Why does a potentially deadly condition that affects 1 in 7 adults fly under the radar, undetected and undiscussed? Join us on this episode of Quality Matters as we explore Chronic Kidney Disease (CKD), a silent killer that's finally coming into the limelight of quality improvement. Our conversation begins with Dr. Ben Oldfield, a primary care physician, who sheds light on the startling prevalence and hidden complexities of CKD. Why is it so hard to detect, even with simple blood and urine tests? And why does this condition, despite its extensive impact, often get lost in the shuffle of busy health care visits? We'll unpack the challenges of diagnosing and managing CKD, exploring the emotional weight the diagnosis carries for patients and the hurdles clinicians face in communicating about it. Discover why CKD is a “loaded term” and how we can shift the narrative from fear to empowerment. But that's not all. We'll also tackle the big picture: How CKD fits into the broader landscape of cardiovascular, kidney, and metabolic (CKM) disorders. Could a holistic approach to these interconnected conditions revolutionize patient care? The conversation ends with Ben's thoughtful observations on how classic literature, particularly epic poetry, helps doctors make sense of health care's emotional complexities, connect with patients and find empowerment in the face of life and death.Join us as we explore how we can move CKD from the sidelines to the front lines of quality improvement, and how collaborative, longitudinal care can make a real difference.Key Quote:“Thinking about chronic kidney disease is a longitudinal process. It's only half the story to make the diagnosis in a snapshot in time. What's the follow up like? How are we doing with the patient over time? Because oftentimes quality measures can look at things in a snapshot in time. Chronic kidney disease really begs us to think more longitudinally.”“Benjamin Oldfield, MDTime Stamps:(02:54) The Silent Nature of CKD(07:05) Quality Measurement and CKD(11:08) The Link Between CKD and Cardiovascular Kidney Metabolic syndrome(13:06) CKD and CKM as Quality Priorities (15:45) Narrative Medicine and Personal InsightsLinks:NCQA Kidney Health ToolkitConnect with Benjamin Oldfield

In EPISODE 182 OF GROWING OLDER LIVING YOUNGER, Dr. Thomas Weimbs and Dr. Gillian Lockitch  discuss the prevalence and management of chronic kidney disease (CKD), which affects one in seven adults in the U.S. Dr. Weimbs highlights the importance of diet and lifestyle in managing CKD, particularly through ketogenic metabolic therapy. He explains that ketosis, induced by a low-carbohydrate diet, can slow the progression of polycystic kidney disease (PKD) by reducing inflammation and improving kidney function. Dr. Weimbs  describes the Ren-Nu program, a three-month nutrition and lifestyle intervention for kidney disease patients, which has shown significant improvements in kidney function and pain reduction for many participants. Whether you're affected by kidney disease, have a family history, or are simply interested in how nutrition and epigenetic strategies can support healthy aging, this episode will provide invaluable insights.  Thomas Weimbs PhD is a leading expert in kidney disease research. He is a professor at the University of California in Santa Barbara (UCSB) where he directs a research laboratory focused on polycystic kidney disease. He received his doctoral degree from the University of Cologne, Germany, and then conducted postdoctoral research at the University of California in San Francisco. In 1999, as an assistant professor, he started his first independent research laboratory at the Lerner Research Institute of the Cleveland Clinic. There, his research on polycystic kidney disease began with the support of a local family affected by PKD and whose family foundation still supports PKD research in Dr. Weimbs' lab today. In 2005, Dr. Weimbs moved his laboratory back to California to join the Department of Molecular, Cellular, and Developmental Biology at UCSB. The laboratory studies molecular mechanisms underlying PKD with an emphasis towards developing new therapies. Recent research has led to the discovery that dietary interventions and natural metabolites strongly slow PKD progression in animal models. This led to the launch of Santa Barbara Nutrients with the goal to make these approaches available for the nutritional management of PKD. GLP-1 Agonists for Chronic Kidney Disease (CKD)

