I get lots of questions on various platforms. But I never get to all of them. So, today is Q and A day. I asked my friend and colleague, Dr. Heidi Flagg to join me because it's always good to get another point of view. Dr. Flagg is, above all, a clinician. She is in her office in NY city, day in and day out, helping women navigate peri menopause and menopause. And unlike many people out there who are new to the scene, Dr. Flagg brings decades of real-world experience to the table. QUESTIONS Do you need to be 12 months without a period to start hormone therapy? I am getting a period every 3-4 months, so I assume I am in perimenopause. I am having a lot of symptoms, so I went to my doctor, and she suggested I go on birth control pills. I'm not even having sex. I don't need birth control. I am 48 years old. I had a hysterectomy 5 years ago because of fibroids. I am now having hot flashes, brain fog, and mood swings. What hormone levels should I check to see if I am ready for hormone therapy? I started using an estrogen patch around 3 months ago. I am still having hot flashes. Do I need to give it more time or do I need more estrogen? Is it OK to substitute a brand name patch with a generic patch? I am 62. My last period was 13 years ago. Is it too late for me to state hormone therapy? Since I started using local vaginal estrogen, I have an accumulation under my clitoral hood. How best to clean it? Is it OK to put vaginal lubricant inside the vagina? Will hormone therapy trigger the recurrence of endometriosis? I have high cholesterol and want to know if Duovee™ is a good product for someone with high cholesterol. Would it be better to use a transdermal estrogen? Is it safe to use systemic estrogen and local estrogen at the same time? I had a hysterectomy last year because of uterine cancer. Can I take estrogen? I am taking low doses of testosterone, and I am pretty sure my voice is getting lower. My doctor said my levels are fine. I thought that only happened with high doses. If I go off, will it go back to how it was? I am 45 years old and scheduled for a hysterectomy because of fibroids that bleed a lot. Is my sex life going to change? Featured Locations: Ohio State Reformatory 100 Reformatory Rd, Mansfield, OH 44905 The Ohio State Reformatory is also rumored to be haunted by a guard who once worked in the East Cell Block, where he was reportedly murdered by an inmate. The guard's ghost has been seen wandering the upper levels, and some visitors have experienced sudden cold spots and the sensation of being watched. Ghost tours of the Reformatory are known to provide EVP sessions and access to areas where paranormal activity is most intense. Moonville Tunnel Hope-Moonville Rd, McArthur, OH 45651 In addition to the railroad worker, some visitors have reported seeing the apparition of a man dressed in 19th-century clothing, believed to be one of the miners from the old town of Moonville. The area surrounding the tunnel is also known for strange misty figures that seem to materialize along the tracks, leading some to believe that the entire ghost town of Moonville might be haunted. Franklin Castle 4308 Franklin Blvd, Cleveland, OH 44113 Another haunting tied to Franklin Castle involves a ghostly child, believed to be the spirit of a Tiedemann child who died in the house. Visitors have reported hearing the sound of a child's laughter and feeling sudden drops in temperature. The secret rooms and passageways within Franklin Castle are also said to conceal a dark history, and some believe the castle's haunted reputation is tied to these hidden areas. Cincinnati Music Hall 1241 Elm St, Cincinnati, OH 45202 The Music Hall is also known for its phantom orchestra—guests have reported hearing the sound of an entire ensemble tuning up and playing music that seems to come from the walls themselves. During a late-night renovation, workers reported strange lights in the grand ballroom, as if a ghostly ball was being held in the empty space. Old Licking County Jail 46 S 3rd St, Newark, OH 43055 In addition to Carl Etherington's ghost, the jail is also haunted by the spirit of a former inmate who was believed to have been wrongfully convicted. Visitors to the jail report hearing the sound of sobbing and have seen a shadowy figure sitting in one of the cells, as if still awaiting release. Some investigators have captured eerie EVPs, with voices calling out for justice. Buxton Inn 313 E Broadway, Granville, OH 43023 