This episode FINALLY exposes how stress is silently destroying your patients' mouths!
In this podcast, Dr. Glenn Clark examines the relationship between adolescent bruxism (teeth grinding) and TMJ disc displacement. He explains how increased deep sleep in adolescents leads to more micro-arousals and frequent bruxism, creating repetitive strain on the TMJ's lateral collateral ligament. Over time, this stress causes ligament deformation and laxity, compromising joint stability and resulting in disc displacement. Podcast created using Notebook LM.

What influences your decision when choosing ceramics? What are the main ceramics nowadays—and do porcelain-fused-to-metal still have a place in dentistry? Are the protocols different for various types of ceramics and crown materials? How important is rubber dam isolation, and is a split dam good enough? In this Back to Basics Protrusive episode, Jaz teams up again with Emma Hutchison, ‘the Protrusive Student', to break down these critical questions and simplify the world of ceramics. From decision-making frameworks to practical rubber dam tips, this episode is packed with insights to elevate your practice. Whether you're a student navigating the foundations or a seasoned clinician revisiting the essentials, this discussion offers a fresh, evidence-based perspective on mastering ceramics in dentistry. https://youtu.be/z4a8Hv6peVU Watch PS013 on Youtube Key Takeaways: Understanding the role of metal ceramic crowns is crucial in modern dentistry. Monolithic ceramics are preferred for posterior restorations due to their strength. Layered ceramics can enhance aesthetics but may compromise strength. Proper crown preparation is essential for successful restorations. Communication with lab technicians is vital for successful bonding. The choice of ceramic material largely depends on the amount of enamel available. Following manufacturer protocols is key to achieving optimal results. Bruxism patients require careful consideration in material selection. Rubber dam isolation is crucial for predictable bonding. Digital scanning requires more aggressive tissue management. Impressions are still valuable, but digital methods are advancing. Leah explains the difference between bruxism and TMJ, highlighting that while TMJ involves the jaw joint, bruxism is about teeth grinding, which can occur consciously or unconsciously, and may be linked to stress or previous oral trauma. She shares her personal experience with both conditions and discusses treatments, which can involve primary care doctors or dentists.

Today I'm going to share the best remedies to help stop teeth grinding at night. Grinding your teeth can lead to issues like: • Excessive wearing of the teeth • Teeth pain • Headaches • Jaw spasms People who smoke, drink alcohol, or take SSRIs have a higher risk of teeth grinding (bruxism). There are a few things involved in teeth grinding at night, but it seems to be largely related to the autonomic nervous system. Bruxism is also associated with a certain variation in genetics. The gene involved is the receptor gene for serotonin. If you have this variation, you'll have to work harder to increase the precursors of serotonin to help build it up. Bruxism can also be triggered by various things, including: • Stress events • Trauma • Excess caffeine consumption The best natural remedies for teeth grinding: 1. Vitamin B1 (nutritional yeast) 2. Vitamin D3 DATA: https://pubmed.ncbi.nlm.nih.gov/33413... https://pubmed.ncbi.nlm.nih.gov/22953... https://www.nature.com/articles/s4159... https://www.nature.com/articles/s4159... https://www.sciencedirect.com/science... https://www.ncbi.nlm.nih.gov/pmc/arti... https://www.ncbi.nlm.nih.gov/pmc/arti... https://pubmed.ncbi.nlm.nih.gov/8990057/ https://www.mdpi.com/2077-0383/10/11/... https://www.ncbi.nlm.nih.gov/pmc/arti...
