Contributor: Nick Hatch, MD Educational Pearls: In the setting of traumatic injury, tranexamic acid (TXA) is given to stabilize clots which minimizes bleeding and decreases risk of hemorrhagic shock   Current TXA dose for trauma is 1 g bolus followed by a 1 g infusion; both doses should be given within 3 hours from time of injury  Due to the split dose and narrow window, patients with complicated care, particularly if they require transfer may miss the infusion dose  Various smaller studies have shown that dosing 2 g initially or 2 g followed by a 1 g infusion produces the same patient outcomes and no additional harm  Receiving hospitals should strive to acquire accurate information regarding previous doses of TXA given and confirm timeline of injury    References Roberts I, Shakur H, Coats T, et al. In this episode, Sun-Wei Guo, a geneticist turned Professor of Gynaecology, reveals his eye-opening research into the pathophysiology of endometriosis. Sun Wei explains how the theory of retrograde flow in endometriosis is necessary but not sufficient to explain the progression of the disease. Prof Guo explains that endometriotic lesions are wounds undergoing repeated tissue injury and repair (ReTIAR). Sun Wei further describes how surprising mediators in the endometriotic microenvironment are responsible for the incomplete repair process that leads to the cellular hallmarks of endometriosis. Hear about Prof Guo's preliminary work on herbal constituents and electrical stimulation that are generating encouraging results in endometriosis. Listen in to discover new non-hormonal targets that can be considered to combat this terrible affliction. Professor Guo received his Ph.D. from the University of Washington and had subsequent stints at University of Michigan and the Medical College of Wisconsin. Well, David Sinclair, a biologist and Harvard genetics professor, had high hopes when he discovered resveratrol, a molecule found in grapes could help reverse the effects of aging like inflammation and cell growth. He claimed one day there would be a pill people could take that would solve all of our aging problems. The media had a field day with his findings and soon resveratrol made its way into wellness as the new fad food. Better yet, you could find resveratrol in wine! But what happens when research studies go wrong? When the findings aren't always what they seem? The story of resveratrol is a great example of trusting the data too soon, before it's replicated over and over by other scientist.