Guests have also reported seeing a ghostly cat that roams the Buxton Inn, believed to have belonged to one of the inn's previous owners. The phantom feline is often spotted lounging by the fireplace, and guests sometimes feel a sudden, inexplicable warmth when it appears. The inn's basement is also rumored to be haunted, with reports of moving shadows and ghostly whispers. The Ridges (Athens Lunatic Asylum) 78 Hospital Dr, Athens, OH 45701 In addition to Margaret Schilling's ghost, some visitors report seeing shadowy figures in the old asylum's graveyard, where thousands of patients are buried in unmarked graves. Paranormal investigators have captured unusual readings and cold spots in the graveyard, which many believe is a hotspot for spiritual activity tied to the asylum's past. Lafayette Hotel 101 Front St, Marietta, OH 45750 The Lafayette Hotel is also known for sightings of a ghostly couple dressed in formal attire, believed to be guests who perished in one of the hotel's fires. Witnesses have seen the couple dancing in the hotel's ballroom, as if reliving a happier time. There have also been reports of old-fashioned music playing in the background, only for it to stop suddenly when someone enters the room. Majestic Theatre 45 E 2nd St, Chillicothe, OH 45601 The Majestic Theatre is also haunted by the spirit of a former performer who is said to linger in the backstage area, still preparing for his final curtain call. Staff have reported hearing the faint sound of a piano playing, as well as disembodied voices singing old show tunes, even when the theater is empty. The theater's dressing rooms are known for their eerie vibes, with makeup mirrors lighting up on their own. Prospect Place Mansion 12150 Main St, Trinway, OH 43842 Visitors to Prospect Place Mansion have also reported hearing the sound of horses' hooves and wagon wheels near the barn, where many enslaved people were hidden during their escape. Some say they've seen the apparition of George Adams himself, standing near the mansion's front door, keeping watch over the historic home. Paranormal investigators have captured strange anomalies in the barn, where a sense of unease often hangs in the air. Bhagawan laid the foundation stone for the first hospital in Puttaparthi - the Sri Sathya Sai General Hospital way back in 1954. That was the beginning of the medicare mission which later had a new and expanded General Hospital and two Super Speciality Hospitals, one each in Puttaparthi and Bengaluru. Even though thousands of poor and needy were served in these Temples of Healing, Bhagawan said that this was not enough. In 2006 He launched the Sri Sathya Sai Mobile Hospital which is literally a 'hospital on the wheels' so that now in stead of the downtrodden coming to the hospital, the hospital could go to the door steps of the poor. Julio Carreño, papá de una de las pacientes que participó, agradeció la vinculación del Hospital con los pueblos indígenas, lo que incentivó que los niños quisieran conocer más acerca de ellos. “Demasiado informativo, creo que es una instancia para que los niños aprendan nuevas cosas de lo que era nuestro tiempo pasado, otras culturas, entonces es bastante gratificante la verdad”. Su visita fue bien recibida también en el Club Escolar del Hospital, donde los niños le formularon varias preguntas y tuvieron oportunidad para probar parte de su gastronomía típica. Así lo indicó Ana María Galaz, encargada del Club Escolar. “Los niños pudieron tener nuevos conocimientos y una experiencia educativa. La lonco le explicó a los niños cómo preparar hierbas, agüitas de hierba, también le mostró los piñones para que degustaran, les mostró los nombres de los trajes típicos mapuche, también nos habló de su familia, de su lengua”. Para la misma lonco, Patricia Antillanca, la experiencia fue enriquecedora y marcó un precedente para seguir trabajando en difundir más acerca de los mapuche en otras unidades del Hospital. “Nosotros vinimos a traer nuestra enseñanza, poder transmitir esa cosmovisión a los pichiqueches (niños), lo que es nuestro atuendo, nuestras joyas, nuestra alimentación, nuestras hierbas medicinales, con lo que nosotros nos curamos, nos sanamos nuestra enfermedad. Y feliz, pues ellos están interesados en saber todo, y ahí uno se da cuenta de que realmente los colegios no están enseñando mucho el mapudungún y las costumbres, y para eso uno tiene la paciencia y la voluntad para enseñarles a los pichiqueches (niños)”.
