In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Enrique Chacón to discuss the SENECA Study: Molecular profiling and SLN. Dr. Chacón works in the Gynecologic Oncology Unit of the Clínica Universidad de Navarra. Dr. Chacón is an active member of the European Society of Gynecologic Oncology where he is the co-chair of the ENYGO Online Education Committee and the national representative of Spain. In 2021 he completed his editorial fellowship in the IJGC, where today he is serving on the Editorial Board of the journal. Highlights: This study, for the first time, reveals significant differences in SLN involvement among more than 2000 patients with early-stage endometrial cancer based on their molecular subtypes, with the p53 abn and MMRd groups having the greatest lymph node involvement. The study defined the risk of SLN involvement for each of the ESGO risk groups. In this sense, the study notes that molecular profiling does not improve the prediction of nodal status with respect to the classical risk factors (FIGO stage and histology). Lymph node staging should not yet be adopted based on molecular profiling as prospective studies are needed to validate whether these differences impact survival (DFS/OS)

Les émeutes qui embrasent la capitale de la Nouvelle-Calédonie depuis le 13 mai ont éclaté sur fond de crise économique. La filière nickel, le poumon du « caillou », est au bord de la faillite. L'an dernier, les cours du métal se sont effondrés de moitié. Résultat, en valeur, les exportations de nickel de la Nouvelle-Calédonie ont chuté de 15% en 2023. Cela a été le coup de grâce pour une filière déjà très mal en point.Pourtant, entre les années 1960 et 2000, le nickel est un moteur de l'économie. C'est grâce à ce minerai que la Nouvelle-Calédonie atteint un niveau de vie comparable à celui de la métropole. Les trois usines actuelles génèrent 10% du PIB et un quart de l'emploi privé. Si la faillite survient, 70 000 emplois seront sur la sellette.L'exploitation demeure un filon d'avenir, parce que la Nouvelle-Calédonie détient les deuxièmes réserves au monde et parce qu'il faut du nickel en quantité pour fabriquer les batteries des voitures électriques. Mais aujourd'hui, l'une des usines de raffinage est à l'arrêt et les deux autres au ralenti.Le nickel calédonien trop cher à produireLe coût de la main-d'œuvre et de l'énergie est prohibitif face à la concurrence féroce de l'Indonésie. Le géant du secteur qui a inondé le marché en 2023. Glencore cherche un repreneur pour l'usine du nord à Koniambo, qui risque de fermer dès le mois d'août. Eramet, l'opérateur historique à Nouméa, avec sa filiale SLN, maintient le site grâce aux crédits consentis par l'État. Au-delà des difficultés conjoncturelles et bien réelles, les opérateurs sont surtout lassés, déroutés par l'absence de consensus politique sur la gestion de la filière.Une gestion politique paralysanteLe métal a, certes, été un moteur de la croissance et une aubaine pour les rentrées fiscales de l'archipel, mais aussi un puissant facteur d'inégalités en faisant grimper le coût de la vie sur l'ensemble du territoire alors que la manne minière a longtemps profité aux seuls habitants de la capitale Nouméa.Dans le cadre des accords de Matignon puis de Nouméa, les indépendantistes obtiennent un rééquilibrage de cette rente minière avec la construction de l'usine de Koniambo au nord et sa gestion par la province qu'ils dirigent. Elle verra enfin le jour en 2013. Mais depuis, les décisions stratégiques sont souvent parasitées, voire empêchées par des polémiques politiciennes.La pauvreté et les inégalités, le revers de la médailleDernière en date : le pacte nickel proposé par Bercy pour réduire les coûts de production a capoté, faute d'accord politique. L'État proposait une enveloppe de 200 millions d'euros à condition que les collectivités ajoutent 66 millions d'euros. C'est l'un des motifs qui les a fait reculer alors que ce coup de pouce est une nécessité absolue pour retrouver de la compétitivité. Il faut dire que leurs finances sont exsangues, avec un taux d'endettement de 150% par rapport aux recettes réelles.Même si le tourisme est quasiment revenu au niveau d'avant le Covid, la Nouvelle-Calédonie panse encore les plaies causées par la pandémie