Imagine a treatment that not only helps manage diabetes but also holds the potential to reverse kidney failure — what was once thought to be impossible. The recent FLOW study has remarkable groundbreaking evidence that GLP-1 receptor agonists can significantly improve kidney function in chronic kidney disease (CKD) patients. And while researchers try to understand the full implications of these findings, one thing is clear: the landscape of kidney disease treatment may never be the same again.  In this podcast, we'll dive into the results of the FLOW trial and explore what they may mean for people at risk for kidney failure. What are GLP-1 Agonists? GLP-1 (glucagon-like peptide-1) agonists are a class of drugs that mimic the effects of the natural hormone GLP-1. They enhance insulin secretion, inhibit glucagon release, and slow gastric emptying, improving blood sugar control. Beyond their primary use in diabetes management, GLP-1 agonists have been studied for various conditions, including obesity and cardiovascular disease prevention. The Study Overview The FLOW trial, which stands for "Fasting and Long-acting GLP-1 Receptor Agonist in Patients with Chronic Kidney Disease," focused on the impact of GLP-1 receptor agonists, specifically semaglutide, on patients with chronic kidney disease (CKD) and type 2 diabetes (those most at risk for kidney failure). Researchers observed a remarkable trend: patients receiving semaglutide demonstrated stabilization of kidney function and signs of renal recovery. This was groundbreaking, as reversing kidney damage has long been considered a near-impossible feat. Study Objectives The primary goal was to determine whether treatment with GLP-1 receptor agonists could improve kidney outcomes in patients with CKD. Study Design Participants: The study enrolled >3500 adults with type 2 diabetes and CKD. Intervention: Participants were randomized to receive either semaglutide or a placebo. Primary Outcomes: The main outcomes included changes in glomerular filtration rate (GFR) and the incidence of major adverse kidney events (kidney failure- need for transplantation or dialysis). Key Findings Renal Function Improvement: Patients on semaglutide showed significant improvements in glomerular filtration rates (GFR), a key indicator of kidney function. Reduced Inflammation: The drugs appeared to lower levels of inflammatory markers associated with kidney damage, suggesting a protective effect. Metabolic Benefits: Improvements in blood sugar control and weight loss were also observed, contributing to overall health and potentially alleviating stress on the kidneys. Adverse Events: The treatment was generally well-tolerated, with a favorable safety profile. Ethical Considerations As the results began to emerge, the ethical landscape of the study shifted dramatically. The control group, which was not receiving semaglutide, was found to be at a significantly higher risk of kidney deterioration. With compelling evidence that the GLP-1 agonist was safe and potentially life-saving, the ethics committee determined withholding treatment from any participants was no longer justifiable. Consequently, the study was halted prematurely to allow all participants access to the medication, prioritizing patient welfare over the continuation of research under an inequitable framework. This decision underscores the ethical responsibility of researchers to ensure that all patients receive the best possible care, especially when clear benefits are identified. Implications for Future Research While the early termination of the study raises questions about the completeness of the data collected, it also opens new avenues for further research. The findings encourage more extensive clinical trials to explore the mechanisms behind the renal protective effects of GLP-1 agonists. Additionally, there is a growing interest in investigating these drugs' long-term effects on kidney health and their potential role in preventing disease progression. As further studies unfold, there is hope that GLP-1 agonists will become a standard part of kidney disease management, potentially transforming outcomes for millions affected by this condition. As the Texas Kidney Foundation (TKF) continues to lead the charge in early detection, education, and support, Reginal shares his insights on the power of community and the importance of amplifying positive stories. Don't miss this thought-provoking conversation on the future of healthcare and the impact of proactive approaches to addressing CKD. About the Texas Kidney Foundation: Texas Kidney Foundation (TKF) is dedicated to the early detection of Chronic Kidney Disease (CKD). Because we know the difficulties that go along with CKD, we use a proactive approach to address the disease. We believe early detection, education, and support will help Texans maintain their quality of life. TKF began because there was a need for patient services in the San Antonio and Rio Grande Valley regions because the local chapter of the nation's largest kidney foundation closed its doors and left the area. Unfortunately, they didn't take kidney disease with them. Luckily a tenacious transplant physician, Francis Wright, MD decided to create a foundation that would stand strong to serve our community, Texas Kidney Foundation (TKF). He gathered together a board of civic-minded people with a connection to renal disease. TKF has continued in that tradition. In 2017 under new leadership TKF began to emerge as a state and national leader within the Kidney space. TKF embarked on the first leg of a statewide campaign called the Kidney C.A.R.E. (Clinician and Adult Renal Education) Campaign. The campaign addresses the lack of knowledge about CKD because the disease is overwhelming families all over Texas. According to the Center for Disease Control, 1 out of 3 adults with diabetes have CKD. Every day in the U.S. 130 diabetics begin treatment for kidney failure. TKF is the voice leading the charge to stop the CKD pandemic. January 8, 2020, The State of Texas reestablished the previously abolished Texas Chronic Kidney Disease Task Force.See omnystudio.com/listener for privacy information.

