On this day in legal history, August 16, 1918, Lothar Witzke was convicted of espionage in the United States on behalf of Germany and became the first German spy convicted during World War I. Lothar Witzke was a junior officer in the German Imperial Navy during World War I, who became a spy and saboteur in the United States and Mexico. After escaping internment in Chile, he reached San Francisco in 1916 and began sabotage activities with another agent, Kurt Jahnke. They were involved in various missions, including suspected connections to significant explosions, though later investigations ruled out their involvement in some cases. Witzke was arrested in 1918 near the Mexican border, convicted, and sentenced to death, but his sentence was commuted to life imprisonment by President Woodrow Wilson. After an act of heroism in prison and diplomatic pressure from Germany, he was pardoned and released by President Calvin Coolidge in 1923. Upon his return to Germany, Witzke was decorated with the Iron Cross and later served in the Abwehr, or German military intelligence, during World War II. DeSantis's background includes a Harvard Law School education and time as a U.S. Navy lawyer, while Trump has been known to criticize and even sue attorneys. Many larger law firms seem uncomfortable supporting Trump, contributing to DeSantis's advantage.DeSantis's fundraising from lawyers has also surpassed other GOP primary candidates, including former Governor Nikki Haley and Senator Tim Scott. His campaign haul includes significant contributions from elite law firms like Sullivan & Cromwell. Hernandez, an MLB umpire since 1993, claimed he had been discriminated against after being passed over for crew chief five times between 2011 and 2018. He also cited a "history of animosity" with Joe Torre, MLB's chief baseball officer at the time of the lawsuit. The court found that Hernandez failed to show a statistically significant disparity in promotion rates, despite a "bottom-line imbalance" between white and minority crew chiefs. It also rejected Hernandez's claim that the judge erred in accepting MLB's reasons for not promoting him, which included a missed call and an "overly confrontational style." This weekend proved to be pretty busy even though we were all properly self-quarantined. We had a few activities for you to try and compete for a prize or two courtesy of Jims Steakout! Lori La Bey the host and founder of Alzheimer's Speaks Radio interviews Jay Benedict, Outreach and Sales Director for Cherrywood Pointe, an Ebenezer managed property.  This new community is located in Roseville, MN just off Lexington Ave just north of County Road C. Jay will discuss a unique support group called "Connections" he created in 2015, for those with memory loss and their care partners which meets monthly.  He also is an active member of the Roseville Alzheimer's and Dementia Community Action Team, which is a grassroots effort making a big impact in it's community as well as surrounding neighborhoods.    Jay has also worked closely with the Ramsey county library to bring the new dementia support system and tools called, "Memory Minders."   Carol Jackson with the library has been leading this cause.  Jay is also working with his community and the Fire Department with a new virtual tool they have developed. Welcome to the newly released Beating Diabetes Podcast with Dr. Michele. This year the podcast will be focusing on all things diabetes. As your host, I’ll be recording solo episodes which will answer some of the most common questions I’m asked as a physician such as “what CAN I eat as a diabetic?”, “how can I lose the extra weight that I’ve put on recently?”, “is it really possible to reverse type 2 diabetes?”, “What can I do to prevent my children from getting diabetes since it runs in the family?” and more. I will also be coaching patients and clients live on-air, and conducting interviews with other diabetic experts. Today Dr. Michele is discussing diabetic neuropathy and how it leads to infections and amputations of your feet.  With the increase in diabetes in recent years, we have seen an increase in many of the complications of diabetes. One of those complications is diabetic neuropathy, which is the numbness of the feet (and sometimes hands) of diabetic patients. And as with everything else in life, there is a trickle down affect when we look at diabetes, and this is why diabetes is such a big deal! Please read the example below: A simple example would go like this: A patient has diabetes for 10 years, develops numbness (diabetic neuropathy) in his feet, develops a blister on his right big toe while wearing new shoes, but because of the numbness, he does not feel the blister and continues to wear the new shoes for over 8 hours in one day. The blister pops on its own and develops into a wound (diabetic foot ulcer) which becomes infected, and after noticing pus and a bad smell coming from his foot, the patient tries to treat it on his own by soaking his foot and applying Neosporin and band-aids at home. He finally comes to see me in the office for a consultation about 3 weeks later when he realizes that it is not improving. When I see the patient, the foot is reddish pink, swollen, warm to the touch, and pus is coming out of the ulcer. I take x-rays of the foot and see that the bone is also infected. Gangrene has not set in yet, but it could happen. This patient is at risk for amputation and further complications down the road. This is a serious complication of diabetes, and unfortunately it is also very common. If you or a family member has diabetes, please schedule an appointment with your podiatrist today to evaluate your foot health and to begin a diabetic foot care program (which will prevent tragedies such as this example). Every 30 seconds, a lower limb is amputated due to a diabetic foot infection.   