Welcome back to Diabetes Dialogue: Technology, Therapeutics, & Real-World Perspectives!The US Food and Drug Administration (FDA) has approved orforglipron (Foundayo), a once-daily oral GLP-1 RA, for chronic weight management in adults with obesity or overweight and ≥1 weight-related comorbidity.1In this episode of Diabetes Dialogue, cohosts Diana Isaacs, PharmD, and Natalie Bellini, DNP, discuss the FDA's decision and the myriad new treatment opportunities afforded by orforglipron's clearance for entry into the market.The approval of orforglipron is supported by the phase 3 ATTAIN clinical development program, which includes 2 global, randomized, double-blind, placebo-controlled trials evaluating efficacy and safety over 72 weeks.¹ The ATTAIN-1 trial enrolled 3,127 adults with obesity or overweight without diabetes, while ATTAIN-2 enrolled more than 1,600 adults with obesity or overweight and type 2 diabetes.1In ATTAIN-1, participants receiving the highest dose of orforglipron achieved a mean weight reduction of approximately 12.4% from baseline at 72 weeks among those who remained on treatment, compared with 0.9% in the placebo group. Reported average absolute weight loss was 27.3 lb versus 2.2 lb, respectively. Across all randomized participants regardless of completion status, mean weight loss was 11.1% with orforglipron compared with 2.1% with placebo.1Secondary outcomes included improvements in cardiometabolic risk markers such as waist circumference, non–high-density lipoprotein cholesterol, triglycerides, and systolic blood pressure. However, detailed peer-reviewed data from ATTAIN-1 and ATTAIN-2 remain limited at the time of reporting, and full efficacy and safety results have not yet been widely published in the medical literature.The safety profile of orforglipron appears consistent with the GLP-1 receptor agonist class. Common adverse events include gastrointestinal symptoms such as nausea, diarrhea, constipation, vomiting, and abdominal pain. A boxed warning highlights a potential risk of thyroid C-cell tumors, based on findings observed with other GLP-1 receptor agonists in rodent studies, though human relevance remains uncertain.1,2The magnitude of weight loss observed with orforglipron in ATTAIN-1 appears clinically meaningful but may be modest relative to leading injectable agents. Cross-trial comparisons should be interpreted cautiously due to differences in study populations and designs. Additionally, treatment persistence rates and discontinuation due to adverse events will be important considerations in real-world use.Ongoing and planned studies are evaluating orforglipron across additional indications, including type 2 diabetes and other cardiometabolic conditions. Will he delay long in helping them? 8I tell you, he will quickly grant justice to them. And yet, when the Son of Man comes, will he find faith on earth?”
Read OnlineJesus told his disciples a parable about the necessity for them to pray always without becoming weary. Luke 18:1In our parable for today, we have the witness of a widow who came to a dishonest judge and continuously begged him for a just judgment. Though the judge did not care about the woman, he eventually rendered a just decision for her because she was so persistent. It's interesting that Jesus used the image of a “dishonest judge” to teach us about persistent prayers being answered. He does so because He wants us to understand that if even those who are dishonest respond to persistence, then so much more will the Just Judge of Heaven respond to persistence.Will God answer any prayer you pray if you offer that prayer day and night, day after day without fail? Does God eventually give into our requests as a parent might give into a child who keeps begging for something? Not exactly. One of the most important qualifiers mentioned in this parable is the word “just.” We read that the woman's plea before the judge was, “render a just decision for me…” At the conclusion of the parable, Jesus gives this interpretation: “Will not God then secure the rights of his chosen ones who call out to him day and night?” When we pray, we ought not pray for whatever we want. We ought not pray for our preference, selfish desires, or our own ideas. We must pray only for the justice of God. When we do so with unwavering perseverance, God will secure our rights and bring forth His justice.Justice, in the mind of God, is not only about righting certain wrongs. The prime example of this is the death of our Lord Himself. Clearly, Jesus was purely innocent and yet He suffered greatly. For that reason, would we conclude that the suffering and death of the Son of God was an injustice? Not really. The reason for this is that justice can be achieved best by mercy. Because Jesus embraced the injustice of His suffering and death and turned it into a free embrace out of love, this “injustice” became a