The Trump administration issued its first major budget document Friday, slashing non-defense discretionary spending by $163 billion — a 23% reduction from 2025 levels — and boosting defense spending by 13%. A fact sheet released by OMB references the administration's targeting of “woke” programs and “weaponized” government. One area that would see a significant boost under the budget is the Department of Veterans Affairs' electronic health record modernization program. The EHRM, whose perpetually plagued rollout has been chronicled in congressional testimony and in various watchdog reports, would be provided with a $2.17 billion funding increase in President Donald Trump's budget, per a summary document released Friday. The VA announced in March that it will have implemented the EHR in 13 facilities by 2026, with the possibility of deployment at all VA health systems as early as 2031. That followed a decision in 2023 to pause the system's implementation to renegotiate the contract with its developer Oracle Cerner and account for safety concerns. Friday's budget summary claimed the VA's EHRM rollout “had stalled under the Biden administration” but is a “top priority effort” for Secretary Doug Collins. The Technology Modernization Fund is shifting its funding model to prioritize the full repayment of new “high-impact” investments across the federal government, the General Services Administration said Friday. GSA's press release said the “strategic” change would provide a “streamlined path to modernization” for agencies by “combining upfront capital with specialized advisory services.” The agency said this “enhanced payment model” was pursued with strengthened longevity for projects in mind. Acting GSA Administrator Stephen Ehikian said in a release that “By ensuring full repayment of our investments, the TMF sends a clear message to federal agencies: focus on high-impact, high-return modernization efforts. Government IT contractor Carahsoft had its Reston, Va., headquarters searched by the FBI Tuesday morning, three sources familiar with the matter told FedScoop. In response to an inquiry about the raid, the FBI confirmed via email that it had “conducted court-authorized law enforcement activity” but declined to comment further. Meanwhile, the Department of Veterans Affairs is once again facing criticism from its inspector general related to its modernized electronic health record program in a pair of new reports published this week. According to one of those, the VA has neglected to put in place the proper controls for its Oracle Cerner electronic health record system to adequately prevent and respond to major incidents. To the uninitiated, interoperability may sound like a surgical term, but it actually refers to how IT systems and other technologies communicate with each other. The goal, of course, is seamless communication to improve efficiency and quality of care, but that's obviously a big challenge. Today on Raise the Line, we dive into this important issue with two people who come at it from different professional perspectives, but who share that goal. Oh, and they also happen to be brothers! Jake Engle is a Senior Director at Oracle Cerner, a supplier of health information technology used at thousands of facilities worldwide. His brother, Dr. Sam Engle, is a pediatric endocrinologist at Children's Wisconsin. On the patient care side, Dr. Engle talks about the value of having a complete patient history in hand prior to appointments to maximize the efficiency of the time spent, but also to avoid doing duplicate tests. “Especially with kids, you never want to have to repeat labs if you don't need to. I feel very strongly about that.” From the tech side, Jake Engle addresses the challenge of trying to synthesize data from multiple sources such as EHRs, insurance records, public health databases or commercial products focused on one niche of healthcare. “I think the healthcare systems are a bit late to the game and it's much a more complicated game.” You'll also learn about efforts to make it easier for patients to access their health data, the need for data standards in the industry and how their personal relationship contributes to this work.
The Department of Veterans Affairs reset its electronic health record modernization program in April, halting all future rollouts of the system, with the exception of the James A Lovell Federal Health Care Center in North Chicago, which will launch in concert with the FEHRM and DHA in March 2024. Since the reset, the agency has focused on improving the functionality and operations at five sites that currently use the new EHR. Its path forward includes a new Oracle-Cerner contract on May 16 to deliver the high-functioning, high-reliability, world-class electronic health record that veterans deserve. Stigma had already been on the decline, and now, available mental health services are on the rise. Some patients are now seeking treatment for the first time, while others are continuing decades-long treatment with better coordinated services. Many don't travel this journey alone. The help their family, friends and caregivers provide is critical—for some, a caregiver's records have provided the only continuity of care. Travis Dalton, general manager of Oracle Health, and Danny Gladden, director of behavioral health for Oracle Health, talk about their personal experiences in this area and how Oracle Cerner and Oracle Health can work together with health systems and clinics to alleviate the administrative burdens of caregivers to provide better care for behavioral health patients. September is Suicide Prevention Awareness Month. In general, 45% of individuals who die by suicide will have seen their primary healthcare provider within one month of their death, but only 20% will have seen a mental health professional. During the last few years, there has been an increased awareness and focus on mental health. Despite the increased availability of mental health apps and virtual connections to providers, the demand for services is still high. Recently, a national three-digit crisis number, 988, was rolled out in the US to help connect anyone in need with suicide prevention and crisis resources. What can health systems and providers do on a larger scale to help prevent suicide? In this episode, Danny Gladden, director of behavioral health at Oracle Cerner, talks with Dr. Caitlin Thompson, clinical psychologist and chief clinical officer at Red Duke Strategies, who formerly served as the national director of suicide prevention at Veterans Affairs. Some key points: - Many cloud deployment start as non mission critical, in a single cloud and organically grew into a giant mess that is hard to untangle, with several design flaws, lack of visibility, security holes and operational/governance nightmares. - CSP by design focus less on networking features as they have to prioritize durability, performance availability and ensure environment is secure. Pace of innovation is slow as they try to recreate 30 years worth of capabilities in a cloud way, which will take time. -Every single customer they met were either multi-cloud already or looking to extend in other clouds.