Hace cerca de 8 años que existe el “Programa de Acercamiento de Medicamentos” en el Hospital Dr. Gustavo Fricke del SSVQP. Hoy, gracias a esta estrategia, hay 2.200 pacientes de comunas retiradas de la V región que evitan acudir al recinto de salud. Este programa está orientado a personas de la tercera edad, con movilidad reducida y que sean pacientes crónicos. Así lo especificó María Eugenia Trujillo, químico farmacéutica y jefa de la Farmacia Externa del Consultorio Adosado de Especialidades. “Farmacia externa se preocupa de preparar medicamentos y enviárselos a pacientes a domicilio y pacientes en CESFAM. Se prioriza a pacientes mayores de 65 años, con movilidad reducida y que tengan algún problema en realidad que les impida venir acá al hospital a la farmacia para retirar sus medicamentos. Del hospital, lo más lejano a que se llega es a Puchuncaví y a Quintero. También estamos a cargo del programa de fibrosis quística infantil que eso se va a Quilpué, Limache, Quillota y Calera, que se envían una vez al mes los medicamentos para los niños. Verónica Villarroel es una de las pacientes beneficiadas con este programa. Ella vive en Puchuncaví y retira sus medicamentos para la artritis una vez al mes en la Casa de la Cultura de Puchuncaví, donde llega a instalarse el equipo en terreno de farmacia del hospital Dr. Gustavo Fricke. “Es buenísimo, porque a veces uno tiene que estar pagando pasajes solamente para ir a buscar medicamentos y en cambio acá es más fácil porque uno está cerca”. Manifestó. Carolina Cisternas retira los remedios para el riñón de su suegra en Puchuncaví, y agradece no tener que viajar hasta Viña únicamente para ello. “A veces uno igual se complica en ir para allá pero que vengan acá está súper bien, porque a veces uno no tiene cómo ir. Dos años tomando remedios que teníamos que buscar allá pero ahora nos llegan acá”. Otro de los usuarios Kiendall Arce, pertenece a la tercera edad y hace años que acercan su tratamiento para la próstata hasta donde reside. “Es súper bueno, porque así los viejos sobre todo, nos ahorramos de ir para allá. A veces andamos a penas, hay días que no puedo ni caminar, entonces, que lo traigan aquí, muy bueno. Pero antes los retiraba allá y después me dieron allá mismo la posibilidad de cambiarme para acá”.

Una docena de pacientes llegaron muy temprano este sábado al Consultorio Adosado de Especialidades del Hospital Dr. Gustavo Fricke para acceder al primero de varios operativos orientados a diagnosticar y gestionar sus audífonos a quienes los perdieron en medio del incendio. Así llegó María Gloria Pérez, acompañando a su mamá, Eva Rosa, de 92 años, a reponer estos valiosos equipos para recuperar su calidad de vida y reconectarse con su entorno: “Me parece muy bien, porque deberíamos tener esta oportunidad siempre. Muchas gracias. Ahora van a hacerse el examen para saber cómo está su problema de audición”. La señora Eva, como los restantes pacientes, accedió a todo lo que requiere, en una sola visita. Tal como lo explica la Dra. Dafne Secul, Subdirectora Médica del Servicio de Salud Viña del Mar Quillota Petorca, “este operativo hoy atenderá a personas que perdieron sus audífonos, personas con hipoacusia, y por lo tanto van a ser vistos hoy día por otorrino, por tecnólogo médico e incluso por el laboratorio que le va a entregar el audífono. De modo que en una atención va a quedar solucionado su problema de audición. Así que estamos muy contentos, con una tremenda generosidad han venido médicos, otorrinolaringólogos, tecnólogos a ponerse a disposición de estos pacientes”. Todo ello, gracias a la alianza entre el Servicio de Salud Viña del Mar Quillota Petorca, el Hospital Dr. Gustavo Fricke, la Clínica Alemana, Desafío Levantemos Chile y la empresa Auditron. La Dra. Tatiana Aldunate, Subdirectora Médica, afirma que “estamos felices de formar parte de esta cadena solidaria, después de esta tremenda tragedia, y ser anfitriones de un equipo tan destacado como otorrinos de la Clínica Alemana, que han puesto su corazón solidario a disposición de los más afectados. Así que felices de poder acogerlos y entregar la infraestructura y el equipo humano nuestro para este desafío”. Tras diversos operativos en diferentes partes del país, la Clínica Alemana se hizo presente a través de sus especialistas. Karen von Schwedler, encargada de Comunidad de la Clínica Alemana, comenta que en el marco de una antigua alianza con Desafío Levantemos Chile, se pusieron a disposición de las necesidades de los damnificados: “Imaginamos lo difícil que es estar en una situación de damnificados, tener que estar en un albergue o en la casa de un familiar y además no escuchar nada porque perdiste tu ayuda técnica, que es súper importante. Entonces, de verdad, por eso además va a ser toda la atención en el mismo día y en siete días los van a llamar para entregar el audífono. Es súper importante poder ayudarlos lo que más se puede ahora”. Por su parte María Francisca Hierro, coordinadora del Área de