Après l'Épinoche et l'Épinochette, place à un autre minuscule poisson de rivière, tout aussi scintillant que les Épinoche(tte)s sont piquantes : l'Ablette. Parée d'une exquise robe argentée, la brillance claire de cette merveilleuse petite créature d'une quinzaine de centimètres se reflète dans son nom : "ablette" vient de "albulus" signifiant "blanchâtre" (bon en vrai c'est pas très flatteur, mais on a un peu l'idée). Eh oui, l'Ablette est comme le Béluga, ce cétacé cousin du Narval couleur neige, une "blanchette" !Si l'Ablette n'est pas réputée pour sa chair, elle est en revanche beaucoup utilisée en tant qu'appât pour de plus gros poissons, et surtout, ses écailles ont été très prisées... du monde du spectacle. En effet, ces petites paillettes nacrées ont pendant très longtemps servi... de fausses perles !_______

Sur le podcast cette semaine, je reçois la physiothérapeute Mary-Lee Blanchette pour parler du traitement aux aiguilles sèches chez les coureurs. Elle prend le temps de nous expliquer les bienfaits de cette technique, les différences avec les techniques de thérapie habituelles en physio et dans quel contexte il peut être intéressant d’avoir ce genre de traitement pour les coureurs. ** Je suis désolée pour la qualité du son de cet épisode. Voici un message que Mary-Lee souhaitait que je vous partage : Bonjour, Je reviens de ma formation d’aiguilles avancée et puis je veux faire une mise à jour concernant les clientèles particulières avec lesquelles nous sommes maintenant autorisée à pratiquer la PPAS tout en prenant des précautions au lieu que ce soit contre-indiqué, voici pourquoi. À ce jour, aucune littérature scientifique peut démontrer que les punctures en physiothérapie sont contre-indiquées pour la femme enceinte et l’enfant, par contre, les précautions suivantes sont à considérer et comme professionnel, nous ne pouvons pas plus avancé qu’il n’existe aucun risque, étant donné la nature invasive de la PPAS (puncture physiothérapique à l’aiguille sèche). Le jugement clinique et la sécurité sont primordiales pour toute intervention. La femme enceinte (ou potentiellement enceinte): Un consentement libre et éclairé de la patiente et l’explication des risques et bénéfices est à faire comme tout autre patient. Au 2e et 3e trimestre c’est possible d’utiliser la PPAS pour une grossesse sans complication ou risque. Il faut vérifier l’état de santé globale au préalable. Éviter le 1er trimestre en raison du risque de fausse couche qui pourrait être faussement attribuée de façon erronée au traitement à la PPAS. En fait, les risques de fausses sont déjà assez augmentés au 1er trimestre avec plusieurs causes confondues et parfois inexplicables, nous voulons donc nous abstenir que la patiente puisse penser que c’est lié à l’utilisation de la PPAS. Au 3e trimestre, les risques immunitaires sont augmentés nous allons alors utiliser plus de mesures prophylactiques lors de l’asepsie. Nous allons utiliser une dose sensorielle faible, une intensité des manipulations faible ainsi que limité le nombre de ponctuer et leur profondeur. Il faut également éviter les points d’Acupuncture à forte réactivités systémiques, et qui, dans l’historique d’acupuncture, peuvent nuèrent à la grossesse. Nous allons également éviter les points à la région sacrée, lombaire et abdominale. Maintenant pour les enfants âgés de moins de 14 ans : Éduquer et valider la compréhension totale de l’enfant dans le choix d’utiliser la PPAS. Obtenir le consentement libre et éclairé du parent ainsi que de l’enfant. L’immaturité et le développement motiva-affectif de l’enfant peut altérer sa perception de la PPAS par rapport à la population adulte, nous n’avons pas le même référentiel et expérience en termes de douleurs vécues. Communication et expression de l’enfant peuvent être limitées selon son âge, sa compétence ou son vocabulaire à pouvoir décrire ses sensations. Nous devons également adapter chaque protocole au gabarit (aux proportions) de l’enfant dans le choix de nos longueurs d’aiguilles et la profondeur des punctures. Les mesures et les repères anatomiques dans le cours ne s’appliquent pas à l’enfant notamment au niveau du rachis ou il y a une plus grande variabilité anatomique par rapport à celui de l’adulte. Il faut également comme physiothérapeute, adapter l’intensité ainsi que la dose de la manipulation. Bref on veut contribuer aux expériences positives de l’enfant vécu par rapport aux aiguilles. Dans notre culture, ce n’est pas quelque chose dont nous sommes fervents généralement comparativement aux populations orientales. Two months into U.S. President Donald Trump's second term, the U.S.-Chinese relationship—the most consequential one in the world by a long stretch—faces new uncertainty. Trump has threatened larger tariffs as China has continued its military buildup and activities in the Taiwan Strait and South China Sea. But Trump has also focused his ire on allied capitals, rather than on Beijing, and talked about making a deal with his “very good friend” Xi Jinping. In a recent essay for Foreign Affairs, Jude Blanchette and Ryan Hass stressed the importance, and highlighted the challenge, of understanding the balance of power with America's top rival. The biggest risk, they argue, is not that Washington will underestimate China's strength, but that it will neglect the sources of its own. Blanchette runs the China Research Center at the RAND Corporation; Hass, a fellow at the Brookings Institution, long worked on China policy at the National Security Council and State Department. They joined editor Dan Kurtz-Phelan to discuss Beijing's assessment of American power, the prospects for a “grand bargain” between Trump and Xi, and whether fears of American decline risk becoming a self-fulfilling prophecy. About 40 years ago a mother brought her 6-month-old child into the hospital. He was covered in bruises. Dr. Victor Blanchette, a pediatric hematologist at the Hospital for Sick Children in Toronto, Canada, met the patient that day and, following a severe hemophilia diagnosis, has treated the patient ever since. Dr. Blanchette recounts how during the patient's childhood in the 80s, the approach to treating hemophilia was reactive, not proactive. This meant that normal childhood activities could lead to devastating bleeds. During this episode, we walk through the history of hemophilia and how its evolution toward preventive care has had an incredible impact on this patient and so many others.