sacrifice of love by which an abundance of mercy was bestowed. Jesus had every right to call down fire from Heaven and to destroy those who sinned against Him. But He had a far better plan. Instead, by choosing to accept the injustice of the Cross, and by freely embracing it with His own will, the injustice was transformed and a far greater good came forth.In our own lives, whenever we are wronged by another, we are often tempted to anger and to desire revenge in the name of justice. We want them to pay for what they did. If you ever feel that way, know that the greatest form of justice is mercy. Know that your free embrace of injustice brings forth the transforming power of God in a way that punishment or retribution could never accomplish. This is the form of justice we must pray for night and day. We must beg God for the ability to accept all sufferings with love, to offer those sufferings as a sacrifice, and to allow them to be transformed into mercy. If this is our persistent prayer, we can be certain that our prayer will be answered. Reflect, today, upon anything for which you desire justice. Is there anything that has been unfair in your life? Do you dwell upon any hurt inflicted upon you by another? As you call those things to mind, know that the power of your persistent prayer has the potential to transform those sufferings into God's mercy. Beg for this gift and know that God will always answer those prayers. Most just Judge, You desire to transform every injustice into mercy by calling us to freely embrace those injustices out of love. This is a high calling, dear Lord, but it is also a glorious one. Please give me the grace I need to follow Your example and to persevere in this form of prayer always. Jesus, I trust in You. Read OnlineJesus was praying in a certain place, and when he had finished, one of his disciples said to him, “Lord, teach us to pray just as John taught his disciples.” Luke 11:1In today's Gospel we are given three teachings on prayer. First, Jesus teaches His disciples what has come to be called “The Lord's Prayer.” Second, He teaches about the importance of persistence in prayer. Third, He teaches about the fruit that comes from correct and persistent prayer.The Church Father Tertullian said that The Lord's Prayer “is truly the summary of the whole Gospel.” Saint Augustine said, “Run through all the words of the holy prayers [in Scripture], and I do not think that you will find anything in them that is not contained and included in the Lord's Prayer.” Saint Thomas Aquinas said, “The Lord's Prayer is the most perfect of prayers.... In it we ask, not only for all the things we can rightly desire, but also in the sequence that they should be desired” (See the Catechism of the Catholic Church #2761–2763).It's amazing to consider what these great teachers of the faith have said about this short prayer. Perhaps because of our familiarity with this prayer we can easily gloss over the depth of its meaning. We can fail to use it as a foundation and model for all of our prayer. One way to correct this tendency is to use The Lord's Prayer for an extended period of time by prayerfully pondering every word slowly and meditatively. Doing so will help open us up to these perfect “instructions” on how we should pray.Immediately after Jesus taught this short prayer, He taught a lesson about persistence in prayer. He taught that we must not simply say a few prayers and leave it at that, giving up if they do not appear to be answered. Instead, we must continuously beg God for His grace until it is bestowed in its fullness.What will we receive from persistent prayer? This is an important point. We ought not go to God with our own wants and desires. We ought not beg Him for things that do not fall within His perfect will. Instead, when our prayer is modeled on the Lord's Prayer, and when it is persistent and grounded in faith, then our prayer will be for what the Father bestows upon us, that is His will alone. We must pray that His Kingdom will come. We must trust that He will provide for our needs. We must seek His forgiveness for our sins, and we must pray that He will protect us from the evil one.Reflect, today, upon that perfect prayer, The Lord's Prayer. Spend time studying it, thinking about each petition, the order in which Jesus laid it out, its simplicity and its clarity. Acknowledge that because we pray The Lord's Prayer so often, we can sometimes miss its true meaning and beauty. Our Lord gave us this prayer for a reason. Make sure that you do all you can to discover its meaning and practice its teaching. Our Father, Who art in Heaven, hallowed be Thy name. Thy kingdom come, Thy will be done, on earth, as it is in Heaven. Give us this day our daily bread and forgive us our trespasses as we forgive those who trespass against us; and lead us not into temptation, but deliver us from evil. Amen.
