“Sometimes you need to lose who you think you are so you can rediscover who you are.” Welcome to Kankakee, IL, at one time ranked the worst city in the U.S. Growing up moving around a lot, with little dreams of a big city, TiRon found himself getting lost in creativity as a way to escape the world around him. He was always drawn to music - not to any genre, but to how it made him feel. His natural curiosity for people and experiencing new things was his way out of his small town, and his mind… Welcome to South Side Chicago, where gangs and violent crimes were at an all time high. But as kids, we don’t always have a concept of danger. For Omari, this was simply his neighborhood, like any where else. Inspired by his loving mother, she introduced him to music at an early age, from Tina Marie to Masta P. But where Omari really got lost was in his drawings and passion for art. Both kids were receptive to the world around them, yet so different how they ingested it. It wasn’t until TiRon moved to LA where his thought was challenged, and he got his own identity. He always wrote and recorded songs, but it was the stage that allowed him to “lose his mind”. For Omari, his drawings would soon turn into writings, and those writings would turn into lyrics. Music was infectious, and it was a matter of time that this new path would lead the way. From online chatrooms and message boards, to bringing a community together to push the culture forward, there’s a very real force you feel when these two brilliant minds come together. You can call it vulnerability, creativity, or authenticity. Or you can just call it freedom. But this is a story about something that gives us purpose, and fulfillment. Health Story Collaborative Podcast Episode 5: Tiron’s Story (Originally aired on the WBUR Commonhealth Blog). In this segment, we hear from Tiron, whose wife, Marie, was featured in our last episode. Marie was diagnosed with Stage IV colorectal cancer in 2007 at the age of 45, and lived bravely with the disease for nine years. In December of 2016, the cancer took her life. In this episode, Tiron offers his perspective on the challenges of marriage and parenting amidst chronic, and ultimately fatal, illness. He demonstrates tremendous flexibility in adjusting to his “new normal,” the new responsibilities and new expectations for the future brought about by his wife’s diagnosis. This podcast is a product of Health Story Collaborative, a nonprofit organization dedicated to harnessing the healing power of stories. The Six Day War of 1967 was fought between Israel and an Arab Coalition (including Jordan, Syria, Egypt, and Iraq).  Israel used the Egyptian blockade of the Straits of Tiron, the massing of Egyptian tanks against the Israeli border, the Syrian shelling of Galilee fishermen, and the Syrian plan to divert drinking water from Israel as reasons for launching their attack. Operation Focus was the Israeli air campaign, which easily wiped out the air forces of the entire Arab Coalition.  Following Operation Focus was a charge eastward into Jordan's West Bank.  The Israelis quickly captured the eastern part of the holy city of Jerusalem.  At the same time, the IDF charged southwards towards Egypt's Sinai peninsula and Gaza Strip.  The IDF, led by future prime minister Ariel Sharon, quickly dispatched the Egyptian army and conquered the entire Sinai region.  On the Syrian Front, the Israelis charged up the Syrian Golan Heights on Day 5 of the Six Day War and took them.  All four of these regions are still in dispute.  Israel returned Gaza and Sinai, but not the Golan Heights or the West Bank. Influence of homocysteine on the interaction between circulating monocytes and endothelial cells

Mild hyperhomocysteinemia is an independent risk factor for the development of coronary artery disease, cerebrovascular disease and peripheral arterial disease. The mechanisms by which hyperhomocysteinemia promotes vascular disease are not completely understood yet. An increasing body of evidence has implicated oxidative stress as being contributory to homocysteine’s deleterious effects on the vasculature. Elevated levels of homocysteine lead to increased generation of superoxide anion in endothelial cells by a biochemical mechanism involving nitric oxide synthase, and, to a lesser extent, by an increase in the chemical oxidation rate of homocysteine and other aminothiols in the circulation. Furthermore, homocysteine has been shown to inhibit the activity of important cellular antioxidant enzymes, like the cellular isoform of glutathione peroxidase or superoxide dismutase, which