So talk to us. What's one piece of advice you can give us this Monday morning? Bob: As we discussed off the air, the one advice I can give you definitively is to join the American Dental Association. And that comes from someone who criticizes them on a regular basis. Michael: So let me ask you this why join them and what do you criticize them about Bob: first of all, let's talk about the positives I don't like to talk about Negativity first, everybody can improve things, right?so why should you join the American Dental Association because for a hundred and sixty three years dentists like myself have contributed to building this huge organizations. We are the fourth largest lobbying power in Washington, DC. Think about it. Hospitals, physicians, all these physical therapists, all these health care organizations.We are the fourth largest. Representatives and senators and politicians know our lobbyists by name. So it is important for us to have proper representation. The government will bring different, ideas to the table. There are a couple of them going on right now that we should be opposing. And we need to have a voice there.So if you do not join the American Dental Association, and if you do not contribute, do not just write a check and go to your offices and say they don't do anything. We need to get involved. We need to have a voice within organized dentistry. It's basically for advocacy. Michael: We're advocating for our patients first. and then the profession. Our oath is to the,patients and then the profession. So that's basically why we should, join. Why do I criticize them? I don't criticize them to weaken them. I criticize them to make them better.Bob: Every, time we, as an advocacy coalition, criticize them is an opportunity for them to correct things and be stronger and attract more people. For me, lack of communication between what the American Dental Association does and the members, that'sbeen an issue. Also lack of focus. To what is important, for the benefit of our patients and the benefit of the profession.I feel like we need to reiterate to them what is important so they can concentrate better. Michael: So you mentioned that we need to participate more and not just cut a check and then, you know what I mean? go back to practicing, but then you also mentioned there's a lack of communication. So do they even listen?Bob: If we participate.They are listening. I guarantee you that they are listening. I have this facebook group dentistry in general. They are part of it. I have heard that they are listening and they are making some changes. Is it fast enough for most people? I don't know. That's why I have created this advocacy coalition that includes 30 people past present American Dental Association leaders and well known people within the dental community to help guide them.We have a platform, a six pillar platform that we have presented to them, and we're going to be at the House of Delegates Trying to contribute positively. I want everybody to understand we're not here to weaken the American Dental Association. This is all for making them stronger. Michael: Gotcha. Okay. So it's more like we're just sometimes, downhearted because it's not moving at the speed of our expectations.Bob: there is a, myth that the ADA can do everything. This is a capitalistic system that we live in, right? Michael: There Bob: are laws and there are rules that when a dentist Criticizes them that oh, why can't we have the insurance companies pay more? That's not the job of the American Dental Association That's illegal.We cannot to have Another company pay us more. These are things that we cannot control. A lot of times dentists criticize the ADA for things that's out of their control. Obviously they're struggling and they see their patients being affected. So we naturally want these things to happen, but we need to be realistic.The way we can affect all the things they want is by making sure we know what legislation to support. what issues we can push through the politicians through legal ways to make a positive effect. Michael: Gotcha. So then what can they control in simple terms and what issues right now do you see most trying to push?Bob: Right now two of the most important things is the dental loss ratio, which is identical to the concept of medical loss ratio that the Affordable Care Act brought for medicine. We want the DLR for dentistry. Unfortunately, ADA, in my opinion, agreed to a plan that is not What we should have agreed to so we need to be outspoken and we need to be involved in creating the better than a loss ratio one that was passed in massachusetts Called question two a lot of this information is on my youtube channel on my facebook group So if people need to find out more and educate themselves contact me educate yourself These are the most important things You that are affecting our profession and our patients.That should be the number one thing. We advocate for patients first, not for ourselves. And the second thing is Medicaid laws are being ignored in every state and people will say, why should I even worry about