than good for these common and chronic health complaints. I often think about what my life might be like now if I’d listened to my gastroenterologist and written off diet, lifestyle and infections as a potential cause of my gut issues. I wonder if by now I’d still be taking anti-inflammatory steroids or if I would have gotten to the point where I needed surgery. A better solution was to take a deep dive into gut testing to uncover the root causes underlying my bloating and diarrhoea. Stool testing allowed me to find amoebic and pinworm infections, urinary organic acids showed yeast overgrowth. Together with some diet and lifestyle changes these problems were treatable with botanical herbs and with just one prescription medicine. My guest today is physician, researcher, author, critical thinker, educator and health enthusiast Dr. Michael Ruscio. Michael is a functional medicine practitioner who emphasizes nutritional and lifestyle solutions to the types of problems described above. He runs a busy practice in Walnut Creek, California where he spends two days a week seeing patients and three days researching and writing. Together with a small team of researchers, Michael is in a unique position to help people through his experience both as a practitioner and researcher. As a result, we're seeing low levels of 25-OH D on a blood test, but that's because it's all been activated, which is potentially immunosuppressive. There's also some evidence that inflammation (certain inflammatory cytokines) upregulates the enzyme CYP27B1, which creates 1,25-OH D.