et l'inflation. Le coût de la vie demeure exorbitant, 30% plus élevé qu'en métropole. In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Andrea Rosati. Mr. Rosati is a consultant at the Department of Gynecologic Oncology at Fondazione Policlinico Universitario Agostino Gemelli in Rome (Italy). He is currently attending a second level master "Gynecologic Oncology International Master" at the Catholic University of the Sacred Heart (Rome, Italy) accredited as a Subspecialty Fellowship by the European Society of Gynaecological Oncology. His main interest areas are gynecological cancer, surgical anatomy, and gynecologic oncology surgery. Highlights: This study evaluated the prevalence of concurrent endometrial cancer in patients with pre-operative diagnoses of atypical endometrial hyperplasia undergoing hysterectomy. Among 460 patients, 47.2% were found to have concurrent endometrial cancer. According to ESGO-ESTRO-ESP classification, 71.4%, were low-risk, 9.7% intermediate, 11.1% high to intermediate and 7.8% high-risk tumors. Positive Lymph node were found in 12 patients, accounting for the 7.6% of concurrent endometrial cancers who underwent SLN biopsy. SLN biopsy allowed for the adjustment of adjuvant therapy in 12.3% of high to intermediate-risk patients without increasing operative time or complication rates.

In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Drs. Giuseppe Cucinella, Andrea Mariani, and Gretchen E. Glaser to discuss ITCs in low-risk endometrial cancer. Giuseppe Cucinella, MD, is a gynecologist working at Istituto Nazionale Tumori IRCSS Fondazione G.Pascale in Naples, Italy (Department of Gynecologic Oncology). During his residency, he worked as a research fellow at Mayo Clinic in Rochester, Minnesota, where he focused on the study of endometrial cancer. He is currently in the second year of the PhD program in "Experimental Oncology and Surgery - Gynecologic Oncology" at the University of Palermo, Palermo (Italy). Dr. Cucinella's clinical research focuses on the diagnosis and treatment of endometrial cancer and minimally invasive surgery in gynecologic oncology. Dr. Andrea Mariani is a Full Professor in the Department of Obstetrics and Gynecology, Division of Gynecologic Surgery, at Mayo Clinic in Rochester, Minnesota. Dr. Mariani is the Gynecologic Oncology Division Chair in the Department of Obstetrics and Gynecology. Dr. Mariani's research interest is endometrial cancer with a special interest in robotic surgery. He is internationally recognized for his contributions in the advancement of surgical and postoperative treatment of endometrial cancer. Gretchen E. Glaser, MD is a Gynecologic Oncologist at Mayo Clinic specializing in complex surgery for malignant and benign conditions, including minimally invasive and open approaches. In addition to her clinical work, Dr. Glaser focuses her clinical research in endometrial cancer diagnosis, treatment, and outcomes as well as quality improvement of surgical care using enhanced recovery after surgery techniques. She also serves as the Vice Chair and Practice Chair for Obstetrics and Gynecology and has a special interested in patient and physician wellness. Highlights: • The aim of our study was to evaluate the prognostic value of SLNs-isolated tumor cells in patients with low-risk endometrial cancer who underwent SLN biopsy and did not receive adjuvant therapy. Outcomes were compared with node negative patients. • Our study shows that recurrence free survival among patients with low-risk endometrial cancer and no adjuvant treatment was significantly worse in patients with SLNs–isolated tumor cells compared with negative nodes. However, we did not observe any significant difference in overall survival. • The presence of SLNs-isolated tumor cells, lymphovascular space invasion, and International Federation of Obstetrics and Gynecology (FIGO) grade 2 were significant risk factors for recurrence (any recurrence and non-vaginal recurrence). • Our group has designed a prospective multicenter cohort study called ENDO-ITC with the aim of definitively establishing the prognostic role of ITC in an otherwise low-risk population who undergo SLN biopsy. In addition, we will evaluate a standardized follow-up plan for these patients.