Salud para la Clínica Alemana de Desafío Levantemos Chile señaló que “es una alianza virtuosa donde cada una de las partes pone su granito de arena para que en conjunto, entre todos, podamos entregar esta ayuda. Sin la colaboración y el apoyo del Servicio de Salud, del hospital, obviamente nada sería posible. Entonces cuando se trabaja mancomunadamente de manera tan linda, tan unida en pro de los demás, sin duda para nosotros es fundamental. Nada lo podríamos hacer solos, todo es un trabajo en conjunto y lo agradecemos de todo corazón”. Parte de la atención de los damnificados estuvo a cargo de los especialistas en otología, como el Dr. Jorge Preisler, que ya ha participado en otros operativos para diagnosticar hipoacusia: “Para nosotros también es importante y yo creo que para nosotros es un privilegio. Nosotros vivimos y trabajamos en una clínica que es distinta y la gente más necesitada está acá. Por lo tanto, tenemos que estar acá, si es posible, como no, encantadísimo”. Uno de los aspectos destacados y que responde a una de las principales políticas impulsadas en materia de salud por el Gobierno, es la gratuidad en la atención para los usuarios de Fonasa en los tramos C y D, con la puesta en marcha del plan Copago Cero, que sólo en el Hospital Dr. Gustavo Fricke, establecimiento base y principal centro de referencia y derivación de la red, alcanzó las 17.924 prestaciones, lo que se tradujo en más de 500 millones de pesos de ahorro a los pacientes afiliados a Fonasa durante el último trimestre de 2022. En paralelo, durante el año 2022, 38.196 personas se hospitalizaron en la red asistencial, siendo las patologías del sistema circulatorio y cardiaco, las enfermedades digestivas, los traumatismos, y enfermedades oncológicas, las principales causas. Tiempos de espera y resolutividad La red asistencial hospitalaria y la Atención Primaria de Salud generaron en 2022 más de 2.327 atenciones médicas, reflejando un aumento de un 26% respecto del año anterior. En cuanto a la gestión de los tiempos de espera asociados a la atención de especialistas y de intervenciones quirúrgicas, consultas de nueva especialidad y la atención odontológica, el Servicio de Salud trabajó en diferentes estrategias como los Centros Regionales de Alta Resolución (CRR), realizando 1.393 cirugías mayores ambulatorias, implementación de extensión horaria a través de Fondos Extraordinarios Transitorios (FET), convenios con CAPREDENA y clínicas privadas. En cuanto al cumplimiento de las garantías GES en la red, el SSVQ logró cumplir con 195.915, lo que corresponde a un 95,5% del total, realizándose además 5.594 cirugías GES con un 77% de cumplimiento. En el ámbito de salud mental y enfermedad mental, la red mantiene 50 mil personas controladas y se realizaron 214.891 atenciones de especialistas, siendo vital fortalecer la psiquiatría adulto e infanto juvenil, las residencias especializadas y lograr la apertura de camas psiquiátricas en la red hospitalaria. En el área de salud oral, se atendió a 289.571 personas en los distintos programas. Entre los principales anuncios destaca la pronta apertura de un servicio con 24 camas de psiquiatría, con personal reclutado, capacitado y motivado, en el Hospital Biprovincial Quillota Petorca. Fortalecer la respuesta ambulatoria de especialidad disponible, considerada esta, como nivel secundario en la complejidad, reforzar las acciones de promoción y detección desde la Atención Primaria de Salud de la red. Todas estas acciones serán articuladas a través de un Modelo de red de Salud Mental que está en desarrollo para finalmente dar respuesta oportuna, pertinente y acorde a las necesidades de las personas, considerando los recursos disponibles. Respecto del abordaje y prevención del cáncer, se ha potenciado un modelo de gestión como red oncológica gestionando el aumento de la actividad quirúrgica, el refuerzo del programa de cuidados paliativos con la consolidación de la telemedicina. También se suministraron 11.231 dosis de vacunación por virus de papiloma humano, 56.683 atenciones de cobertura PAP y 16.452 exámenes de mamografías. Inversiones en salud En cuanto a las inversiones en salud, durante el año 2022 el servicio de salud ejecutó más de 11 mil millones de pesos en obras finales de la etapa uno del Hospital Dr. Gustavo Fricke correspondientes a adecuaciones funcionales, habilitación de equipos y Tecnologías de la información, En la construcción del Hospital Provincial Marga Marga, se ejecutaron más de 12 mil millones de pesos, además de la ejecución presupuestaria del Hospital Biprovincial Quillota Petorca con más de 7 mil millones para el inicio total de operación con ejecución en obras complementarias, equipos y Tics.

Priya Prakash, Annie Polcari, Lea Hoefer, and William McKinley.Sharnae Phagan is the trauma program manager at UChicago Medicine and was an ED trauma nurse for over 20 years. Dr. Priya Prakash is a fellowship-trained surgeon who specializes in trauma, critical care and emergency surgery.