Many of us hold our tongues or clench our teeth to protect ourselves from saying what you really want to say. Or feel. And for most of us, that works and we go about our day. But if you are clenching and grinding your teeth on a daily basis there is going to be a problem. Sometimes with your jaw. Or wearing away your teeth. Or having headaches or something worse. In this episode, a look at what jaw clenching and teeth grinding and some of the things you can do to reduce the symptoms. The medical term is called bruxism. I do want to mention that I did not make any references to medication. That is because there can be non-anxiety related forms of bruxism. There can be structural problems with the jaw or mouth. If that is the source of the problem, then that is more of a dental treatment issue. I'm looking at jaw clenching and teeth grinding from a stress and anxiety point of view. Join me for a summary looking at the use of botox for deep bite management and bruxism. This was an interesting lecture by Dr Nan Hatch from Seattle, which was presented at this year's AAO 2023. She explored the use of Botox for masseter hypertrophy and the evidence around it. Masseter hypertrophy when combined with bruxism can result in: o Long term changes in function o Fixation breakages o Orofacial pain, tmd, mobility How does it work? · Injection of neurotoxin into muscle · Temporary partial paralysis through chemical denervation · Most common Botulinum toxin or Botox · Mechanism of action o Block acetylcholine (neuromuscular transmission) release. o Also inhibit pain sensory neuron How to inject · Extra oral – use of facial landmarks · Intra-oral Use MRI / EMG guided injection What are the effects · Anticipate change facial aesthetics · Greater facial contouring achieved with higher dosage Review Wu 2023 · Last up to 180 days · Greater complications with greater dosage Anticipated change from botox · Treatment for masseteric hypertrophy o 35 units to masseter , two injections o 91% reported improvement headaches o Duration 25 months Decrease bite force Ahn 2007 · 25 units to each massenteric muscle · Mean bite force 51kg /cm using bite block attached to a transducer o Reduced to 30-36kg/cm for 8 weeks (29%-41% reduction) o After 41kg after 12 weeks , no longer statistically significant · Significantly reduced bite force up to 8 weeks Potential adverse effects · Chewing weakness · Sunken cheeks – high volume · Headaches · Sagging skin · Asymmetry · Paradoxical Bulging – miss masseter and affect other muscles · Distant spread of toxin · Speech disturbance · Muscle fiber atrophy 4-6 weeks, remover 4-6 weeks Chemical denervation protocol · 25 units per muscle, both masetter and temporalis · Interval 4-5 months and patient response · 3-5 serial injections o Some patients changes can be permanent

The coffee appeared to have been resealed–which I almost hoped meant it contained some some sort of magical additive that would allow me to overcome the sort of rut I had been so unsettled in; it really had seemed the last roommate had done somewhat of a number on me–the bite marks on my left hand now turned to a perfect crescent-shaped scar, forming a remarkable C-shaped half circle, and I knew now that no one could be trusted, as she truly had been possessed by The Devil, or some sort of Demon, or she had been paid or hired by some agency to attack me, as the way she had lowered the room temperature to freezing so that I had been sitting in the cold hours before our fight–and of course, had chosen such a night which I had intended a well-deserved feast, after weeks of malnourishment, unintentional fasting, and still with all of my might trying to the best of my abilities to train for at least an hour per day with overall success in the endeavor–it seemed something eerily odd about the encounter, and though I considere myself a paranoid conspiracy theorist at best, there was still something shattering about the experience, so much so that I felt I had been at once dragged into Hell, and also somehow elevated–having captured the experience inadvertently all on tape, having forgotten that I had even been recording for the first place, still collecting bits and pieces for the album I was trying to make, but taking my time on–it had to be worth all of the work I had so far put into it–which was a lot, and it still seemed that something was trailing behind me–I remained neutral entirely to the matter, not to deconstruct such bizarre circumstances as one thing or another, either good or bad–but I had become irritated by now at the very least, as my new roommate, though sweet, was too sweet, and I found it too good to be true that she appeared to be so clean, neat, and tidy that she wasn't as well up to something, or also hired by someone–seemingly to collect information, or to study me . “Not too much…” I thought to myself, filling the filter only so slightly as to test its contents, and careful not to upset my nervous system; I was, actually still on edge, but today in particular; it seemed everyone that I collided with wanted to know something strange, as if if they already knew who I was–and still in the haze of all that had happened, i let go to expectation, embracing myself as I knew only I could, and finally allowed myself the understanding that I was, actually, entirely alone–with no friends, no family, and no allies–but certainly no trust to be had. “What do you know about Shamans?” My new roommate asked; This was entirely the wrong way to go about opening up a polite conversation with me, as I had had discovered that along with my estranged and abusive ex-husband, and also my former roommate, had excruciatingly noisy Bruxism–and though I had been working closely with the spirits to undo anything which might have been a curse, demonic spell, or hex–I had never not believed in coincidences at any other time in my life: I thought “She must