Saint Lawrence Nurseries is a specialized tree nursery in New York state, renowned for its expertise in cold-hardy fruit and nut trees with a committment to sustainable and organic practices. The nursery, which was originally founded in the 1920s by Fred L. Ashworth of Heuvelton, NY, was initially more of a hobby and experimental station for Ashworth. Bill MacKentley met Fred in 1971 and they developed a strong, almost familial relationship. After Fred's death in 1977, Bill and his wife Diana purchased the land in Potsdam, NY, where Fred had done much of his grafting and breeding. In 1981, they produced the first official St. Lawrence Nurseries catalog. For 34 years, the MacKentleys cultivated Saint Lawrence Nurseries into a unique and well-loved business, carrying on Fred Ashworth's passion for growing and propagating fruits and nuts suitable for northern climates. The MacKentley, were planning their retirement and phasing out their business around 2015. Connor Hardiman who began working at Saint Lawrence Nurseries years earlier stepped in to take over rather than allowing the nursery to close. He and his wife Alyssa are the new stewards of Saint Lawrence Nurseries and are helping to herald in a new era for the Nurseries. Do note that many refer to Saint Lawrence Nurseries as SLN. The Ups and Downs of Climate and Test Winters Citing from a Cornell University study for 1981, that stated the the the previous 40 years had been the best years of agriculture (good weather patterns) in the world. Of particular note is that moving forward from 81 onward, one can now expect that the climate was returning to normal. Note that this was serval years before climate change was being discussed to the degree that it is today. Says Bill, “We have got to get our act together [in regards to climate change]. Who is Fred L. Ashworth Born in 1898, Ashworth studied the trees that survived the 1938 kill off of apples and studied them very closely. He began the nursery in the 1920s. He also worked with nut trees, potatoes and inspired Bill's career at Saint Lawrence Nurseries. In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Núria Agustí to discuss SLN mapping in ovarian cancer. Dr. Agustí specializes in gynecologic oncology at Hospital Clinic of Barcelona. She is currently a PhD researcher at the University of Barcelona and engaged in a postdoctoral fellowship at MD Anderson Cancer Center, USA. She recently completed the IJGC Editorial Fellowship program. Highlights: There is a high heterogeneity in SLN mapping techniques, emphasizing the need for standardized protocols and further research in this area. The study reports a sentinel lymph node detection rate of 93.3% in patients with early-stage ovarian cancer with a 100% negative predictive value. The study suggests that the combination of radiotracer and indocyanine green for tracer injection into the utero-ovarian and infundibulo-pelvic ligaments may yield the best detection rates for SLNs in ovarian cancer. Additional ongoing and well-defined studies and consideration of oncologic outcomes before implementing SLN detection as a routine practice for ovarian cancer staging is highly needed.

In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Vito Andrea Capozzi to discuss SLN alone in high-risk endometrial cancer. Dr. Capozzi works at the Department of Medicine and Surgery of the University Hospital of Parma, Italy. He shows special interest in gynecologic oncology, oncologic surgery, and translational medicine. Highlights: Sentinel lymph node mapping alone in high-risk endometrial cancer patients is an oncologically safe technique over a median observational time of 31 months. No significant difference in overall survival and disease-free survival was observed between sentinel lymph node alone and Lymphadenectomy groups. Patients with histologically proven nodal metastasis had overlapping survivals and disease-free survival in the sentinel lymph node alone and lymphadenectomy groups.

