In this special episode, RAPM Focus takes a break from its usual format of discussing specific research projects to discuss instead how research in health care is increasingly vital yet often hindered by escalating costs and time constraints. Editor-in-Chief Brian Sites, MD, discusses these challenges with Alexander (Xan) Abess, MD, and Jonathan Weed, MD. Traditional, large scale, randomized controlled trials are resource intensive, making them challenging for clinicians to conduct. Dr. Abess is an anesthesiologist at Dartmouth Hitchcock Medical Center and assistant professor of anesthesiology and perioperative medicine at Geisel School of Medicine at Dartmouth. He has previously served as the vice chair of quality and safety, department of anesthesiology and perioperative medicine, and is actively involved with the Multi-Center Perioperative Outcomes Group (MPOG). He is the founder and Director of IDEA Lab (Innovation, Development, and Entrepreneurship in Anesthesiology). Xan has a wide range of clinical experience (private practice, academic medicine, military medicine) and medical device development experience, having previously started his own medical device company. He understands well the challenges of medical innovation and truly enjoys helping others achieve their development goals while also attempting to improve the quality of health care delivery and simultaneously addressing our broader economic and workforce challenges. Dr. Weed is a clinical assistant professor of anesthesiology at Dartmouth Hitchcock Medical Center. He is a graduate of Tulane Medical School and completed his anesthesiology residency at Beth Israel Deaconess Medical Center, followed by a fellowship at Dartmouth Hitchcock Medical Center in regional anesthesia. He spent several years as a residency program director at Tulane Medical Center before returning to Tulane Medical Center in 2021. To start, can you give us an overview of Tulane Medical's program focusing on its more distinctive aspects and elements? [2:11] Sure, Tulane School of Medicine is right in the heart of downtown New Orleans, Louisiana in the south. Our focus is mainly on really helping our community here in New Orleans. Now we have students that come from all over. The mission of our school is healing communities, and that's what everything about our school is focused on, whether it's curriculum, whether it's extracurricular activities, whether it's research, it's all focused on healing communities and we're trying to do that here in the New Orleans community. It's interesting that you say you're focused on healing communities, not individuals. Normally you think about doctors treating individuals, but this is a little bit different focus. [2:55] When we say healing communities, yes, individuals are part of the community, but as a whole, our goal is to really help the community because communities help themselves. It can't be this big institution that's talking down to the community, talking to individuals, lecturing communities, lecturing individuals about what they need to do. It has to be a partnership, and so it helps that Tulane is really partnering with the community in the community. Our students are living in the community. Our doctors and faculty are living in the community, so they have a vested interest in this. And so yes, individuals are part of the community, but overall we're really trying to focus on healing communities. What's new at Tulane since we last spoke? It's almost exactly two years ago. [3:49] I would say there are a couple of new things that we have going on. It doesn't really pertain per se to the medical school, but it will affect us. In the fall we have a new nursing program that started, and the good thing about that is, once it goes again with our mission of healing communities,

I'm so happy to speak with Dr. Nick Hallowell, a renowned board-certified child, and adult psychiatrist and the world's leading expert on ADHD. He graduated from Harvard College and Tulane Medical School and was a faculty member at Harvard Medical School for 21 years. He founded the Hallowell ADHD centers in several major US cities and has helped thousands of individuals with ADHD lead happy and productive lives. Dr. Hallowell is also a New York Times best-selling author, having written 20 books on various psychological topics, and has appeared on several popular TV shows, including 20/20, 60 Minutes, and Oprah. We also discuss some of the upsides of having ADHD, especially when it is well-managed. The final message is that people need to learn the truth about getting treatment, as it can significantly improve their lives. The diagnosis and treatment should be stories of good news. [08:21] Dyslexia is being slow to read and spell your native language. [11:27] ADHD causes problems with executive function like planning and organizing. [12:15] The upside of ADHD is being creative, entrepreneurial, intuitive, visionary, and more. It's having a race car brain and bicycle brakes. It's not a deficit of attention. It's an abundance of attention. [13:37] When people have ADHD, their brains go really fast like a Ferrari. [15:21] Dr Hallowell has ADHD himself, and he's treated thousands of people for ADHD. He also has medical credentials. [17:37] There isn't a center devoted for people who have addiction and ADHD. [19:41] Getting ADHD treated would enable those people to get more done with less effort. [20:16] People need to learn the truth about getting treatment, because it's a good thing. After diagnosis, your life will get better. [20:46] ADHD is frequently misdiagnosed. On today's episode #280 we will be speaking with someone I was referred to back in 2014, from Jeff Kleck, from EP #246[i]. He was the educator who