be some kind of spy.” Perhaps a psychologist intended to collect information after the ridiculous amount of synchronicities and circumstances which had become otherwise outwardly apparent to the exterior world in whatever realm I happened to have found myself in– not that it mattered, as I considered all to be done to me was to eventually to be returned to sender, and the extreme amounts of poverty, homelessness abandonment, lovelessness, hunger, and overall dehumanizing Hell that I had otherwise been put through to simply be a test of wills, a sign of better days, or, merely, a subhuman existence in which my place in the universe had been deemed somewhat ‘useless'. In any event, I still hadn't wished for any of it to be cast upon even my worst enemies, if they existed–and because I didn't believe them to exist at all in any form besides my darkest and most unhinged, ludacris self–unraveling at the core and grasping at the throes of suicide simply for the supposed peace and rest of ‘non-existence'. My new roomate often and out of nowhere brought up things that I had been psychologically tortured with–and at the very least I knew of course that I was indeed being watched and perhaps even studied–and I knew undoubtedly that whatever information I gave her would without any doubt be used against me–and so, once more, I toiled away at The Festival Project, the mockery of my own judgment a stone cast upon the glass house in which I lived, at least in my mind–not shielded from but subject to whatever cruelty my very own mind and consciousness saw fit to suffer through in this lifetime, without promise or evidence of relief in the next– it was, afterall–an infinite eternity; Loveless, penniless, and without a soul or song in the world to love again. Occlusal appliances are commonly prescribed for the management of Bruxism - but they might be doing more harm than good if you have not screened for an airway issue. Dr. Aditi Desai discussed the link between airway and bruxism- could an airway problem cause bruxism? Why is it advised NOT to have a standard occlusal appliance if there is an airway issue? Did you know there are three levels of diagnosis for Sleep Bruxism? The 'Possible' Bruxist, the 'Probable' Bruxist and the 'Definite' Bruxist - in this episode Dr. Aditi Desai who also featured in PDP 139 on sleep disordered breathing and sleep apnea, will explain this and when it may be relevant to sleep apnoea. https://youtu.be/DozqYGEPNxY Watch PDP149 on Youtube The Protrusive Dental Pearl: Parafunctional Screening Sheet - A simple PDF that you can look for in terms of your extra-oral examination, intra-oral examination and the history - to give a clue that a patient might be a bruxist in just 2 minutes - download below: Parafunctional-ScreenDownload Download and Sign in to the Protrusive App on iOS and Android and head over to the freemium version of this episode and on The Protrusive Vault for those Protrusive Premium members (where you can get full CE or CPD Certificate by answering a few questions) Highlights of this episode 5:21 The Protrusive Dental Pearl6:28 Dr. Aditi Desai's Introduction9:17 Airway in Dentistry12:49 Lack of studies with regards to diagnosing sleep bruxism16:32 Signs to look for to a possible bruxist and how to communicate with them27:01 Nomenclature of sleep bruxism30:51 Learning points to assess airway35:43 Cases that caused the patient to become apnoeic Get in touch for different Board of Sleep Medicine: UK: British Academy of Dental Sleep Medicine (BADSM)USA: American Academy of Dental Sleep Medicine (AADSM)Australia: Australian Academy of Dental Sleep Medicine If you enjoyed this episode, you may also like Airway – Dentistry's Elephant in the Room with Prof Ama Johal

I'm Jaz Gulati and thanks so much for coming onto the app for part two or what was quite a brilliant part one with Dr. Sandra Hulac. Her energy is just absolutely amazing. Jaz's Introduction:Now, just a recap, we left you on a bit of a cliffhanger last time. We talked about, I asked Sandra about orthodontics. You know, how often does orthodontics come into it? Is it every single time? And the answer she gives is really pragmatic. So you'll hear that or see that in just a few minutes. But I just want to do a bit of a recap, what we talked about last time, right? So, a constricted chewing pattern is someone who really wants to bite here, but the teeth meet together here, so the condyle gets sort of pushed back, if you like. Right. So for this patient, the centric relation is actually further forward. So who's at risk? People who've got upright or retroclined upper incisors, right? And their jaw wants to be further forward, right? Maybe cuz of growth, the jaw wants to be further forward, which is why Sandra said that people who've had orthodontics and upper pre-molars extracted. The maxilla gets smaller if you like, and mandible start continues to grow and those patients may be at risk. So if you see those kind of traits, then maybe we shouldn't be using like a leaf gauge for centric relation on those patients. We should be using like a deprogramming appliance Lucia Jig or a Kois deprogrammer. If you fancy that one because that's what Dr. Sandra does. And what you find with these patients with a CCP is yes, their jaw gets shifted back, but you might get more wear on their edges. And that's not from bruxism. That's because the outside in movement. Right? Their jaw wants to be further forward. Now, this is similar. The cousin of this is the frictional chewing pattern who their condyle isn't getting forced back, right? Their MIP is okay. It's just that as they're chewing the outside in is causing this. So, let's say that's normal, the incisal edges are not worn here. Can you see? They're a bit jaggedy over here, right? It's just, to over here and over here. We see this all the time and let me show you some photos actually on th...