Medicaid? I'm fee for service. I'm a PPO. I don't take Medicaid. Medicaid laws dictate.every insurance law that there is. As Medicaid goes, so does the private insurance. Michael: Gotcha. So we should be focused more, especially if we're heavily insurance, but like Medicaid. Bob: We shouldn't be focused on it, but we should look at states to enforce Medicaid laws that are there. I have a YouTube video on this subject that goes into details of what it means.All we want is to prove to politicians that this is good for the patients and let's enforce it. Michael: Okay. And so doing this. The first steps, if we're like, ah, should we join? Should we not join is joining, right? Continue to join, continue to be a part of it. And then now, if we wanted to start having our input into it, we do what?Bob: The alternative is what? Sitting on social media complaining about our problems, right? And what's going to happen? Nothing. Zero is accomplished by not joining, So joining and getting involved is important, and then support the coalition. Dentistry in General Advocacy Coalition will be out there presenting ideas.It's not a, organization that collects money. I'm covering all the costs because I don't want people to think that this is a profit generating organization. This is to help younger dentists. For years to come. We want to make sure the profession is protected and our patients are protected.Michael: Gotcha. So right now, Bob, me a percentage from one to a hundred? How much of a percentage do you feel protected and your patients protected? Bob: Listen, we are fighting different wars on different fronts. The corporate intrusion into healthcare is something that everybody knows about, We have insurance companies. That's another corporation intruding into our profession and into our patients well being. So for me, to put a percentage on it, I'm an optimist, right? I'm a, Glass half full. I say something that a lot of dentists rolled their eyes at me. We are a lot like. Hairstylist, Then doctors. Why? Because you will drive two hours to see your hairstylist The personal relationship dentists form with their patients is very important to them. So I still believe we have a chance. of keeping corporate intrusion at bay.We can't prevent it. It's there, However, if you create this professional and compassionate relationship with your patients, they will travel to come to you, but they also pay out of pocket to be out of network with a lot of these insurances. So it's important to understand that. So I would say We are still 80 percent strong, my friend.Some people may be less optimistic as I am, but we want to make sure we keep it at 80 percent and we're not going to go down the road medicine took. In 2010, he received the ADA's Golden Apple Award, the highest award bestowed by the Association, for outstanding leadership in mentoring. Dr. Kessler served as president of the Colorado Dental Association from 2014 to 2015 and was instrumental in the formulation and implementation of an adult Medicaid dental benefit in Colorado. Dr. Kessler speaks nationally on well-being and leadership topics to help dentists live their best lives. Dr. Kessler has received fellowships from the American College of Dentists, the International College of Dentists and the Pierre Fauchard Academy. He has testified before both the U.S. Congress and Colorado state legislature in an effort to help enact laws that serve the profession and the public. Dr. Brett Kessler, a dentist in Denver, Colo., is the new President-Elect of the American Dental Association (ADA). Dr. Kessler has been an active volunteer leader and member of the ADA for 26 years. Since 2019, Dr. Kessler has represented the 14th District on the ADA Board of Trustees, and he has chaired several ADA committees, including the Diversity and Inclusion Committee, the Audit Committee and the Science and Research Institute, on which he is currently serving his second term. Dr. Kessler's vast ADA leadership involvement also extends to the House of Delegates, the Dental Wellbeing Advisory Committee and the Council of Dental Benefit Programs, where he was Vice Chair. Additionally, he has served as a Board of Trustees Liaison to the Council on Advocacy for Access and Prevention, Council of Communications and Council on Dental Practice. He was also awarded the Golden Apple award for “Outstanding Leadership in Mentoring” in 2010, which is the highest award bestowed from the ADA.Dr. Kessler is also involved in numerous other organizations, including the Metropolitan Denver Dental Society, the Colorado Prosthodontic Society, the American College of Dentists, the International College of Dentists, the Pierre Fauchard Academy and the Colorado Dental Association Foundation, where he was a Founding Chair. He served as president of the Colorado Dental Association from 2014-2015, and prior to his term, he held a plethora of other leadership roles within the organization. Dr. Kessler graduated from the University of Illinois Chicago College of Dentistry and completed his General Practice Residency at Northwestern Memorial