The proliferation of unnecessary medical tests and procedures not only harms patients but costs the United States billions of dollars every year. Between 2019 and 2021, Medicare spent as much as $2.4 billion on unnecessary coronary stents alone. At some hospitals, it's estimated that more than half of all stents are unwarranted. And the medical system is not fit for purpose: designed to deliver health care not health. Much of medicine doesn't do what it is supposed to do: improve health. That is the view of orthopaedic surgeon, Ian Harris. Ian believes too many drugs are being prescribed, too much surgery is being performed, and there are too many unhelpful tests, scans, and overdiagnosis. The 'business' of medicine, he says, is taking precedence over what the science tells us. We should go back to the first principles of the Hippocratic oath and ‘first, do no harm'.

Dr. Greene wil be discussing the impact of overtreatment "successes" in the field of temporomandibular disorders. Dr. Greene has been involved for many years in clinical research and teaching in the area of temporomandibular disorders and orofacial pain. From 1965-1984 he worked with Dr. Daniel Laskin in the UIC College of Dentistry TMJ and Facial Pain Research Center. From 1986 to 1995 he worked at Northwestern University Dental School with Dr. Harold Perry as co-director of their TMD Clinic. Since returning to UIC in 1995, Dr. Greene has served as Director of Orofacial Pain Studies. He was a Clinical Professor in the Department of Orthodontics, and he has contributed many articles and lectures to the orthodontic specialty in the USA and abroad. He has published over 150 articles, book chapters, and abstracts, and was co-editor of a multi-author book about temporomandibular disorders in 2006. A new multi-author book on TMDs co-edited by Dr. Greene and Dr. Laskin was released in 2013. In 2015 Dr. Greene was one of four co-editors on a book entitled “TMD and Orthodontics”. He is a Life Member of the American Academy of Orofacial Pain as well as the International Association for Dental Research. He retired from UIC in 2018 and is currently Clinical Professor Emeritus.

In this health care podcast, I speak with Dr. Marty Makary about his new book, which is entitled The Price We Pay: What Broke American Health Care—and How to Fix It. I could not recommend this book more highly. It’s a page turner for hospital execs trying to do the right thing, employers trying to do right by their employees, insurance carriers looking for better ways to actually drive health care value, and doctors and nurses who are feeling burnout because they see their organizations demanding them to do things misaligned with their mission to do the best they can by patients. Dr. Makary tells me in this interview that his intent with this book was to shine light on some of the issues, mainly around the price we—as patients, taxpayers, employers, basically all of us—pay. Dr. Makary says that understanding the situation is the first step toward navigating and redressing it. The Price We Pay gives multiple examples of egregious pricing. I’m going to split these examples into two categories: First, your basic price gouging, including surprise billing and what amounts to predatory pricing done at scale. The second category are high total prices because the services rendered were some shade of unnecessary. So high prices based on the price of the unit, and then high prices based on the number of units delivered. Dr. Makary and I talk about both challenges in this health care podcast. We also talk about the multiple instances where doctors and nurses and others are doing the right thing and really working hard to correct issues.