Do you blame bruxism for every time you observe attrition? As you know, I'm no stranger to occlusal appliances, but often they may be inappropriate for the patient who is causing their wear during FUNCTION and not so much during parafunction. Maybe it's time for us to start looking at different aetiologies of attrition and this is what the wonderful Dr. Sandra Hulac breaks down for us in this banger of an episode. We also discussed the differences between frictional and constricted chewing patterns, which are often confused with each other. We share some case examples and discuss how to correctly diagnose these types of chewing patterns. https://youtu.be/ao0sqY6lXZ4 Check out this full episode on YouTube Download Protrusive App on iOS and Android and Claim your Verifiable CPD/CE by answering a few questions + You can get EARLY ACCESS to the episode + EXCLUSIVE content The Protrusive Dental Pearl: Acknowledge, understand, and believe the fact that often our patient's centric relation (CR) is NOT more distal/posterior to their maximum intercuspation (MIP) - it can actually be anterior to their MIP! Now we know that sounds like a big task and a huge ask, but we did it. I think we did it. OBAB:We did it. We finally made OBAB, Occlusion Basics and Beyond. And we've really, really worked our butts off to give you an occlusion course that is gonna be applicable to real world dentistry. So, what's included in this pre-launch deal? We've got five different things for you. First of all is the OBAB starter kit. We're gonna send you a starter kit so you can start implementing the concepts we're gonna teach you straight away on Monday. It's got a Huffman leaf gauge we imported from the US and this is our favorite leaf gauge. It's also got a pack of shim stock in it, so you don't have to use your fat fingers every time. We're gonna send you a pair of Miller forceps as well. The start kit is worth a hundred pounds and we'll start shipping it once the course access begins on 7th of April. I think really anyone interested in occlusion, whether you are at the beginning of your career in the middle, or even getting towards the end would learn a huge amount from this particular program. The second benefit of this pre-launch deal is we're gonna give you 500 pounds off of the cost of the course, and you can take our word for it, that we are never gonna price it this low ever again. And this course truly has an unbelievable return on investment. The third benefit of the pre-launch deal is that instead of getting 12 months of access, we're gonna extend that. So you get two whole years of oab and that's a no extra charge, and we're gonna be adding lots of new cases and content as we go. I felt like I finally understood topics that I just struggled to wrap my head around for years. And that's purely down to the way in which the content's delivered. The fourth benefit of this pre-launch deal is you'll get one fully mentored case with us incl...

Probably about 90% of the patients I see down at that muscular end as you say. Jaz's Introduction:What happens to our TMD patients when conservative care fails? Like you've done your patient education, you've given him the best occlusal appliance, you've worked alongside your TMJ physiotherapist, you've been through exercises, and you've even counseled them about the importance of recognizing awake bruxism, a huge player, and all this stuff isn't working. What happens next? Well, depending on your diagnosis, the next step for some patients will be see a maxillofacial surgeon, but not any old maxillofacial surgeon, you ideally want to send someone who's got an interest in TMJ and TMD. So I've got today a private physician, private maxillofacial surgeon in the UK, who exclusively treats TMD. So what this guy doesn't know about surgery and TMJ. And what happens in the latter parts once conservative care fails, how the referrals manage, when should we refer these patients, which patients are suitable for referral to Maxfax, once conservative care fails. Let me give you a clue, if your primary diagnosis is muscular, then really, you know, really need to go and exhaust conservative care and the physio and by the way, most TMDS are of a muscular nature that myalgia and myofascial pain and there's no real scope for surgery when it comes to muscles that are upset. That's when we really to give the best conservative care we can and involve a pain specialist sometimes potentially Botox and lots more which we will discuss. Now if you want to learn more about occlusal appliances, bruxism as a GDP as a restorative dentists who wants to just not be afraid of doing a TMJ exam when the patient comes into an emergency slot and they're complaining of pain from their jaw,

In this episode Dr. Karen Raphael shares her research on the areas of psychological comorbidities in chronic pain, the relationship of sleep bruxism and temporomandibular disorders as well as the use of botulinum toxin in the management of orofacial pain conditions. Dr. Karen Raphael is a PhD-level psychologist and epidemiologist whose work over the last 35 years has focused on the role of psychosocial and stress-related factors in medically unexplained pain. As she was completing postdoctoral training in psychiatric epidemiology at Columbia University, she met Dr. Joseph Marbach, an orofacial pain specialist and renowned clinical researcher. Most of their close collaboration for a dozen years until his passing in 2001 involved studying processes underlying chronic temporomandibular disorders. After serving on faculty at Columbia University (in Public Health, Psychiatry and the Dental School), she continued research on orofacial pain and other forms of medically unexplained pain at Rutgers/Newark Medical and Dental Schools, where she was promoted to Professor. Her work broadened to encompass areas ranging from translational pain psychophysics to pain epidemiology. She continued methodologically-driven work on biases in both patient reporting and clinical observation. Since 2009, Dr. Raphael has held a primary appointment at New York University College of Dentistry, serving as Professor in the Department of Oral & Maxillofacial Pathology, Radiology and Medicine. Her research on stress and muscle activity expanded to include use of polysomnographic studies and awake stress-reactivity studies to examine the role of bruxism in temporomandibular disorders. Dr. Raphael's writings inspired an international consensus committee of orofacial pain experts to reconsider whether bruxism is a disorder requiring treatment. In addition to serving as Principal Investigator on a dozen grants funded primarily by the NIH, Dr. Raphael has published approximately 100 peer-reviewed manuscripts and has served on numerous editorial boards and grant review committees. Her most recent funded research has examined potential bone-related risk from use of Botulinum Toxin to treat temporomandibular disorders.