He received his PhD on new technologies for endometriosis surgical removal from the University of Bologna in 2021. He is an Advisory Board Member of the ESGE (European Society for Gynaecological Endoscopy) and member of the Special Interest Group in Robotics. His research interests are focused on minimally invasive surgery (laparoscopic, robotic and hysteroscopic) for endometriosis and endometrial cancer. Dr. Giulia Rovero graduated in Medicine in 2017 at the University of Florence. She is currently a fifth-year resident at the Division of Gynecology and Pathophysiology of Human Reproduction, Bologna, Italy, directed by Prof. Renato Seracchioli. In 2022/23 she spent twelve months at the Lariboisiere Hospital in Paris focusing on gynecologic oncology. She is developing her research activity in collaboration with Dr. Raimondo in the field of minimally invasive surgery for endometrial cancer. Highlights: In endometrial carcinoma (EC) patients, the rate of sentinel lymph node (SLN) mapping failure ranges from 20% to 25%. Pooled data assessing predictive factors of SLN mapping failure in EC patients undergoing SLN biopsy through the cervical injection of indocyanine green (ICG) are lacking. ICG dose 30 kg/m2, menopausal status, adenomyosis) nor surgical history is significantly associated with SLN mapping failure in EC patients. Deep myometrial invasion, FIGO grade 3, non-endometrioid histotype, and lymphovascular space invasion are not significantly associated with SLN mapping failure in EC patients.

In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Devon Abt to discuss SLN mapping in endometrial hyperplasia. Devon is a fourth-year Resident in Obstetrics & Gynecology at Beth Israel Deaconess Medical Center/Harvard Medical School currently applying into Gynecologic Oncology fellowship. Highlights: - Fewer than 30% of patients with preoperative diagnosis of endometrial intraepithelial neoplasia had concurrent endometrial cancer on final pathology. Endometrial stripe ≥20mm was associated with 2 times the risk of concurrent endometrial cancer. - Routine SLND in all patients with preoperative diagnosis of endometrial intraepithelial neoplasia is not cost-effective and would result in overtreatment. - Endometrial stripe may be a criterion for selectively using a sentinel lymph node algorithm in patients with preoperative diagnosis of endometrial intraepithelial neoplasia who are poor candidates for lymphadenectomy or at institutions where frozen section is not available or reliable.

Rachel and Christie talk with Emily Holl of the Sibling Support Network about the challenges for siblings of kids with disabilities both visible and invisible. Emily talks about SibShops, a worldwide program, that offers support to siblings of kids with special needs. Emily also offers tips on how parents can best support these siblings and help them voice their own feelings and fears. Emily Holl is the Director of the Sibling Support Project. Emily is a social worker, author, trainer, and sibling. Since 2003, she has worked in the disability field and has provided workshops, training, and groups for siblings, families, and individuals with disabilities. Emily has served as a board member of the national Sibling Leadership Network (SLN), and she founded sibsNY, the New York state chapter of the SLN. Emily has written about her own sibling experiences in blogs, magazines and books such as Thicker than Water. Emily was an author and a co-editor of The Sibling Survival Guide: Indispensable Information for Adult Brothers and Sisters of People with Disabilities, published by Woodbine House in 2014. Dr. Ferguson is a Professor in the Division of Gynecologic Oncology in the Department of Obstetrics and Gynecology at the University of Toronto and a member of the Division of Gynecologic Oncology at University Health Network/Sinai Health Systems. She is the Director of Research for the Division of Gynecologic Oncology at Princess Margaret Cancer. Dr. Ferguson completed her residency in Obstetrics and Gynecology at University of Toronto and a fellowship at Memorial Sloan-Kettering Cancer Centre in Gynecologic Oncology. Current research program includes prospective surgical trials in cervical and endometrial cancer, evaluation of patient reported outcomes and molecular characterization of endometrial cancer. Highlights: + Diagnostic accuracy of SLN technique is excellent in high grade endometrial cancer – high sensitivity (low false negative rate), high NPV , DR and bilaterality rate + Based on this is a viable option to surgical staging in this population (clinical stage 1, high grade endometrial cancer) + High proportion (26%) of node positive would be missed if SLN technique was not performed
