Episode 224: Community Health Workers Dr. Arreaza: Today we will discuss a topic that, frankly, every single person listening, whether you're a medical student, a resident, a nurse, a family doctor, or any primary care provider, needs to really understand. We're talking about community health workers (CHWs). We are joined by our stellar medical student; you may be familiar with her voice from previous episodes about insomnia. Moira, welcome, please introduce yourself.  Moira: I want to be upfront about why Community Health Workers matter to you specifically. If you've ever felt frustrated that your patient with uncontrolled diabetes keeps missing appointments because they can't get a ride, or that your heart failure patient was readmitted because nobody checked whether they could afford their medications, then you already understand the problem that CHWs are designed to solve. Dr. Arreaza: We're going to give you the definition of a CHW, the evidence behind their effectiveness, how they fit into your care team, the return on investment, and practical steps for integrating them into your practice. We have pulled information from a lot of peer-reviewed sources, and we want to share them with you. So, Moira, let's start with the basics. What exactly is a community health worker? Moira: Great question, and it's one that even literature struggles with, because there are so many titles for this role. Community Health Worker is an umbrella term that encompasses more than 20 different titles including outreach workers, promotores or promotoras de salud, community health representatives, lay health workers, peer educators, patient navigators, and many more. The American Public Health Association defines CHWs as frontline public health workers who are trusted members of or have an unusually close understanding of the communities they serve. Arreaza: And that trust is so important in health care. CHWs are not physicians. They are not nurses. They do not diagnose or prescribe. But they are like a bridge connecting the medical environment, social services, and the community to reduce gaps in healthcare delivery.  Moira: Exactly. Growing up on a sheep and beef farm, he has seen solar technology become cheaper and more powerful, making it a practical choice for modern agriculture. The organisation focuses on providing impartial information to help farmers make strategic decisions.The Solar on Farms ProgrammeTo provide producers with up-to-date data, EECA has established 37 demonstration farms across various sectors and regions. These farms monitor performance and share the unvarnished truth about the installation process, helping other growers understand the benefits and barriers without the typical marketing noise.Building Resilience with BatteriesWhile solar saves money from day one, batteries provide critical resilience. Chris notes that a battery can prevent the chaos of power cuts during milking and highlights a 2025 Southland case study where a farm continued operating through major storms. While the upfront cost is higher, the ability to milk through an outage is a major risk mitigant.Sizing and System CostsInvestment levels vary significantly based on farm type. Systems range from small 10,000 dollar setups for sheep and beef units to large 500,000 dollar investments for high-use dairy and irrigation sheds. The goal is to right-size the system to match on-farm usage, as exporting excess power to the grid often yields lower financial returns.Avoiding Common PitfallsFarmers must consider dust management on panels, especially near unsealed roads, as it can significantly affect output. Another key consideration is the complexity of multiple electricity connections (ICPs). Chris recommends ensuring solar is installed on the meter with the highest load to maximise the return on investment.The Future: Electrification by 2035Looking ahead, Chris predicts a major shift toward the electrification of farm vehicles and heavy machinery. This transition will reduce reliance on long, volatile supply chains for liquid fuels, replacing them with locally produced on-farm energy that is visualised and managed through smart apps.Final Advice: Start Saving TodayChris encourages farmers not to wait for the next big technological leap. Solar technology is already at a compelling price point, and as soon as a system is switched on, it begins paying for itself. The best time to start locking in energy costs and building operational resilience is now.

We talked about other items that have gotten too expensive. Hot Tea: Ella Langley had several eye surgeries where her eyeballs had to be removed and put back. Michael J. Fox confirmed that he is NOT dead after CNN posted a video and article claiming that he was. Two guys in Florida were arrested for driving with a dead alligator strapped to the top of their car. Joey used AI to update his bio and was very proud of it. Nancy and Karly told him that it sounded like a robot wrote it. The Academy of Country Music Award nominations came out yesterday! Our favorite news is that Emily Ann Roberts is nominated for New Female Artist of the Year! Lucky 7 for $50 to Old South Candy Thomas Rhett Facetimed Ella Langley because his grandma didn’t understand her song Choosin’ Texas. We had older people call and let us explain songs that they didn’t understand. We explained McArthur, Fast Car, and Worst Way. All Bones Considered: Laurel Hill Stories #085 for April 2026   Philadelphia's City Hall doubles as an art gallery and sculpture garden. More than 250 of Alexander Milne Calder's statues grace the massive walls of the building. But at ground level on the plaza outside the building there are seven more statues, two of them equestrian.  Matthias William Baldwin was a precision jeweler turned locomotive manufacturer who gave away most of his wealth. His factory manufactured 75,000 railroad engines before it closed down. John McArthur, Jr., was a Scottish architect who specialized in massive buildings. City Hall in its time was the largest building in the Western Hemisphere.  William Bullitt was a transplanted southern lawyer who believed in both slavery and secession, yet he was chosen to rewrite the city's constitution in the 1870s.  Samuel Perkins was the Director of the Public Buildings who ruled over the proceedings with an iron fist and was rewarded with a private joke in the south entry portico.

