As two doctors with cultural backgrounds from Sri Lanka and China, we opened up about the unique challenges (and opportunities!) we face when it comes to food, family, and health. This one's close to my heart

The obesity epidemic isn't about calories-in-calories-out. Instead, chronic stress and high insulin levels from refined carbs and sugar are drivers of fat storage. And the type of fat stored matters, since visceral fat accumulation leads to worsening metabolic health. Dr. Robert Lustig and Mike Haney discuss the different types of fat and how they are stored and burned; what thin on the outside, fat on the inside means; whether people can be healthy at any size; environmental factors that contribute to obesity; and what people can do to manage weight. In this episode we are learning about TOFI - thin on the outside and fat on the inside. Just because someone in slim or "skinny" does not mean that they are healthy. Especially if that person does not have a lot of muscle. TOFI can be a high risk factor for several metabolic diseases and sometimes the only way to find out if you are at risk is through testing. Episode #144 In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community: I've known for a long time how neuroprotective ketones are for the brain, but, wow, what ketones do for the heart is amazing, especially the failing heart. Could you elaborate, Dr. Fung? [02:15] Do you need to be in ketosis to burn fat and does being in at least a low level of nutritional ketosis help you go into autophagy sooner? [04:39]  Why does it take such a long time to become fat adapted? [07:20] I've heard that fasting “turns on” survival genes. Can you tell us a little more about this? [09:25] What are your thoughts about the new recommendation to carb load before surgery to “calm” someone? [11:12] Is there any correlation between fasting and pulse rate? [12:58] I have a diabetic friend who is participating in a study on duodenal mucosa resurfacing (DMR) as a treatment for T2D. Does Dr. Fung have any take on this pretty new procedure or explanation of how and why it works? [13:17] If a person wants the benefit of therapeutic fasting (like autophagy) but they do not have adequate body fat for longer fasts (for example, a TOFI person with a fatty liver, or somebody at 30% body fat who needs a lot of autophagy for an autoimmune condition), How should they eat on eating days to be able to maintain a therapeutic fasting schedule like an alternate-day fasting protocol? [16:33] What are the signs of a histamine intolerance and can it get worse over time? Does fasting help? [17:53] If autophagy is mostly switched on by the presence of mTor, could a person (in principle) eat a zero protein diet and eat exclusively fat but still ramp up autophagy enough for some protein recycling to aid loose skin reduction, macular problems, skin tags, etc.? [18:45] How do you get rid of visceral fat? [20:22] How long does the effect of dairy on insulin last? [21:45] Why is it easier for women to do longer fasts after they've had a small amount of tubers during the last meal before their fast? [24:41] Please note that you need to be a member of the TFM Community to submit questions to the Q&A webinars with Dr. Fung but you can submit questions to our regular Q&A episodes here: https://bit.ly/TFMPodcastQs Learn More About Our Community: https://www.thefastingmethod.com Join our FREE Facebook Group: https://bit.ly/TFMNetwork Watch Us On YouTube: https://bit.ly/TFMYouTube Follow Us on Instagram: @fastingmethod This podcast is for educational purposes only and is not a substitute for professional care by a doctor or other qualified medical professional. This episode is brought to you by Rupa Health and Athletic Greens. It's no secret that being overweight or obese can increase your risk for numerous health problems, including stroke, some cancers, heart disease, and depression. What many people don't realize, however, is that we can suffer all these problems even if we're normal weight or underweight. Scientists have even coined a phrase for this phenomenon: “thin on the outside, fat on the inside,” or TOFI. Instead of being “overweight,” these people are “under-lean” (i.e., they don't have enough muscle) and have a little extra weight around the middle, or “belly fat.” TOFIs have the metabolic features of a prediabetic obese person: Low muscle mass, inflammation, high triglycerides, low good cholesterol, high blood sugar, and high blood pressure. In today's episode of my series I'm calling Health Bites, I am talking about “skinny fat,” why it can be worse than obesity, and how to address it. Janice has a great foundation after going through the Virgin Diet and is a