Nutrition has vast effects on individual and population health—impacting obesity rates, chronic conditions, and quality of life. Yet despite its proven, wide-reaching effects, nutrition remains largely overlooked as a social determinant of health. Dr. Livia Santiago-Rosado, MD, Commissioner of the Dutchess County (NY) Department of Behavioral and Community Health, joins Dr. Enrique Enguidanos to discuss this important topic and how communities can work together to make small tweaks and big impacts. Bio:Dr. Santiago-Rosado has been Commissioner of the Dutchess County Department of Behavioral Health since her appointment in December 2021. Following completion of a BA in Biological Anthropology summa cum laude from Harvard University, she attended medical school at Columbia University. Having completed residency training at Mount Sinai and Elmhurst Hospitals in New York City, has practiced Emergency Medicine in New York for over two decades, working in emergency departments (EDs) in diverse settings including urban, suburban, rural, public and private. She holds an academic appointment as Distinguished Visiting Professor at New York Medical College and Adjunct Associate Professor at the Touro School of Osteopathic Medicine.Dr. Santiago has dedicated her career to working with and enhancing the care of underserved populations. Prior to moving to Dutchess County, she spent 15 years as a clinical leader in New York City's public hospital system, Health + Hospitals (H+H), where she helped found a novel ED Care Management program for highly vulnerable patients and the first ED-based observation unit in H+H. Prior to her current appointment, Dr. Santiago was the Medical Director and inaugural Chair of Emergency Medicine at Vassar Brothers Medical Center in Poughkeepsie, NY. Dr. Santiago is an active member of the New York State Association of County Health Officials (NYSACHO) and sits on its Emerging Issues committee. She also currently serves as a Councilor representing New York at the American College of Emergency Physicians (ACEP) and has served two terms on the Board of Directors for the New York Chapter of ACEP. She also serves on ACEP practice committees at both the State and national levels. Throughout her career, she has led and participated in policy development, education, informatics and data analytics, quality improvement, change management, and research in Emergency Medicine, social determinants, and population health at the local, state, and national levels. Lastly, since local health commissioners in New York are required to have a master's degree in public health (or related field) in addition to a medical doctorate, she is currently enrolled in the MPH program at the University at Albany.

Kanamalar travmalarla ilişkili ölümlerin önde gelen nedenidir ve bu hastaların yarısından fazlası hastane öncesi dönemde ölmektedir. Bu nedenle üç dernek (American College of Surgeons Committee on Trauma, the American College of Emergency Physicians [ACEP], and the National Association of EMS Physicians [NAEMSP]) bir araya gelerek, hastane öncesinde kanama kontrolü teknikleri ve hemostatik resüsitasyon konusunda ortak rehberlik sunmak ve aynı zamanda ilk yardımcılara da yol gösterici olmak için bir konsensüs bildirgesi yayınladılar. BİLDİRGE ORTAK KARAR NOKTALARI Elbette kanamalı hastaların kanama kontrolü için halen ilk seçenek olarak doğrudan baskı uygulanması öneriliyor. Direkt basının hastaların çoğunda kanamayı etkili olarak kontrol altına aldığı vurgulanmış. Kanama kontrolü için öncelikle kanamanın kaynağının ve hayati tehlike oluşturup oluşturmadığının belirlenmesi önerilmiş. Diğer öneriler; Doğrudan bası ile kanama kontrol altına alınamıyor veya alınması mümkün değilse, yarayı sarmak için gazlı bez ve/veya hemostatik emdirilmiş pansumanların kullanılması ve sıkıştırılabilir arteriyel kanamayı kontrol altına alabilmek için ekstremitelerde turnike kullanılması önerilir. Gövdeye birleşim bölgelerinden (boyun, omuz/koltuk altı ve kasık) kaynaklanan kanamalar doğrudan bası ve yara içi tampon ve/veya bir birleşim bölgesi -KAVŞAK turnikesi- (yalnızca koltuk altı veya kasık için) kullanılarak kontrol edilebilir. Yara içi tampon, yaranın içindeki damarlara doğrudan basıncı artırır. Bir travmatik yarayı doldurmak için temiz bir bez, gazlı bez veya hemostatik içeren pansuman derinlemesine ve sıkı bir şekilde yaraya bastırılır. Yara tamamen dolana kadar tampon uygulanırken doğrudan bası yapılmaya devam edilir. Tampon sonrasında yara bir pansumanla kapatılmalı ve her iki elle ciddi baskı uygulanarak başlangıçta hemostaz sağlanana kadar baskı sürdürülmelidir  Saçlı deri yaralanmaları ciddi hayati tehlikeli kanamalara neden olabilir ve doğrudan baskı uygulanarak veya hızla dikiş atılarak kontrol edilebilir. Hayati tehlike oluşturan dış kanamalar için kanama kontrol algoritması aşağıdaki şekilde gösterilmiştir. NOT: Bu noktada taktik tıbbında geçmişte de sık kullanılan 'yara içi tampon' önerisine dikkat çekmek istiyorum. Kanama doğrudan bası ile kontrol alınamadığında ikinci basamak müdahale olarak ekstremitelerde turnike uygulaması, bunun mümkün olmadığı vücut birleşim yerlerinde ise tampon ya da kavşak turnikesi kullanılması önerilmiş. 3. Lokal hemostatik içeren örtüler: Lokal hemostatik içeren örtüler faktör konsantratörleri, mukoadezifler ve prokoagülanlar içerebilir. ÖNERİ: Hemostatik emdirilmiş örtülerin en az 3 dakika doğrudan baskı uygulandıktan sonra uygulanması önerilmiş. 