Welcome to Talk Tuesdays. This is part 2 with Dr. Coleman. Today we are talking about Breast Cancer Surgery.What options are there for breast cancer surgery?There are different types of cancer so first things will depend on what kind of cancer you have. Not all abnormalities that need surgery are cancer and not all cancer operations mean you will lose your whole breast. Again, don’t panic, there are a lot of options for surgery. You will need to discuss everything with your physician to determine what is the best course of action for you, but we will discuss some options. Breast conservation: this is also known as a lumpectomy or partial mastectomy. This is where a small-ish incision is made and the area of concern is removed with a small portion of normal breast tissue surrounding it. This is sometimes done using wire localization, but more recently this has moved to using radioactive seeds and such that can be located using a probe in the operating room. If a wire is used, this is placed by a radiologist into or close to the area of concern and secured/covered so as to help prevent dislodgment prior to surgery. Depending on if you have DCIS or invasive cancer will determine if you need any lymph nodes from your axilla (AKA armpit) removed. Mastectomy is where the whole breast is removed. There are different types of mastectomies out there. A simple mastectomy removes the nipple/areola/ and all breast tissue. A modified radical mastectomy is the same as a simple mastectomy except in this operation all of the lymph nodes in your axilla are removed as well. There are nipple sparing and skin sparing mastectomies as options also. All of these mastectomies can follow with breast reconstruction by a plastic surgeon either during the same operation or at a later date depending on the type of cancer you have and the other treatments that are needed after surgery. Understand that there are risks and benefits of each type of surgery and what may be best for you may not be the option you desire. What are lymph nodes and why are they removed?The lymph nodes often times are said to “filter your blood”. If you have breast cancer that cancer is going to “drain” from your breast to those lymph nodes. If tumor is found in those nodes that is a sign of metastatic cancer. That’s why it is important to evaluate the lymph nodes in certain types of cancer, especially invasive cancer. This can be done in 2 ways - a sentinel lymph node biopsy or an axillary dissection. A sentinel lymph node biopsy can be done with breast conservation or mastectomy. When done with breast conservation a separate incision is made in the armpit to access the lymph nodes. With a mastectomy the same breast incision is used. A radioactive tracer is injected by nuclear medicine prior to surgery as well as a blue dye in the operating room. Both of these tracers help find the “sentinel” node. At least 3 nodes need to be taken in most cases and sometimes it will be more than that depending on whats seen in the OR. An axillary dissection involved removing all of the lymph nodes in the axilla. This is usually done at the time of mastectomy through the same incision. Sometimes it will be done after a SLN biopsy has shown metastatic disease that needs to be removed. But all the tissue is removed in this area. There are nerves and blood vessels there that can be injured and can make it difficult to lift your arm or cause your shoulder blade to stick out, but injury is not super common. What happens after surgery?Depending on the type of surgery you had you may be able to leave that day or you may stay in the hospital. After a mastectomy you will have a drain. if you had an axillary dissection you will likely have 2 drains. You typically go home the day after surgery and drains will come out at your next clinic appointment where the pathology from the OR will be discussed in more detail. You will see an oncologist to discuss any need for chemotherapy and radiation. Depending on the cancer if breast conservation was done and there are “positive margins” you will then need more surgery and the details of that can be discussed at that time. She helps coordinate