Following a merger of Forum Energy Metals and Baselode Energy backed by the Ore Group, the company has consolidated its efforts on two flagship projects: the Aberdeen property in the Thelon Basin and the Hook project in Saskatchewan.Dr. Hunter brings significant credibility to the venture, having worked on Cameco's exploration team during the previous uranium boom and examined multiple world-class deposits including McArthur, Cigar, Dawn Lake, and Fox Lake. Her geological expertise centers on recognizing subtle alteration signatures in blind uranium deposits—a critical skill in frontier exploration where deposits lack surface expression.The company is currently deploying its $7 million treasury across both projects. At Hook in Saskatchewan, two drill rigs are operating with a $2.5 million budget, testing for resource expansion near existing mineralization. The flagship Aberdeen program will commence in June with a planned 10,000+ meter drilling campaign focused on the Loki target, where recent drilling intersected intense alteration across the entire sandstone column—similar to signatures seen above world-class Athabasca Basin deposits.Geiger achieved a significant technical milestone by intersecting uranium at the unconformity for the first time in the northeast Thelon Basin. While grades of 100-200 ppm remain sub-economic, this validates the geological model and confirms uranium-bearing hydrothermal systems are present. The next critical step is discovering high-grade mineralization over significant widths, which would fundamentally alter perceptions of the Thelon's potential.The investment thesis rests on first-mover positioning in an underexplored district with geological similarities to the Athabasca Basin, experienced technical leadership, and significant upside leverage. As Dr. Hunter noted, "The big deposits will be found there new ones big ones shallow ones." Today, my guests represent a cross-generational journey through the ever-evolving world of jazz.  Alex McArthur is a singer and storyteller whose resume reads like that of a musician three times her age. A winner of multiple JazzBuffalo Vocalist of the Year awards, Alex brings a distinct soulfulness to any music she lends her voice to, whether she's singing something she wrote herself, or flexing her interpretive muscles on the works of legends like Ella Fitzgerald, Stevie Wonder, and Mavis Staples.  Alex is joined today by a man who has acted as a significant mentor in her musical journey.  Pianist George Caldwell is credited by Alex as, quote, “the first person I ever attempted to sing jazz in front of, and the person who encouraged me to explore vocal jazz and improvisation in the first place.”  In his role as Director of the Student Jazz Ensembles and Music Piano Performance Instructor at the University at Buffalo, George has employed his decades of experience as a Grammy-winning professional musician to help mold the musical imaginations of countless young artists-in-the-making.  Those decades of experience include tenures with the Count Basie Orchestra, the Duke Ellington Orchestra, tap dancer and choreographer Saivon Glover, R&B legend Ruth Brown, and right here in Buffalo, the awe-inspiring Star People, a Miles Davis Repertory Ensemble.  Currently, Alex and George are in the midst of performing Lady Day at Emerson's Bar & Grill, a recreation of Billie Holiday's final performance, with Alex inhabiting the role of Lady Day, and George acting as Musical Director.  The show, a MusicalFare production, is running at Shea's Smith Theatre in Buffalo's Theatre and Entertainment District, through February 15. RIOTS IN NORTHERN IRELAND BECOME MORE KINETIC.// -----BEGIN TEARLINE------International Events-Middle East: Indications and warnings are growing regarding an unknown threat to the United States throughout the region. Reports have emerged indicating that the US Navy base in Bahrain is at an increased defensive posture, and that non-essential staffers and their families are currently being evacuated from the American Embassies in Baghdad, Kuwait, and Bahrain. A few moments ago the UKMTO office also issued a very vague warning statement, advising mariners to be advised of increasing tensions through the Strait of Hormuz.AC: At the moment, none of this is confirmed, but the rumors are trickling out anyway, since this appears to be a very time-sensitive threat, whatever it is. Nobody knows what's going on for sure, but a lot of diplomatic stations and military bases are at an increased level of readiness right now, which looks to be related to a very specific threat of some kind. Shooting from the hip (without any solid detail to go on), this could be related to an imminent Iranian nuclear test, or it could be related to Israel launching pre-emptive strikes to prevent such. Either way, the threat is extremely vague, but the heightened readiness seems real enough.Northern Ireland: Protests transitioned into rioting throughout several cities overnight, from a few different perspectives. Originally, riots broke out over the weekend in response to immigration issues and the assault of a child. However, as unrest has spread, opportunistic criminals have taken advantage of the fray to conduct looting that was unrelated to the initial incident.-HomeFront-USA: Counter-ICE protests have emerged in several major cities around the nation. So far, most of the more disruptive events have taken place in Los Angeles, Dallas, Austin, Seattle, Chicago, Atlanta, NYC, Providence, and Denver.Ohio: This morning evacuations were ordered for Vinton County after an industrial accident occurred at a local explosives factory in McArthur. Local authorities state that a very large tank of nitric acid began leaking at Austin Powders this morning, prompting the creation of a substantially large evacuation zone. The FAA has also placed a 30-mile Temporary Flight Restriction over the facility as the contents of the tank continue to present a hazard to the general area.-----END TEARLINE-----Analyst Comments: Last night the city of Los Angeles implemented a curfew for the city center, in an attempt to quell the rioting. Mass arrests were made in the DTLA area, however a state of civil unrest remains as demonstrations and protests have remained fairly persistent over the past week.Regarding the protest events for this weekend nationwide, roughly 1,655 events are scheduled for June 14th, most of which are being conducted and organized by the "No Kings" cause. Of course, it is highly likely that not all of these 1,600+ events will take place.