Hospital. He has a wide variety of practice modalities, including hospital setting, employee dentist, practice owner in a fee-for-service startup, chief dental officer at a Dental Support Organization and currently practices in a restorative Preferred Provider Organization.Dr. Kessler has been an educator at the University of Colorado School of Dental Medicine and University of Michigan. A national wellness and leadership speaker, he is also a five-time Ironman triathlete, ultra-marathoner and avid trail runner. In addition to his work as a clinician, he has been involved with volunteer organizations which promote substance abuse advocacy and wellbeing. It's a seismic shift that has the potential to fundamentally change the way we approach not only pediatric dental care, but total health for our children. Airway-centric dentistry focuses on the root causes and can address the underlying issues that lead to sleep apnea, snoring, sleep deprivation, and yes, cavities and tooth decay. Driven first by her intuition as a mother to children with chronic health issues, Dr. Lauren Ballinger, who is a board-certified pediatric dentist, was not satisfied by the traditional Western medicine options of addressing the symptoms. She knew there was a better way, and there was no going back. Dr. Lauren experienced what she describes as her "breakthrough," leaving her family's practice to take a great leap on a new venture: Good To Grow Airway Pediatric Dentistry. Quotes from Mercury-Free by Teresa Franklin PhD and James Hardy DMD (2021):What makes [mercury} amalgam scrap? If it is not in the tooth, it is called scrap. Environmental agencies must be contacted for proper disposal methods of that little piece of amalgam that didn't go in the tooth. If amalgam is unsafe to put in the trash can, how can it be safe in the mouths and bodies of an estimated 100 million Americans, which translates to 1/3 of the population? It is not safe and in fact, based on a conservative estimate in a population-based trial, over 67 million Americans exceed the safety exposure level set by the EPA.89The World Health Organization (WHO) studied mercury exposure to humans from air, water, food and amalgam fillings, and concluded that the largest source of exposure was, by far, dental amalgams. The WHO also concluded that there was no safe minimum dose of mercury. “Symptoms are known to occur, at least among some of the population, at every level of exposure 97,281.”In the U.S., mercury has been banned from interior and exterior latex paint. Mercury has also been banned in many pesticides. Mercury has been banned from batteries, smoke detectors, children's toys, eyedrops, from most vaccines, and its use in thermometers and thermostats is being drastically reduced and will soon be nonexistent. Other products that contained mercury such as mercurochrome and calomel, have been banned altogether. Mercury use has been banned for a reason - because it is highly toxic. But….explain to me why mercury has still not been banned in the most personal environment, the mouth. It makes one wonder who has the most powerful lobby in Washington D.C.: the paint industry, the pesticide industry or the ADA?Dr. Franklin was quite sick for a long time. She had all four health issues that people can get with dental appliances:metal allergiesmercury toxicitya battery effect between the metals implanted in the body that likely causes amalgams to release mercury more rapidlychronic infectionAs she gradually figured out what was happening, she started insisting on proper treatments despite denial and foot-dragging by her dentists and doctors. Orthopedists are paid poorly for screw removal, and dentists are brainwashed to ignore amalgams by the American Dental Association (ADA).As Dr. Teri had her dental work removed and replaced with modern “composite” materials over several years, her health progressively and dramatically improved. One of her experiences was the draining of a chronic, nearly asymptomatic abscess. It was invisible on a “cone beam” dental X-ray. This was shockingly stinky, and a globule of mercury was found inside. Another time, patch testing of her skin proved that she had certain allergies to her metal orthopedic implants. Her local orthopedists told her she was crazy, so she flew across the country to get her these taken out by Scott Schroeder, my last podcast guest. One of those instances is when his son, Simon, got bone infection. [3:33] The problem with taking too much antibiotics Antibiotics wipes out microbiomes which is 70-80% of your immune system and it creates immune problems later. After World War II, we all though antibiotic is our savior but we've learned a lot this past few decades. Antibiotics created more disease than the number of lives it saved. People get slapped with antibiotics when they have pneumonia or sinus infections. Antibiotics cannot fight viral infections. Because of this, a lot of superbugs and resistant bacteria that are now killing people. [5:21] Dr. Dan's journey to becoming a health practitioner Dr. Dan loves the book Doctor Dan, the Bandage Man