handed me a bunch of books off his book shelf and urged me to move in the direction of educational neuroscience, to make a bigger impact with my work. He didn't give me our next guest's book, (so I figured it must be important to him) but he told me to write down “Dr. Janet Zadina[ii]” and study her. Now, I don't ignore anything someone tells me to do, especially if there is learning involved, so I wrote down Dr. Zadina's name, and immediately followed her work back in 2014. I had no idea at the time that Dr. Zadina was one of THE leaders, spearheading educational neuroscience in the country, before it was even called this, and now fast forward to last September, when Jeff Kleck and I finished our interview, he asked me “Did you ever interview Dr. Janet Zadina?” I just looked at him thinking “How did I miss that?” remembering it was important. I remembered standing in his office, holding a bunch of books he asked me to read, and then we went to his computer and he pulled up all the FREE resources and most up to date research on Janet Zadina's website[iii] that he told me to review. It was Dr. Zadina's work that helped me to begin in this field of educational neuroscience, making the connections to the brain and learning. Today's guest, Dr. Janet Zadina, we will soon discover, a former high school and college teacher and cognitive neuroscientist whose background, expertise, energy, and humor all took her to international acclaim. I can't believe it was almost 10 years ago that an educator who found your work, passed it onto me and without even knowing it, you sparked something to help me get to where I am today with educational neuroscience at the heart of everything I think about, and do. Isn't that crazy to think of the impact you've had on others around the world? Where have we reached you today? Are you in New Orleans? INTRO: Before we get to your questions, I just wonder how many people do you run into who share stories like I did with you, about how learning from you ignited something in them somehow? If we connect the dot looking backwards in your life, we all have people and experiences that shift our direction in some way. I wonder who crossed your path that took you on this pathway to becoming a neuroscientist? What was the defining moment? Spear heading anything new is never easy but I've heard some horror stories from past guests who really struggled when they begin in this field. As a pioneer in educational neuroscience, I wonder, what were some of the biggest challenges that you faced? Why did you make the switch from doing MRI research in the lab to actually speaking to educators? So now we are all well into the digital age, and most of the books I read are on my phone, in my Kindle reader, but I've got a select few books that sit on the corner of my desk for me to reflect back to when I need some ideas or inspiration. What should we know about reading books digitally versus actually holding a print edition that we can highlight and refer back to often? There were a many important AHA moments within the chapters of your book Multiple Pathways to the Student Brain but the first passage that stuck out to me was “this books stems from my decade of speaking to teachers….at the time I graduated, I was the only person, so far as I knew, using the term educational neuroscience.” Looking back now, where is this discipline now, from where you saw it in those beginning days? What was your “mission” when you began speaking about the brain and learning and writing your book? Was it that you wanted “to help teachers at all levels” to understand this information that they were craving to learn? Do you have anything exciting coming up with your presentations? When I first began learning about the brain, I found some of your early TED TALKS and presentations where I started to connect stress and the brain to its impact on our students' learning in the classroom. What motivated you to transition to speaking primarily about the effects of anxiety, high stress, and trauma on the brain and learning? What is one take-away for teachers from your work on this topic? In other words, what might be one of the most important things they can do to help with their stress? In this episode, I am excited to welcome back Dr. Ned Hallowell back on the show, this time we are talking about his new book ADHD 2.0. We do a deep dive into a couple of key areas in his book: Why he likes to change the name from ADHD to VAST (Variable Attention Stimulus Trait), he explains why we do not have a deficit of attention. How if we manage ADHD it can enhance our lives and can become your superpower. The main focus of this episode is Dr. Ned’s explanation of the regions of the brain called Task Positive Network and Default Mode Network and when these regions are not working effectively we go into extreme negative self-talk. He talks about a few simple but effective ways of managing negative self-talk. Dr. T.E. Ross came to Hattiesburg in 1892 and set up an office on Main Street. In this 1975 oral history interview, his son, Dr. T.E. Ross, Junior, recalls his father’s decision to move their family from Neshoba county. Before vaccines and antibiotics, the only way to stop infectious diseases was through quarantine. Dr. Ross recounts how his father was blocked from returning home during a yellow fever outbreak. Dr. Ross graduated from Tulane Medical School in 1918. He remembers the circumstances that led him to set up his practice in Hattiesburg like his father. PODCAST EXTRA: W.S.F. Tatum was a successful Hattiesburg timber magnate who served as mayor in the 1920s and 30s. Dr. Ross describes the soft-spoken businessman as a frugal, yet good and generous man, who disliked ostentatious displays of wealth.