may contribute to homocysteine’s induced oxidant stress. The resulting increase in reactive oxygen species leads to decreased bioavailability of the endothelium-derived signaling molecule nitric oxide via oxidative inactivation and thereby induces endothelial dysfunction. This seems to play a central role in the molecular mechanisms underlying the effects of homocysteine on vascular function. Hyperhomocysteinemia not only leads to endothelial dysfunction but also promotes the development and propagation of atherosclerotic lesion in atherosclerosis-prone animal models. As the recruitment of circulating monocytes to the vessel wall plays a crucial role in the process of atherosclerosis, the purpose of this study was to examine the influence of homocysteine on the interaction of endothelial cells with monocytes. Exposure of endothelial monolayers to D,L- and L-homocysteine resulted in a time- and dose-dependent increase in adherent THP-1 cells by upregulating ICAM-1 expression on endothelial cells. L-cysteine and D-homocysteine had no effects. This indicates that the stimulatory effect is specific for the naturally occurring L-stereoisomer and rather a biochemical than a chemical effect. The increased endothelial expression of ICAM-1 seems to be mediated by increased activation of the nuclear transcription factor NF-kB, as shown by increased nuclear translocation of NF-kB in homocysteine-incubated endothelial cells. In accordance, inhibition of NF-kB translocation by a synthetic inhibitor Bay 11-7082 significantly diminished homocysteine-induced ICAM-1 expression and adhesion of monocytes to endothelial cells. In addition, incubation of monocytes with D,L- homocysteine and L-homocysteine resulted in significant increase in the number of adhering monocytes to unstimulated endothelial monolayer by upregulating the expression of beta-2 integrins. Furthermore, homocysteine-incubation of endothelial cells and monocytes resulted in a dose-dependent and significant increase in the intracellular generation of reactive oxygen species. In support of the role of increased oxidant stress for the above mentioned effects, treatment of endothelial cells with the superoxide scavengers MnTBAP or Tiron together with homocysteine abolished homocysteine-induced monocyte adhesion, ICAM-1 expression and the nuclear translocation of NF-kB. Incubation of THP-1 monocytes with Tiron abolished homocysteine-induced beta-2 integrin expression on these cells and adhesion to unstimulated endothelial cells. These findings suggest that superoxide anion radicals mediate homocysteine’s effects on endothelium-monocyte interactions. In addition to previous studies that indicated that a significant source of reactive oxygen species in homocysteine-treated endothelial cells might be endothelial nitric oxide synthase, experiments using inhibitors of nitric oxide synthase in THP-1 cells indicated that nitric oxide synthase-dependent generation of superoxide anion also occurs in homocysteine-incubated THP-1 cells. This mechanism may contribute to homocysteine-induced oxidant stress. The information generated from these studies may be helpful in designing intervention strategies aimed at inhibiting the generation of reactive oxygen species in the vasculature that is associated with signaling events of monocyte recruitment and infiltration involved in atherosclerosis.

Einfluß der endothelialen Autakoide NO und PGI2 auf die Permeabilität endothelialer Gap Junctions

Die Anpassung der Gewebsdurchblutung an die unterschiedlichen Bedarfssituationen, setzt ein koordiniertes Verhalten der Gefäße im mikrovaskulären Gefäßnetz voraus. Diese Koordination der vasomotorischen Reaktionen im mikrovaskulären Gefäßsystem, ist möglicherweise auf die interzelluläre Kommunikation der Endothelzellen angewiesen. Die Endothelzellen und glattten Muskelzellen der Blutgefäße sind über Gap Junctions gekoppelt, auch eine myoendotheliale Kopplung wird diskutiert. Dadurch können Signale in Form von Ionen (und damit Änderungen des Membranpotentials) oder kleinen Moleküle über solche interzellulären Kanäle entlang der Endothelzellschicht weitergegeben werden. Völlig unbekannt ist aber, ob die Permeabilität dieser endothelialen Gap Junctions reguliert wird. Deshalb wurde in dieser Arbeit untersucht, ob vom Endothel gebildete lokal wirksame Gewebshormone (Autakoide, wie NO und Prostacyclin) die Durchlässigkeit der Gap Junctions beeinflussen. Hierzu wurde in konfluenten Kulturen von humanen umbilikalvenösen Endothelzellen (n=190) die Ausbreitung der Farbstoffe Carboxyfluoresein und Calcein nach Injektion in eine