Being a pre-dental college student and a wrestler: His interest was always health Cutting weight/calories didn't make sense to him Reading fitness magazines and muscle and fitness: How to keep your muscle and be as lean as possible Cutting calories don't work. You lose strength. In Ocean City Maryland, Dr. Pompa ran a marketing sales crew and they all ended up on the Pompa Diet [10:30] Life as Dr. Pompa knows it came into a screeching haul As a chiropractor and a cyclist, Dr. Pompa was in a great shape until he got sick. From fatigue and anxiety, he had insomnia, panic attacks and he got allergic to everything he was eating. He can't even watch a movie. He became irritable, angry, or simply someone he didn't know. In 2005, he started getting his life back. He and his wife grew their family. They adopted two kids who lost their parents tragically and they had another son. [14:42] Dr. Dan Pompa continues his personal story of mercury poisoning Searching the internet and reading literature, he came up with him having Mad Hatter Disease. However, blood test results came back negative. A very bright endocrinologist advised him to do another test, one that is based on tissues not blood, and found out he has mercury poisoning. Chronic mercury poisoning from Amalgam Fillings: from Pain to Purpose Silver fillings in amalgam fillings contain 50% mercury The golf crown creates an electrical current with the silver fillings and made the mercury flow out of the fillings Studies have shown that the number of silver fillings you have in your mouth is proportional to the number of mercury they find in your brain on autopsies. Mercury accumulates in the brain through time, particularly in hypothalamus pituitary which controls your thyroid and your adrenals. Dr. Pompa had to get the mercury out of his brain safely and correctly to ultimately get his life back [20:44] Detox downstream analogy The analogy: If you're downstream trying to fix the river and get fish to repopulate the stream of the river, while upstream, they're still dumping mercury or lead into the river, you're never ever going to get the fish to take population again. All of those detoxes are too far downstream to what the real problem is. Metals, mold and hidden infections are the top three big ones that people miss upstream. Detox at the cellular level is the ultimate solution. [23:56] Why are dentists still doing root canals and still put silver fillings in people's mouths? There was a documentary called Root Cause featuring opinions from doctors and dentists from around the world on the health effects of the root canal procedure. The American Dental Association (ADA) urged Netflix, Amazon and other companies to remove Root Cause, which they believed is a documentary spreading misinformation. A hundred percent (100%) of root canals is infected yet dentists are still doing them because it is the cash cow of dentistry. There's a growing group of dentists called Biological Dentists who are not doing root canals anymore. A good place to find a safe dentist is at IAOMT: The International Academy of Oral Medicine and Toxicology These dentists know how to take fillings out correctly. If you had them out, the mercury vapor turns into inorganic mercury in your brain where it is locked for life, unless you do the right detox. Fortune has done a great job reporting on this trend: I know how lobbyists make sure Americans don't get dental care–I was one of them Three powerful trade groups in Washington lobby against getting dental coverage for American dental patients –America's Health Insurance Plans(AHIP), the Better Medical Alliance, and the American Dental Association (ADA) kill bills to expand medical coverage/access to include dental care This is despite the fact that adding coverage for dental care to the new Build Back Better is the #1 request The primary reason Medicare hasn't covered dental care since its inception in 1965 (except when oral health problems become so severe they require hospitalization) is that organized dentistry staunchly opposed it. main concern–as it was 56 years ago–is that Medicare would not reimburse dentists to their satisfaction. The American Dental Association (ADA) - the largest association of dental professionals in the country - are fighting against the proposal to add dental benefits to Medicare. Last week we emailed our list of Medicare for All supporters, asking if any seniors with challenges getting the dental care they need would be willing to share their story with us. We weren't prepared for the flood of 150 stories that came in, or how heartbreaking they would be. Sandy in Turtle Creek, Pennsylvania: “I'm a 64yr old woman who [is] shut in my apartment. Instead of having friends, going places, doing things, I don't leave my home because I have no teeth. After a horrific divorce, I lost my teeth, and cannot afford dentures. It has caused a deep depression, and loneliness. I'm 64, which I don't feel like should be the end. I should be going to church, having friends, going places, but can't. Due to the embarrassment and shame of not having teeth. I'm