Terence Bendixson was the Guardian's planning correspondent in the 1960s when he wrote a piece that propelled him into walking campaigning. Alexander Repenning created AgentSheets, an environment to help kids develop computational thinking skills. It wrapped an unusual computational model with an even more unusual user interface. Adapting to life with a disability is a challenge many of us won't face. This week our Host and CEO, Stuart Elliott had the pleasure of speaking to Dr Amit Patel, talking about his journey with an unexpected disability; we hope to enlighten our listeners with his incredible story and how he learned to navigate the world in a completely new way. Dr Amit is an Accessibility Consultant, Motivational Speaker, Writer, and Disability Activist. Amit lost his vision overnight in November 2013, amid the prime of his career as a Trauma Doctor. Since accepting his sight loss, he has been a powerful advocate for disability rights. In 2020 he wrote his first book Kika & Me, which explores his experiences of being visually impaired and how much having a guide dog changed his perception. The Office for National Statistics have released some disability stats that some are finding a little surprising. The number of people who declared themselves disabled in the 2021 Census didn't go up in the way people expected, while the number of unpaid carers, unexpectedly, also went down. Carer and founder of the We Care Campaign, Katy Styles, and academic Miro Griffiths, from the famous Centre for Disability Studies at Leeds University, try to work out why. Dr. Amit Patel's recent autobiography revealed how he scoured the world for a solution to his pending sight loss but eventually failed. This week Anna welcomes back Dr Amit Patel and his famous Guide Dog, Kika. We chat about a new programme on Cbeebies called Dog Squad which is all about teaching children just how amazing dogs are in helping their owners. For Dr Amit, Kika is his eyes. There are four other superhero dogs featured: Tinks, Sylvie, Meagaidh and Diesel. They all have amazing jobs! We chat about how the TV show also aims to break down preconceptions on disabilities, and explain why dogs can help people live independent lives. Giving an educational view on dogs and how they help people is going to inspire the next generation to understand dogs, and look at disability in a new light. Dr Amit talks about how he is an Ambassador for Guide Dogs having had his life transformed by Kika, and he speaks out as to why Assistance Dogs should never be refused access; how some venues, taxi firms and more refuse Kika entrance which is against the law. He graduated from the University of Liverpool in 1997 and gained membership of the Royal Faculty of Dental Surgeons in 2000. He went on gain a master's in periodontology and clinical dentistry at Guy's and St Thomas' Dental Institute. He is a specialist in periodontics and an honorary clinical lecturer at the University of Birmingham Dental School. Amit is an ambassador for the Clean Implant Foundation, which aims to establish a science-based quality benchmark for implants. In 2018, he was voted one of dentistry's 50 most influential figures.

He invites Dr. Amit Patel and Dr. Ranko Miocinovic to share their experiences with focal ablation using the Nanoknife System from AngioDynamics. If prostate cancer is suspected due to an elevated PSA and a suspicious MRI scan, a prostate biopsy is necessary to confirm the diagnosis. Both doctors prefer to perform their biopsies through a transperitoneal approach in an office setting with local anesthesia or ProNox. If a positive biopsy for prostate cancer is obtained, additional imaging to stage the lesion and radiation oncology is helpful in prognosing and treating the cancer, respectively. Focal ablation is a new minimally invasive surgical technique that allows destruction of the cancerous part of the prostate gland without having to destroy or remove the entire gland. Intermediate risk prostate tumors yield the best results with focal ablation, specifically in the context of preventing post-operative sexual and voiding dysfunction. Once the focal ablation sensors localize the tumor in the gland, a variety of ablation techniques can be utilized. Both Dr. Patel and Dr. Miocinovic use the NanoKnife System, an irreversible electroporation system that uses an electric current to break up cell membranes. Proponents of the NanoKnife System believe that it causes less peripheral destruction because it preserves connective tissue and minimizes destruction of nerves. Evidence also suggests that focal ablation using the NanoKnife system lowers the rate of scar tissue formation thereby lowering the rate of erectile dysfunction, improves protection of the urethra, causes less swelling of the prostate, lowers the risk of post-operative retention, and requires shorter operating time. Finally, this method of ablation allows for consequent follow up surgeries if necessary. Next, the doctors discuss NanoKnife procedural techniques, such as surgical approaches, and using ultrasound-guided probe placement. When using this type of ablation, it is important to monitor the wattage of the NanoKnife carefully in order to prevent the ablation from causing thermal destruction. Finally, both doctors discuss the future possibility of adding focal ablation procedures as a first line therapy to the AUA guidelines on treating prostate cancer.