einzelne Endothelzelle in die benachbarten Endothelzellen untersucht. Es konnte gezeigt werden, daß der injizierte Farbstoff tatsächlich nur über interzelluläre Kanäle von einer Zelle zur nächsten gelangt. Diese Kanäle werden von Connexinen gebildet, denn ein Peptid, das das Aneinanderdocken der Connexine verhindert, reduzierte die Ausbreitung des fluoreszierenden Farbstoffs. Daher kann mit dieser Methode tatsächlich die Kopplung der Zellen über Gap Junctions untersucht werden. Die erhobenen Daten zeigen, daß die Anzahl der fluoreszierenden Zellen nach Hemmung der NO-Synthase mit Nw-nitro-L-Arginin (L-NA, 30µmol/L) um bis zu 29% zunahm, während die anschließende erneute Freisetzung von NO durch zwei differente NO-Donoren (SNAP bzw. SNP, 1 µmol/L) die Zahl der fluoreszierenden Zellen wieder auf den Ausgangswert reduzierte oder sogar unterhalb den, der unbehandelten Kontrollzellen senkte. Diese durch NO hervorgerufene Wirkung blieb in Anwesenheit des Hemmstoffes der löslichen Guanylatcyclase ODQ (10 µmol/L) oder der Radikalfänger Tiron und Superoxiddismutase unverändert. Dies weist daraufhin, daß es sich bei dieser durch NO hervorgerufenen Hemmung um einen direkten Effekt von NO handelt, der weder über die Bildung von cGMP noch über eine gesteigerte Peroxynitritproduktion vermittelt wird. Auch eine Hyperpolarisation der Endothelzellen durch den Aktivator von KATP-Kanälen HOE234 (1 µmol/L) hatte keinen Einfluß auf die Kopplung der Zellen. Im Gegensatz dazu hatte NO in Anwesenheit der Hemmstoffe der Tyrosinphosphatase Orthovanadat (100 µmol/L) und Phenylarsinoxid (1 µmol/L) keinen Einfluß mehr auf die endotheliale Kommunikation via Gap Junctions. Dagegen führte die Behandlung der Zellen mit dem Tyrosinkinase Inhibitor Genistein (100 µmol/L) zu einer deutlichen Reduktion der endothelialen Kopplung (-14%), die mit der Wirkung von NO vergleichbar war. Daraus läßt sich schließen, daß die durch NO hervorgerufene Wirkung auf die interzelulläre Kommunikation über eine Verminderung der Tyrosinphosphorylierung vermittelt zu werden scheint. Außerdem zeigen diese Daten, daß Prostacyclin die endotheliale Kopplung signifikant steigert, und das diese Wirkung über das gebildete cAMP vermittelt wird. Denn nicht nur das Prostacyclin Analogon Iloprost (1 µmol/L), sondern auch der Aktivator der Adenylatcyclase Forskolin (30 µmol/L), verbesserte die Ausbreitung des Farbstoffes signifikant . Schließlich zeigen die Ergebnisse auch, daß die beiden vom Endothel gebildeten Substanzen sich gegenseitig in ihrer Wirkung auf die endothelialen Gap Junctions beeinflussen können. Die erhobenen Daten zeigen erstmals eine Rolle von NO und Prostacyclin in der Regulation der Permeabilität endothelialer Gap Junctions. Diese Regulationsmöglichkeit und die Auswirkungen einer vermehrten oder verminderten Kopplung der Endothelzellen wirft zahlreiche neue Fragestellungen auf z. B. hinsichtlich der Pathophysiologie der coronaren Herzkrankheit oder auch des arteriellen Hypertonus und bietet damit auch die Möglichkeit zur Entwicklung neuer Therapiemöglichkeiten.

Hyperhomocysteinemia is an independent risk factor for the development of atherosclerosis. An increasing body of evidence has implicated oxidative stress as being contributory to homocysteines deleterious effects on the vasculature. Elevated levels of homocysteine may lead to increased generation of superoxide by a biochemical mechanism involving nitric oxide synthase, and, to a lesser extent, by an increase in the chemical oxidation of homocysteine and other aminothiols in the circulation. The resultant increase in superoxide levels is further amplified by homocysteinedependent alterations in the function of cellular antioxidant enzymes such as cellular glutathione peroxidase or extracellular superoxide dismutase. One direct clinical consequence of elevated vascular superoxide levels is the inactivation of the vasorelaxant messenger nitric oxide, leading to endothelial dysfunction. Scavenging of superoxide anion by either superoxide dismutase or 4,5-dihydroxybenzene 1,3-disulfonate (Tiron) reverses endothelial dysfunction in hyperhomocysteinemic animal models and in isolated aortic rings incubated with homocysteine. Similarly, homocysteineinduced endothelial dysfunction is also reversed by increasing the concentration of the endogenous antioxidant glutathione or overexpressing cellular glutathione peroxidase in animal models of mild hyperhomocysteinemia. Taken together, these findings strongly suggest that the adverse vascular effects of homocysteine are at least partly mediated by oxidative inactivation of nitric oxide.