the Council’s communications efforts (including its social media, website, publications, and community outreach events) to inform Tennesseans with disabilities and families about the issues that impact their lives. Emma also has 24 year old younger twin brothers, Evan and Brendan; Evan has autism and lives in a supported living apartment with the help of a provider agency. Emma leads the Tennessee Adult Brothers and Sisters (TABS) statewide sibling support network for brothers and sisters of individuals with disabilities, launched by the TN Council and Vanderbilt Kennedy Center for Excellence in Developmental Disabilities over a decade ago. She also serves as co-chair of the Chapter Development Committee of the national Sibling Leadership Network (SLN). TABS is TN’s chapter of the SLN. Martin Kelly is best known to SLN listeners for authoring the book "Fender The Golden Age" along with his brother photographer Paul Kelly and previous SLN participant Terry Foster. We visited him outside London where he lives with his wife Sarah Cracknell singer of Saint Etienne for this interview which covers his history and fascination with vintage guitars and amplifiers and his career as a musician, manager, label owner and producer of films. In dieser retrospektiven klinischen Studie sind die Daten von 183 Patienten aufgearbeitet, bei denen in einem Zeitraum von 2005 bis 2011 eine inguinale oder axilläre TLAD durchgeführt worden war. Es wurde auf drei Themenschwerpunkte eingegangen. 1. TLAD-Chirurgisches Management: Die Gesamtkomplikationsrate der TLAD betrug 40,56 % (axillär 30,84 %, inguinal 54,79 %). 76,71 % der postoperativen Komplikationen mussten erneut operativ oder konservativ behandelt werden. Eine Lymphozelenbildung (34,31 %), Lymphfistelung (20,59 %) und Wundheilungsstörungen (22,55 %) stellten die häufigsten Komplikationen dar. Als postoperative Risikofaktoren wurden die inguinale Dissektion (Koeffizient 1,096, p-Wert 0,002) sowie ein NM (Koeffizient 0,912, p-Wert 0,025) identifiziert. Die Krankenhausverweildauer wurde mit steigendem Patientenalter sowie bei einer inguinalen im Vergleich zu einer axillären TLAD verlängert. 2. Risikofaktoren für einen positiven NSLN (bei positivem SLN): Ein höheres pT-Stadium führte zu einem größeren Risiko für einen NSLN-Befall (Koeffizient 0,179, p-Wert 0,0259). 3. Beeinflussende Faktoren der MMS und des rezidivfreien Überlebens (bei positivem SLN): Je höher der Breslow-Index war, desto größer war das Sterberisiko (HR 1,204, Koeffizient 0,186, p-Wert 0,0088). Patienten mit regionärer Metastasierung wiesen im Vergleich zu einer fehlenden regionären Metastasierung ein geringeres Überleben auf (p-Wert < 0,0001). Das Patientenkollektiv mit verzögerter TLAD (keine SLNB, TLAD bei klinischer Auffälligkeit) zeigte diesbezüglich die niedrigste Überlebensrate (p-Wert < 0,0001). Ein positiver NSLN (HR 2,470, Koeffizient 0,904, p-Wert 0,0174) war der bedeutendste Prognosefaktor bezüglich des rezidivfreien Überlebens.

Dr Fowble talks to ecancertv about the role of radiation in the treatment of the axilla in the setting of neoadjuvant chemotherapy for breast cancer. There is considerable controversy regarding the appropriate surgical procedure and its timing for evaluating the status of the axillary nodes in the neoadjuvant setting. Sentinel lymph node (SLN) surgery following initial chemotherapy avoids an axillary dissection and its associated morbidity in clinical N0 patients who have pathologic negative SLN. The potential advantage of neoadjuvant chemotherapy is the ability to adjust surgical and radiation treatment based on pathologic response and thereby minimise the morbidity of combined therapy.

Sentinel lymph node (SLN) surgery may provide a less-invasive alternative to axillary lymph node dissection (ALND) for patients with node-positive breast cancer. Dr Judy Boughey explains that the majority of women with node-positive breast cancer currently undergo ALND as a treatment, while SLN surgery is typically used for patients diagnosed with node-negative disease. However, results of the Z1071 study suggest that treating node positive patients with neoadjuvant chemotherapy can eradicate disease in the lymph nodes and allow successful treatment with SLN surgery. Dr Boughey discusses the results of this study, and outlines the levels of adverse effects and the effectiveness with which SLN surgery can correctly identify nodal status following neoadjuvant therapy.

