In this episode of the Wealth Planning for the Modern Physician podcast, host David Mandell sits down with two highly respected medical leaders—Dr. Mark Figgie, Chief Emeritus of the Surgical Arthritis Service at the Hospital for Special Surgery (HSS), and Dr. Joel Press, Physiatrist-in-Chief at HSS—to explore their personal and professional journeys in medicine. Dr. Press shares how his diverse early experiences led him to physiatry, while Dr. Figgie recounts his balance between science, business, and a lifelong pursuit of education, including earning an MBA during his fellowship. The conversation shifts into a discussion of mentorship at the Hospital for Special Surgery, where both physicians have played key roles in developing a cross-disciplinary mentorship model to support younger doctors. Their focus extends beyond clinical guidance to include career development, personal financial planning, and wellness—areas often overlooked in traditional medical training. This approach fosters institutional collaboration and a stronger culture at HSS. Finally, the episode dives into physicians' relationships with industry. Dr. Figgie offers candid advice on contracts, intellectual property rights, and ethical considerations when working with device manufacturers. He emphasizes the importance of legal review and staying true to what benefits patients. Dr. Press adds insight into personal financial trust, suggesting that doctors build a "board of directors" for trusted guidance. The episode closes with a preview of future initiatives aimed at supporting physicians earlier in their careers through financial education and mentorship. Key Insights: Dr. Press discovered physiatry through a unique summer program, showing the value of early exposure to diverse specialties. Dr. Figgie pursued an MBA during fellowship, believing in the power of financial literacy for physicians. Mentorship at HSS now includes financial, administrative, and personal development components—not just clinical. A cross-specialty mentorship program allows different departments (e.g., orthopedics and radiology) to learn from one another. Many young physicians lack basic financial planning tools, leading to under-preparedness later in their careers. HSS has created a repository of recorded educational sessions covering mortgages, insurance, investing, and more. Dr. Figgie cautions against blindly signing industry contracts without legal review, especially when IP is involved. Relationships with medical device companies often include consulting, design input, or teaching—but require transparency. Dr. Press emphasizes the importance of building a trusted "personal board of directors" to help with life and career decisions. Future initiatives at HSS may include subsidized financial checkups for new physicians to kickstart long-term planning. In this Episode I am joined by John Organ, Chief Emeritus of the U.S. Geological Survey's Cooperative Fish and Wildlife Research Unit Program, to discuss the North American Model of Wildlife Conservation. John begins by telling us about the people that were responsible for consolidating the model, He then describes the ecological and historical context that ultimately led to modern wildlife management. We go back to the late Pleistocene to provide a sense of what the mega-faunal community looked like prior to the arrival of humans on the continent. We then step forward in time to the wildlife fauna of the North American frontier, and outline the main species that were hunted, exploited, and those that were driven to extinction or near extinction. John tells us about the early advocates of wildlife conservation, and describes their major legislative and institutional milestones. We then turn to detailing the seven tenets of the North American Model. We close-out the episode discussing what John sees as the three biggest threats to hunting, and given that hunters make up about 5% of the population in the NA, and that it would almost certainly be outlawed if the question of its legitimacy were put to a simple referendum,I ask John what he would advise hunters to do to encourage support for hunting in the non-hunting community.

It was more than 13 years ago that a veterinarian introduced Dr. Gregory Lutz to platelet-rich plasma (PRP). Dr. Lutz had a lame horse suffering from a tendon injury. With the help of a portable ultrasound, the veterinary physician drew the horse's blood, spun it in a centrifuge, and injected it into the tendon, right there in the stall. Within a few weeks, the horse was running around like a pony. That inspired Dr. Lutz to research this treatment for his own patients. With more than 30 years of clinical experience under his belt, Dr. Lutz has proven himself to be a driving force in today's regenerative medicine movement. He has co-authored more than 60 scientific publications on the topic of PRP and continues to research the power of this treatment to improve outcomes.“We have to share research, speak openly about our findings, report the good and the bad and the ugly, so that people are aware of the risks, and see if we can keep moving this forward,” says Dr. Lutz. “It's been a very exciting journey to finally find something that I think has staying power.” In this episode of Back Talk Doc, host Dr. Sanjiv Lakhia and Dr. Lutz discuss PRP treatments for low back pain and Dr. Lutz's new book.

Before it was Hartford Hospital's Institute of Living, it was the Hartford Retreat for the Insane. Today Hank Schwartz, joins us to share the history of this Connecticut mental health center, 200 years after its opening. Although asylums have a reputation for inhumane treatment towards the mentally ill, the Hartford Retreat was a pioneer in treating all of its patients with respect and dignity. We hear how that philosophy informs how the Institute operates today. What questions do you have about the history of asylums in our state? GUESTS: Dr. Hank Schwartz - Psychiatrist and Chief Emeritus of Hartford Hospital's Institute of Living Daniel Bergner - contributing writer for the New York Times Magazine and author of the new book, The Mind and the Moon: My Brother's Story, The Science of Our Brains and the Search for Our Psyches Support the show: http://wnpr.org/donate See omnystudio.com/listener for privacy information.
