It's still seen as a taboo subject – something that our guest, Thrive Licensed Practitioner, Cathryn Giles, is passionate about changing.  Cathryn is an Early Years Transition Worker in the Education Service at Cardiff Council. As part of her role, Cathryn does outreach works with schools in Cardiff to support pupils who are having problems with continence and toileting. In today's episode, special guest Joan Morgernstern explains why toileting is no different. An educator, parent coach, and author, Joan has developed a kinder and more effective approach to “potty mastery.” Tune in to hear how to shift your mindset and language around toileting so you can empower children to learn this new skill. Essential Takeaways • Adults must shift their mindset around toileting and view it as supporting children's skill development, the same way we support them in reaching other milestones. We need to let go of the stress and pressure around toileting and remember that our children will eventually master this skill too. • Growing up is a stressor for many young children. Instead of telling children they're “a big kid now,” Joan encourages parents to tell children, “You have a new important job. You're a body boss now.” • The two key skills of potty mastery are 1) Listening to your body and 2) Taking care of your body. Before toileting, normalize the way kids are already using these skills (e.g., listening to their bodies when they're hungry or thirsty and taking care of their bodies by bathing, washing their hands, and going to sleep). • Celebrate missteps as an essential part of the learning process. Through every other milestone, children's missteps were celebrated. When they took their first step and immediately fell, for example, the adults in the room cheered. When toileting comes around and missteps are not celebrated or even neutralized, the shift is abrupt and scary to children. • Often, toilet training can be children's first experience with shame, leading to issues with performance anxiety. When we take a kinder, more empowering approach, we teach children that they have a support system behind them when they encounter challenges. And if it takes a bit longer to overcome a challenge, it's okay, because they're moving in the right direction. In this episode we answer all your potty training questions!Potty training can be confusing and overwhelming as a parent, and something that can lead to judgment from others if our kids don't do it at the "right" time." The average age kids are potty trained is 27 months, but is typical anytime between 18 months and 3 years old.It's important to wait to see some signs that your child is ready to start potty training, such as:1. Interested in the whole process2. Ability to tell you before or after he's gone3. Desire to imitate othersSometimes when parents introduce the potty chair when kids are 2 or 3 yeras old, it causes a lot of fear and avoidance of the entire process. Some suggestions to address this include:1. Introdoce the potty chair as soon as your child is mobile2. Put them on the chair before and after every bath3. Celebrate every small victory (dance party, singing, praise, etc.)Conversations about potty training often lead to questions about toileting issues, such as ongoing bedwetting, sneaky behavior related to toileting, smearing feces, etc. It's crucial that toileting issues do not receive consequences, other than natural consequences, such as helping clean up the mess, take the sheets to the washing machine, etc. Toileting problems are often the first sign of emotional distress for our kids, suggesting they don't have the ability to or don't feel safe enough to communicate what is going on with them or what they're feeling in a given situation.Subscribe, rate, and review us on Podchaser, Apple,  or wherever you listen to podcasts!We're planning a Q&A episode!! Nocturnal enuresis is involuntary urination while asleep. This can be very challenging for kids with autism, and their families and carers. Most kids aren’t able to stay dry through the night until they’re 5 or older because their bladders are too small, they lack muscle control, or they sleep too soundly to sense when their bladders are full. The best thing for you and your family to do would be to try to tackle nighttime toileting. However, I am aware that some of you have tried everything available and had no luck. I have put some bedding strategies below to try and make it slightly more manageable. Toilet training your child is a big task even for neuro-typical children. Parents often procrastinate over when to start and we keep delaying it for whatever reason we can think of! My biggest concern for children with ASD is if they are not out of nappies by five years of age then they often start to lose muscle control and can end up with long term bowel and bladder issues. Toileting is not something that the child will grow into or get better with age. We all need to action ASAP – and this includes schools. If a child is attending pre-school or school when the parents are toilet training then we all need to get behind the toileting programme and provide consistency all day every day until the child has mastered it. Malena Fryar "Use your critical thinking skills!” Do any of you remember hearing that being drilled in nursing school? How many of us are actually make critical decisions for your patients or are you deferring all decisions to the providers?  Really think about this. Decisions in triage - nurses do everything in this domain.  Most ED's don't even have a provider up in triage. We seem to do okay in this area.  But then the patient comes in to get worked up and now ALL decisions to the providers.  Why??!  Why do you have to stop making decisions? You know what’s going on - You were trained for this! As nurses, we are the ones at the bedside - who better to answer these questions, than us?! Nursing Autonomy ED nursing is awesome because we have autonomy - why give it away? Aside from the ED, the only other unit that has these privileges are the ICUs. Remember, we deal with ICU/critical patients - why not have the same autonomy? I encourage you to make clinical decisions in your practice. Yes, you may need orders for things but honestly, there shouldn't be an issue if the provider puts the order in 1-10 minutes later but gives a verbal order in the meantime. Don't let your patient suffer or crash over this. Especially not in your critically ill patient. Examples NPO status (needs procedure/waiting for labs/radiology) Toileting  (Can this patient walk to the bathroom?) Pain medication Labs (remind the doctor to put in the order, or even better have a protocol where you can just order them yourself!) i.e. Running gases for BIPAP patients repeat troponin levels BMP/ICU Venous panel levels for DKA Drips  (titrate them!) Respiratory interventions (bipap/intubation/vents) - learn how to adjust Changes in Vital Signs (is this concerning or is this a watch and wait situation?) Initiate an EKG I’s & O’s Multiple IV Lines I believe that we have relinquished a lot of our decision-making capabilities as an emergency nurse - so I’m making a stance and let’s take it back. Let's own our ED practice - we see and do a lot. So stop deferring decisions you can make as a nurse. Communicate with your providers. And let’s work together to get these sick patients better. Traditional toilet training Toilet training is a funny thing in that there are two completely different issues to understand. Firstly there are those puppies (and sometimes dogs) who simply need some help in the area of being reminded where we would like them to go pee pee and poo poo, (this is what we commonly know as toilet training). Read more about it and watch the videos on this free Video Course here: Puppy Potty Training. This issue is usually quite straightforward to solve, however there is often a lot of patience involved. But it has very little, if anything, to do with being the pack leader. Toilet training as a behavioural issue Dogs are pack animals and when a dog believes that they are the pack leader a whole host of issues can arise surrounding the area of toileting. Often a dog will have been toileting very happily for several years when suddenly you return to a very large poo in the middle of the carpet and it is completely out of character. This is what I would call a behavioural issue surrounding mixed messages about being the pack leader. Here is a list of just a few of the signs that suggests that your dog is in need of something other than more basic toilet training: Toileting in the house when you leave (even if it is for a short period) Marking your possessions such as shoes, bed, the children’s clothes Urinating outside then coming inside and finishing off Finding poo or wee right in the middle of the room, almost like your dog is trying to tell you something When you get these sorts of issues, no amount of training is going to sort things out. Rather you need to understand where your dog is coming from and why they are doing what they are doing. It’s about understanding what your dog is thinking and why. Then the solution will become obvious. Inside the podcast I have included lots of tips and help and advice about training puppies.