African American women with early stage, invasive breast cancer were 13 percent less likely than Caucasian women with the same diagnosis to receive a minimally invasive technique, axillary sentinel lymph node (SLN) biopsy, years after the procedure had become the standard of surgical care, according to research from MD Anderson Cancer Center. The older technique, axillary lymph node dissection (ALND), is associated with a higher rate of lymphedema and worse outcome. Dalliah Black, M.D., assistant professor in Surgical Oncology and Benjamin Smith, M.D., assistant professor in Radiation Oncology at MD Anderson, discuss their findings.

Der bisherige Konsens, dass bei melanompositivem SLN-Befund eine CLND indiziert ist, wird zunehmend in Frage gestellt, da sich dabei selbst mit aufwändiger Diagnostik nur in 15-30% der Dissektate noch weitere Lymphknotenmikrometastasen finden lassen. Noch existiert aber kein neuer Konsens, bei welchen Melanompatienten mit SLN-Befall auf die erheblich morbiditätsbehaftete CLND verzichtet werden kann. Die vorliegende Daten- und Verlaufsanalyse bei insgesamt 276 Patienten mit jeweils mindestens einem melanombefallenen SLN aus zwei großen deutschen Hautkliniken (Augsburg und Göttingen) zielt darauf ab, geeignete Entscheidungskriterien für oder gegen eine CLND bei künftigen Melanompatienten zu erarbeiten. Als Voraussageparameter sowohl für die CLND-Befunde als auch für die Metastasierungsverläufe erwies sich die von Starz mikromorphometrisch definierte S-Klassifikation der in der aktuellen AJCC-Klassifikation berücksichtigten Anzahl der befallenen SLN als weitaus überlegen. Jeweils mindestens eine Melanommikrometastase in den auch immunhistochemisch aufgearbeiteten CLND-Dissektaten ließ sich nur bei 12% der S I Patienten und bei 16% der S II Patienten nachweisen. Im Gegensatz dazu lag bei S III der Prozentsatz der Dissektate mit Mikrometastasen bei 50%. Bei insgesamt 76 S I / S II Patienten, die sich nach ausführlicher Aufklärung gegen eine CLND entschieden hatten, ergab die Kaplan-Meier-Auswertung für den Nachbeobachtungszeitraum von fünf Jahren eine Manifestationsrate regionärer Lymphknotenmetastasen von 14%. Dies entsprach fast exakt den Erwartungen aus den o.g. histologischen Non-SLN-Untersuchungsergebnissen der anderen S I / S II Patienten mit CLND. Interessanterweise blieb aber auch diese Patientengruppe trotz CLND nicht komplett verschont von regionären oder postregionären Lymphknoten-metastasen im weiteren Verlauf. Die 5-Jahres-Rate betrug 10,5% und war im Log-Rank-Test nicht signifikant niedriger als bei den S I / S II Patienten ohne CLND. Die für denselben Zeitraum errechnete Fernmetastasierungsrate lag für S I / S II Patienten ohne CLND bei 14%, für jene mit CLND bei 19,5%, die melanombedingte Sterberate ohne CLND bei 6%, mit CLND bei 19%. Auch diese jeweiligen Unterschiede waren im Log-Rank-Test nicht signifikant. Die Anzahl der S III Patienten ohne CLND ist mit insgesamt elf aus beiden Städten für eine valide statistische Aussage zu klein. In diesem Fall muss auf die Ergebnisse aus laufenden multizentrischen Studien, speziell des Multicenter Selective Lymphadenectomy Trial 2 (MSLT-2) und der ähnlich von der Arbeitsgemeinschaft Dermatologische Onkologie (ADO) konzipierten deutschen Studie, gewartet werden. Vorläufig spricht die o.g. CLND-Datenlage mit einer Non-SLN-Metastasen-Detektionsrate von ca. 50% für eine Beibehaltung der CLND-Indikation bei dieser Patientengruppe. Demgegenüber deuten die Erfahrungen bei S I / S II Patienten auf einen zweifachen therapeutischen Effekt der nur gering invasiven SLNE hin: 1. Sie erspart der Mehrzahl dieser Patienten die spätere klinische Manifestation regionärer Lymphknotenmetastasen und potentiell daraus sich entwickelnder, meist fataler Fernmetastasen. 2. Sie kann ihnen mehrheitlich auch die Morbidität einer radikalen regionären Lymphknotendissektion ersparen.