In 1822, the Hartford Retreat for the Insane was chartered as one of the first mental health centers in the United States, and the first hospital of any kind in CT. In 2022, the CHS is exploring of the story of mental health in our state. Recently, the CHS invited Dr. Harold I (Hank) Schwartz to talk about the history of the Hartford Retreat, renamed the Institute of Living in the 20th century. His presentation took us through the state of mental health care in the early 1800s, the reasons for the founding of the Retreat, and its place in the development of modern psychiatry. Dr. Schwartz, is the Psychiatrist-in-Chief Emeritus at the Institute of Living/Hartford Hospital and formerly served as Vice President, Behavioral Health at Hartford Healthcare. He is Professor of Psychiatry at the University of Connecticut School of Medicine and Adjunct Professor of Psychiatry at the Yale University School of Medicine. Last night on March 9th, The MGH Center for Neuroscience of Psychedelics welcomed award winning author and Professor Michael Pollan to join Dr. Jerry Rosenbaum and team to celebrate the launching of this exciting and novel effort at Mass General. The conversation was introduced by social change activist and philanthropist Dick Simon who appropriately held up Pollan's 2018 book "How to change your mind." It was clear that although psychedelics for mental health have been proposed and studied for about 100 years, Simon referred to what many people in the field call time periods of "BP and AP" or Before Pollan and After Pollan. Since his 2018 book, the field continues to evolve and Pollan mentioned that a Center like this is just the right place to continue to explore and create new treatments. He said that although he expected to meet some resistance by mainstream psychiatry, on the contrary he found at MGH a willingness and openness to move forward. He noted that the "return of psychedelics" to Harvard just makes sense and is a big event, given the rich yet complicated history here.Pollan emphasized that as clinicians and researchers, many focus on the negative changes that can occur in the brain from development forward in life. However, what are the possible ways to creating positive change? A key theme was the idea that how we change the brain is not likely facilitated by the chemicals we offer the brain, rather it is experience that can change the brain. Towards the conclusion of the discussion, Pollan mentioned that (stuck) with me - saying that this entire field has been very promising, but with the right investment in resources, education and research, could be profound. There is so much mental illness that is not diagnosed and not treated the way it can be. The veteran population as we know has significant rates of PTS and depression, with only 50% seeking help. Two medications with the most evidence and work supporting the use in this kind of experience driven treatment are…psilocybin and MDMA. Currently, Home Base is looking at PTS with MDMA in a new study. In collaboration with the new Center at MGH, we hope to bring more to the table for the veteran population so we can help to target stress, depression, anxiety and other mental health conditions our veterans struggle with.So how do we deliver that experience, how do we show the evidence or the state of the science, and how do we educate and endorse the public that there are indeed so many people that can be helped in this way? I was lucky to talk with a team of people leading this effort at MGH: Dr. Jerry Rosenbaum, The Director of the new Center and Psychiatrist-in-Chief Emeritus at MGH, Dr. Franklin King (Education and Training), and Dr. Rick Doblin, a leader in psychedelic research and policy, and founder of MAPS, or Multidisciplinary Association for Psychedelic Studies. We will take a step back in time to the beginning of this journey in using psychedelics to augment experience, talk about the process of this novel treatment, and highlight the new MGH Center for Neuroscience of Psychedelics that Home Base is collaborating with. At the end of the day, the overarching goal is alleviating the pain of mental illness in both civilian and military populations and combating these complicated diseases.Thank you for joining us for this special episode on new ways to look at the brain, and treatment of mental health conditions, specifically PTS as it relates to our veterans at Home Base. Did you know that physicians, as a group, have lower work-life satisfaction compared to the general population? The consequences of burnout are borne both by the individual physician and the health care system. The recognition of burnout as a serious individual and institutional challenge is leading to an increased need to design, implement, and evaluate systems that promote well-being. Moderator Richard L. Dr. Michael Gazzaniga - Left Brain vs. Right Brain. We often hear that if we're creative we must be "right-brained" but if we're logical we must be "left-brained". Science tells us that each hemisphere controls certain cognitive functions, so it only makes sense that there is a dominant side that gives us our tendencies - but is it true? Are we either "left-brained" or "right brained"? Or better yet, what happens when you disconnect the two regions from each other entirely? For over 40 years, our guest this week has been studying patients who have had their left and right brain disconnected via surgery, and he is here to set the record straight. Michael Gazzaniga, is one of the leading researchers in cognitive neuroscience and is the worlds top expert on split-brain research. Michael is a professor of psychology at the University of California, Santa Barbara, and the author of Tales from Both Sides of the Brain: A Life in Neuroscience. He received a Ph.D. in psychobiology from the California Institute of Technology, where he worked under the guidance of Roger Sperry, with primary responsibility for initiating human split-brain research. In his subsequent work he has made important advances in our understanding of functional lateralization in the brain and how the cerebral hemispheres communicate with one another. Gazzaniga founded the Centers for Cognitive Neuroscience at the University of California, Davis and at Dartmouth College, the Neuroscience Institute, and the Journal of Cognitive Neuroscience, of which he is the Editor-in-Chief Emeritus. Gazzaniga was a member of U.S. President George W. Bush's Council on Bioethics. He was also the Director of the Law and Neuroscience Project, a project to study the intersection of law and neuroscience. "The brain is built for us all to make decisions to achieve goals." - Michael Gazzaniga Quotes from Michael: What we learn in this episode: Is there such things as left brain and right brain people? Why can our brain function fairly normally when it is essentially cut in half? What is split brain research? What happens when our brain is split in the middle (split brain surgery), disconnecting the left and right hemisphere?