If you or a family member has diabetes, do NOT hesitate in scheduling an appointment with your podiatrist today to avoid a diabetic foot infection which can lead to amputation. You do not need to be a statistic if you have diabetes. There are many preventative measures that can be administered by your podiatrist such as Diabetic Shoes and a Diabetic Foot Care Program. Diagnosed: 23.1 million people Undiagnosed: 7.2 million people Prevalence of diabetes among different age groups Age 18-44 years: 4.6 million  Age 45-64 years: 14.3 million Age 65 years or older: 12.0 milion  Men:  15.3 million Women: 14.9 million *This total included: 132,000 children and adolescents younger than age 18 years (0.18% of the total U.S. population younger than age 18 years). 193,000 children and adolescents younger than age 20 years (0.24% of the total U.S. population younger than age 20 years). About 5% of people with diabetes are estimated to have type 1 diabetes.  Prevalence of diabetes by race/ethnicity among people 20 years or older The prevalence was higher among Native Americans/Alaska Natives (15.1%), non-Hispanic blacks (12.7%), and people of Hispanic ethnicity (12.1%) than among non-Hispanic whites (7.4%) and Asians (8.0%) Among people of Hispanic ethnicity, Mexicans had the highest prevalence (13.8%), followed by Puerto Ricans (12.0%), Cubans (9.0%), and Central/South Americans (8.5%) Among Asians, Asian Indians had the highest prevalence (11.2%), followed by Filipinos (8.9%), and Chinese (4.3%). Other Asian groups had a prevalence of 8.5% Prevalence varied significantly by education level, which is an indicator of socioeconomic status. Specifically, 12.6% of adults with less than a high school education had diagnosed diabetes versus 9.5% of those with a high school education and 7.2% of those with more than a high school education Prevalence of Prediabetes An estimated 33.9% of U.S. adults aged 18 years or older (84.1 million people) have prediabetes, based on their fasting glucose or A1C level. Nearly half (48.3%) of adults aged 65 years or older had prediabetes. Among adults with prediabetes, 11.6% reported being told by a health professional that they had this condition. Age-adjusted data indicated that more men (36.6%) than women (29.3%) had prediabetes. Prevalence of prediabetes was similar among racial and ethnic groups. Risk Factors for Complications Smoking: 15.9% of adults are current smokers, and 34.5% had quit smoking but had a history of smoking at least 100 cigarettes in their lifetime. Overweight and Obesity: 87.5% of adults were overweight or obese, defined as a body mass index (BMI) of 25 kg/m2 or higher. Physical Inactivity: 40.8% of adults were physically inactive, defined as getting less than 10 minutes a week of moderate or vigorous activity in each of the physical activity categories of work, leisure time, and transportation. High Blood Pressure: 73.6% of adults had systolic blood pressure of 140 mm Hg or higher or diastolic blood pressure of 90 mm Hg or higher, or they were on prescription medication for high blood pressure. High Cholesterol (Hyperlipidemia): 58.2% of adults aged 21 years or older with no self-reported cardiovascular disease but who were eligible for statin therapy were on a lipid-lowering medication. 66.9% of adults aged 21 years or older with self-reported cardiovascular disease who were thus eligible for statin therapy were on a lipid-lowering medication. High Blood Glucose (Hyperglycemia): • 15.6% of adults had an A1C value higher than 9%. Complications of Diabetes:  Amputations More than 60 percent of non-traumatic lower-limb amputations in the United States occur among people with diabetes. Nearly 82,000 non-traumatic lower-limb amputations are performed among people with diabetes each year. Non-Hispanic blacks are 2.7 times as likely to suffer from lower-limb amputations as non-Hispanic whites. Kidney Disease Among U.S. adults aged 20 years or older with diagnosed diabetes, the estimated crude prevalence of chronic kidney disease (stages 1–4) was 36.5%. Among those with diabetes and moderate to severe kidney disease (stage 3 or 4), 19.4% were aware of their kidney disease. Approximately 53,000 people develop end-stage renal disease with diabetes as the primary cause each year. Preventing Diabetes Complications A podiatric physician, a doctor focusing on the treatment of diabetic foot and ankle conditions, plays an integral role in a diabetes management team. Diabetes can affect many parts of the body and can lead to serious complications such as blindness, kidney damage, and lower-limb amputations. Working together, people with diabetes and their health care providers, such as a podiatric physician, can reduce the occurrence of these and other diabetes complications. Comprehensive foot care programs can reduce amputation rates by 45 percent to 85 percent. Research in the United States and abroad has found that lifestyle changes can prevent or delay the onset of type II diabetes among high-risk adults.  Lifestyle interventions included diet and moderate-intensity physical activity, such as walking for 2.5 hours each week.  