In 1845, European potato fields from Spain to Scandinavia were attacked by a novel pathogen. But it was only in Ireland, then part of the United Kingdom, that the blight's devastation reached apocalyptic levels, leaving more than a million people dead and forcing millions more to emigrate. In Rot, historian Padraic X. Scanlan offers the definitive account of the Great Famine, showing how Ireland's place in the United Kingdom and the British Empire made it uniquely vulnerable to starvation. Ireland's overreliance on the potato was a desperate adaptation to an unstable and unequal marketplace created by British colonialism. The empire's laissez-faire economic policies saw Ireland exporting livestock and grain even as its people starved. When famine struck, relief efforts were premised on the idea that only free markets and wage labor could save the Irish. Ireland's wretchedness, before and during the Great Famine, was often blamed on Irish backwardness, but in fact, it resulted from the British Empire's embrace of modern capitalism. In 1845 a water mold named Phytophthora Infestans which afflicts potato and tomato plants began to spread across Europe, killing potatoes from Sweden to Spain. “The potato blight caused crisis everywhere it appeared in Europe,” writes my guest Padraic X. Scanlan; “in Ireland, it caused an apocalypse.” In 1845, a third of the United Kingdom's population lived in Ireland; an 1841 census had counted a population of 8.2 million. In the next six years, 1 million of them would die from famine related causes; another 1.5 million had emigrated. The 1851 census totaled the Irish population at 6.5 million, and the population of Ireland would continue to decline for another 100 years. “Although the labouring poor ate potatoes throughout northern and western Europe, only Ireland experienced demographic collapse during and after the blight pandemic.” And the consequences of the famine were more than demographic. It frayed or destroyed communal and familial relationships, and must have led to long-lasting psychological trauma. Padraic X. Scanlan is an associate professor at the Centre for Industrial Relations and Human Resources and the Centre for Diaspora & Transnational Studies at the University of Toronto. The Rich Zeoli Show- Full Episode (02/07/2025): 3:05pm- On Friday, President Donald Trump met with Japanese Prime Minister Shigeru Ishiba and held a joint press conference from the White House—where Ishiba announced he will increase his country's investment in the U.S. amounting to $1 trillion. The two world leaders also revealed plans for a joint venture which would send American oil to Japan. 3:40pm- Democrats continue to panic over the Trump Administration's threat to dismantle and restructure the United States Agency for International Development (USAID). On Tuesday, the administration announced that many employees within the agency will be placed on leave—with many contractors being laid off. Secretary of State Marco Rubio explained that many of USAID's activities are currently “conflicting” with State Department priorities. According to reports, USAID has been merged with the State Department. 3:45pm- As the Trump Administration begins to reconstruct USAID, Congressman Brian Mast posted a video to social media highlighting some of USAID's most outrageous expenditures. Rep. Mast's list includes: $15 million for contraceptives to the Taliban, $20k for drag shows in Venezuela, $450k to promote atheism in Nepal, $1 million to unionize gig workers in Brazil, $55k for a climate change presentation in Argentina, $350 million to build pier in Gaza, and $450k to help Indonesian coffee companies become more climate and gender friendly. 3:50pm- Make Straws Great Again: In a post to Truth Social, President Donald Trump announced that he plans to sign an executive order reversing the Biden Administration's initiative to ban plastic straws. Complicating fact of current life, it's becoming increasingly obvious that in order to stand up a practice that can take advantage of value-based care payments—payments where primary care docs mainly at this time can get paid more and likely more fairly to care for patients well—you need a lot of infrastructure. You need data, you need tech, you need a team. Translation: You need money, maybe a lot of money, to invest in all of this. These are the external realities that hit anyone trying to do right by patients from every direction. But on the other hand (or maybe different fingers on the same hand), as Dr. Amy Scanlan says in this healthcare podcast, physicians are the backbone of this system. Dr. Scanlan talks in the interview today about the opportunity, and maybe the responsibility, that physicians have here for patients; but also the Eric Reinhart article comes up again about rampant physician moral injury (unpaywalled link with my compliments). Right now might be a great time to read something from Denver Sallee, MD. He wrote to me, and he wrote, “Like many physicians, I did not have much understanding of the business side of medicine, as I mistakenly thought as long as I helped take great care of patients that I was doing my job. More recently, it became apparent to me that by ceding the management of medicine to nonclinical administrators and to companies interested primarily in value extraction for the benefit of shareholders that I needed more education in order to truly help patients.” Today as aforementioned, I'm talking with Amy Scanlan, MD, who is chief medical officer of the clinically integrated network (CIN) that is the joint venture between Intermountain Health and UCHealth in Colorado. We talk about what it's like to be in the kind of messy middle of transformation to integrate care in a clinically integrated network, trying to figure out how to help physician practices and the CIN itself navigate the external environment in a way that empowers different kinds of practices at different points in their transformation journey that empowers physicians to be in charge, and considering clinical and financial outcomes (ie, the business of healthcare).