regular user of Reignite Wellness All-In-One Shakes, but she still has goals of achieving a flat tummy and excellent sleep. JJ explains how these two objectives are connected (not sleeping well impacts your belly fat), and shares the simple tweaks Janice can make to achieve her goals. You'll learn how to avoid being TOFI (thin on the outside, fat on the inside), the importance of weight-bearing exercises and a protein-first approach to food when it comes to building muscle as you age, how to adjust meal timing for more restful sleep, and the specific measurements to track and monitor to meet your body-composition goals. Plus, JJ tackles the controversial topic of snacking! Everyone wants to sneak in their favorite snacks, but is it really serving your health? The confusion over diet or nutrition is because patients have no idea what to eat. It's not a lack of money or a busy schedule. A physician assistant turned culinary and nutrition educator, Shefaly Ravula provides a personalized approach to her patients to develop their diet plan. When her father, a vegetarian and a thin man, had a heart attack, she was left perplexed. It was an example of TOFI - thin outside, fat inside. The experience made her immerse herself to study nutrition for cardiac health. She also studied cardiology and even lipidology. She attended different conferences on integrative medicine, functional medicine, and culinary medicine. She now focuses on teaching food-as-medicine cooking classes. Her desire is to help address diseases with the right food to eat. Episode #34 In this Fasting Q&A episode, Megan and Nadia answer your fasting questions: 1. I'd like to fast primarily for therapeutic reasons (mental health and autophagy especially) and aim to do three 42s. My issue is raging ketones! How high is too high when it comes to BK and at what point should I break a fast for this reason? [01:18] 2. What is the best protocol for weight loss? [11:58] 3. I am a TOFI and a pre-diabetic. What fasting can I do to reverse my T2D without losing weight? I noticed when I do extended fasting of 42 hours I lose too much weight. [21:18]  4. The one issue that I'm having the hardest time with is my mood during an extended fast (longer than 24 hours). I feel extremely irritable, agitated, quick tempered, & anxious, which is not how I normally am. I don't know if it's my cortisol levels, or what. Is this common? My guest on this week's Feel Better Live More podcast is Dr Robert Lustig, Professor of Paediatric Endocrinology at the University of California. He's a leading public health expert who has long been exposing the myths of modern medicine and the food industry. His passion is communicating how sugar and ultra-processed food is fuelling the chronic disease epidemic that we are all facing today. Obesity, type 2 diabetes, metabolic syndrome, heart disease and so much more are caused, in Rob's view, by the foods that we are eating. In his latest book, Metabolical: The Lure and the Lies of Processed Food, Nutrition and Modern Medicine, he outlines what he calls the ‘hateful eight' – the eight root causes in our body that underlie all chronic disease. He explains how food and sugar impact on them and, most importantly, suggests strategies to counteract them. In this conversation, Rob explains why sugar is so damaging and explains that just like alcohol, our bodies can cope with sugar in small amounts. But in excess will end up in the liver and ultimately trigger us to get sick. Rob's decades of clinical experience and research has led to his bold and compelling assertion that the answer to all chronic disease can be found in real food. His solution? To ‘protect the liver, feed the gut'. I think these 6 words are an elegant way of summarising the nutritional advice that all of us should consider taking on board in order to improve our health and wellbeing. We talk about what constitutes ‘real food' and how different levels of food processing are classified. Rob explains why sugar-sweetened drinks are so disastrous for health, particularly in children – and why diet drinks do just as much damage, if not more. We also talk about TOFI (thin on the outside, fat on the inside) and fascinatingly, Rob outlines the three types of fat gain that we can all experience: subcutaneous (which you can see); visceral (stress-related fat around the middle), and liver fat. It's only the first of these that you're likely to notice – but it's the latter two which we really need to fix, especially as they're already appearing in kids. This conversation is full of mind-blowing facts and insights but it's also really empowering and contains simple, practical tips that all of us can use to improve our lives.