Background: Sentinel lymph node biopsy (SLNB) has become the standard care for melanoma and is an important diagnostic procedure. It has been doubted whether lymphoscintigraphy detects the correct sentinel lymph node (SLN) when excision of the tumor and SLNB are not performed at the same time. This would imply that this sequential approach may have an increased risk of undetected micrometastases resulting in a worse outcome. Objective: The purpose of the present study was to compare the outcome of melanoma patients having received excision of the tumor and SLNB either at the same time or consecutively. Methods: A total of 854 patients with cutaneous melanoma were enrolled in this retrospective study between September 1996 and November 2007. Disease-free (DFS) and overall survivals (OS) were estimated using the Kaplan-Meier product limit method and were analyzed by the log rank test. Results: No statistically significant difference was found regarding DFS, progression rates and OS in patients with primary tumor excision and SLNB at the same time compared with patients with excisional biopsy of primary tumor and SLNB at different times. Conclusion: These data suggest that excisional biopsy of the primary tumor does not prevent the correct SLN mapping in melanoma patients. Copyright (C) 2010 S. Karger AG, Basel

Introduction: Having in mind the promising results of lymphoscintigraphy and intraoperative gamma probe application for the detection of sentinel lymph nodes (SLN) in malignant melanoma, breast and penis cancer, we tried to identify the SLN in prostate cancer by applying a comparable technique. Materials and Method. 350 patients with prostate cancer were examined after providing informed consent. The day before pelvic lymphadenectomy technetium-99m nanocolloid was transrectally injected into the prostate under ultrasound guidance. A single central application was done per prostate lobe in most cases. Activity attained 90400 MBq, and the total injected volume was about 2-3 ml. Hereafter, lymphoscintigraphy was carried out. Those lymph nodes having been identified as SLN by means of gamma probe detection and lymphoscintigraphy were removed intraoperatively. Later, most of the cases had different types of pelvic lymphadenectomy. SLN received serial sections and immunohistochemistry, non-SLN step sections. Results: 335 patients showed at least 1 SLN in lymphoscintigraphy. 24.7% had lymph node metastases. In 2 patients, metastases in non-SLN were found without at least one SLN being affected (false-negative patient). Conclusion:Our experience suggests that the SLN identification is not only feasible in breast cancer and malignant melanoma, but also in prostate cancer with a comparable technique. The activity of the external (motor) branch of the superior laryngeal nerve (SLN), innervating the cricothyroid muscle, was recorded in the greater horseshoe bat,Rhinolophus ferrumequinum. The bats were induced to change the frequency of the constant frequency (CF) component of their echolocation signals by presenting artificial signals for which they Doppler shift compensated. The data show that the SLN discharge rate and the frequency of the emitted CF are correlated in a linear manner.
The motoneurons innervating the laryngeal muscles were localized in the rufous horseshoe bat,Rhinolophus rouxi, using the HRP method. HRP was applied to the cricothyroid muscle and to the cut end of the recurrent laryngeal nerve. Labeled motoneurons were found in two completely separated regions of the nucleus ambiguus. The motoneurons innervating the cricothyroid muscle via the superior laryngeal nerve (SLN) are located withinn the ventrolateral portion of the nucleus reaching the caudal pole of the motor nucleus of the facial nerve. The motoneurons innervating the other intrinsic laryngeal muscles via the recurrent laryngeal nerve (RLN) are situated in the caudal half of the nucleus ambiguus. The innervation is strictly homolateral.