4. Turnike için Öneriler Hayati tehlike oluşturan kanamaları olan hastaların ekstremite travmalarında, vaktinde uygulanan turnike sağkalım şansını artırır. Bu nedenle ticari turnikelerin hızlı erişilebilecek yerde bulundurulması önerilmiş. Bu rehberde el yapımı turnikeler yerine ticari turnikelerin tercih edilmesinin üzerinde durulmuş. Dr. Schmitz highlights the implementation challenges, including that many insurance companies are not reimbursing physicians fairly or even showing up to arbitration, as the law requires. Christina Miller, MD, FACEP, is double board-certified in emergency and integrative medicine. After spending ten years in the emergency department and serving as president of Colorado ACEP (American College of Emergency Physicians), she radically changed her practice to health promotion, due to her own health concerns and being overwhelmed by the amount of chronic disease and illness seen in the ED. She studied nutritional science and plant-based nutrition, completed a fellowship in integrative medicine, and now focuses exclusively on nutrition and lifestyle changes to get to the root cause of disease and illness, while promoting wellness and longevity in each individual. Instead of treating emergencies in the ED, she is now helping to prevent and reverse chronic illnesses and empower people to live with vitality. She enjoys teaching plant-based, nutrient-dense cooking classes to both groups and individuals, and helped establish a free clinic outside of Aspen, Colorado for the underserved. She lectures locally, has spoken around the country with Younger Next Year best-selling author Chris Crowley, as well as in corporate wellness programs promoting good nutrition and healthy lifestyle. She has contributed significantly to the nutrition chapter for the American College of Emergency Physicians (ACEP) handbook and wrote wellness-promoting tips for the emergency physician. She was also selected to be a member of the Pitkin County Board of Health, where she helped with policy and public health concerns. She is now proud to be part of the start-up company, Plantbasedtelehealth.com, offering plant-based telemedicine services to patients across the country, reaching people who otherwise wouldn't have access. Her goal is to continue to share the information and the resources necessary to help empower people to take back their health, prevent and even reverse chronic illness, and create long-lasting true health and vitality. One of her favorite mantras is, “Together we are healthier”. On a personal note, I also have my own story, as I was diagnosed with lupus in 2009,. I had initially presented with joint pains, which rapidly progressed to heart, lung, blood vessel, and kidney involvement. I was on 6 medications, including high dose prednisone and immunosuppressants, and chemo drugs. I had to quit my job in the ER at this time, because I was so immunocompromised. It was a very depressing and dark time in my life, and I was scared for my health. This is when I googled diet for lupus patients, and learned about plant-based whole food diets. I transitioned immediately after learning about it, but my healing was still a long course. I was not one of those people who went plant-based and all of my medical problems went away. I tweaked the diet quite a bit to a full autoimmune healing diet, developed a comprehensive mind-body program to calm my nervous system and stress hormones, improved my sleep, and made other lifestyle adjustments. I also had to eliminate toxins completely and overhaul many aspects of my life. It's been quite a journey, and I live a very different, much more awake, aware, and present life, for which I'm ultimately grateful. Dr. Baruch Fertel is the Director of Operations for the Emergency Services Institute and all of the Cleveland Clinic Emergency Departments. He has a strong interest in high reliability, process improvement, utilization, standardization, opioid stewardship and Peer Review/QA. Dr. Fertel is also Associate Chief Quality Officer- Regulatory Affairs for Enterprise Quality & Safety where he provides clinical oversight and leadership for Cleveland Clinic's interaction with various regulatory agencies, as well as oversight of professional practice evaluation (Peer Review, OPPE, FPPE). Previously, he was Medical Director in the IT Division/ Clinical Systems Office with oversight over the Electronic Health Record (EPIC-ASAP) and other aspects of Clinical Informatics for the Emergency Services Institute and was also the Quality Improvement Officer for the Emergency Services Institute. Clinically, Dr. Fertel sees patients in both the General and Pediatric ED's. He is a teacher to residents, PA and medical students. He holds the academic rank of Associate Professor of Emergency Medicine at the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University and was also part of the faculty at the PA program at Case Western. Academic accomplishments include more than 80 published abstracts, manuscripts and book chapters. In addition he has made numerous media (TV/Radio/Print) appearances locally and nationally. He is an active member of the American College of Emergency Physicians (ACEP) both nationally and locally, sits on the ACEP Membership Committee, ACEP EM Practice Committee and is Chair of the Ohio ACEP Government Affairs Committee. Perhaps 100 young people may be prescribed opioid pain killers for extracts wisdom teeth or a broken bone and only a handful may feel a strong pull of addiction after a single pill.  The problem is we do not know who that vulnerable handful is.  