My Clinical Experience As you many know by now, I’ve been practicing medicine for over 20 years, and in that time I have seen thousands of diabetic patients. My practice is specialized to focus on diabetic care and to prevent diabetic limb loss and amputations. I have discovered that most of my patients are not educated much about how to prevent complications of diabetes or how to reverse diabetes by their primary care physicians. I spend much of my time educating my patients about diabetes, what they should and should not be eating (and it’s not just about table sugar), how they can start to lose the stubborn weight and bloated belly by making a few simple changes in their daily routines, and how to save their eyes, kidneys, and feet from the damage that diabetes can do. My passion is to help pre-diabetics, borderline diabetics, women who have been diagnosed with prenatal diabetes (research has found this to be a precursor to type II diabetes), and people who have been diagnosed with metabolic syndrome or insulin resistance, reverse their diagnosis and prevent full blown type II diabetes from taking over their lives. Interview with Eleanor Goldfield This week we spoke with Eleanor Goldfield, who is an activist on the far left and a proponent of creative activism, a stance that centers creative approaches to direct action with the aim of leading people to interrogate their own politics on an emotional and psychological level. Vinegar often plays an essential role in the food we eat. We use it in everything from baking to braising to pickling. But, author Michael Harlan Turkell writes that vinegar is "underappreciated and little understood." For his new book Acid Trip: Travels in the World of Vinegar: With Recipes from Leading Chefs, Insights from Top Producers, and Step-by-Step Instructions on How to Make Your Own, Turkell set out to give vinegar its due. He traveled the world, learning how countries from Japan to France make and use vinegar. He also collected recipes from chefs who are using vinegar in exciting, different and delicious ways. OEUFS EN MEURETTE, FROM BERTRAND A UBOYNEAU, BISTROT PAUL BERT, PARIS, FRANCE SERVES 4 This dish takes the concept of bourguignon sauce and uses it to poach eggs. What you’re left with is the same rich stock, adding the decadence of a creamy egg yolk, with a side of toast to sop it all up. Bertrand, always in need of acidity, uses a portion of red wine vinegar in place of some of the red wine, which gives a much lighter quality to a dish that usually invites a postprandial nap, and instead has you feeling like conquering the day ahead. ¼ pound (115 g) THICK SMOKED BACON, cut into lardoons 1 tablespoon BUTTER ¼ pound (115 g) WHITE PEARL ONIONS, peeled, tops and bottoms trimmed 1 clove GARLIC, crushed ¼ pound (115 g) BUTTON MUSHROOMS, cleaned, cut into quarters 3 cups (720 ml) RED WINE, such as Burgundy, Beaujolais, Cabernet 1 branch THYME 1 cup (240 ml) RED WINE VINEGAR 4 EGGS, kept in shell, cold BLACK PEPPER PARSLEY LEAVES, optional TOAST and BUTTER   In a large saucepan over medium heat, render the bacon for 5 to 7 minutes, until it’s just browning but not burning. If it’s cooking too fast, lower the temperature. Pour out all but about 1 tablespoon of the fat (reserve the excess to cook with another time) and set the bacon aside (you’ll add it back in later, so try not to snack on it too much). Add the butter, onions, and garlic and cook for about 1 minute, until aromatic. Lower the heat to medium-low, add the mushrooms and cook for another 2 minutes, stirring occasionally. Add the red wine, scrape the bottom of the pan to release the fond, and add the thyme. Bring back to a simmer and cook for 45 minutes, or until reduced by a third. Add the red wine vinegar and continue to cook for another 30 minutes. (If it’s too acidic for your taste, add ¼ cup water at a time until it’s not.) To poach the eggs, either in the pot of sauce itself (if you don’t mind a few stray pieces of egg white) or in a separate pot of water, bring the liquid to a bare boil. Make a small pinprick on the larger end of each egg, place in the liquid, and cook for 30 seconds (a Julia Child tip); this is just to set the whites. Remove the eggs and crack them into individual small bowls. Slide the eggs back into the pot to poach them. If you like a soft yolk, cook for only a few minutes. Using a slotted spoon, remove the eggs and set aside. In individual serving bowls, evenly distribute the onion and mushroom mixture, then pour a bit of the sauce, enough to cover an egg, into the bowl as well. Place the eggs into the bowls and garnish with the bacon, freshly cracked black pepper, and parsley, if using. Bon appetit! Tuesday, October 18th, 2016, continued Let me preface this second part of the story of today’s journey with full credit and a note of gratitude to Robin Pokorski, who worked with The Margaret Sanger Papers Project for a time. As with the MSPP overall, I found Pokorski’s project Mapping Margaret Sanger to be an absolutely invaluable resource... …I exit Grand Central Station, where I’ve just returned to Manhattan from the Sanger clinic site in Brownsville, Brooklyn. I follow the signs to the exit which lets me out right underneath the Chrysler Building at 405 Lexington Ave, just north of 42nd St. I’ve long been curious about this building but for one reason or another, had never made it here. It’s fully as handsome on the outside and lovely on the inside as I’ve heard. It shoots up to the sky enthusiastically and towers overhead with almost aggressive confidence and optimism. I love its Art Deco style, and I’m excited to see all the wonderful architecture and art of this period that this trip will take me to... Pokorsky writes, ‘At 4.30pm on April 20, 1939, Sanger met with Bill Melon at the Chrysler Building.’ I have not been able to discover who Melon was or why she met him here yet; I await her response to my inquiry....