never going to be able to afford nice dentures, so this has become my life.” [Impact on mental health, social life.]Mary in Devine, Texas: “My husband and I both have dental problems. I had my bottom teeth pulled out in hope of getting affordable dentures. I went to see about them and the price is always three to six thousand dollars. It is outrageous… I don't have insurance. My husband has Medicare but it doesn't cover dental... We both can't eat nuts, dried fruit or even salads. We would like to eat healthy. We are unable to have protein foods. I have [even] choked. I mostly eat very soft foods and make smoothies. It is quite painful. It saddens me because of [the] lack of concern and our quality of life is affected.” [Impact on broader health - ability to eat healthy.]Janus in Champaign, IL (edited down): “I am a 66-year-old woman who worked for the University of Illinois.. for 25 years. During that time, I had good dental coverage and saw the dentist every year to have my teeth cleaned. In 2000, I started my own business, and could only get healthcare coverage by joining the local farm bureau. That policy did not include dental care… [and] I had to go in debt just to keep insurance coverage until the Affordable Care Act was finally enacted... And then I turned 65. I was forced off of the ACA policy and into Medicare… I have not had a dental cleaning for 15 years and see no future in which I will be able to get that kind of care for myself… Now that I am a senior, I expect a safety net to catch me in times of, oh say a pandemic! My business is caring for clients' homes and pets when they travel. No one is traveling. I made $4,000 last year... It's not looking any better for 2021...” The Medicare for All movement has been fighting to expand Medicare, including finally adding comprehensive vision, hearing, and dental benefits. Because of our pressure, this demand has actually been taken up in Congress, despite the fact that President Biden's proposal for this bill didn't include any Medicare expansions. But as Medicare expansion works its way through the committee process, the proposed benefits are shockingly poor for dental coverage. Today, Medicare Part B doesn't cover any routine cleanings, regular care, teeth replacement, etc. Basically, no preventative care, only emergency care as it relates to other medical treatment (https://www.healthmarkets.com/content/medicare-part-b-dental-coverage) On the plus side, the House proposal would cover routine care,

Dr. Robert Stanley has been interested in dentistry since spending hours in his grandfather's dental office in Chicago as a child. Dr. Cynthia K. Brattesani joined the worms in podcast studio 1-A for the latest podcast. After meeting her and chatting with her via Skype, we can easily say Cynthia is our favorite Italian dentist. She is a very successful dentist in the Bay Area— and we can totally see why! Not only is she very knowledgeable, focused, driven— all of those make for a great dentist, of course. Listen NowThe Medicare program, now in its 51st year, still does not cover oral/dental health care such as exams, X-rays, cleanings, fillings, tooth extractions and dentures. (Medicare will cover an oral health procedure if it is incent to a serious accident or disease, for example, for surgery to treat fractures of the jaw or face or if you have oral cancer and need dental services necessary for radiation treatments.) This is unfortunate when you consider for example: poor oral/dental health worsens overall health; less than five percent of older Americans have dental insurance of any kind; one-third of adults over 65 have untreated dental caries and over 40 percent have periodontal disease; the Affordable Care Act did not name adult oral/dental benefits as an "essential health benefit"; an overwhelming majority of adults believe dental coverage should be part of overall health coverage; for all of CMS's "innovation" demonstrations (now numbering well over 50) there are none that address improving oral/dental health for seniors; and, oral/dental health disparities are, according to the CDC, "profound." During this 20-minute conversation Dr. Vujicic provides his assessment of the oral/dental health of American seniors, his understanding of why the Medicare program still does not cover routine oral health care and what can be done to improve access and (insurance) coverage of oral health for seniors or Medicare eligible individuals.Dr. Marko Vujicic is the Chief Economist and Vice President of the Health Policy Institute at the American Dental Association (ADA). Prior to joining the ADA Dr. Vujicic was a Senior Economist at The World Bank and also a Health Economist with the World Health Organization in Geneva, Switzerland. Dr. Vujicic is the lead author of the book, "Working in Health" and has authored additional essays and book chapters on various health policies. He is published in the New England Journal of Medicine, Health Services Research, Health Affairs and other policy and scholarly journals. Dr. Vujicic is also a visiting professor at Tufts University in Boston.