Welcome to another episode of Diary of a Kidney Warrior. This podcast episode is the first in a series that takes a deep dive into the world of pregnancy and Chronic Kidney Disease (CKD). Our expert guest, Dr. Jan, examines the nuances of a kidney damage diagnosis of the unborn baby during pregnancy and the onset of uncertainties that this health revelation carries. In this comprehensive understanding of CKD, Dr Jan addresses the considerable challenges associated with diagnosing the disease. The episode further explores the emotional turmoil parents endure upon their child's CKD detection and the importance of a multidisciplinary support team surrounding the affected family. As they delve deeper, we grasp the implications of a baby with one or two compromised kidneys, the risks, and its effect on their life quality. The narrative examines how both kidneys damaged by CKD can escalate to later disease stages, potentially necessitating kidney replacement therapy - dialysis or transplantation. The discussion unfolds touching upon the areas of neonatal nephrology, the significant impact of CKD on a child's growth, development, and learning abilities, with emphasis on the importance of kidneys for overall body health. Furthermore, Dee and Dr Jan explore the opportunities and limitations of neonatal dialysis, illuminating the complexities of medical interventions in newborns, especially those at extremely low birth weights. In conclusion, Dr Jan echos the paramount importance of acknowledging these uncertainties and advocate for extensive patient support, peer groups, and committed charities. Final remarks introduce useful resources, such as InfoKid, to aid informed decision-making in healthcare, hence underscoring the need for ample education and a robust support system for CKD patients and caregivers.   For updates and more, follow Dee Moore on:   Instagram: www.instagram.com/diaryofakidneywarrior  Facebook: www.facebook.com/diaryofakidneywarrior  Twitter: www.twitter.com/diaryofakidneyw  Tik Tok: @diaryofakidneywarrior  Youtube: https://www.youtube.com/channel/UChGUfib7lu9eKENlLJ6lafw Pinterest: https://www.pinterest.co.uk/diaryofakidneywarrior/   Join the Diary of a Kidney Warrior Family Mailing list to get new episode reminders, episode links, Vlogs, newsletters and more!   diaryofakidneywarrior.getform.com/rjv47   Join the Diary of a Kidney Warrior Family Transcript Email Mailing list to get new episode reminders, episode links, Vlogs, newsletters and more!   https://diaryofakidneywarrior.getform.com/ojp1p   infoKID provides information for parents and carers about kidney conditions in babies, children and young people.   https://infokid.org.uk/   If you were affected by any of the issues raised in this episode contact:   Kidney Care UK   •           Website: www.kidneycarekuk.org •           Email: info@kidneycareuk.org •           Tel: 01420 541 424 •           Facebook: www.facebook.com/kidneycareuk.org •           Instagram: @kidneycareuk •           YouTube: https://www.youtube.com/channel/UCeqQTdAsEzXphqjHVtcTD-A   To sign up to receive the Kidney Matters Quarterly Magazine Email: info@kidneycareuk.org

So, after he recovered from a kidney transplant and had just started getting his life back in order, he found himself back in the hospital fighting to avoid amputation. He talks to hosts Kym McNicholas and Dr. John Phillips about his life and limb-saving journey that is certain to give others hope that it's possible to live a longer, better quality of life, if only you stand up and advocate for what you feel is right for your care.

While not without risk, Randy thinks RNLX could be even bigger if successful as they are attacking the much larger Chronic Kidney Disease (CKD) market, and he discusses all of the signs he sees that the company is about to inflect.Randy's first appearance on Amyris (AMRS): https://www.youtube.com/watch?v=zCKxUHfCoQQ&t=1sChapters0:00 Intro1:20 RNLX overview8:55 Where does RNLX fall into CKD?14:00 Why now is the time for RNLX in CKD20:40 How RNLX can operate even without MICT24:30 More on RNLX and Medicare25:30 When will RNLX get FDA approval?33:40 What if RNLX doesn't get FDA approval?37:30 RNLX's CEO and founder, James McCullough43:40 Why is RNLX so under the radar?46:40 Comping RNLX to Theranos50:00 Randy's closing thoughts

What do you do when your life and health change in an instant?  In this highly-anticipated episode (part 2 of a 2-part series), my friend Andre Wallace shares his journey with kidney failure, and the many lessons he learned along the way.  He walked diligently with God on some of his darkest days, while also actively transforming his health.  The transparency and eloquence he lends to his journey will surely touch hearts and change lives.African Americans constitute more than 35% of all patients in the U.S. receiving dialysis for kidney failure, but only represent 13.2% of the overall U.S. population. (www.kidney.org)Simply, there is not enough outrage over the danger posed to the health of Blacks, especially Black men, by chronic disease.  While there are a multitude of complex factors at play, the fact remains that Black men are dying at an alarming rate from the complications of diabetes and high blood pressure.  Chronic Kidney Disease (CKD), or kidney failure, is often the result of years of uncontrolled blood pressure and/or diabetes.  (Though there are other causes, these are the most common, by far.) When it starts, much like high blood pressure, it is largely silent.  Our bodies are very resilient and will maintain our kidney function, the filtration system for the blood, at all costs.  Accordingly, many people with kidney disease do not have severe (or recognizable) symptoms until it is very advanced and they are in need of dialysis.  About Dre:Andre Wallace is a man of God, a father of 3 and a mentor to many.  He is a barber instructor at the Ohio State Barber College and Cosmetology.  He is a gifted artist and vocalist, who also aspires to explore filmmaking and podcasting.  Follow Dre on:IG: @drecassoartFollow Pivot & Bloom on:FB: Pivot & BloomIG: @pivotandbloompodcasthttps://pivotandbloom.buzzsprout.com