That is why prevention of exposure is important. It does not mean prohibition of opioids, but it does mean innovations in pain management that avoid opioids when possible. The opioid epidemic has a silver lining. It engaged the medical community in solution. All medical specialties have been activated in the issue of addiction and pain. And the medical community is rallying – with solutions and innovation in managing pain better with less addiction. Alexis M. LaPietra, DO, FACEP Dr. Alexis LaPietra is System Chief for Pain Management and Addiction Medicine as well as the Fellowship Director of the Emergency Medicine Pain Management Fellowship and Emergency Medicine Addiction Medicine Fellowship at St Joseph's Health in NJ. Evie Marcolini, MD is an Assistant Professor of Emergency Medicine and Neurocritical Care.  She has clinical appointments in the Department of Emergency Medicine and the Department of Neurology; and is core faculty in the Emergency Department as well as the Division of Neurocritical Care and Emergency Neurology at Yale. She has board certifications in Emergency Medicine and Neurocritical Care, and is fellowship-trained in Surgical Critical Care from the R Adams Cowley Shock Trauma Center in Baltimore, Maryland.   Evie is Medical Director for the SkyHealth Critical Care helicopter transport service that is shared between Yale-New Haven Health System and Northwell Health. She is a Fellow of the American College of Critical Care Medicine, American Academy of Emergency Medicine (AAEM) and American College of Emergency Physicians (ACEP), and is a member of the Board of Directors for AAEM. She is the past Chair of the Critical Care Section of ACEP, and active in all three societies, with a goal of bringing emergency physicians to the forefront of critical care in all realms. Evie is an active educator and speaker, with an interest in neurocritical care topics as they relate to emergency medicine and bioethics as it relates to emergency medicine and critical care patients. She has, and continues to lecture nationally and internationally, and has won the ACEP Junior Faculty Teaching Award. She has been active as faculty for Wilderness Medical Associates International since 1992, and teaches wilderness medical courses to medical students, residents, faculty and allied health professionals nationally and internationally, as well as having served as Faculty Advisor to wilderness medicine interest groups at the University of Maryland and Yale. As Emergency Rooms and Hospitals quickly get overwhelmed by ill patients, the Urgent Care and Primary Care setting play critical roles in appropriately triaging patients that can go home vs those that need to be transferred to the Emergency Department for further evaluation and treatment. The ability to act as levies to the large number of patients during this pandemic will help preserve the ED and Hospital “resources” for those critically ill patients that need it most.  In order to help UC and PC providers make these decisions the College of Urgent Care Medicine (CUCM) and the American College of Emergency Physicians (ACEP) have published a set of criteria that break suspected Covid-19 patients into 2 categories. This is a technique through which you can optimize your existing skills in venous access." ~Bret Nelson How to Start a Nurse Ultrasound IV Training Program?! Nurse Driven Program Nurses interested in ultrasound should be the course director(s). Doesn't necessarily have to be the unit Nurse Educator. If working with MD, have multiple Course Directors or MD can be there to help out initially. Nurses can and should drive this program. Nurses teaching nurses a new skill offers insight that providers aren't able to provide. Only takes 1-2 Nurses to drive practice change within a unit! Multidisciplinary Course Directors and/or Instructors (RN and MD) Both nurses and physicians bring a lot of their own skills to the table. Combining skills, we learn from each other and ultimately become better at obtaining IV access with ultrasound use. ED Initiative for ED Nurses and ED Culture Utilizing Multidisciplinary Support A positive initiative that improves patient care. We created a culture where once a nurse finishes competency, they are able to place ultrasound-guided IV access on their own. Sometimes, you run into trouble and usually it's because the patient has limited veins, so they are still able to ask for help. Have another person look with the ultrasound. It's okay to ask for help! This has also increased more collegial respect between disciplines and increased teamwork. Nurse buy-in and Physician buy-in We decided to make this program optional for nurses. Currently we only have this program for our adult population in the ED. Nurses love the program and so do our patients! Support from Nursing and Physician Leadership Talk to your Nursing and Physician Directors of your department. Keep them in the loop. I have found Leadership to be extremely supportive and helpful. Nursing Policy and Nursing Education Written policy for nurses is important to have in writing within your department/institution. Work with your Nursing Director and/or Nursing Education in order to develop policy if it's not already written or change/expand existing policy. Know your policy. Some may restrict this skill to nurses working in specific departments.

Empowering nurses with an advanced skill Nurses are able to identify a patient as a hard stick and obtain IV access with ultrasound without waiting for a physician to place the IV. Nurses who need to develop their IV skills have a skill to strive for. Patient Satisfaction Nurses are able to identify patients who are a hard stick. Patients have reduced failed attempts prior to an ultrasound guided placed IV access. Essentially, patients get stuck less but get good results. Patients know they are a hard stick - they are happy. Who Should be getting USG PIV?
